    Clinical impact of bladder biopsies with TUR-BT according to cytology results in patients with bladder cancer: a case control study

    <p>Abstract</p> <p>Background</p> <p>There seems to be no consensus concerning taking bladder biopsies during transurethral resection of bladder tumor (TUR-BT). We investigate the clinical significance of bladder biopsy with TUR-BT and the relationship between urinary cytology and the biopsy results.</p> <p>Methods</p> <p>We reviewed a total of 424 patients with non-muscle invasive bladder cancer treated with TUR-BT between 1998 and 2005. Of the total, 293 patients also underwent a bladder biopsy. Biopsies from suspicious-appearing urothelium (N = 59) and those from normal-appearing urothelium (N = 234) were evaluated separately.</p> <p>Results</p> <p>Bladder cancer was observed in 23 cases (39.0%) who underwent a biopsy of suspicious-appearing urothelium. Among these 23 cases, 9 cases with visible tumor resection had carcinoma in situ (CIS) only in the biopsies from suspicious-appearing urothelium. Urinary cytology was negative in 3 of the 9 cases. Bladder cancer was observed in 26 cases (11.1%) who underwent a biopsy of normal-appearing urothelium. Of them, 5 cases with visible tumors had CIS only in the multiple biopsies from normal-appearing urothelium. Urinary cytology was positive in all of the 5 cases. No upstaging or upgrading cases were found in these patients by the addition of these two types of biopsy. Furthermore, therapy was not altered in these patients. With or without bladder biopsy was not a significant factor for tumor recurrence in either the univariate or multivariate analysis.</p> <p>Conclusions</p> <p>Based on the results, it is concluded the multiple biopsies from normal-appearing urothelium are not necessary in patients with negative cytology results because of the low detection rate and lack of influence on therapeutic decisions. Meanwhile, biopsy of suspicious-appearing urothelium is needed in patients with negative cytology results in order to detect CIS due to staging properties. This result supports a recent EAU guideline.</p

    Magnetic resonance (MR) imaging assessment for glossopharyngeal neuralgia : value of three-dimensional T2-reversed MR imaging (3D-T2R) in conjunction with other modes of 3D MR imaging

    Background: To retrospectively examine the usefulness of gray-scale reversal imaging of T2-weighted images (3D-T2R) in conjunction with other modes of 3D MRI for preoperative assessments in patients with glossopharyngeal neuralgia (GPN) due to neurovascular compression. Material/Methods: Imaging findings on 3D-T2R, constructive interference in steady state (CISS), and MRA were analyzed with reference to operative charts in 10 patients with GPN. Results: Offending vessels were associated with the posterior inferior cerebellar artery (PICA) in 9 of 10 patients (90%). Eight of the 10 patients (80%) had offending vessels located at the supraolivary fossette. Of those eight patients, six (75%) had a shift of the ipsilateral vertebral artery to the affected side. Five (42%) and seven (48%) contact points were associated with the root entry/exit zone and the peripheral nerve system segment, respectively. In six of nine contact points (67%), 3D-T2R demonstrated the pathomorphological features at the contact points better than CISS. Conclusions: The offending vessels were mostly associated with posterior inferior cerebellar arteries, were frequently located at the supraolivary fossette, and had attachments at the root entry/exit zone and at the peripheral segment of the glossopharyngeal nerve, which was well demonstrated on 3D-T2R

    Enhancement of low-frequency spin-orbit-torque ferromagnetic resonance signals by frequency tuning observed in Pt/Py, Pt/Co, and Pt/Fe bilayers

    DC voltages via spin rectification effect (SRE), VDC, under microwave irradiation are investigated for three platinum (Pt)/ferromagnetic metal (FM) bilayer structures: Pt/Ni₈₀Fe₂₀, Pt/Co, and Pt/Fe. At the microwave frequency region lower than the resonant frequency, large VDC is obtained at zero DC magnetic field for all devices. In frequency dependence just around the resonant frequency, sharp rise and drop of magnitude in VDC are observed. These behaviors are well explained by the numerically calculated magnetic susceptibility. It is also found that the magnitude of VDC is strongly dependent on the slope of magnetoresistance spectrum. These findings lead to developments of sensitive detection technique for nano-scale magnetization switching

    AbstractA cDNA clone for porcine liver proline-β-naphthylamidase was isolated and sequenced. The deduced amino acid sequence of 567 residues was highly homologous with those of carboxylesterases (EC 3.1.1.1) previously reported for other species. In addition, proline-β-naphthylamidase purified from porcine liver was shown to have strong activity towards p-nitrophenylacetate, a representative substrate for carboxylesterases. These results suggest that proline-β-naphthylamidase is identical with carboxylesterase

    Observation of gigantic spin conversion anisotropy in bismuth

    Whilst the g-factor can be anisotropic due to the spin-orbit interaction (SOI), its existence in solids cannot be simply asserted from a band structure, which hinders progress on studies from such the viewpoints. The g-factor in bismuth (Bi) is largely anisotropic; especially for holes at T-point, the g-factor perpendicular to the trigonal axis is negligibly small (< 0.112), whereas the g-factor along the trigonal axis is very large (62.7). We clarified in this work that the large g- factor anisotropy gives rise to the gigantic spin conversion anisotropy in Bi from experimental and theoretical approaches. Spin-torque ferromagnetic resonance was applied to estimate the spin conversion efficiency in rhombohedral (110) Bi to be 17%, which is unlike the negligibly small efficiency in Bi(111). Harmonic Hall measurements supports the large spin conversion efficiency in Bi(110). This is the first observation of gigantic spin conversion anisotropy as the clear manifestation of the g-factor anisotropy. Beyond the emblematic case of Bi, our study unveiled the significance of the g-factor anisotropy in condensed-matter physics and can pave a pathway toward establishing novel spin physics under g-factor control.Comment: 28 pages, 7 figure

Here, we describe a case wherein the patient&nbsp;accidentally swallowed a 7-cm-sized mouthguard that could not be removed by emergency upper gastrointestinal endoscopy but was&nbsp;subsequently removed via the anus after a period of watchful waiting

    本校では特別支援学級と複式学級の教科による交流及び共同学習の導入や,学級交流や縦割り学習,運動会等の校内行事での交流などに取り組んできている。本研究は,これまでの成果を基に,通常の学級に在籍する児童の多様性や障害の理解を促す実践を交流及び共同学習や教科学習の中で行い,その効果を検証することを目的とした。特別支援学級の担任ではない教員が,特別支援学級の指導案を作成することを通して,日頃の交流だけではとらえられない特別支援児童の実態に気づき,支援の在り方について検討する。また実際に授業を行うことで特別支援領域に関する見方・考え方を活用し,それぞれの学級の児童支援に生かし,共有できるようにした。授業を行う教員の研究教科が算数科であることから算数科で授業を行うこととし,都合5時間の特別支援学級教員による算数科の授業を観察,児童の実態把握を行い,聞き取りも含め授業構想を行っていった。実際の授業から,子どもたちの中に価値に関する概念が育成されているかどうかの違いがあることに気付く。価値概念の育成や獲得方法を明らかにしていくことによって小学校の教科の指導をより充実させていくことができると考えた。Do the practice of promoting an understanding of the children of diversity and disorder enrolled in regular classes in the exchange and collaborative learning and subject learning, was aimed to verify the effect.     Dysgeusia during cancer treatment and dietary intervention

    For patients with cancer, malnutrition is one of the most serious problems. Cancer treatments, such as chemotherapy and radiotherapy, are effective for metastasis and tumor reduction as adjuvant therapy at the perioperative stage, in addition to prolonging the life of a patient and providing a radical cure. On the other hand, loss of appetite that is induced by the above treatment sometimes worsens the nutritional health of a patient. Therefore, it confers prolonged hospitalization and a delay in additional chemotherapy or surgery. Moreover, other side effects besides the loss of appetite due to chemoradiotherapy(hair loss, nausea, vomiting, and diarrhea, among others)can lead to worse nutritional health. Among these side effects, taste disorder is a major factor of decreasing meal intake and is a severe problem occurring frequently during treatment. However, its fundamental reasons, remedial measures, and treatments have not been established yet. In this article, we will report the previous basic research and clinical problems of dysgeusia that occurs during cancer treatment, and introduce a nutritional approach to preventing or improving dysgeusia

    The novel heart-specific RING finger protein 207 is involved in energy metabolism in cardiomyocytes

    A failing heart shows severe energy insufficiency, and it is presumed that this energy shortage plays a critical role in the development of cardiac dysfunction. However, little is known about the mechanisms that cause energy metabolic alterations in the failing heart. Here, we show that the novel RING-finger protein 207 (RNF207), which is specifically expressed in the heart, plays a role in cardiac energy metabolism. Depletion of RNF207 in neonatal rat cardiomyocytes (NRCs) leads to a reduced cellular concentration of adenosine triphosphate (ATP) and mitochondrial dysfunction. Consistent with this result, we observed here that the expression of RNF207 was significantly reduced in mice with common cardiac diseases including heart failure. Intriguingly, proteomic approaches revealed that RNF207 interacts with the voltage-dependent anion channel (VDAC), which is considered to be a key regulator of mitochondria function, as an RNF207-interacting protein. Our findings indicate that RNF207 is involved in ATP production by cardiomyocytes, suggesting that RNF207 plays an important role in the development of heart failure
