Accelerated heart rate is a risk factor of general and cardiovascular mortality in different populations. The classical
treatment to achieve heart rate reduction by beta-blockers and calcium channel antagonists from the group of nondihydropyridines
(verapamil and diltiazem), is often limited by contraindications or adverse reactions. Ivabradine is
a unique, modern drug which mechanism of action is blocking the If current in the sinoatrial node. This translates into
a reduction in heart rate — a parameter associated with poor prognosis in patients with heart failure (HF). The following
document contains the opinion of an expert group summarizing current knowledge on ivabradine and its use in population

Factors affecting the choice of therapeutic regimens in asthma and chronic obstructive pulmonary disease patients and patient adherence to the therapies prescribed
Introduction: Chronic obstructive pulmonary disease (COPD) and asthma are the most common chronic respiratory
diseases worldwide. At the moment, there is no information about the preferences of Polish specialists as regards
the treatment of asthma and COPD or factors influencing the choice of particular treatment regimens.
Aim: To determine the treatment options most commonly used by experienced pulmonologists and allergists for
asthma and COPD and to identify the factors affecting the choice of a particular therapy.
Material and methods: The survey included 224 doctors (pulmonologists and allergists) across Poland and concerned
patients diagnosed with asthma (n = 4358) and COPD (n = 3062).
Results: In the case of asthma, the most common therapy applied was inhaled glucocorticosteroids and long-acting
\beta2 agonists. According to 27.2% of doctors, combination therapy was used in 70-80% of patients while 23.7%
declared that the proportion of patients receiving such a treatment exceeded 80%. In the case of COPD, anticholinergics
were most frequently prescribed when inhaled glucocorticosteroids and long-acting \beta2 agonists had proved
insufficient. According to 21% of specialists, the percentage of patients treated so was 41-50%, while 19% declared
the use of this treatment in 71-80% of patients.
Conclusions: The most common treatments for asthma and COPD in Poland are inhaled glucocorticosteroids and
long-acting \beta2 agonists. The main factors influencing treatment decisions are the current GINA and GOLD recommendations
as well as patients' age, comorbidities, and price of treatment

Kserostomia u pacjentów geriatrycznych. Problemy i możliwości terapeutyczne — przegląd literatury
Fizjologiczne zmiany związane z procesem starzenia, choroby przewlekłe czy farmakoterapia przyczyniają się do szczególnie istotnego w grupie pacjentów geriatrycznych problemu, jakim jest kserostomia. W artykule zebrano i omówiono najczęstsze przyczyny kserostomii w populacji pacjentów geriatrycznych ze szczególnym uwzględnieniem najczęściej występujących w tej grupie wiekowej chorób oraz przyjmowanych leków. Opisano możliwe powikłania kserostomii, a także możliwości przełożenia się tego zjawiska na stan zdrowia i jakość życia. Przedstawiono również możliwości terapeutyczne (farmakologiczne i niefarmakologiczne) w terapii kserostomii z uwzględnieniem roli lekarza podstawowej opieki zdrowotnej zarówno w zapobieganiu samej kserostomii, jak również jej diagnostyki i skuteczności leczenia
Antihypertensive treatment among 75 thousand patients in primary care settings - POZ-NAD study
Wstęp Wyniki badań epidemiologicznych wskazują na
wciąż niezadowalającą skuteczność leczenia nadciśnienia
tętniczego. W zaleceniach ESH/ESC z 2007 roku
podkreślono znaczenie obniżania ciśnienia tętniczego
do wartości 140/90 mm Hg i niższych oraz do wartości
co najmniej poniżej 130/80 mm Hg u chorych z nadciśnieniem
tętniczym i współistniejącą cukrzycą lub rozpoznaną
chorobą układu sercowo-naczyniowego. Celem
prezentowanego dużego, ogólnopolskiego badania
było dokonanie charakterystyki chorych na nadciśnienie
tętnicze, pozostających pod opieką lekarzy podstawowej
opieki zdrowotnej (POZ).
Materiał i metody Do badania włączono chorych na
nadciśnienie tętnicze w wieku 18 lat i więcej. Na
podstawie badania podmiotowego oraz badania
przedmiotowego lekarz wypełniał specjalnie przygotowany
kwestionariusz, który obejmował dane
z badania przedmiotowego oraz podmiotowego, a także
wyniki wybranych badań laboratoryjnych. Ryzyko
rozwoju cukrzycy oceniono na podstawie karty
FINDRISC.
Wyniki Do badania włączono 74 745 pacjentów (śr.
wiek 59,7 ± 11,5 rż., kobiety - 58,5%). Średnia wartość
wskaźnika masy ciała w badanej grupie wyniosła
28,8 kg/m2. Otyłość brzuszna występowała u prawie
połowy chorych. U 23,1% i 6,5% chorych ryzyko
rozwoju cukrzycy określone za pomocą karty FINDRISC
było odpowiednio wysokie i bardzo wysokie.
Średnia liczba przyjmowanych leków hipotensyjnych
w badanej grupie wyniosła 1,9 ± 1,0 leku, mediana
- 2 leki. W badanej grupie 60,1% chorych
przyjmowało 2 i więcej leki hipotensyjne, natomiast
3 i więcej leki hipotensyjne przyjmowało 28,5% pacjentów.
U osób bez rozpoznanej choroby układu
sercowo-naczyniowego lub cukrzycy typu 2 docelowe
wartości ciśnienia tętniczego (< 140/90 mm Hg) stwierdzono
u 33,3% chorych, natomiast stężenie cholesterolu
frakcji LDL 115 mg/dl i mniej u 35,1% chorych. U chorych
z rozpoznaną chorobą układu sercowo-naczyniowego
lub cukrzycą typu 2 docelowe wartości ciśnienia
tętniczego (< 130/80 mm Hg) stwierdzono u 7,0% pacjentów,
natomiast stężenie cholesterolu frakcji LDL
100 mg/dl i mniej u 21,7% chorych.
Wnioski W prezentowanym badaniu wykazano, że
nadciśnienie tętnicze jest źle kontrolowane u znacznego
odsetka chorych. Niższe, zalecane w obowiązujących wytycznych wartości docelowe, stwierdzono
u mniej niż 10% pacjentów z cukrzycą typu 2 lub
chorobą układu sercowo-naczyniowego. Uzyskane
wyniki wskazują na niewystarczające stosowanie się
do obowiązujących zaleceń w praktyce klinicznej.Background It has been reported that hypertension is still
inadequately controlled in the majority of patients. Recent
ESH/ESC guidelines (2007) emphasize that blood pressure
should be lowered below 140/90 mm Hg in all hypertensive
patients and below 130/80 mm Hg in hypertensive subjects
with diabetes or associated cardiovascular disease. The aim
of present study was to analyze clinical characteristics of
hypertensive subjects in primary care settings in a large sample
of patients in Poland.
Material and methods The study included hypertensive
subjects aged 18 years or more. Based on medical history
and physical examination family practitioners filled questionnaires.
Selected laboratory investigations were also included.
The risk of diabetes was established based on Finnish
diabetes risk score (FINDRISC).
Results 74 745 hypertensive subjects were included in the study
(mean age 59.7±11.5 years, females 58.5%). Mean body mass
index was 28.8 kg/m2. Abdominal obesity was found in almost
half of the patients. In 23.1% i 6.5% patients diabetes risk based
on FINDRISC score was high and very high. Mean number of
antihypertensive drugs was 1.9 ± 1.0, median 2 drugs. In the
studied population, 60.1% of patients received 2 or more antihypertensive
drugs. 3 or more antihypertensive drugs were used in 28.5% of patients. In patients without diabetes or established
cardiovascular disease target blood pressure levels were achieved
in 1 of 3 patients. LDL cholesterol serum concentration of
115 mg/dl or lower was found in 35.1% of patients. In patient
with diabetes or established cardiovascular disease target blood
pressure levels (< 130/80 mm Hg) were achieved in 7.0%
of patient. LDL cholesterol serum concentration of 100 mg/dl
or lower was found in 21.7% of patients.
Conclusions In the large sample of hypertensive subjects
in the primary care settings, target blood pressure levels were
achieved in the minority of patients. Lower target blood pressure
levels recommended by recent guidelines were achieved
in less than 10% of patients with diabetes or established
cardiovascular disease. The implementation of current recommendation
is insufficient in primary care settings
Weight stigma and fat phobia in Poland – attitudes towards people living with obesity and the level of knowledge about obesity among the social media internet respondents and medical professionals
IntroductionObesity often subjects individuals to stigmatization, impacting self-esteem, contributing to depression, social isolation, and even exacerbating weight gain. Our research aimed to evaluate weight stigma, fat phobia, their expressions, and obesity-related knowledge among social media internet respondents and medical practitioners in Poland.MethodsConducted through Computer-Assisted Web Interview (CAWI), our study employed the Fat Phobia Scale (FPS) and tailored questions, analyzing 1705 questionnaires.ResultsThe respondents averaged a score of 3.60 ± 0.62 on the FPS. Interestingly, men exhibited higher stigma levels than women. Variables like BMI, residency, and interactions with people having obesity did not significantly impact stigma levels. Approximately 74.0% of respondents found individuals with obesity less attractive than those with normal weight, while 32.2% identified obesity as a cause of shame. Only 69.1% were aware of the BMI-based obesity diagnosis criterion.ConclusionGiven limited knowledge of Poland’s weight stigma landscape, our research yields crucial insights for shaping social campaigns and enhancing educational initiatives in obesity management for healthcare professionals. Further studies will be instrumental in addressing patient and practitioner needs effectively

The strategies to support the COVID-19 vaccination with evidence-based communication and tackling misinformation
COVID-19 vaccinations are about to begin in various countries or are already ongoing. This is an unprecedented operation that is also met with a loud response from anti-vaccine communities—currently using all available channels to manipulate public opinion. At the same time, the strategy to educate on vaccinations, explain their mechanism of action, and build trust in science is subdued in different world parts. Such actions should go much beyond campaigns promoting the COVID-19 vaccines solely on the information provided by the health institutions and national authorities. In this paper, actions provided by independent expert groups needed to counteract the anti-vaccine propaganda and provide scientific-based information to the general public are offered. These actions encompass organizing groups continuously communicating science on COVID-19 vaccines to the general public; tracking and tackling emerging and circulating fake news; and equipping celebrities and politicians with scientific information to ensure the quality of messages they communicate, as well as public letters, and statements of support for vaccination by healthcare workers, recognized scientists, VIPs, and scientific societies; and no tolerance to false and manipulated claims on vaccination spread via traditional and social media as well as by health professionals, scientists, and academics. These activities should be promptly implemented worldwide, regardless of the current status and availability of the COVID-19 vaccine in a particular region. If we are about to control the pandemic for the sake of public benefit, it is high time to collectively speak out as academic and medical societies with support from decision-makers. Otherwise, the battle will be lost to those who stand against scientific evidence while offering no feasible solution to the problem

Expert opinion of the Heart Failure Association of the Polish Society of Cardiology, the College of Family Physicians in Poland, and the Polish Society of Family Medicine on the peri discharge management of patients with heart failure
Despite advances in the treatment of heart failure (HF), the rate of hospitalisation for exacerbations of the disease remains high. One of the underlying reasons is that recommended guidelines for the management of HF are still too rarely followed in daily practice. Disease exacerbation requiring inpatient treatment is always a factor that worsens the prognosis, and thus signals disease progression. This is also a key moment when therapy should be modified for HF exacerbation, or initiated in the case of newly diagnosed disease. Inpatient treatment and the peri‑discharge period is the time when the aetiology and mechanism of HF decompensation should be established. Therapy should be individualised based on aetiology, HF phenotype, and comorbidities; it should take into account the possibilities of modern treatment. According to the recommendations of the European Society of Cardiology (ESC), patients with HF should receive multidisciplinary management. Cooperation between the various members of the multidisciplinary team taking care of patients with HF improves the efficiency and quality of treatment.
