    Sustainability of Italian families' food practices: Mediterranean diet adherence combined with organic and local food consumption

    The present research intends to contribute to the literature on sustainable food consumption by investigating the present-day dietary patterns of Italian households to assess the degree of adherence to the Mediterranean diet together with the level of organic and local food consumption. The analysis is based on data from the Italian National Institute of Statistics survey “Aspects of Daily Life” for the year 2014; this is a Multipurpose Survey conducted in Italy with 44,984 individuals across 18,864 households. A Heckman two-step probit model was implemented to estimate equations to assess the impact of Italian household composition and adherence to the Mediterranean diet upon organic and local food consumption. The study's findings show that both those households with children and those with a higher degree of adherence to the Mediterranean diet have higher probabilities of buying both organic and local products, while increasing household size reduces the likelihood of buying local products. Furthermore, good economic status and a higher level of education increase the probability of consuming organic food. Some of the differences highlighted are of low amplitude; however, these results offer novel and original insights for Italy and support the need to for both policy makers and marketers to implement a wide range of initiatives aimed at educating consumers about, and facilitating access to sustainable food products

    Diagnosis of primary headache in children younger than 6 years: A clinical challenge

    Background: Criteria defined by the International headache Society are commonly used for the diagnosis of the different headache types in both adults and children. However, some authors have stressed some limits of these criteria when applied to preschool age. Objective: Our study aimed to describe the characteristics of primary headaches in children younger than 6 years and investigate how often the International Classification of Headache Disorders (ICHD) criteria allow a definitive diagnosis. Methods: This retrospective study analysed the clinical feature of 368 children younger than 6 years with primary headache. Results: We found that in our patients the percentage of undefined diagnosis was high when either the ICHD-II or the ICHD-III criteria were used. More than 70% of our children showed a duration of their attacks shorter than 1 hour. The absence of photophobia/phonophobia and nausea/vomiting significantly correlate with tension-type headache (TTH) and probable TTH. The number of first-degree relatives with migraine was positively correlated to the diagnosis of migraine in the patients (p<0.001). Conclusions: Our study showed that the ICHD-III criteria are difficult to use in children younger than 6 years. The problem is not solved by the reduction of the lowest duration limit for the diagnosis of migraine to 1 hour, as was done in the ICHD-II

    Identification of platelet hyper-reactivity measured with a portable device immediately after percutaneous coronary intervention predicts in stent thrombosis

    Introduction: Platelet hyper-reactivity, despite a standard anti-thrombotic therapy, is a recognized risk factor for recurrent myocardial ischemia and in-stent thrombosis following PCI. We have investigated whether this detrimental condition, measured by collagen-epinephrine closure times (CEPI-CT) with the Platelet Function Analyzer (PFA-100) device could predict IST defined as the composite of cardiovascular death or myocardial infarction. Materials and methods: CEPI-CT was measured in 256 consecutive patients with stable angina (n=103) or ACS (n=153) 30?8 h after PCI (T0) and 1 month later (T1). All patients were followed up for a mean period of 9 months. Platelet hyperactivity was defined as a CEPI-CTb190 s. Results: Baseline CEPI-CTb190 s was associated with a higher rate of death or MI (LogRank χ2 =4.23, p=0.039) as compared with CEPI-CTN190 s (4.6% vs. 0.7%). Multivariable analysis after adjustment for other risk factors confirmed that baseline CEPI-CTb190 s was an independent correlate for death or MI (Hazard ratio 6.981, p=0.008).     Sequencing of NOTCH1, GATA5, TGFBR1 and TGFBR2genes in familial cases of bicuspid aortic valve

    BACKGROUND: The purpose of our study was to investigate the potential contribution of germline mutations in NOTCH1, GATA5 and TGFBR1 and TGFBR2 genes in a cohort of Italian patients with familial Bicuspid Aortic Valve (BAV). METHODS: All the coding exons including adjacent intronic as well as 5(′) and 3(′) untranslated (UTR) sequences of NOTCH1, GATA5, TGFBR1 and TGFBR2 genes were screened by direct gene sequencing in 11 index patients (8 males; age = 42 ± 19 years) with familial BAV defined as two or more affected members. RESULTS: Two novel mutations, a missense and a nonsense mutation (Exon 5, p.P284L; Exon 26, p.Y1619X), were found in the NOTCH1 gene in two unrelated families. The mutations segregated with the disease in these families, and they were not found on 200 unrelated chromosomes from ethnically matched controls. No pathogenetic mutation was identified in GATA5, TGFBR1 and TGFBR2 genes. CONCLUSIONS: Two novel NOTCH1 mutations were identified in two Italian families with BAV, highlighting the role of a NOTCH1 signaling pathway in BAV and its aortic complications. These findings are of relevance for genetic counseling and clinical care of families presenting with BAV. Future studies are needed in order to unravel the still largely unknown genetics of BAV

    ODINet is a research and development project, approved as part of the Regional Operational Programme through the European Regional Development Fund 2007-2013. The project involves the construction of a semantic search engine prototype able to catalog the data in a ontological graph, to extract the most relevant information depending on user requests and return them in a highly usable way. The application domain concerns the social, economic and health, in order to cover most of the data held by public bodies in the national context. The focus of this report is in the first place the description of the semantic components of the platform, emphasizing how the ontologies have been used to build an index in the form of graph. We also present a description of our semantic searches and finally an analysis of the results obtained in the final stage of the ODINEt prototype testing

    Late plasma exosome microRNA-21-5p depicts magnitude of reverse ventricular remodeling after early surgical repair of primary mitral valve regurgitation

    Introduction: Primary mitral valve regurgitation (MR) results from degeneration of mitral valve apparatus. Mechanisms leading to incomplete postoperative left ventricular (LV) reverse remodeling (Rev–Rem) despite timely and successful surgical mitral valve repair (MVR) remain unknown. Plasma exosomes (pEXOs) are smallest nanovesicles exerting early postoperative cardioprotection. We hypothesized that late plasma exosomal microRNAs (miRs) contribute to Rev–Rem during the late postoperative period. Methods: Primary MR patients (n = 19; age, 45–71 years) underwent cardiac magnetic resonance imaging and blood sampling before (T0) and 6 months after (T1) MVR. The postoperative LV Rev–Rem was assessed in terms of a decrease in LV end-diastolic volume and patients were stratified into high (HiR-REM) and low (LoR-REM) LV Rev–Rem subgroups. Isolated pEXOs were quantified by nanoparticle tracking analysis. Exosomal microRNA (miR)-1, –21–5p, –133a, and –208a levels were measured by RT-qPCR. Anti-hypertrophic effects of pEXOs were tested in HL-1 cardiomyocytes cultured with angiotensin II (AngII, 1 μM for 48 h). Results: Surgery zeroed out volume regurgitation in all patients. Although preoperative pEXOs were similar in both groups, pEXO levels increased after MVR in HiR-REM patients (+0.75-fold, p = 0.016), who showed lower cardiac mass index (–11%, p = 0.032). Postoperative exosomal miR-21-5p values of HiR-REM patients were higher than other groups (p < 0.05). In vitro, T1-pEXOs isolated from LoR-REM patients boosted the AngII-induced cardiomyocyte hypertrophy, but not postoperative exosomes of HiR-REM. This adaptive effect was counteracted by miR-21-5p inhibition. Summary/Conclusion: High levels of miR-21-5p-enriched pEXOs during the late postoperative period depict higher LV Rev–Rem after MVR. miR-21-5p-enriched pEXOs may be helpful to predict and to treat incomplete LV Rev–Rem after successful early surgical MVR

    The 8 and 9 September 2002 flash flood event in France: a model intercomparison

    Within the framework of the European Interreg IIIb Medocc program, the HYDROPTIMET project aims at the optimization of the hydrometeorological forecasting tools in the context of intense precipitation within complex topography. Therefore, some meteorological forecast models and hydrological models were tested on four Mediterranean flash-flood events. One of them occured in France where the South-eastern ridge of the French “Massif Central”, the Gard region, experienced a devastating flood on 8 and 9 September 2002. 24 people were killed during this event and the economic damage was estimated at 1.2 billion euros. To built the next generation of the hydrometeorological forecasting chain that will be able to capture such localized and fast events and the resulting discharges, the forecasted rain fields might be improved to be relevant for hydrological purposes. In such context, this paper presents the results of the evaluation methodology proposed by Yates et al. (2005) that highlights the relevant hydrological scales of a simulated rain field. Simulated rain fields of 7 meteorological model runs concerning with the French event are therefore evaluated for different accumulation times. The dynamics of these models are either based on non-hydrostatic or hydrostatic equation systems. Moreover, these models were run under different configurations (resolution, initial conditions). The classical score analysis and the areal evaluation of the simulated rain fields are then performed in order to put forward the main simulation characteristics that improve the quantitative precipitation forecast. The conclusions draw some recommendations on the value of the quantitative precipitation forecasts and way to use it for quantitative discharge forecasts within mountainous areas

    Genetic variants in the immunomodulatory gene mannose-binding lectin 2 (MBL2), were associated with risk, severity, and frequency of viral infections. In a case-control setting, we investigated the association of MBL2 functional polymorphisms with Human Papillomas Virus (HPV) infection. No differences between cases (HPV+) and controls (HPV-) were found in the distribution of each single genotypes or allele. Haplotype analysis did not show any difference between HPV+ and HPV- groups

    Predictive value of less than moderate residual mitral regurgitation as assessed by transesophageal echocardiography for the short-term outcomes of patients with mitral regurgitation treated with mitral valve repair

    <p>Abstract</p> <p>Background</p> <p>Traditionally, in patients with mitral regurgitation (MR) a successful mitral valve repair is considered when residual MR by post-pump transesophageal echocardiography (TEE) is less than moderate or absent. Little is known about the prognostic value of less than moderate (mild or mild-to-moderate) residual MR for the early outcome of patients treated with mitral valve repair.</p> <p>Methods</p> <p>Eligible for this study were patients undergoing isolated mitral valve repair. Patients with moderate or severe residual MR after valve repair were excluded. The primary endpoint of the study was the composite of death or need of reintervention.</p> <p>Results</p> <p>A total of 98 patients (54 with no residual MR-Group 1, and 44 with less than moderate residual MR-Group 2) were analyzed. Of these, 72% presented with New York Heart Association (NYHA) 3/4, and 38% were women. The primary endpoint of the study occurred in 3 (5.5%) patients in Group 1 and 6 (13.6%) patients in Group 2 MR (<it>P </it>= 0.31). There was a trend toward a higher incidence of use of inotropic drugs post-interventional (<it>P </it>= 0.12), and a longer hospital stay among patients with less than moderate residual MR (<it>P </it>= 0.18).</p> <p>Conclusion</p> <p>In our study population, patients with less than moderate residual MR had a trend toward a higher risk of early adverse outcomes as compared with patients with no residual MR by post-pump TEE. Studies with a larger patient population and longer follow-up data may be useful to better define the clinical significance of residual mild MR after mitral vale repair.</p
