    Self-efficacy for coping. Utility of the Cancer behavior inventory (Italian) for use in palliative care

    Background: Newer models of palliative and supportive cancer care view the person as an active agent in managing physical and psychosocial challenges. Therefore, personal efficacy is an integral part of this model. Due to the lack of instruments in Italian to assess coping self-efficacy, the present study included the translation and validation of the Italian version of the Cancer Behavior Inventory-Brief (CBI-B/I) and an initial analysis of the utility of self-efficacy for coping in an Italian sample of palliative care patients. Methods: 216 advanced cancer patients who attended palliative care clinics were enrolled. The CBI-B/I was administered along with the European Organization for Research and Treatment of Cancer Quality of Life Questionnaire-Core 30 (EORTC QLQ-C30), the Mini Mental Adjustment to Cancer Scale (Mini-MAC), the Cancer Concerns Checklist (CCL), and the Hospital Anxiety and Depression Scale (HADS). The Eastern Cooperative Oncology Group Performance Status (ECOG-PS) ratings of functional capacity were completed by physicians. Results: Factor analysis confirmed that the structure of the CBI-B/I was consistent with the English version. Internal consistency reliability and significant correlations with the EORTC QLQ-C30, Mini-MAC, and HADS supported the concurrent validity of the CBI-B/I. Differences in CBI-B/I scores for high versus low levels of the CCL and ECOG-PS supported the clinical utility of the CBI-B/I. Conclusions: The CBI-B/I has strong psychometric properties and represents an important addition to newer model of palliative and supportive care. In order to improve clinical practice, the CBI-B/I could be useful in identifying specific self-efficacy goals for coping in structured psychosocial intervention

    The antiphospholipid syndrome (APS) is an autoimmune disease characterized by vascularthrombosis, recurrent abortions and persistent elevation of antiphospholipid antibodies. Since its first description in 1983, a wide variety of dermatological manifestations have been associated with this syndrome, most frequently livedo reticularis and skin ulcers. The presence of antiphospholipid antibodies has been described in several diseases such as autoimmune diseases, infections, neoplasms and secondarily to the use of certain drugs. We report the case of a 72-year-old diagnosed with Rheumatoid Arthritis and secondary APS, for its unusual relationship.El Síndrome antifosfolípido (SAF) es una patología autoinmune que se caracteriza por presentar trombosis vascular, abortos recurrentes, así como una elevación persistente de los anticuerpos antifosfolípidos. Desde su primera descripción en el año 1983, una amplia variedad de manifestaciones dermatológicas han sido asociadas a este síndrome, siendo las más frecuentes el livedo reticularis y las úlceras cutáneas. La presencia de los anticuerpos antifosfolípidos ha sido descrita en diversas patologías como en enfermedades autoinmunes, infecciones, neoplasias y de forma secundaria al uso de determinados fármacos.     Leprosy is a chronic infectious disease with a broad spectrum of clinical manifestations. Luciophenomenon and necrotic erythema nodosum leprosum are two vasculonecrotic reactions that can mimic cutaneous vasculitis in leprosy. Although they can be distinguished through clinical and histopathological features, both are often used as synonyms. Lucio Phenomenon, a severe immunecomplex mediated skin reaction, is a necrotizing skin condition that occurs in patients with non-nodular or diffuse leprosy which occurs in patients with little or no resistance to infection. Vasculonecrotic reactions due to erythema nodosum occur in other forms of leprosy different from the diffuse form. We present the case of 3 patients with lepromatous leprosy who presented vasculonecrotic reactions. The differential diagnosis between Lucio phenomenon and necrotic erythema nodosum leprosum is important since both entities have different treatments. In a patient with cutaneous vasculitic lesions, infectious etiologies including leprosy, should always be considered.La lepra es una enfermedad crónica con un amplio espectro de manifestaciones clínicas. Las reacciones lepromatosas vasculonecróticas que incluyen al fenómeno de Lucio y el eritema nodoso necrótico (EN) pueden simular una vasculitis cutánea. A pesar de que pueden distinguirse a través de las características clínicas e histopatológicas, ambos a menudo se utilizan como sinónimo. El Fenómeno de Lucio (FL), reacción cutánea grave y distintiva mediada por inmunocomplejos, es una afección cutánea necrosante que ocurre en pacientes portadores de Lepra no nodular o difusa que se produce en aquellos pacientes con poca o nula resistencia a la infección. Las reacciones vasculonecróticas (VN) por eritema nodoso se presentan en otras formas de la lepra diferentes a la forma difusa. Presentamos el caso de 3 pacientes con lepra lepromatosa que presentaron reacciones vasculonecróticas, la identificación entre FL y EN necrótico es importante ya que ambas entidades tienen tratamientos diferentes.     O aumento considerável da expectativa de vida dos pacientes infectados pelo HIV na era do tratamento antirretroviral de alta potência, resulta em importantes alterações metabólicas e osteoarticulares decorrentes do prolongado tempo de infecção viral e desse tratamento. As complicações ortopédicas mais frequentes são as alterações da mineralização óssea, a osteonecrose, síndrome do túnel do carpo e capsulite adesiva glenoumeral, com padrão de apresentação clínica, evolução natural da doença e resposta terapêutica diferentes daqueles da população geral. Os relatos da literatura são iniciais e a experiência do serviço multidisciplinar do Instituto de Ortopedia e Traumatologia da USP permite avanço no conhecimento das diversas patologias envolvidas e o desenvolvimento de protocolos de tratamento adequados a esses diagnósticos.The considerable increase of the life expectancy of HIV-infected patients in the age of highly-powerful antiretroviral treatment results in important metabolic and bone-joint changes resulting from a long-lasting viral infection time and from this treatment. The most common orthopaedic complications are bone mineralization changes, osteonecrosis, carpal tunnel syndrome and gleno-humeral adhesive capsulitis, with different clinical presentation features, natural disease progression and therapeutic response compared to the overall population. Literature reports are initial, and the experience of the multidisciplinary service of the University of São Paulo's Institute of Orthopaedics and Traumatology enables us a more in-depth knowledge about the various pathologies involved and the development of treatment protocols that are appropriate to these diagnoses

    Worldwide trends in underweight and obesity from 1990 to 2022: a pooled analysis of 3663 population-representative studies with 222 million children, adolescents, and adults

    Background Underweight and obesity are associated with adverse health outcomes throughout the life course. We estimated the individual and combined prevalence of underweight or thinness and obesity, and their changes, from 1990 to 2022 for adults and school-aged children and adolescents in 200 countries and territories. Methods We used data from 3663 population-based studies with 222 million participants that measured height and weight in representative samples of the general population. We used a Bayesian hierarchical model to estimate trends in the prevalence of different BMI categories, separately for adults (age ≥20 years) and school-aged children and adolescents (age 5–19 years), from 1990 to 2022 for 200 countries and territories. For adults, we report the individual and combined prevalence of underweight (BMI <18·5 kg/m2) and obesity (BMI ≥30 kg/m2). For schoolaged children and adolescents, we report thinness (BMI <2 SD below the median of the WHO growth reference) and obesity (BMI >2 SD above the median). Findings From 1990 to 2022, the combined prevalence of underweight and obesity in adults decreased in 11 countries (6%) for women and 17 (9%) for men with a posterior probability of at least 0·80 that the observed changes were true decreases. The combined prevalence increased in 162 countries (81%) for women and 140 countries (70%) for men with a posterior probability of at least 0·80. In 2022, the combined prevalence of underweight and obesity was highest in island nations in the Caribbean and Polynesia and Micronesia, and countries in the Middle East and north Africa. Obesity prevalence was higher than underweight with posterior probability of at least 0·80 in 177 countries (89%) for women and 145 (73%) for men in 2022, whereas the converse was true in 16 countries (8%) for women, and 39 (20%) for men. From 1990 to 2022, the combined prevalence of thinness and obesity decreased among girls in five countries (3%) and among boys in 15 countries (8%) with a posterior probability of at least 0·80, and increased among girls in 140 countries (70%) and boys in 137 countries (69%) with a posterior probability of at least 0·80. The countries with highest combined prevalence of thinness and obesity in school-aged children and adolescents in 2022 were in Polynesia and Micronesia and the Caribbean for both sexes, and Chile and Qatar for boys. Combined prevalence was also high in some countries in south Asia, such as India and Pakistan, where thinness remained prevalent despite having declined. In 2022, obesity in school-aged children and adolescents was more prevalent than thinness with a posterior probability of at least 0·80 among girls in 133 countries (67%) and boys in 125 countries (63%), whereas the converse was true in 35 countries (18%) and 42 countries (21%), respectively. In almost all countries for both adults and school-aged children and adolescents, the increases in double burden were driven by increases in obesity, and decreases in double burden by declining underweight or thinness. Interpretation The combined burden of underweight and obesity has increased in most countries, driven by an increase in obesity, while underweight and thinness remain prevalent in south Asia and parts of Africa. A healthy nutrition transition that enhances access to nutritious foods is needed to address the remaining burden of underweight while curbing and reversing the increase in obesit
