Histopathologic parameters as predictors of response to endoscopic sinus surgery in nonallergic patients with chronic rhinosinusitis
OBJECTIVE: To estimate the predictable value of histopathologic parameters in chronic rhinosinusitis (CRS) for response to endoscopic sinus surgery (ESS). - - - - - STUDY DESIGN: Symptomatology was rated in 100 patients prior to as well as 12 and 24 months after surgery. Specimens taken during the procedure were examined and scored for goblet cells, subepithelial thickening, mast cells, and eosinophils. Multiple regression analysis was performed to predict the total score of subjective symptoms before treatment by histopathologic parameters. The correlation between histopathologic parameters and postoperative symptoms was then evaluated. - - - - - RESULTS: Goblet cells were the best predictor correlating with 5 symptoms. Subepithelial thickening correlated with 4 symptoms. Mast cell infiltration correlated with 3 symptoms. Eosinophilic infiltration correlated with only one symptom (P<0.05). - - - - - CONCLUSION: Certain histopathologic parameters in CRS are predictive of favorable response to ESS. - - - - - SIGNIFICANCE: Pathologic evaluation may help the ENT surgeon to predict the persistence of certain CRS symptoms after ESS, even in patients at low risk for surgical failure. - - - - - EBM rating: C-4
Traps of multi-level governance. Lessons from the implementation of the Water Framework Directive in Italy
During recent decades, different patterns of multi-level governance (MLG) have spread across Europe as a consequence of Europeanisation of public policies, which have increasingly adopted decentralized and participatory procedures conceived as a tool of more effective and accountable policy-making. It appears, however, that the implementation of operational designs based on MLG may be rather problematic and it does not necessarily bring to the expected performance improvements. Referring to the case of the EU Water Framework Directive (2000/60/EC), which conceives the creation of new multi-level institutional settings as a key tool for enacting a new holistic approach to water management and protection, this article explores the difficulties that the implementation of such settings has brought in Italy, despite some favorable pre-conditions existing in the country. Evidence is provided that along with institutional and agency variables, the implementation effectiveness of MLG arrangements promoted by the EU can be challenged by their inherent characteristics
Academic freedom in Europe: reviewing UNESCOâs recommendation
This paper examines the compliance of universities in the European Union with the UNESCO Recommendation concerning the Status of HigherâEducation Teaching Personnel, which deals primarily with protection for academic freedom. The paper briefly surveys the European genesis of the modern research university and academic freedom, before evaluating compliance with the UNESCO recommendation on institutional autonomy, academic freedom, university governance and tenure. Following from this, the paper examines the reasons for the generally low level of compliance with the UNESCO Recommendation within the EU states, and considers how such compliance could be improved
Close Packing of Atoms, Geometric Frustration and the Formation of Heterogeneous States in Crystals
To describe structural peculiarities in inhomogeneous media caused by the
tendency to the close packing of atoms a formalism based on the using of the
Riemann geometry methods (which were successfully applied lately to the
description of structures of quasicrystals and glasses) is developed. Basing on
this formalism we find in particular the criterion of stability of precipitates
of the Frank-Kasper phases in metallic systems. The nature of the ''rhenium

How primary care can contribute to good mental health in adults.
The need for support for good mental health is enormous. General support for good mental health is needed for 100% of the population, and at all stages of life, from early childhood to end of life. Focused support is needed for the 17.6% of adults who have a mental disorder at any time, including those who also have a mental health problem amongst the 30% who report having a long-term condition of some kind. All sectors of society and all parts of the NHS need to play their part. Primary care cannot do this on its own. This paper describes how primary care practitioners can help stimulate such a grand alliance for health, by operating at four different levels - as individual practitioners, as organisations, as geographic clusters of organisations and as policy-makers
Ab-initio theory of NMR chemical shifts in solids and liquids
We present a theory for the ab-initio computation of NMR chemical shifts
(sigma) in condensed matter systems, using periodic boundary conditions. Our
approach can be applied to periodic systems such as crystals, surfaces, or
polymers and, with a super-cell technique, to non-periodic systems such as
amorphous materials, liquids, or solids with defects. We have computed the
hydrogen sigma for a set of free molecules, for an ionic crystal, LiH, and for
a H-bonded crystal, HF, using density functional theory in the local density
approximation. The results are in excellent agreement with experimental data.Comment: to appear in Physical Review Letter

Pulsed thermal treatment of carbon up to 3000âŻÂ°C using an atomic absorption spectrometer
An atomic absorption spectrometer unit fitted with a graphite furnace module is used to perform high temperature treatment on three carbonized polymers: polyvinyl chloride (PVC), polyvinylidene chloride (PVDC) and polyacrylonitrile (PAN). Using short pulses up to 45âŻs, we heat small samples to a maximum of 3000âŻÂ°C. High-resolution transmission electron microscopy and X-ray diffractometry are used to track the growth of crystallites in the materials as a function of the heating temperature. We observe the well-known behaviour of large crystalline graphite growth in PVC-derived samples and the formation of curved graphitic layers in PVDC- and PAN-derived samples. This graphite furnace atomic absorption spectrometer approach is an attractive alternative to conventional laboratory-scale graphite furnaces in research of high temperature treatment of carbon and other refractory materials

Present-day quantum field theory can be regularized by a decomposition into
quantum simplices. This replaces the infinite-dimensional Hilbert space by a
high-dimensional spinor space and singular canonical Lie groups by regular spin
groups. It radically changes the uncertainty principle for small distances.
Gaugeons, including the gravitational, are represented as bound fermion-pairs,
and space-time curvature as a singular organized limit of quantum
non-commutativity.

Folding of graphene slit like pore wallsâa simple method of improving CO2 separation from mixtures with CH4 or N2
We report for the first time a detailed procedure for creating a simulation model of energetically stable, folded graphene-like pores and simulation results of CO2/CH4 and CO2/N2 separation using these structures. We show that folding of graphene structures is a very promising method to improve the separation of CO2 from mixtures with CH4 and N2. The separation properties of the analyzed materials are compared with carbon nanotubes having similar diameters or S/V ratio. The presented results have potential importance in the field of CO2 capture and sequestration
Molecular dynamics simulation of radiation damage cascades in diamond
Radiation damage cascades in diamond are studied by molecular dynamics simulations employing the Environment Dependent Interaction Potential for carbon. Primary knock-on atom (PKA) energies up to 2.5 keV are considered and a uniformly distributed set of 25 initial PKA directions provide robust statistics. The simulations reveal the atomistic origins of radiation-resistance in diamond and
provide a comprehensive computational analysis of cascade evolution and dynamics. As for the case of graphite, the atomic trajectories are found to have a fractal-like character, thermal spikes are absent and only isolated point defects are generated. Quantitative analysis shows that the instantaneous maximum kinetic energy decays exponentially with time, and that the timescale of the ballistic phase has a power-law dependence on PKA energy. Defect recombination is efïŹcient and independent of PKA energy, with only 50% of displacements resulting in defects, superior to graphite where the same quantity is nearly 75%
