Univerzita Karlova, 1.lékařská fakulta Obor: Nutriční specialista Vedoucí práce MUDr. Robert Zajíček, Ph.d Abstrakt V teoretickém úvodu je popsána historie popáleninových úrazů, včetně historie výživy, kritéria dělení popáleninových úrazů, přednemocniční péče a role jednotlivých členů multidisciplinárního týmu, především role nutričního terapeuta. Extrémní hypermetabolismus, který přetrvává i více než rok od úrazu, modifikuje doporučení pro výživu, nejen z hlediska potřeby energie a makronutrientů, ale i mikronutrientů. Je kladen důraz na optimální formu a cestu podání, což se odráží v konkrétních dietních opatřeních na Klinice popáleninové medicíny. Práce shrnuje specifické aspekty výživy pacientů s rozsáhlými termickými úrazy. Cílem praktické části bylo porovnat dynamiku sérových hladin selenu u dvou skupin pacientů při rozdílném typu dávkování a cestě podání ve vztahu k nutriční péči u kritických popáleninových úrazů. V praktické části jsou srovnávány dvě skupiny dospělých pacientů s rozsáhlým popáleninovým traumatem. Special emphasis is given to specifics of pediatric patients, who in many aspects react differently. The introduction focuses on history of burn medicine including nutrition, classification of burn injuries, prehospital care and role of individual members of the therapeutical multidisciplinary team logically centring on role of the dietitian. Extreme hypermetabolism, persisting a year or longer after the insult, influences nutritional recommendations including energy, macronutrients (protein, fats and carbohydrates) and micronutrients (minerals, vitamins and trace elements). Mode of administration is influenced as well. Specific facts are shown how these characteristics are reflected in daily dietetic therapy at the Clinic of Burn Medicine of the Prague Vinohrady Faculty Hospital (FNKV).     Involvement of the arms during ski-mountaineering according to different slope inclinations.

    Title of the thesis: Involvement of the arms during ski-mountaineering according to different slope inclinations. Objectives: The objective of the bachelor thesis is to find out the difference of strain makes to poles during ski-mountaineering depending on slope and constant speed. Methods: The study is designed like a pilot study. 8 persons were chosen for this measurement. It was an experiment. The research was realized in a laboratory. Results: Results of study confirm our scientific question. Pressure in the poles increased when the angle of slope is increasing during ski- mountaineering. Angle 10ř of slope showed at an average 42,3% more pressure makes to poles than angle 0ř. Angle 20ř of slope showed at an average 12,8% more pressure makes to poles than angle 10ř. Key words: Ski-mountaineering, strain-gauge transducer, pressureNázev práce: Zapojení paží při chůzi na skialpinistických lyžích v různých sklonech svahu Cíl práce: Cílem bakalářské práce je zjištění rozdílu v zatížení holí při chůzi na skialpinistických lyžích v závislosti na sklonu svahu a konstantní rychlosti. Metoda: Práce je koncipovaná jako pilotní studie. Pro měření bylo vybráno 8 probandů. Jednalo se o experiment, který proběhl formou laboratorního testování. Výsledky: Výsledky výzkumu potvrzují vědeckou otázku, že se zvětšujícím úhlem svahu a konstantní rychlostí se zvyšuje působení tlaku na skialpinistické hole. Při sklonu 10ř bylo v průměru dosaženo o 42,3% většího působení tlaku na hole než při sklonu 0ř. U sklonu 20ř jsme dosáhli v průměru o 12,8% většího působení tlaku na hole než při sklonu 10ř.     Case report: Susac syndrome—two ends of the spectrum, single center case reports and review of the literature

    Susac syndrome is a rare and enigmatic complex neurological disorder primarily affecting small blood vessels in the brain, retina, and inner ear. Diagnosing Susac syndrome may be extremely challenging not only due to its rarity, but also due to the variability of its clinical presentation. This paper describes two vastly different cases—one with mild symptoms and good response to therapy, the other with severe, complicated course, relapses and long-term sequelae despite multiple therapeutic interventions. Building upon the available guidelines, we highlight the utility of black blood MRI in this disease and provide a comprehensive review of available clinical experience in clinical presentation, diagnosis and therapy of this disease. Despite its rarity, the awareness of Susac syndrome may be of uttermost importance since it ultimately is a treatable condition. If diagnosed in a timely manner, early intervention can substantially improve the outcomes of our patients

    Diplomová práce pojednává o nejčastějších příčinách školní nezralosti a jejich aspektech. Teoretická část charakterizuje a definuje pojmy spjaté se školní zralostí, jako je zraková a sluchová percepce, motorika, komunikace a matematické představy, a s nimi spojenou problematiku v předškolním období. Zaměřuje se také na legislativu týkající se vstupu dítěte do základní školy. Praktická část analyzuje jednotlivé metody odhalující školní nezralost. Na základě analýzy dětských prací a sumarizace výsledků ověřuje objektivnost vytvořeného testu a zjišťuje nej častější příčiny školní nezralosti. Z pozorování a rozhovoru zjišťuje sociální nezralost. V závěru shrnuje poznatky z jednotlivých metod a dílčích závěrů kapitol.The thesis deals with the most frequent causes of school immaturity and their aspects. The theoretical part characterises and defines concepts linked with school maturity as the auditory and visual perception, kinetics, communication and mathematical imagination and the adherent problematics of the preschool period. It also focuses on the legislation pertaining the admission of the child to grammar school. The practical part analysis individual methods detecting the school immaturity. On the basis of analysing the childern work and summarisation of its results it is verifying the objectivity of the created test and also investigates the most frequent causes of school immaturity. On the basis of observation and dialogues it traces the social immaturity.     This thesis investigates the coppice-making ability of sessile oak (Quercus petraea agg.) as a part of project TARMAG II in the area of ŠLP Masaryk Forest Křtiny near the village Soběšice. It describes the relationship between diameter of stump, bark thickness, thickness of tree rings and volume exhibitions left in the area with sprout quantity. Than examine interrelationships among volume retained exhibitions, influence of tree rings and bark with biomass increment sprouts. The research area consists of sixteen sub - areas, where each of them was examined 8 stumps. Based on field measurements in winter 2013 and 2014, measured data, which were subsequently analyzed by statistical methods in the program R ( R DevelopmentCore Team , 2010). It was found, that diameter of stump do significant effect on sprouting capacity. The coppice-making ability of oak decreases with diameter of stump. It was demonstrated dependence among volume retained exhibitions and weight and increase in biomass

    A proposal of insert production technology for an injection mold

    This bachelor thesis is focused on the design of technology for the production of a removable insert into an injection mould. The main aim of the thesis is to elaborate the technological process of production of this component. In the introduction part of the thesis a research of technologies of production of injection moulds is carried out. The work also includes a description of the mold parts and their functions. Furthermore, the basic properties of the materials which are most commonly used for the production of inserts are described. The work is focused on the identification of mould defects in production and their possible causes. The thesis also describes the manufacturing technologies for the production of moulds.The practical part focuses on the design of a new pattern of replaceable component, the selection of tools, machines and gauges for the production of a new design of replaceable insert. A detailed assessment of the available options and the selected optimal production variants to achieve the quality and accuracy of the product is made. On the result of the proposed technical requirements, a technological procedure for the production of the insert to the injection mould. This process includes a precise description of the individual manufacturing operations, including material selection, drawing documentation, selection of gauges and tools, and programming of tool trajectories. Using these toolpaths, the production time for the selected machines was determined. The found results are compared with the production time of the same inserts of other designs and the possibilities of rationalizing the production process to achieve maximum production efficiency are discussed
