    Thermal and resonance neutrons generated by various electron and X-ray therapeutic beams from medical linacs installed in polish oncological centers

    AbstractBackgroundHigh-energy photon and electron therapeutic beams generated in medical linear accelerators can cause the electronuclear and photonuclear reactions in which neutrons with a broad energy spectrum are produced. A low-energy component of this neutron radiation induces simple capture reactions from which various radioisotopes originate and in which the radioactivity of a linac head and various objects in the treatment room appear.AimThe aim of this paper is to present the results of the thermal/resonance neutron fluence measurements during therapeutic beam emission and exemplary spectra of gamma radiation emitted by medical linac components activated in neutron reactions for four X-ray beams and for four electron beams generated by various manufacturers’ accelerators installed in typical concrete bunkers in Polish oncological centers.Materials and methodsThe measurements of neutron fluence were performed with the use of the induced activity method, whereas the spectra of gamma radiation from decays of the resulting radioisotopes were measured by means of a portable high-purity germanium detector set for field spectroscopy.ResultsThe fluence of thermal neutrons as well as resonance neutrons connected with the emission of a 20MV X-ray beam is ∼106neutrons/cm2 per 1Gy of a dose in water at a reference depth. It is about one order of magnitude greater than that for the 15MV X-ray beams and about two orders of magnitude greater than for the 18–22MeV electron beams regardless of the type of an accelerator.ConclusionThe thermal as well as resonance neutron fluence depends strongly on the type and the nominal potential of a therapeutic beam. It is greater for X-ray beams than for electrons. The accelerator accessories and other large objects should not be stored in a treatment room during high-energy therapeutic beam emission to avoid their activation caused by thermal and resonance neutrons. Half-lives of the radioisotopes originating from the simple capture reaction (n,γ) (from minutes to hours) are long enough to accumulate radioactivity of components of the accelerator head. The radiation emitted by induced radioisotopes causes the additional doses to staff operating the accelerators

    The Influence of COVID-19 Pandemic Lockdown on the Physical Performance of Professional Soccer Players: An Example of German and Polish Leagues

    [Abstract] The purpose of this study was to investigate whether the in-season pandemic lockdown influenced physical match performance in professional soccer players who participated in the competition of German Bundesliga and Polish Ekstraklasa. The data from 306 games for German Bundesliga and 296 games for Polish Ekstraklasa were divided into before- and after-lockdown periods. The physical performance of German Bundesliga teams after the 63-day COVID-19 lockdown did not differ significantly from that in the first part of the season. In contrast, Polish Ekstraklasa teams, after the 81-day lockdown, displayed significantly shorter total distances (p < 0.001), shorter high-intensity running distances (p = 0.03), and fewer high-intensity actions (p = 0.02). Moreover, when the effective playing time was considered, teams covered significantly less relative total distance (p < 0.001) and relative high-intensity running distance (p = 0.02). The results of the current study suggest that physical performance during the matches of the German Bundesliga was not influenced by the COVID-19 lockdown, contrary to those of the Polish Ekstraklasa. This difference could have been caused by different break lengths and different restrictions implemented in these countries during the pandemic lockdown

    Test of production of 99Mo/99mTc by means of typical medical linear accelerators used in teleradiotherapy

    The test of production of the 99Mo/99mTc complex by means of Varian medical linear accelerators used in teleradiotherapy was performed. The targets made of the natural molybdenum were irradiated by high-energy therapeutic 20 MV X-ray beam. 99Mo was produced mainly in the photonuclear reaction of 110Mo(; n)99Mo. The obtained specific activities are relatively small in the saturation state (from 155.4 kBq/g to 163.2 kBq/g) because only the high-energetic part of the spectrum of 20 MV X-ray beam covers the energy range of the 110Mo(; n)99Mo reaction cross section. The new application of the 99Mo/99mTc complex of the low activity was suggested using the molybdenum nanoparticle technology

    Cantrell’s pentalogy – a case report

    Pentalogy of Cantrell is a rare multiple congenital malformation syndrome characterized by a combination of features: a midline supraumbilical abdominal wall defect, a defect of the lower sternum, a defect of the diaphragmatic pericardium, deficiency of the anterior diaphragm, and congenital cardiac anomalies. This congenital defects could be diagnosed as early as in the first trimester of pregnancy. The complexity of this anomaly, especially coexistent heart defects, determines the way and order of surgical treatment and commonly is a prognostic factor. A case of male newborn with prenatal diagnosed omphalocele and diaphragmatic hernia is reported in the present work. The child was operated within two hours after birth. Intraoperativelly a significant defect of diaphragmatic pericardium and heart surrounded by a small bowel were discovered. The defect of diaphragm was sutured and a drainage of left pericardial cavity was put in place. Afterwards, the abdominal wall defect was sutured after transferring organs into a peritoneal cavity. Six days after the procedure cardiac sonography was performed and it indicated the presence of hemodynamically insignificant congenital intracardiac defect – atrial septal defect. Up-to-date psychomotor development of the boy, currently five months old, is correct. Prenatal diagnosis of the complexity of congenital defects and possibility of fetus condition monitoring allows to plan precisely the time and way of the delivery, as well as to decide the treatment method of the newborn. Due to the absence of hemodynamic disorders and in spite of the present cardiac defect, an early surgical treatment of our patient was possible

    The effect of mid-season coach turnover on running match performance and match outcome in professional soccer players

    [Abstract]: The aim of this research was to examine the physical match performance and match outcome before and after coach turnover in professional soccer teams compared with a control group containing coaches working continuously for 3 consecutive seasons. Analysed data included 325 performances of teams led by dismissed coaches (DC), 313 of teams led by new coaches (NC), and 580 of teams led by unchanged coaches (UnC). Variables, such as average number of collected points, total distance (TD), total distance per minute (TD/min), high-speed running (HSR), sprinting and number of high-intensity runs (NHIR), were analysed in the last 15 games of DC and 15 matches of NC. These periods were divided into 3 blocks out of 5 matches (5-match blocks). NC collected a significantly higher number of points than DC (p = 0.015), whereas average points collected by UNC were significantly greater than DC (p < 0.001 and NC (p = 0.012). TD and TD/min for DC were significantly lower (p < 0.001) compared with both NC and UnC, whereas significant differences in HSR (p = 0.003) and NHIR (p = 0.03) were identified between DC and NC. The current study reported that mid-season coach turnover may result in short-term improvement in team results and physical match performance. However, this effect disappears after a period of approximately 5 games

    The influence of physical performance on technical and tactical outcomes in the UEFA Champions League

    Background: To clarify does physical performance affect success in highest-level soccer, the purpose of the present study was to identify differences in technical-tactical performance (TP) between teams covering high and low run- ning performance (RP) during the UEFA Champions League (UCL) matches. Methods: The RP and TP data were collected from UCL group stage matches in the 2020/21 season. RP variables included total distance covered (TD), high intensity running (HIR), total distance when in ball possession (TDB), and high intensity running when in ball possession (HIRB). TP variables included goal chances, shots, shots on target, passes, accurate passes, key passes, key passes accurate, crosses, crosses accurate, counter attacks, counter attacks with a shot, high pressing, high pressing successful, low pressing, low pressing successful, tackles, tackles successful, entrances to the opponent’s box, total actions, and successful actions. K-means cluster analysis method was used to classify teams covering (i) low and high TD, (ii) low and high HIR, (iii) low and high TDB, (iv) low and high and HIRB. Linear mixed models were used to identify differences in teams’ TP according to their RP. Pearson’s correlations were used to establish direct association between team TP and RP. Results: Similar TP were observed whether teams covering high or low TD/HIR. Teams covering greater TDB/HIRB had more goal chances, shots, shots on target, passes, accurate passes, key passes, accurate key passes, crosses, successful high pressing, entrances to the opponent’s box, total actions, and successful actions were observed (all moderate to very large effect sizes. Significant association between specific TP variables and TDB/HIRB were evidenced (Pearson’s r = 0.35–0.96, all p < 0.05). Conclusion: Covering greater TDB and HIRB may allow more frequent execution of fundamental TP which are con- sidered essential for match success, indicating that RP when team has ball in possession is important determinant of success in highest-level soccer.     The effects of high-intensity interval training at the anaerobic and psychomotor fatigue thresholds on physiological parameters in young soccer players: a prospective study

    This study aimed to investigate the effects of a 4-week specific high-intensity interval training (HIIT) program performed between the anaerobic threshold (ANT) and the psychomotor fatigue threshold (PFT) on physiological parameters in 14 professional soccer players at the under-17 level. The first and second stages of the research protocol included a treadmill running exercise with increasing load and six 3-min four-versus-four games of soccer with a 3-min break between games. Players then participated in a training microcycle involving three specific HIIT exercises twice per week for 4 weeks, after which they repeated stages one and two, followed by an assessment of changes. The measurement of lactate (LA) determined ANT, whereas the choice reaction time (CRT) indicated PFT among other selected physiological parameters. The repeated-measure analysis of variance (ANOVA) compared mean values for the examined variables using Bonferroni post hoc test. It demonstrated significantly increased maximal oxygen consumption (VO2 max) from 45.9 ± 3.0 to 48.7 ± 2.6 at the ANT and from 49.1 ± 3.4 to 52.0 ± 3.6 on the PFT after 4 weeks of training. A significant increase in the running speed (RS) at both thresholds and heart rate (HR) at the ANT (p ≤ 0.05) was also recorded. Moreover, the players exceeded their intensity of effort during ANT while playing four-versus-four soccer matches, but they did not reach intensity during PFT. In conclusion, the findings of the study demonstrated that both thresholds shifted toward higher loads and the proposed specific HIIT effectively increased the exercise capacity of soccer players

    Estimation of diagnostic value of the middle cerebral artery peak systolic velocity in prediction of fetal anemia in pregnancies complicated by alloimmunisation

    Abstract The use of the middle cerbral artery peak systolic velocity (PSV) for the noninvasive diagnosis of fetal anemia in pregnancies complicated by alloimmunisation has the potential to reduce the number of invesive procedures. Objectives: The study was undertaken to determine the detection of fetal anemia by fetal middle cerebral artery peak systolic velociy (MCA PSV). Material and methods: 31 fetuses with red cell alloimmunisation were evaluated with Doppler ultrasongraphy. On the basis of ROC (AUC) analysis the cutoff point of MoM=1.215 with the highest sensitivity and specivicity was established. We examined the relation between MoM=1.215 and neonatal hemoglobin level and the maternal antibody titre in the indirect antiglobulin test. Sensitivity, specificity, positive and negative value and statistical significance were calculated. Conclusions: Data reported to date suggest that a threshold of 1.215 multiples of the median can be used to better diagnostic of fetal anemia

    Impact of marathon performance on muscles stiffness in runners over 50 years old

    IntroductionThe research examines the relationship between marathon performance and muscle stiffness changes from pre to marathon in recreational runners aged 50+ years.MethodsThirty-one male long-distance runners aged 50–73 years participated in the experiment. The muscle stiffness of quadriceps and calves was measured in two independent sessions: the day before the marathon and 30 min after the completed marathon run using a Myoton device.Results and DiscussionThe 42.195-km run was completed in 4.30,05 h ± 35.12 min, which indicates an intensity of 79.3% ± 7.1% of HRmax. The long-term, low-intensity running exercise (marathon) in older recreational runners and the low level of HRmax and VO2max showed no statistically significant changes in muscle stiffness (quadriceps and calves). There was reduced muscle stiffness (p = 0.016), but only in the triceps of the calf in the dominant (left) leg. Moreover, to optimally evaluate the marathon and adequately prepare for the performance training program, we need to consider the direct and indirect analyses of the running economy, running technique, and HRmax and VO2max variables. These variables significantly affect marathon exercise

    Optimization of Image Quality in Digital Mammography with the Response of a Selenium Detector by Monte Carlo Simulation

    Mammography machines must meet high requirements to ensure the quality of the generated images. On the other hand, due to the use of ionizing radiation, there is a need to minimize the dose received by patients. To optimize both of these parameters (dose and image quality), the response characteristics of image detectors and, depending on the composition of the breasts, the physical contrast of the examined structures should be considered. This study aimed to determine the optimal voltage values for a given breast thickness during imaging with the use of a selenium image detector. Analysis was carried out using the Monte Carlo simulation method with the GEANT4 code. Our results reveal that the combination of Mo anode together with Mo filtration (the system recommended in analog mammography) was the least favorable combination among those used in digital mammography machines with a selenium detector. Moreover, the use of Rh filtration instead of Mo was advantageous regardless of the thickness of the breast and resulted in a significant improvement in image quality with the same dose absorbed in the breast. The most advantageous solution was found to be an X-ray tube with a W anode. The highest values of the image quality-to-dose ratio were observed for breasts with dimensions ranging from 53 mm to 60 mm in thickness. Lower image quality was observed for breasts with smaller dimensions due to high breast glandularity, resulting in the deterioration of the physical contrast
