In het kader van het in opdracht van het Ministerie van Welzijn, Volksgezondheid en Cultuur verrichte wetenschappelijk onderzoek 'De Nederlandse geneesmiddelenmarkt in observatie' is door een projectgroep een studie verricht naar structuur en werking van de Nederlandse extramurale geneesmiddelenmarkt. 1 Doel van het onderzoek was het verkrijgen van inzicht in het functioneren van deze markt, ten einde op basis daarvan voorstellen te kunnen doen tot kostenbeheersing; hierbij vormden het behoud van de kwaliteit en de continuiteit van de geneesmiddelenvoorziening het uitgangspunt

Het beslissingsproces van de huisarts bij de keuze van een geneesmiddel, zoals het uit dit empirisch onderzoek naar voren kwam, kan als volgt worden getypeerd. In het merendeel van de gevallen (ca. 90%) beperkt de arts zich tot de informatie die hij al in zijn geheugen heeft. Vaak wordt hierbij direct een keuze gemaakt uit een kleine verzameling: 4 à 5 produkten die 'bovenaan in het geheugen' zitten. De structuur volgens welke de produkten in het geheugen van de arts zijn opgeslagen, wordt in de eerste plaats bepaald door de chemische eigenschappen en werking. Spécialités fungeren hierbij als de 'knooppunten' van de cognitieve structuur. Bij het onderscheid maken tussen de produkten spelen 3 à 4 criteria een rol. Naast de genoemde ook: prijs, effectiviteit en Nederlands fabrikaat. Voorkeuren in voorschrijven worden vooral bepaald door bijwerking en effectiviteit, en minder door prijs en produktsoort (spécialité versus locopreparaat). De analyse van het beslissingsproces van de huisarts, zoals in dit onderzoek is gebeurd met behulp van algemene modellen van menselijke besluitvorming en het consumentengedrag, biedt perspectief voor verdere analyse van de keuze van geneesmiddelen door artsen

A quantitative description of skyrmions in ultrathin ferromagnetic films and rigidity of degree ±1\pm1 harmonic maps from R2\mathbb{R}^2 to S2\mathbb{S}^2

We characterize skyrmions in ultrathin ferromagnetic films as local minimizers of a reduced micromagnetic energy appropriate for quasi two-dimensional materials with perpendicular magnetic anisotropy and interfacial Dzyaloshinskii-Moriya interaction. The minimization is carried out in a suitable class of two-dimensional magnetization configurations that prevents the energy from going to negative infinity, while not imposing any restrictions on the spatial scale of the configuration. We first demonstrate existence of minimizers for an explicit range of the model parameters when the energy is dominated by the exchange energy. We then investigate the conformal limit, in which only the exchange energy survives and identify the asymptotic profiles of the skyrmions as degree 1 harmonic maps from the plane to the sphere, together with their radii, angles and energies. A byproduct of our analysis is a quantitative rigidity result for degree $\pm 1$ harmonic maps from the two-dimensional sphere to itself

Warehouse design and control: framework and literature review

In this paper we present a reference framework and a classification of warehouse design and control problems. Based on this framework, we review the existing literature on warehousing systems and indicate important gaps. In particular, we emphasize the need for design oriented studies, as opposed to the strong analysis oriented research on isolated subproblems that seems to be dominant in the current literature

De geneesmiddelenmarkt in observatie

Er wordt veel geld uitgegeven aan medicijnen. In 1985 verstrekten apothekers en huisartsen voor ongeveer f. 3 mrd. (incl. honorarium) aan geneesmiddelen. Deze uitgaven zijn in de periode 1980-1985 fors toegenomen. Bovendien zijn geneesmiddelen in alle EGlanden, uitgezonderd West-Duitsland, aanmerkelijk goedkoper dan in ons land. In dit artikel wordt de Nederlandse geneesmiddelenmarkt onder de loep genomen. De auteurs constateren dat er bij zowel de verstrekkers als de gebruikers van medicijnen onvoldoende prikkels aanwezig zijn tot kostenbeheersing. De auteurs hebben hiervoor een aantal suggesties in petto

Theory of magnetic field-stabilized compact skyrmions in thin film ferromagnets

We present a micromagnetic theory of compact magnetic skyrmions under applied magnetic field that accounts for the full dipolar energy and the interfacial Dzyaloshinskii-Moryia interaction (DMI) in the thin film regime. Asymptotic analysis is used to derive analytical formulas for the parametric dependence of the skyrmion size and rotation angle, as well as the energy barriers for collapse and bursting, two processes that lead to a finite skyrmion lifetime. We demonstrate the existence of a new regime at low DMI, in which the skyrmion is stabilized by a combination of non-local dipolar interaction and a magnetic field applied parallel to its core, and discuss the conditions for an experimental realization of such field-stabilized skyrmions

Changes in trabecular bone, hematopoiesis and bone marrow vessels in aplastic anemia, primary osteoporosis, and old age

Retrospective histologic analyses of bone biopsies and of post mortem samples from normal persons of different age groups, and of bone biopsies of age- and sex-matched groups of patients with primary osteoporosis and aplastic anemia show characteristic age dependent as well as pathologic changes including atrophy of osseous trabeculae and of hematopoiesis, and changes in the sinusoidal and arterial capillary compartments. These results indicate the possible role of a microvascular defect in the pathogenesis of osteoporosis and aplastic anemia
