    Validation study of the Functional Assessment Scale for Acute Hamstring injuries in Spanish professional soccer players

    Objective: To cross-culturally adapt and validate the Functional Assessment Scale for acute hamstring injury for professional Spanish-speaking soccer players. Design: Clinical measurement study. Cross-cultural adaptation was conducted following international recommendations. Indicators of validity, reliability and responsiveness are provided. Subjects: The Spanish version of the Functional Assessment Scale for acute hamstring injury scale was administered to 165 participants: 45 professional soccer players with acute hamstring muscle injury diagnosis, 40 healthy subjects, 40 individuals at-risk for a hamstring muscle injury and 40 patients with injuries of the lower limb other than hamstring muscle injury. Main measures: The Functional Assessment Scale for acute hamstring injury. Reference measures: Spanish version of the Quality of Life Short-Form 36 questionnaire (SF-36) and the Lower Limb Functional Index (LLFI). Results: Cronbach’s alpha (internal consistency) for the Spanish version of the Functional Assessment Scale for acute hamstring injury scale was >0.8. The intraclass correlation coefficient using the two-way random model (ICC2,1) (test–retest) was 0.993 (95% confidence interval (CI): 0.991–0.995; P < 0.05). In the exploratory factor analysis, a one-factor solution explained 85% of the variance. Subjects with hamstring muscle injury scored significantly lower than the other groups in the Spanish version of the Functional Assessment Scale for acute hamstring injury scale (P < 0.001). The Spanish version of the Functional Assessment Scale for acute hamstring injury scale score within the hamstring muscle injury group showed moderate and significant correlations with SF-36 physical components (Spearman’s rs > 0.6; P < 0.001), and LLFI score at baseline (rs = 0.42; P < 0.01). The standard error of measurement (SEM) and minimum detectable change threshold (MDC95%) were 2.6 and 7.2 points, respectively. The responsiveness indicators have an effect size of 3.62, and the standardized response mean is 3.24. Conclusion: The Spanish version of the Functional Assessment Scale for acute hamstring injury scale showed satisfactory psychometric properties. It can be considered a reliable and valid instrument to assess the functional impact of acute hamstring muscle injury in professional Spanish-speaking football players

    Muscle and intensity based hamstring exercise classification in elite female track and field athletes: implications for exercise selection during rehabilitation

    Background: Hamstring injuries are common in many sports, including track and field. Strains occur in different parts of the hamstring muscle but very little is known about whether common hamstring loading exercises specifically load different hamstring components. The purpose of this study was to investigate muscle activation of different components of the hamstring muscle during common hamstring loading exercises. Methods: Twenty elite female track and field athletes were recruited into this study, which had a single-sample, repeated-measures design. Each athlete performed ten hamstring loading exercises, and an electromyogram (EMG) was recorded from the biceps femoris and semitendinosus components of the hamstring. Hamstring EMG during maximal voluntary isometric contraction (MVIC) was used to normalize the mean data across ten repetitions of each exercise. An electrogoniometer synchronized to the EMG was used to determine whether peak EMG activity occurred during muscle-tendon unit lengthening, shortening, or no change in length. Mean EMG values were compared between the two recording sites for each exercise using the Student’s t-test. Results: The lunge, dead lift, and kettle swings were low intensity (<50% MVIC) and all showed higher EMG activity for semitendinosus than for biceps femoris. Bridge was low but approaching medium intensity, and the TRX, hamstring bridge, and hamstring curl were all medium intensity exercises (≥50% or <80% MVIC). The Nordic, fitball, and slide leg exercises were all high intensity exercises. Only the fitball exercise showed higher EMG activity in the biceps femoris compared with the semitendinosus. Only lunge and kettle swings showed peak EMG in the muscle-tendon unit lengthening phase and both these exercises involved faster speed. Conclusion: Some exercises selectively activated the lateral and medial distal hamstrings. Low, medium, and high intensity exercises were demonstrated. This information enables the clinician, strength and conditioning coach and physiotherapist to better understand intensity- and muscle-specific activation during hamstring muscle rehabilitation. Therefore, these results may help in designing progressive strengthening and rehabilitation and prevention programs

    A 90 minute soccer match induces eccentric hamstring muscles fatigue

    Background/Purpose of the study. Hamstring injuries are the most prevalent muscle injuries in both amateur and professional soccer (football) players. Eccentric strength deficits and muscle strength imbalances have been associated with an increased risk of muscle injuries. The current study is a non-randomised observational study investigating whether fatigue significantly reduces peak eccentric strength of the hamstring muscle group. Methods. Twenty-two right leg dominant professional football players participated in this study. None were injured or were rehabilitating from an injury at the time of testing. The eccentric strength of the hamstring muscles was determined using the Nordic exercise on a Norbord device prior to a 90 minute soccer match and after its end. Results. There was a significant decrease in eccentric strength at the end of the match (before the match: M = 306.91, SD = 59.26; after the match: Mean = 277.77 ± SD = 60.35 p =.000, Cohen’s d =.50). The imbalance in eccentric hamstring muscle strength between dominant and non-dominant limb before the match remained unchanged after its end. Conclusions. A professional football match significantly impacts on the ability of players to produce high speed eccentric strength in the hamstring muscle groups. Resistance to fatigue and eccentric strength, particularly at high speeds, are considerable factors in conditioning of professional soccer players. Eccentric muscle strength fatigue of the hamstring muscle group after a 90 min soccer match may provide a possible explanation for the greater risk of hamstring injuries under fatigue conditions. © 2021, CIC Edizioni Internazionali s.r.l..     Higher frequency of hamstring injuries in elite track and field athletes who had a previous injury to the ankle - a 17 years observational cohort study

    Inversion injury to the ankle and hamstring injuries are common problems in most sports. It is not known whether these injuries constitute a predisposing factor or a precursor of injury or re-injury of these anatomical locations. Therefore, we wished to test the hypothesis that a previous inversion ankle injury exerted a significant effect on the chance of an athlete suffering from a subsequent ipsilateral hamstring injury and vice versa. In an observational cohort study over 17 years (1998-2015), 367 elite track and field athletes, were grouped according to their first traumatic isolated ankle or hamstring injury. Fifty athletes experienced both injuries. The Mann-Whitney U and Chi-square tests (p < 0.05) were performed to test possible associations of ankle and hamstring injury with age, gender, athletics discipline, grade, and type of antecedent injury. Athletes with a preceding ankle injury had a statistically significantly higher chance of experiencing a subsequent hamstring injury compared with athletes who had experienced a hamstring injury as their first traumatic event (x 2 = 4.245, p = 0.039). The proportion of both ankle and hamstring injury events was not statistically different between female (18%) and male (11%) athletes. Age and grade of injury did not influence the proportion of ankle and/or hamstring injury events. There is a statistically significantly higher frequency of hamstring injuries in elite track and field athletes having experienced a previous ankle ligament injury

    Periodization in Anterior Cruciate Ligament Rehabilitation: New Framework Versus Old Model? A Clinical Commentary

    The physiological and psychological changes after anterior cruciate ligament reconstruction (ACLR) do not always allow a return to sport in the best condition and at the same level as before. Moreover, the number of significant re-injuries, especially in young athletes should be considered and physical therapists must develop rehabilitation strategies and increasingly specific and ecological test batteries to optimize safe return to play. The return to sport and return to play of athletes after ACLR must progress through the recovery of strength, neuromotor control, and include cardiovascular training while considering different psychological aspects. Because motor control seems to be the key to a safe return to sport, it should be associated with the progressive development of strength, and cognitive abilities should also be considered throughout rehabilitation. Periodization, the planned manipulation of training variables (load, sets, and repetitions) to maximize training adaptations while minimizing fatigue and injury, is relevant to the optimization of muscle strengthening, athletic qualities, and neurocognitive qualities of athletes during rehabilitation after ACLR. Periodized programming utilizes the principle of overload, whereby the neuromuscular system is required to adapt to unaccustomed loads. While progressive loading is a well-established and widely used concept for strengthening, the variance of volume and intensity makes periodization effective for improving athletic skills and attributes, such as muscular strength, endurance, and power, when compared with non-periodized training.     Ball heading and subclinical concussion in soccer as a risk factor for anterior cruciate ligament injury.

    Soccer players have a high risk of anterior cruciate ligament (ACL) injury, a potentially career-ending event. ACL rupture has been linked with abnormal neuromuscular control in the lower limb. Additionally, heading the ball with the unprotected head during game play is increasingly recognized as a major source of exposure to concussive and sub-concussive repetitive head impacts. This article provides a hypothesis of potential connection of ACL injury with ball heading in soccer players. The study reviews literature sources regarding the impact of neurocognitive alterations after ball headings in ACL injuries. Poor baseline neurocognitive performance or impairments in neurocognitive performance via sleep deprivation, psychological stress, or concussion can increase the risk for subsequent musculoskeletal injury
