    Femoral tunnel formation using reference anatomical structures of the femoral intercondylar space during anterior cruciate ligament reconstruction

    Purpose: a clinical rationale for using reference structures of the intercondylar space to determine the femoral footprint of the ACL. Material: sixty one patients with ACL deficiency who underwent anatomic anterior cruciate ligament reconstruction using single-bundle «bone-patellar tendon-bone» autograft. Methods: radiography and radiometry, computed tomography, arthroscopy, arthrometry. Results: lateral intercondylar and lateral bifurcate ridges can be verified arthroscopically with a frequency of 94% and 48%. Orientation of the femoral tunnel formed using reference structures is more horizontal in coronal plane and has increased sagittal slope compare with traditional technique. Conclusions: The lateral intercondylar and lateral bifurcate ridges can be used as reference structures for femoral tunnel formation during anatomical anterior cruciate ligament reconstruction

    Shotgun metagenomic data on the human stool samples to characterize shifts of the gut microbial profile after the Helicobacter pylori eradication therapy

    © 2017 The Authors The shotgun sequencing data presented in this report are related to the research article named “Gut microbiome shotgun sequencing in assessment of microbial community changes associated with H. pylori eradication therapy” (Khusnutdinova et al., 2016) [1]. Typically, the H. pylori eradication protocol includes a prolonged two-week use of the broad-spectrum antibiotics. The presented data on the whole-genome sequencing of the total DNA from stool samples of patients before the start of the eradication, immediately after eradication and several weeks after the end of treatment could help to profile the gut microbiota both taxonomically and functionally. The presented data together with those described in Glushchenko et al. (2017) [2] allow researchers to characterize the metagenomic profiles in which the use of antibiotics could result in dramatic changes in the intestinal microbiota composition. We perform 15 gut metagenomes from 5 patients with H. pylori infection, obtained through the shotgun sequencing on the SOLiD 5500 W platform.     Lessons from the Whole Exome Sequencing Effort in Populations of Russia and Tajikistan

    © 2016, Springer Science+Business Media New York.In contrast with the traditional methods applied to assessment of population diversity, high-throughput sequencing technologies have a wider application in clinical practice with greater potential to find novel disease-causing variants for multifactorial disorders. Widely used test panels may not meet their goal to diagnose the patient’s condition with a full reliability since this method often does not take into account the population frequencies of analyzed genetic markers. Here, we analyzed 57 male individuals of five ethnic groups from Russia and Tajikistan using the whole exome sequencing technique (Ion AmpliSeq Exome), which resulted in detecting more than 299,000 single nucleotide polymorphisms. Samples formed clusters on the PCA plot according to the geographical location of the corresponding populations. Thereby, the methodology of whole-exome sequencing, in general, and the Ion AmpliSeq Exome panel, in particular, could be positively applied for the purposes of population genetics and for detection of the novel clinically relevant variants

    Effects of posture and venous insufficiency on endothelial-dependent and -independent cutaneous vasodilation in the perimalleolar region

    Objectives: To assess the effects of posture, endothelial function and venous insufficiency on cutaneous microvascular vasodilator function in the gaiter area, in particular defining factors which may affect microangiopathy and ulcer formation. Methods: Endothelial-dependent and –independent vasodilator responses to incremental-doses of acetylcholine (Ach) and sodium nitroprusside (SNP) were evaluated in the perimalleolar region in the supine and standing positions in middle-aged patients with isolated superficial venous insufficiency (ISVI) (n=25) and health controls (n=28) using laser Doppler fluximetry (LDF) and iontophoresis of vasodilators. Results: The venoarteriolar reflex (vasoconstriction on standing) was equally present in both groups, and reduced the vasodilator responses to SNP in the upright position (e.g for patients with ISVI, peak SNP response was 82+11 PU [standing] vs 123+15 PU [supine]). The presence of ISVI had no effect on endothelial vasodilator function in the supine position, but on standing cutaneous reactivity to Ach was significantly reduced (e.g peak Ach response 69+8 PU [ISVI] vs 109+11 PU [controls], p<0.003). Conclusions: Upright posture impairs cutaneous endothelial-dependent vasodilation in the gaiter area of patients with ISVI. This may be of clinical and prognostic utility in identifying which patients with uncomplicated ISVI are at highest risk of tissue breakdown and ulcer formation in the gaiter area

    Cytokine Levels in the Serum of Patients with Chronic Kidney Insufficiency Before and After Hemodialysis

    © 2016, Springer Science+Business Media New York.Chronic kidney insufficiency (CKI) is often the end point of a broad range of chronic kidney diseases and characterized with decreasing number of functionally active nephrons. Pathophysiological CKI is characterized by decreased glomerular filtration, which leads to accumulation of life-threatening toxic metabolites. Hemodialysis is the main therapeutic measure aimed to prolong patient’s life until kidney transplant is available. The goal of this study is to analyze serum level of 21 cytokines in CKI. We have found that the serum level of several (IL-2Rα, IL-3, IL-12 (p40), IL-16, IL-18, HGF, MIF, CSF-1, MCP-3, CXCL12, SCF, IFN-α2, LIF, ÎČ-NGF, and CXCL1) cytokines and chemokines was upregulated in CKI without hemodialysis as compared to controls (p = 0.005). Interestingly, serum cytokines were also upregulated in serum of CKI patients who received hemodialysis. Upregulated cytokines are associated with inflammation and activation of Th1 lymphocytes. We suggest that hemodialysis has limited effect on serum cytokine levels. It could be concluded that therapeutic effect of hemodialysis is not associated with removal of inflammatory cytokines from circulation. Further studies will help better define the underlying cause of an increased inflammation in CKI and identify the laboratory criteria for anti-inflammatory therapy

    © 2016 The Authors.Polymerization of the oligosaccharides (K units) of complex capsular polysaccharides (CPSs) requires a Wzy polymerase, which is usually encoded in the gene cluster that directs K unit synthesis. Here, a gene cluster at the Acinetobacter K locus (KL) that lacks a wzy gene, KL19, was found in Acinetobacter baumannii ST111 isolates 28 and RBH2 recovered from hospitals in the Russian Federation and Australia, respectively. However, these isolates produced long-chain capsule, and a wzy gene was found in a 6.1 kb genomic island (GI) located adjacent to the cpn60 gene. The GI also includes an acetyltransferase gene, atr25, which is interrupted by an insertion sequence (IS) in RBH2. The capsule structure from both strains was→3)-α-D-GalpNAc-(1→4)-α-D-GalpNAcA-(1→3)-ÎČ-D-QuipNAc4NAc-(1→, determined using NMR spectroscopy. Biosynthesis of the K unit was inferred to be initiated with QuiNAc4NAc, and hence the Wzy forms the ÎČ-(1→3) linkage between QuipNAc4NAc and GalpNAc. The GalpNAc residue is 6-O-acetylated in isolate 28 only, showing that atr25 is responsible for this acetylation. The same GI with or without an IS in atr25 was found in draft genomes of other KL19 isolates, as well as ones carrying a closely related CPS gene cluster, KL39, which differs from KL19 only in a gene for an acyltransferase in the QuiNAc4NR synthesis pathway. Isolates carrying a KL1 variant with the wzy and atr genes each interrupted by an ISAba125 also have this GI. To our knowledge, this study is the first report of genes involved in capsule biosynthesis normally found at the KL located elsewhere in A. baumannii genomes

    Characterization of Dysbiotic Changes of Skin Microbiota in Contact Sports Athletes

    © 2017, Springer Science+Business Media New York.Contact sports athletes often suffer from various skin disorders (inflammatory diseases of bacterial and fungal origin, atopic dermatitis, psoriasis, etc.) resulting in long breaks in training which ruin athletic performance. Wrestling implies intense skin-to-skin contact that creates perfect conditions for transmission of the infectious agents. Following the standard rules of hygiene (showering and handwashing directly after each competition and training) does not exclude the possibility to get an infection from sparring partner. To characterize the skin microbial composition of wrestlers who do not have current manifestation of any skin disorders, the metagenomic analysis was performed. Absolute predominance of Bacillus genus in metagenomic profiles of wrestlers’ skin was observed in contrast with the existing literature data. Classic microbiological approaches allowed to detect hemolytic forms of microorganisms. Wrestlers’ skin appeared to be colonized with hemolytic bacilli, whereas the non-wrestler athletes did not have such bacteria on their skin. Such dysbiotic shifts in the microbial community may cause the emergence of skin diseases. Revealed properties could help to design highly effective antiseptics for the contact sports hygiene

Pharmacotherapy including antibiotics use is one of the factors leading to dysbiosis. To evaluate the influence of antibiotics use on intestinal microbiota metagenomic profiles of stool, samples of 74 patients before and after Helicobacter pylori—eradication therapy—were analyzed. Evaluation of taxonomic diversity changes based on Shannon index and Bray-Curtis metrics allows to range patients according to mild, moderate, and severe risk of disturbance of intestinal microbiota pathological conditions
