    Free amino acids in human palatine gland secretions

    Human palatine gland saliva (HPS) was collected from young adult subjects (mean age 22 yr) of both sexes by gentle mechanical stimulation. The HPS contained 9-13 of the common amino acids within the range of 2--850 nmol/ml, but most samples had low or no concentrations of arginine, tryptophan and proline. Methionine and cysteine were not reliably detected. The molar ratios of most amino acids varied between subjects, but the ratio of leucine to isoleucine was remarkably constant (mean +/- SD = 1.86 +/- 0.28; N = 28).     Xylitol Chewing Gums and Caries Rates: A 40-month Cohort Study

    Dental caries is a pandemic infectious disease which can affect the quality of life and consumes considerable health care resources. The chewing of xylitol, sorbitol, and even sugar gum has been suggested to reduce caries rates. No clinical study has simultaneously investigated the effectiveness of these gums when compared with a group receiving no chewing gum. A 40-month double-blind cohort study on the relationship between the use of chewing gum and dental caries was performed in 1989-1993 in Belize, Central America. One thousand two hundred and seventy-seven subjects (mean age, 10.2 years) were assigned to nine treatment groups: one control group (no supervised gum use), four xylitol groups (range of supervised xylitol consumption: 4.3 to 9.0 g/day), two xylitol-sorbitol groups (range of supervised consumption of total polyols: 8.0 to 9.7 g/day), one sorbitol group (supervised consumption: 9.0 g/day), and one sucrose group (9.0 g/day). The gum use during school hours was supervised. Four calibrated dentists performed the caries registrations by means of a modified WHO procedure. The primary endpoint was the development of an unequivocal caries lesion on a non-cavitated tooth surface. Compared with the no-gum group, sucrose gum usage resulted in a marginal increase in the caries rate (relative risk, 1.20; 95% confidence interval, 0.96 to 1.49; p = 0.1128). Sorbitol gum significantly reduced caries rates (relative risk, 0.74; 95% confidence interval, 0.6 to 0.92; p = 0.0074). The four xylitol gums were most effective in reducing caries rates, the most effective agent being a 100% xylitol pellet gum (relative risk, 0.27; 95% confidence interval, 0.20 to 0.36; p = 0.0001). This gum was superior to any other gum (p < 0.01). The xylitol-sorbitol mixtures were less effective than xylitol, but they still reduced caries rates significantly compared with the no-gum group. DMFS analyses were consistent with these conclusions.     Monopolar tonsillotomy versus cold dissection tonsillectomy in children : Prospective study on postoperative recovery

    Objectives: To compare postoperative self-reported recovery results with monopolar tonsillotomy and cold dissection tonsillectomy in children. To evaluate the feasibility of the monopolar technique in tonsillotomy. Methods: Children Results: Altogether 166 patients were recruited; 103 (62%) returned the questionnaire. The first pain-free day with tonsillotomy was day 5 and with tonsillectomy day 11. After tonsillotomy, patients returned to normal activities faster, e.g. they were able to eat normally 6.5 days earlier than tonsillectomy patients. During the first postoperative week weight dropped after tonsillectomy, but not after tonsillotomy. The length of home care was 6 days with tonsillotomy and 10 days with tonsillectomy. The incidence of postoperative hemorrhage (including minor bleedings at home) was 14% after tonsillotomy and 32% after tonsillectomy. Hemorrhages needing interventions were 0% with tonsillotomy and 2% with tonsillectomy. Conclusion: Children operated on with monopolar tonsillotomy recovered faster and had less postoperative hemorrhage than those undergoing tonsillectomy. They were able to return earlier to daycare/school and their caregivers back to work. Recovery results with monopolar tonsillotomy were equal to other tonsillotomy techniques reported in the literature, hence the monopolar technique can be considered an alternative method to perform tonsillotomy.Peer reviewe

    Collagenolytic and phosphatase activity in the rat mandible after functional protrusion

    The effect of chronic mandibular protrusion on the collagenolytic and phosphatase activity of several mandibular bone sites and the condylar cartilage was evaluated. Ninety-three male Sprague-Dawley rats were equally divided into two experimental and one control group. One experimental group wore a protrusive appliance for 2 weeks, the other for 4 weeks. All animals were killed at 59 days of age. Collagenolytic, alkaline and acid phosphatase activities were determined in the condylar cartilage, the subchondral bone and condylar neck, and in the gonial angle and coronoid process. In the cartilage and subchondral bone, the protrusive appliance caused a reduction in collagenolytic and alkaline phosphatase activity. In the condylar neck, it caused a large increase in collagenolytic activity and a decrease in alkaline phosphatase activity in both experimental groups. In the gonial angle and coronoid process, the appliance increased the collagenolytic activity only in the 2-week group. In the 4-week group, the alkaline phosphatase and collagenolytic activities were not different from the activities in those tissues in the control animals. Thus a protrusive appliance induced quantitative changes in enzyme activities in condylar cartilage and mandibular bone. The increase in collagenolytic activity (representing increased bone resorption) occurred typically in areas of muscle attachment and might have been the result of the neuromuscular changes induced by the protrusive appliance. The recovery to normal values of collagenolytic activity in the coronoid process and gonial angle of the 4-week group suggests that at these sites the muscles (and subperiosteal bone) might have adapted to their new biomechanical environment after the longer period of appliance wear.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/28165/1/0000617.pd

    Cauliflower ear among Finnish high-level male wrestlers and judokas is prevalent and symptomatic deformity

    Our research aimed to study the prevalence, concerns, and treatment practices related to cauliflower ear among Finnish wrestlers and judokas. In total, 32 Finnish wrestlers and 31 judokas completed a questionnaire at training sessions or at a competition. All participants were adults competing at the national or international levels. We also took lateral digital photographs of participants' ears. A senior author graded the overall appearance of the auricles on a scale from 0 to 5 (0 = normal auricle, 5 = extreme cauliflower ear). Cauliflower ear was more prevalent among male athletes (84%, 46/55) than female athletes (0%, 0/8, P <.001). Almost all (96%) had sought treatment for an auricular hematoma. The most prevalent treatment modality was needle aspiration (96%). Most (76%) had received treatment from individuals not representing the healthcare profession. Only one athlete reported receiving successful treatment. No complications from treatment were reported. Almost all participants (96%) reported some symptom from the cauliflower ear, typically pain. None regretted their cauliflower ear(s), and 41% of athletes with cauliflower ear considered it desirable. Cauliflower ear is a common and symptomatic deformity among high-level Finnish wrestlers and judokas. Despite the symptoms, it is accepted and sometimes even desired among the athletes.Peer reviewe

    A combined study of heat and mass transfer in an infant incubator with an overhead screen

    The main objective of this study is to investigate the major physical processes taking place inside an infant incubator, before and after modifications have been made to its interior chamber. The modification involves the addition of an overhead screen to decrease radiation heat losses from the infant placed inside the incubator. The present study investigates the effect of these modifications on the convective heat flux from the infant’s body to the surrounding environment inside the incubator. A combined analysis of airflow and heat transfer due to conduction, convection, radiation and evaporation has been performed, in order to calculate the temperature and velocity fields inside the incubator before and after the design modification. Due to the geometrical complexity of the model, Computer-Aided Design (CAD) applications were used to generate a computer-based model. All numerical calculations have been performed using the commercial Computational Fluid Dynamics (CFD) package FLUENT, together with in-house routines used for managing purposes and User-Defined Functions (UDFs) which extend the basic solver capabilities. Numerical calculations have been performed for three different air inlet temperatures: 32, 34 and 36ºC. The study shows a decrease of the radiative and convective heat losses when the overhead screen is present. The results obtained were numerically verified as well as compared with results available in the literature from investigations of dry heat losses from infant manikins

    Saliva Stimulants and the Oral Health of Geriatric Patients

    Root-surface caries (RSC) has been recognized as a specific and important dental disease. Significant advances have been made in the pathology and microbiology of RSC, and the need to standardize the guidelines for recording RSC data has been recognized. Researchers have emphasized the increasing impact RSC will have on the geriatric population, especially since the methods to treat and prevent this disease are limited. The purpose of this study was to investigate the possibility of limiting RSC in a Veterans Administration (VA) patient population, using polyol-containing saliva stimulants that were voluntarily consumed by residents of a VA Medical Center (VAMC) over a period of from six to 30 months.
