    Flat-top temperature tuning response in periodically-poled nonlinear crystals

    Second harmonic generation via periodically-poled nonlinear materials offers an efficient means of generating high-quality visible light that would be otherwise unattainable with traditional laser sources. While this technology has the potential for implementation in many mass-industrial applications, temperature stability requirements of 0.1 deg.C can make packaging with a pump source problematic. Using our high fidelity poling technique we have achieved precise placement of poled domains in Lithium Niobate based on the resulting mathematical models. These initial devices provide more than 4 deg.C flat-top temperature stability, albeit with a corresponding loss in operational efficiency. Our aim is to implement improved designs in magnesium-doped Lithium Niobate for packaging with near-room temperature diode-based pump sources, as could be applied towards RGB TV and projector applications

    100 GHz electrically tunable planar Bragg grating via nematic liquid crystal overlay towards reconfigurable WDM networks

    Novel liquid crystal-based integrated optical devices with >140GHz electrical tuning are presented for application towards reconfigurable wavelength division multiplexing (WDM) networks. Initial results with Bragg wavelength tuning covering five 25GHz WDM channel spacing have been achieved with 170V (peak-to-peak) sinusoidal voltages applied across electro-patterned ITO-covered glass electrodes placed 60µm apart. These prototype devices were fabricated using direct UV grating writing, with an evanescent field coupling into a liquid crystal overlay through an etched window. Electrically controlled liquid crystal birefringence modifies the waveguide effective index, resulting in Bragg wavelength shift. Merck 18523 nematic liquid crystals are used, exhibiting compatible refractive index values to that of silica (no=1.44, ne=1.49 at lambda=1550nm). Homeotropic alignment of the liquid crystal is provided by application of a surfactant layer.The inherent refractive index sensitivity of our etched direct-UV-written structures allows observation of previously unreported liquid crystal surface-behaviour, such as multi-threshold points during variation of the applied field. Continued optimisation based on evanescent field penetration, electrode layout, and surface interaction will allow implementation towards a variety of novel liquid crystal applications and devices. For example, a cascaded architecture of these integrated liquid crystal devices operating at different Bragg wavelengths would pave the way towards true colorless add/drop modules for dense optical networks

    Combining Survival and Toxicity Effect Sizes from Clinical Trials: NCCTG 89-20-52 (Alliance)

    Background: How can a clinician and patient incorporate survival and toxicity information into a single expression of comparative treatment benefit? Sloan et al. recently extended the ½ standard deviation concept for judging the clinical importance of findings from clinical trials to survival and tumor response endpoints. A new method using this approach to combine survival and toxicity effect sizes from clinical trials into a quality-adjusted effect size is presented.Methods: The quality-adjusted survival effect size (QASES) is calculated as survival effect size (ESS) minus the calibrated toxicity effect sizes (EST) (QASES=ESS-EST). This combined effect size can be weighted to adjust for the relative emphasis placed by the patient on survival and toxicity effects.Results: As an example, consider clinical trial NCCTG 89-20-52 which randomized patients to once-daily thoracic radiotherapy (ODTRT) versus twice-daily treatment of thoracic radiotherapy (TDRT) for the treatment of lung cancer. The ODTRT vs. TDRT arms had median survival time of 22 vs. 20 months (p=0.49) and toxicity rate of 39% vs. 54%, (p<0.05). The QASES of 0.18 standard deviations translates to a quality-adjusted survival difference of 5.7 months advantage for the ODRT arm over the TDRT treatment arm (22(16.3) months), p<0.05). Similar results are presented for the four possible case combinations of significant/non-significant survival and toxicity benefits using completed clinical trials.Conclusions: We used a novel approach to re-analyze clinical trial data to produce a single estimate for each treatment that combines survival and toxicity data. The QASES approach is an intuitive and mathematically simple yet robust approach

    Postinfective bowel dysfunction following Campylobacter enteritis is characterised by reduced microbiota diversity and impaired microbiota recovery

    Objectives Persistent bowel dysfunction following gastroenteritis (postinfectious (PI)-BD) is well recognised, but the associated changes in microbiota remain unclear. Our aim was to define these changes after gastroenteritis caused by a single organism, Campylobacter jejuni, examining the dynamic changes in the microbiota and the impact of antibiotics. Design A single-centre cohort study of 155 patients infected with Campylobacter jejuni. Features of the initial illness as well as current bowel symptoms and the intestinal microbiota composition were recorded soon after infection (visit 1, 80 days later (visits 2 and 3). Microbiota were assessed using 16S rRNA sequencing. Results PI-BD was found in 22 of the 99 patients who completed the trial. The cases reported significantly looser stools, with more somatic and gastrointestinal symptoms. Microbiota were assessed in 22 cases who had significantly lower diversity and altered microbiota composition compared with the 44 age-matched and sex-matched controls. Moreover 60 days after infection, cases showed a significantly lower abundance of 23 taxa including phylum Firmicutes, particularly in the order Clostridiales and the family Ruminoccocaceae, increased Proteobacteria abundance and increased levels of Fusobacteria and Gammaproteobacteria. The microbiota changes were linked with diet; higher fibre consumption being associated with lower levels of Gammaproteobacteria. Conclusion The microbiota of PI-BD patients appeared more disturbed by the initial infection compared with the microbiota of those who recovered. The prebiotic effect of high fibre diets may inhibit some of the disturbances seen in PI-BD.Peer reviewe

    Long-Term Assessment of the Effects of COVID-19 and Isolation Care on Survivor Disability and Anxiety

    We conducted an assessment of disability, anxiety, and other life impacts of COVID-19 and isolation care in a unique cohort of individuals. These included both community admissions to a university hospital as well as some of the earliest international aeromedical evacuees. Among an initial 16 COVID-19 survivors that were interviewed 6-12 months following their admission into isolation care, perception of their isolation care experience was related to their reporting of long-term consequences. However, anxiety and disability assessed with standard scores had no relationship with each other. Both capture of the isolation care experience and caution relying on single scoring systems for assessing long-term consequences in survivors are important considerations for on-going and future COVID-19 and other pandemic survivor research
