Vergleich der offenen und der transurethralen radikalen Prostatektomie bei Patienten uÌber 70 Jahren im Hinblick auf die onkologischen Langzeitergebnisse
In der vorliegenden Arbeit wurde die Stellung der radikalen transurethralen
Prostatektomie als Alternative zur offen retropubischen Prostatektomie untersucht.
Hierzu wurden die perioperativen Daten von Patienten gesammelt, die zwischen 2002
und 2008 an der UniversitĂ€tsklinik fĂŒr Urologie TĂŒbingen offen retropubisch und an der
Reuter-Klink Urologie Stuttgart transurethral prostatektomiert worden waren. Im
Anschluss wurden die Patienten kontaktiert und zum onkologischen Verlauf ihrer
Erkrankung befragt, die Zeitspanne nach OP sollte nicht unter 4 Jahren betragen.
Insgesamt erhielten 360 Patienten Eingang in die Studie, die in ein offen und ein
transurethral versorgtes Kollektiv getrennt wurden. Durch Bildung einer
wissenschaftlichen Kontrollgruppe der 70-72jÀhrigen Patienten konnte der
Störeinfluss einer ungleichen Altersverteilung ausgeschlossen werden. Die
Auswertung zeigte, dass die Kollektive im Hinblick auf ihren prÀoperativen
Gesundheitszustand und ihre Tumorlast vergleichbar waren. Im perioperativen Verlauf
war der transurethrale Eingriff zwar schneller als die offen-retropubische Operation,
jedoch mit einer lÀngeren Hospitalisierungsdauer und hÀufigeren Mehrfacheingriffen
zum Erreichen der Tumorfreiheit verbunden. Die Nachbeobachtung, in der 281
Patienten erreicht werden konnten, zeigte ein hÀufigeres biochemisches Rezidiv und
einen signifikant hÀufigeren Gebrauch antihormoneller Therapie in der transurethral
behandelten Gruppe. Auffallend war auch, dass zwar in beiden Gruppen kaum ein
Patient an einem Prostatakarzinom verstarb, das GesamtĂŒberleben der transurethral
therapierten Patienten jedoch hochsignifikant kĂŒrzer war.
In einer abschlieĂenden Beurteilung lĂ€sst sich sagen, dass die transurethrale
Prostatektomie aus rein chirurgischer Sicht durchaus mit der konventionellen offen-
retropubischen Variante vergleichbar scheint. Da sie aber auch Mehrfacheingriffe,
einen hĂ€ufigeren Einsatz hormonablativer Therapie und ein hochsignifikant kĂŒrzere
GesamtĂŒberleben mit sich bringt, muss sie kritisch gesehen werden. Ihr Einsatz sollte
einem genau definierten Patientenkollektiv vorbehalten sein
Experimental demonstration of fiber-accessible metal nanoparticle plasmon waveguides for planar energy guiding and sensing
Experimental evidence of mode-selective evanescent power coupling at
telecommunication frequencies with efficiencies up to 75 % from a tapered
optical fiber to a carefully designed metal nanoparticle plasmon waveguide is
presented. The waveguide consists of a two-dimensional square lattice of
lithographically defined Au nanoparticles on an optically thin silicon
membrane. The dispersion and attenuation properties of the waveguide are
analyzed using the fiber taper. The high efficiency of power transfer into
these waveguides solves the coupling problem between conventional optics and
plasmonic devices and could lead to the development of highly efficient
plasmonic sensors and optical switches.Comment: 1 file, 3 figure
Segmentation of RV in 4D Cardiac MR Volumes using region-merging graph cuts
Non-invasive quantitative assessment of the right ventricular anatomical and functional parameters is a challenging task. We present a semi-automatic approach for right ventricle (RV) segmentation from 4D MR images in two variants, which differ in the amount of user interaction. The method consists of three main phases: First, foreground and background markers are generated from the user input. Next, an over-segmented region image is obtained applying a watershed transform. Finally, these regions are merged using 4D graph-cuts with an intensity based boundary term. For the first variant the user outlines the inside of the RV wall in a few end-diastole slices, for the second two marker pixels serve as starting point for a statistical atlas application. Results were obtained by blind evaluation on 16 testing 4D MR volumes. They prove our method to be robust against markers location and place it favourably in the ranks of existing approaches
Low-loss fiber accessible plasmon waveguide for planar energy guiding and sensing
A metal nanoparticle plasmon waveguide for electromagnetic energy transport utilizing dispersion engineering to increase lateral energy confinement via a two-dimensional pattern of Au dots on an optically thin Si membrane is described. Using finite-difference time-domain simulations and coupled-mode theory, we show that phase-matched evanescent excitation from conventional fiber tapers is possible with efficiencies >90% for realistic geometries. Energy loss in this waveguide is mainly due to material absorption, allowing for 1/e energy decay distances of about 320 ”m for excitation at telecommunication frequencies. This concept can be extended to the visible regime and promises applications in optical energy guiding, optical sensing, and switching
ISLES 2015 - A public evaluation benchmark for ischemic stroke lesion segmentation from multispectral MRI
Ischemic stroke is the most common cerebrovascular disease, and its diagnosis, treatment, and study relies on non-invasive imaging. Algorithms for stroke lesion segmentation from magnetic resonance imaging (MRI) volumes are intensely researched, but the reported results are largely incomparable due to different datasets and evaluation schemes. We approached this urgent problem of comparability with the Ischemic Stroke Lesion Segmentation (ISLES) challenge organized in conjunction with the MICCAI 2015 conference. In this paper we propose a common evaluation framework, describe the publicly available datasets, and present the results of the two sub-challenges: Sub-Acute Stroke Lesion Segmentation (SISS) and Stroke Perfusion Estimation (SPES). A total of 16 research groups participated with a wide range of state-of-the-art automatic segmentation algorithms. A thorough analysis of the obtained data enables a critical evaluation of the current state-of-the-art, recommendations for further developments, and the identification of remaining challenges. The segmentation of acute perfusion lesions addressed in SPES was found to be feasible. However, algorithms applied to sub-acute lesion segmentation in SISS still lack accuracy. Overall, no algorithmic characteristic of any method was found to perform superior to the others. Instead, the characteristics of stroke lesion appearances, their evolution, and the observed challenges should be studied in detail. The annotated ISLES image datasets continue to be publicly available through an online evaluation system to serve as an ongoing benchmarking resource (www.isles-challenge.org).Peer reviewe
