    Endovascular aneurysm repair (EVAR) has been accepted as an alternative to traditional open surgery in selected patients. Now it is a widely accepted standard. In case of contraindications for open repair, after accomplishing including criteria for EVAR, the patient can be treated by this method. Despite the minimally invasiveness of this treatment, several complications may occur during or after EVAR. Complications arise from the limitations of the method and improper patient selection. We report a case of patient with heart failure and complications after EVAR

Miniinvasive procedures give the opportunity to plan thehybrid approach. In this way, all the advantages and opportunities of chosen methods can be used, minimising their complications.Multidisciplinary cooperation during the whole process of treatment, including out-patient care is crutial.     Antiarrhythmic drugs for pharmacological cardioversion of atrial fibrillation and sex differences: Insights from the CANT II Study

    Background: Data on sex differences in terms of action of antiarrhythmic agents (AADs) are limited. This study aimed to evaluate the clinical profile of patients with atrial fibrillation (AF), and efficacy and safety of AADs used for pharmacological cardioversion (PCV) of AF.Methods: This research was a sub-analysis of the retrospective multicenter Cardioversion with ANTazoline II (CANT) registry, which comprised 1365 patients with short-duration AF referred for urgent PCV with the use of AAD. Patients were categorized according to and compared in terms of clinical parameters and PCV outcomes. The primary endpoint was return of sinus rhythm within 12 hours after drug infusion, and the composite safety endpoint involved bradycardia <45 bpm, hypotension, syncope, or death.Results: The sex distribution of patients qualified for PCV was even (men, n = 725; 53.1%). Females were older and more symptomatic and had higher CHA2DS2-VASc scores, higher prevalence of tachyarrhythmia, and higher use of chronic anticoagulation. The overall efficacy (71.4% vs. 70.1%; P = 0.59) and safety (5.2% vs. 4.6%; P = 0.60) of PCV was comparable in men and women. Amiodarone (68.3% vs. 65.9%; P = 0.66) and antazoline (77.1% vs. 80.0%; P = 0.19) had similar efficacy in men and women, but propafenone had a lower rate of rhythm conversion in men (64.7% vs. 79.3%; P = 0.046). None of the assessed AADs differed in terms of safety profile in both sexes.Conclusion: Female patients with AF have different clinical profiles but similar efficacy and safety of AADs as compared to male participants. Propafenone has significantly lower efficacy in men, which requires further investigation

    Mechanical thrombectomy in acute stroke – Five years of experience in Poland

    Objectives Mechanical thrombectomy (MT) is not reimbursed by the Polish public health system. We present a description of 5 years of experience with MT in acute stroke in Comprehensive Stroke Centers (CSCs) in Poland. Methods and results We retrospectively analyzed the results of a structured questionnaire from 23 out of 25 identified CSCs and 22 data sets that include 61 clinical, radiological and outcome measures. Results Most of the CSCs (74%) were founded at University Hospitals and most (65.2%) work round the clock. In 78.3% of them, the working teams are composed of neurologists and neuro-radiologists. All CSCs perform CT and angio-CT before MT. In total 586 patients were subjected to MT and data from 531 of them were analyzed. Mean time laps from stroke onset to groin puncture was 250±99min. 90.3% of the studied patients had MT within 6h from stroke onset; 59.3% of them were treated with IV rt-PA prior to MT; 15.1% had IA rt-PA during MT and 4.7% – emergent stenting of a large vessel. M1 of MCA was occluded in 47.8% of cases. The Solitaire device was used in 53% of cases. Successful recanalization (TICI2b–TICI3) was achieved in 64.6% of cases and 53.4% of patients did not experience hemorrhagic transformation. Clinical improvement on discharge was noticed in 53.7% of cases, futile recanalization – in 30.7%, mRS of 0–2 – in 31.4% and mRS of 6 in 22% of cases. Conclusion Our results can help harmonize standards for MT in Poland according to international guidelines

    W mojej prezentacji pragnę przedstawić kontekst organizacyjny i społeczny zmian w systemie zarządzania wdrażanych w Akademii Muzycznej w Bydgoszczy. W wystąpieniu odpowiem na pytania: • Jaka jest specyfika wdrażania zmiany w uczelni? • Z jakimi problemami się mierzymy? • Jak zaangażować pracowników w zarządzanie procesami? • Jak zorganizować pracę, aby z sukcesem wdrażać Lean Management? W wystąpieniu przedstawię działania podejmowane w mojej Uczelni w ciągu ostatnich kilku lat, ze szczególnym uwzględnieniem projektu w zakresie Lean Management.The aim of this session is to present how to prepare o good programme for cultural change in a university. I will discuss the potential of Lean Culture Maturity Questionnaire and result of the survey on lean Culture maturity conducted in the university in Poland. The two approaches will be presented: the approach demonstrated by university top management and the approach of a lean facilitator. In the presentation, we would like to focus on methodology, but also we will show and discuss the general results of lean culture maturity evaluation

    Lean management and the change of work culture : the case of a Polish university

    Purpose: The main goal of this paper is to identify the process of changes that occur in universities as a result of the implementation and functioning of the lean management concept in the area of work culture. There is a lack of such research focused on higher education institutions, especially arts universities, which have their own specific rules based on the master-student relationship. The educational process is tailored to the needs of students, teachers and society at large. Consequently, this form of education is not only very unique, but also very expensive. In research on higher education, there is a theoretical gap concerning the definition of work culture and its perception by university employees, as well as the elements, factors and consequences of the Lean implementation process. Study design/methodology/approach: The study was based on a used questionnaire and interviews. An additional method was an analysis of the university’s documentation and the researchers’ notes. Statistical methods were used to analyse the participants’ responses provided in the questionnaire. Data were collected as part of a research project carried out at the Academy of Music in Bydgoszcz during the years 2019-2022. Findings: The preliminary results of the research show that the implementation of the Lean concept changes the work culture at the university in different ways. The changes are perceived as either positive or negative. They are mainly related to technical, organisational and social dimensions. The positives are that people started to use new tools, e.g. Kanban, the relations among them strengthened, cross functional communication became more intensive and effective, and the willingness to take a risk related to improvements increased. The changes could also be perceived as oppression and compulsion caused by the pressure sourced by managers and colleagues. Research limitations/implications: Limitations are related to the applied research methods (opinions can be subjective). In addition, the article describes only a case of applied solutions (no possibility of assigning the results to any other organisation). Originality/value: This is the first ever research conducted in Poland on this subject. The findings provide new insights into the relation between the work culture and Lean implementations
