    Tooth reattachment and palatal veneer on a multidisciplinary approach of crown fractures in upper central incisors

    Dental trauma is more common in young patients and its sequelae may have great impact on the esthetics, functions, and phonetics. This paper reports a case of trauma in both central incisors in a young 17-year-old patient who was treated using adhesive tooth fragment reattachment on tooth 2.1 and a palatal indirect composite veneer on tooth 1.1. Regarding the available literature and fracture extension, the treatment approach proposed for this case provided good functional and esthetic outcomes. Clinical and radiographic results after 1 year were successful.     Fine-tuning multilevel modeling of risk factors associated with nonsurgical periodontal treatment outcome

Overall, 37 patients (24 males and 13 females) with moderate-to-severe chronic periodontitis underwent NSPT. Follow-up visits at 3, 6, and 12 months included measurements of several clinical periodontal parameters. Data were sourced from a previously reported database. In a total of 1416 initially affected sites (baseline PD ≥ 4 mm) on 536 teeth, probing depth (PD) and clinical attachment loss (CAL) reductions after NSPT were evaluated against known risk factors at 3 hierarchical levels (patient, tooth, and site). For each post-treatment follow-up, the variance component models fitted to evaluate the 3-level variance of PD and CAL decrease revealed that all levels contributed significantly to the overall variance (p < 0.001). Patients who underwent NSPT and were continually monitored had curative results. All 3 hierarchical levels included risk factors influencing the degree of PD and CAL reduction. Specifically, the type of tooth, surfaces involved, and tooth mobility site-level risk factors had the strongest impact on these reductions and were highly relevant for the success of NSPT.info:eu-repo/semantics/publishedVersio

    Partial recording protocols performance on the assessment of Periodontitis severity and extent: bias magnitudes, sensibility, and specificity

    This is an open access article under the CC BY-NC-ND license (http://creativecommons.org/licenses/by-nc-nd/4.0/).Objectives: This study aimed to assess bias magnitudes, sensibility, and specificity of particular partial-mouth recording protocols (PRPs) to estimate periodontal clinical measurements and periodontal status. Methods: Estimates of mean clinical attachment loss (MCAL) and mean probing depth (MPD) were calculated for 15 different PRPs and were compared to full-mouth recording protocol (FRP) data from 402 Portuguese. Biases, relative biases and intra-class correlations for all PRPs were evaluated. Bland–Altman plots and Receiver Operating Characteristic / Area Under the Curve (ROC/AUC) analysis were used to assess the sensitivity and specificity for each PRP periodontal diagnosis. Results: Regarding MPD, Half RD6 UR/LL and RD6 UL/LR had the lowest bias observed with 0.00 mm (-0.22% and 0.22%, respectively) and all full-mouth PRPs significantly produce an underestimation. Concerning MCAL, the Half MB-B-DL UR/LR had the lowest bias observed with 0.01 mm (0.16%). Excluding CPITN, Full-Mouth PRPs outperforms in average Half Mouth PRPs correlations. The Half RD6 UR/LL had the highest AUC (0.96) with 95.5 and 97.1% of sensitivity and specificity, respectively. Conclusions: Three half-mouth PRPs (Half MB-B-DL UR/LR, Half RD6 UR/LL and Half RD6 UL/ LR) protocols can be used to estimate periodontal clinical measurements with limited bias, and high sensitivity, specificity, and concordance. All full-mouth PRPs failed to estimate pocket depth means, and for clinical attachment loss, they present less ability then half-mouth partial protocols, despite presenting high sensitivity levels.info:eu-repo/semantics/publishedVersio

    Study of periodontal health in Almada-Seixal (SoPHiAS): A cross-sectional study in the Lisbon Metropolitan Area

This population-based cross-sectional study included 1,064 randomized participants (aged 18 to 95 years, 617 females/447 males). Sociodemographic, behaviours and medical information were recorded. Periodontal conditions were assessed with a full-mouth circumferential periodontal examination. It was used the American Association of Periodontology/European Federation of Periodontology 2017 case definitions. A logistic regression analysis was applied to ascertain hypothetical risk factors towards periodontitis. The prevalence of periodontitis was 59.9%, with 24.0% and 22.2% of the participants exhibiting severe and moderate periodontitis, respectively. The risk of periodontitis significantly increased with age (OR = 1.05, 95% CI: 1.04–1.06), for active and former smokers (OR = 3.76 and OR = 2.11, respectively), with lower education levels (OR = 2.08, OR = 1.86, for middle and elementary education, respectively) and with diabetes mellitus (OR = 1.53). This study confirms a high burden of periodontitis in the target (Portuguese) sub-population. The findings provide a comprehensive understanding that will empower appropriate national public oral health programmes and population-based preventive actions.info:eu-repo/semantics/publishedVersio

    Periodontitis Impact in Interleukin-6 Serum Levels in Solid Organ Transplanted Patients: A Systematic Review and Meta-Analysis

    Esta revisión sistemática tuvo como objetivo investigar la influencia de la periodontitis en los niveles séricos de IL-6 postrasplante en pacientes trasplantados de órganos sólidos en comparación con sujetos sanos. Se realizaron búsquedas en cuatro bases de datos (PubMed, Scholar, EMBASE y CENTRAL) hasta febrero de 2020 (PROSPERO CRD42018107817). Se incluyeron estudios de casos y controles y de cohortes sobre la asociación de los niveles séricos de IL-6 con el estado periodontal de los pacientes después de un trasplante de órganos sólidos. El riesgo de sesgo de los estudios observacionales se evaluó mediante la Escala de Newcastle-Ottawa (NOS). Se realizaron metanálisis de efectos aleatorios minuciosamente. La evaluación GRADE proporcionó evidencia de calidad. Cuatro estudios de casos y controles cumplieron los criterios de inclusión (274 receptores de trasplantes y 146 controles sanos), todos de bajo riesgo de sesgo. Los metanálisis revelaron niveles de IL-6 significativamente más altos en pacientes trasplantados que en individuos sanos con evidencia de baja calidad (Diferencia de medias (DM): 2.55 (intervalo de confianza (IC) del 95%: 2.07, 3.03)). Los pacientes trasplantados con periodontitis tienen niveles séricos de IL-6 más altos que los pacientes trasplantados sin periodontitis con evidencia de calidad moderada (DM: 2,20 (IC del 95%: 1,00; 3,39)). Encontramos evidencia de baja calidad de niveles más altos de IL-6 que los pacientes sanos en pacientes con trasplante de corazón y riñón. En estos pacientes trasplantados, hubo evidencia de calidad moderada de que la periodontitis se asocia con niveles séricos más altos de IL-6. Four databases (PubMed, Scholar, EMBASE, and CENTRAL) were searched up to February 2020 (PROSPERO CRD42018107817). Case-control and cohort studies on the association of IL-6 serum levels with a periodontal status of patients after solid organ transplantation were included. The risk of bias of observational studies was assessed through the Newcastle-Ottawa Scale (NOS). Random effects meta-analyses were thoroughly conducted. GRADE assessment provided quality evidence. Four case-control studies fulfilled the inclusion criteria (274 transplant recipients and 146 healthy controls), all of low risk of bias. Meta-analyses revealed significantly higher IL-6 levels in transplanted patients than healthy individuals with low-quality evidence (Mean Difference (MD): 2.55 (95% confidence interval (CI): 2.07, 3.03)). Transplanted patients with periodontitis have higher serum IL-6 levels than transplanted patients without periodontitis with moderate quality evidence (MD: 2.20 (95% CI: 1.00, 3.39)). We found low-quality evidence of higher IL-6 levels than healthy patients in patients with heart and kidney transplant. In these transplanted patients, there was moderate quality evidence that periodontitis is associated with higher IL-6 serum levels. Future research should consider the impact of such a difference in organ failure and systemic complications
