    Ischemic Cerebrovascular Disease In Childhood: Cognitive Assessment Of 15 Patients.

    The aim of this study was to evaluate and to compare the cognitive function of children with ischemic cerebrovascular disease (ICVD). Fifteen children, 7 girls and 8 boys, aged 7.9 to 16.1 years, were evaluated by Piaget's clinical method. The control group was composed by fifteen children whose ages, sex and socioeconomic conditions were similar to those of the ICVD group. The cognitive function evaluation of the ICVD group showed that most of the children (10/15) were under their age group. The SPECT was performed on 14 children with ICVD and the results showed that 8/9 children with hypoperfusion presented a poor cognitive estimation and 3/5 children with normal perfusion demonstrated an adequate performance. We conclude that ICVD in childhood may compromise cognition; therefore, it needs a follow up of acquisitions during alI the stages of development.62802-

    We studied the clinical aspects of 100 consecutive premature newborns with and without intraventricular and periventricular hemorrhage (IPVH).The diagnosis of IPVH was obtained by ultrasonic scans of the skull during the first week of life and at the age of one month. Forty eight percent of newborns with IPVH had abnormal results, and there was a significant correlation with the neurological evaluation in 85% of the infants. The probability of normality for a child with no associated brain abnormalities was 72%, whereas for a child of the same gestational age with associated brain abnormalities was 48.7%.Foram estudados os aspectos clínicos de 100 recém-nascidos prematuros, com e sem hemorragia periventricular-intraventricular (HPIV). O diagnóstico foi obtido através de exames ultra-sonográficos de crânio, realizados durante a primeira semana de vida e na idade corrigida de um mês.Quarenta e oito por cento dos recém-nascidos com HPVI evidenciaram resultados anormais, sendo a correlação significativa com a evolução neurológica, em 85% das crianças. Crianças que não apresentaram anormalidades cerebrais associadas tiveram 72% de probabilidade de manter a evolução normal, enquanto para crianças da mesma idade gestacional porém com anormalidades cerebrais associadas, a probabilidade foi 48,7%.36637

Quinze crianças com idade entre 7,9 e 16, 1 anos foram avaliadas pelo método clínico de Piaget, sendo 8 do sexo masculino. Outras 15 crianças, com idade, sexo e nível sócio-econômico similares aos do grupo propósito compuseram o grupo controle. A avaliação das funções cognitivas mostrou que a maioria das crianças (10/15) do grupo DCV-I apresentou defasagem para a faixa etária. Catorze crianças do Grupo DCV-I realizaram o SPECT. Comparando-se o resultado deste exame com a avaliação cognitiva, verificou-se que a maioria das crianças com hipoperfusão (8/9) apresentou déficit na avaliação cognitiva e 3/5 com perfusão normal tiveram desempenho adequado. Concluímos que a DCV-I na infância pode comprometer a cognição da criança, indicando a necessidade do acompanhamento evolutivo das aquisições em todas as etapas do desenvolvimento.80280

    Doença cerebrovascular na infância: investigação neuropsicológica em 14 casos

    There are few studies about the development of a child after a cerebrovascular accident (CVA), and they usually describe problems such as diminishing in intellectual capacities, difficulties in linguistic and visual-motor skills, as well as in spatial organization and integration. In this study, there were 28 children participating, being 14 placed in the experimental group (EG) after clinical diagnosis and ischemic CVA imaging, and other 14 children without past history of CVA, who formed the control group (CG). The neuropsychological research protocol included an intelligence test, a visual-motor coordination test, human figure drawing, a cortical functions battery and the medical records of the children from the EG. The analysis of the results of this study revealed that the best performances after the CVA are related to the shortest time of functional recovery; CG presented better performances than EG in all the instruments used, in cognitive, perceptual and motor skills. It has been noticed that CVA may lead to intellectual reduction in case of a recurrence of the vascular insult.Nos poucos estudos acerca do desenvolvimento da criança após o acidente vascular cerebral (AVC) são descritos problemas como rebaixamento intelectual, dificuldades na capacidade lingüística, viso-motora, na organização e integração espacial. Neste estudo fizeram parte 28 crianças, sendo 14 pertencentes ao grupo experimental (GE) com diagnóstico clínico e de imagem de AVC isquêmico. Outras 14 crianças sem história de AVC que formaram o grupo controle (GC). O protocolo de investigação neuropsicológica constou de teste de inteligência, teste de coordenação viso-motor, desenho da figura humana, bateria de funções corticais e prontuário médico das crianças do GE. A análise dos resultados deste estudo mostrou que os melhores desempenhos após o AVC, estão relacionados ao menor tempo de recuperação funcional; houve desempenho superior do GC sobre o GE em todos os instrumentos utilizados, nas áreas cognitiva, perceptual e motora. Constatou-se ainda que o AVC pode levar a rebaixamento intelectual, quando ocorre um quadro de repetição do insulto vascular.414

    The purpose of this study is to associate neuropsychological evaluation with neuroimaging results in children with cerebral tomography indicating ischemic cerebrovascular disease (ICVD). Neuroimaging, neurological exams and neuropsychological instruments were used to evaluate five children. The study revealed that the cognitive and perceptive skills in two children were normal and motor sequele in four cases. The rhythm, visual and speech receptive skills remained unchanged. In four cases the SPECT exam showed regions with hypoperfusion and in four cases the EEG was normal. Neuropsychological, neurological and image indication some degree of sequele demonstrating the importance of follow up of children who had suffered cerebrovascular disease.Este estudo teve como objetivo relacionar os dados da avaliação neuropsicológica com exames de neuroimagem em crianças com doença cerebrovascular isquêmica (DCVI) comprovada pela tomografia de crânio. Foram avaliadas cinco crianças através de instrumentos neuropsicológicos, exame neurológico e neuroimagem. O estudo revelou habilidades cognitivas e perceptivas preservadas em duas crianças e sequela motora em quatro. Não houve alterações nas habilidades de ritmo, visual e fala receptiva. SPECT apresentou regiões com hipoperfusão em quatro casos e EEG normal em quatro. Constatou-se comprometimento sequelar neuropsicológico, neurológico e de imagem; o trabalho mostrou a importância do acompanhamento de crianças pós distúrbio cerebral vascular.38638

    Neurologic semiology in a sample of hearing impaired children

    A random sample of 42 sensorioneural hearing impaired children (severe and bilateral) was studied, from special classes in Campinas, with chronological ages varying between 4 and 7 years old. The children of this sample were compared with two control groups of 42 children of the same chronological age, from regular classes of private and public schools. All of them were submitted to the traditional neurological examination. Hearing impaired children showed differences as to head circumference and muscle tonus. In the other examined items we found motor hyperactivity, cerebellar and ocular syndromes although there were no significant differences between the groups.Foi constituída uma amostra aleatória de 42 crianças deficientes auditivas neuros-sensoriaís congênitas, profundas e bilaterais, com idade cronológica variando entre 4 e 7 anos, que frequentavam classes de habilitação da cidade de Campinas. As crianças propostas foram comparadas com dois grupos controles de 42 crianças, da mesma faixa etária, de classe comum. Todas foram submetidas ao exame neurológico tradicional. Verificou-se que os aspectos que demonstraram diferenças foram o perímetro craniano e o tono muscular. Nos demais itens avaliados, mostrou-se hiperatividade motora, síndrome cerebelar e síndrome ocular, porém não houve diferença significativa entre os dois grupos.34134

    Evaluation of the static equilibrium in a sample of hearing impaired children

    A random sample was organized with 42 children with congenital sensorineural auditory deficit (severe and bilateral) from special education schools in Campinas. Chronological ages ranged from 4 to 7 years of age. This sample was compared with two control groups of 42 children of the same chronological age but attending normal classes. All were submitted to 9 tests of the set of static equilibrium of the Evolutive Neurologic Examination. It was observed that in the position of equilibrium with opened eyes (test 8) there were no differences between the groups. In the Romberg position, the hearing impaired children of 4, 5 and 6 years of age showed a significant decrease in the ability to do the test (test 9); those of 7 years of age had equal ability whether of the control or the deficient groups. Hearing impaired children were less able to do other static equilibrium tests (heel-toe or one foot or tip-toe standing tests, with opened or closed eyes: tests 10, 12, 16, 20, 24, 25 and 26). The use of detailed neurological semiology allowed us to support the alteration of the static equilibrium in this group of hearing impaired children.Foi constituída amostra aleatória de 42 crianças deficientes auditivas neurossen-soriais congênitas, profundas e bilaterais, com idade cronológica variando entre 4 e 7 anos, que frequentavam classes de habilitação da cidade de Campinas. As crianças propostas foram comparadas) às de dois grupos controles de 42 crianças, da mesma faixa etária, que apresentavam bom rendimento escolar em classe comum. Todas foram submetidas a 9 provas do setor de equilíbrio estático do Exame Neurológico Evolutivo. Verificou-se que, na posição de equilíbrio com olhos abertos, não houve diferença entre os grupos. Na posição de Romberg, foi significativamente menor o número de deficientes auditivos que a realizaram aos 4, 5 e 6 anos; aos 7 anos foi executada por igual número de controles e deficientes auditivos. As demais provas foram realizadas por número significativamente menor de deficientes auditivos nas seguintes posturas: artelho-calcâneo com olhos abertos ou fechados (provas 24 e 25), permanecer num pé só (provas 12 e 16) ou nas pontas dos pés (provas 10 e 16). O uso de semiologia neurológica detalhada permitiu documentar as alterações do equilíbrio estático nesse grupo de crianças deficientes auditivas.34635

    Fibrodisplasia ossificante progressiva: relato de caso

    Fibrodysplasia ossificans progressiva is a rare genetic disease characterized by widespread soft tissue ossification and congenital stigmata of the extremities. We report on a male child followed for ten years since the age of 3 years and 9 months, when the diagnosis was made. He was born with bilateral hypoplasic hallux valgus and ventricular septal defect, corrected by transsternal approach when 32 months old. Restriction of neck mobility followed and foci of ectopic ossification appeared. Four crises of disease exacerbation were treated with oral prednisone and/or other antiinflammatory drugs. Sodium etidronate 5 to 10 mg/kg/day was prescribed intermittently during about six years but was discontinued due to osteopenia. The disease course has been relentless, with severe movement restriction including the chest wall. A review showed few similar case reports in the Brazilian literature. We revisit the criteria for diagnosis and the essentials of management and treatment.Fibrodisplasia ossificante progressiva � uma doen�a gen�tica rara, caracterizada por ossifica��o disseminada em tecidos moles e estigmas cong�nitos nas extremidades. Relatamos sobre uma crian�a do sexo masculino, branca, que vem sendo acompanhada desde seus 3 anos e 9 meses quando diagnosticamos a doen�a. Nascida com halux valgo hipopl�sico bilateral e defeito do septo interventricular, submeteu-se a corre��o cir�rgica atrav�s de esternotomia, aos 32 meses de vida. Seguiram-se progressiva restri��o de movimentos cervicais e aparecimento de n�dulos duros e dolorosos em regi�es paravertebrais c�rvico-tor�cicas. Em dez anos, assistimos a quatro epis�dios bem definidos de exacerba��o da doen�a, com acr�scimos de ossifica��o ect�pica. Nessas ocasi�es, prescrev�amos prednisona oral e/ou antiinflamat�rios n�o hormonais. Etidronato de s�dio, 5 a 10 mg/kg/dia, foi utilizado em per�odos de dois ou tr�s meses, com intervalos de dois meses, durante seis anos. Osteopenia obrigou-nos a interromper o uso do quelante. A evolu��o tem sido inexor�vel, com limita��o intensa de movimentos tor�cicos e corp�reos. A revis�o da literatura brasileira revelou poucos casos semelhantes. Enfatizamos os crit�rios para diagn�stico e princ�pios b�sicos de tratamento.34234

    Central Auditory Processing Outcome After Stroke In Children.

    To investigate central auditory processing in children with unilateral stroke and to verify whether the hemisphere affected by the lesion influenced auditory competence. 23 children (13 male) between 7 and 16 years old were evaluated through speech-in-noise tests (auditory closure); dichotic digit test and staggered spondaic word test (selective attention); pitch pattern and duration pattern sequence tests (temporal processing) and their results were compared with control children. Auditory competence was established according to the performance in auditory analysis ability. Was verified similar performance between groups in auditory closure ability and pronounced deficits in selective attention and temporal processing abilities. Most children with stroke showed an impaired auditory ability in a moderate degree. Children with stroke showed deficits in auditory processing and the degree of impairment was not related to the hemisphere affected by the lesion.72680-
