In Vitro Gene Expression Dissected: Chemostat Surgery for Mycobacterium Tuberculosis
A unique approach, combining defined and reproducible in vitro models with DNA
microarrays, has been developed to study environmental modulation of mycobacterial
gene expression. The gene expression profiles of samples of Mycobacterium tuberculosis,
from independent chemostat cultures grown under defined and reproducible
conditions, were found to be highly correlated. This approach is now being used to
study the effect of relevant stimuli, such as limited oxygen availability, on mycobacterial
gene expression. A modification of the chemostat culture system, enabling largevolume
controlled batch culture, has been developed to study starvation survival.
Cultures of M. tuberculosis have been maintained under nutrient-starved conditions
for extended periods, with 106 ā 107 bacilli surviving in a culturable state after
100 days. The design of the culture system has made it possible to control the environment
and collect multiple time-course samples to study patterns of gene expression.
These studies demonstrate that it is possible to perform long-term studies and obtain
reproducible expression data using controlled and defined in vitro models
Pelvic irradiation for stage II ovarian carcinoma
Over a 20-year period, 34 patients with FIGO stage II ovarian carcinoma were treated with postoperative pelvic irradiation at the University of Michigan. Complications of radiation treatment were minimal. The overall actuarial disease-free 5-year survival was 53%. This was not significantly different for substages IIA, IIB, or IIC. Patients with well-differentiated tumors had a significantly better survival than patients with moderate or poorly differentiated tumors (P < 0.05). Stage IB carcinoma of the cervix with lymph node metastases
From January 1, 1946, through January 1, 1986, at the University of Michigan, 60 patients were treated with Stage IB (FIGO) carcinoma of the cervix with lymph node metastases. Clinical variables were examined with regard to long-term survival. Prognostically significant variables were (1) highest level of lymph node involvement and (2) tumor histology. Patients with common iliac or periaortic node involvement had an actuarial survival at 5 years of 14%; 5-year survival for patients with only pelvic node involvement (external iliac, hypogastric, or obturator) was 68%. Patients with adenocarcinoma or adenosquamous histology had an actuarial survival at 5 years of 16%; 5-year survival for patient with squamous cell carcinoma was 71%. Overall survival for all patients with lymph node involvement was 61%. Estimating the economic impacts of climate change on 16 major US fisheries
Observational evidence shows marine species are shifting their geographic distribution in response to warming ocean temperatures. These shifts have implications for the US fisheries and seafood consumers. The analysis presented here employs a two-stage inverse demand model to estimate the consumer welfare impacts of projected increases or decreases in commercial landings for 16 US fisheries from 2021 to 2100, based on the predicted changes in thermally available habitat. The fisheries analyzed together account for 56% of the current US commercial fishing revenues. The analysis compares welfare impacts under two climate scenarios: a high emissions case that assumes limited efforts to reduce atmospheric greenhouse gas and a low emissions case that assumes more stringent mitigation. The present value of consumer surplus impacts when discounted at 3% is a net loss of ) in the low emissions case and 278ā901 million by 2100
Combination cisplatin and dichloromethotrexate in patients with advanced or recurrent cervical cancer: A preliminary report
The chemotherapy combination of cisplatin and dichloromethotrexate was administered to 14 patients with measurable cervical cancer. Six (46%) have had a complete response for 6+, 7, 11, 14, 15, and 21 months. Four (30%) have had a partial response for 1, 6, 10, and 11 months. Two patients (15%) have had stable disease for 6 and 11 months. Only one patient failed to respond to this combination. In six of these patients, the measurable mass was located in a previously radiated area. This combination was well tolerated with only three instances of severe toxicity. The gynecologic oncologist in academic departments : Report of survey
Now that the subspecialty of gynecologic oncology is well established within the specialty of obstetrics and gynecology, it seems timely to evaluate the pros and cons, the strengths and weaknesses of such a program as it interrelates with other programs in an academic department. A survey is presented which reflects the beliefs of both members and candidate members of the Society of Gynecologic Oncologists on such issues as gynecologic oncologists as chairmen of departments; teaching demands; time commitments to patient care and research in an academic institution; and surgical privileges for gastro-intestinal and urologic procedures in various hospitals. Financial and budgetary items are also discussed. The effects of early removal of indwelling urinary catheter after radical hysterectomy
Radical hysterectomy has long been a primary mode of therapy for selected gynecologic malignancies. The lower urinary tract is an area associated with complications following this procedure. Lack of satisfactory reflex micturition and urinary retention, diminished bladder sensation, infection, and fistula formation are common adverse sequelae. Prolonged indwelling catheterization is a cornerstone of postoperative management after radical hysterectomy. An alternative regimen consisting of early postoperative catheter removal, with a strict voiding schedule, and intermittent self-catheterization (ISC) for postvoid residuals (PVR) was prospectively investigated. Intermittent self-catheterization was initiated only if the PVR 12 hr after catheter removal was greater than 75 ml. Twenty-six patients who underwent radical hysterectomy were studied. Catheters were removed between the fifth and ninth postoperative day. Eighteen patients (69%) had PVRs less than 75 ml at 12 hr and were successfully managed with a strict voiding schedule only. Eight patients (31%) had 12-hr PVRs greater than 75 ml and were managed with a strict voiding schedule and ISC until the PVR was less than 75 ml for two consecutive voids. These patients were evaluated with fluorourodynamics and none had an abnormal study. Compared to 25 historical control patients, study group median indwelling catheter duration was less (6.0 days compared to 30.0 days) with no increase in postoperative complications. On the basis of these data, early removal of indwelling urinary catheters after radical hysterectomy appears to be an acceptable alternative to long-term catheterization.Peer Reviewedhttp://deepblue.lib.umich.edu/bitstream/2027.42/29044/1/0000077.pd
