    認知症高齢者グループホームにおけるサブ共用空間の計画に関する研究

    Residents in the "group homes" for the elderly people with dementia spend a lot of time in the common activity areas. It is well recognized that these common spaces are playing important roles in the group home life. Besides the legally required common areas such as dining rooms and living rooms, some group homes have additional sub-common activity areas. At three group homes in Hokkaido, we have conducted the time study on daily activities of the residents (where and what the residents are doing), employing the behavior observation method as well as the interviews with the care staff. Purpose of this study is to clarify the influences on the life style of the residents coming from the different kinds of physical environments. Based on the findings, we are to establish a basic guideline for the planning of the sub-common spaces in the future group homes. On the survey day, behavior maps were drawn every 10 minutes from 7 a.m. to 8 p.m. Some of the results obtained from this study are shown below. Unexpectedly, the sub-common spaces were not so well utilized by the residents. Therefore, it is quite difficult to determine if these sub-common spaces are giving influence on the life style of the residents or not. With the better and more active utilization of such sub-common spaces by the group home managements, these spaces could play more important roles for the residents. Due attention should also be paid to the physical environments when such sub-common areas be included in the space planning of the group homes

    Recognition of the "team medical-care" in medical field.

    チーム医療に対して高い関心が寄せられていることから,チーム医療として認識される具体的な事柄や各職種に対する認識,その利点や職種間の認識の違いなどを明らかにする目的で,岡山県内の病院の看護師,診療放射線技師,臨床検査技師,計652名に対してアンケート調査を行った。有効回答者数470名で,その内訳は看護師207名,診療放射線技師91名,臨床検査技師172名であった。結果はチーム医療と認識される具体的な事柄については,異業種間カンファレンスがチーム医療を促進する事柄として認識されているが,整備・実施されていないことがうかがえた。各職種に対する認識については,職種別・経験年数別にみても,医師,看護師,理学・作業療法士はチーム医療のメンバーとして認識されている割合が高かった。また,経験年数別では,10~14年の人までは統計上ややばらつきがみられるが,経験年数15年以上の人はあらゆる職種をチーム医療のメンバーであると考え,その重要性を認識していることがわかった。チーム医療の利点については,どの職種も共通して「患者中心の医療」と考えている割合が高かった。Recently, the "team medical-care" has been received increasing attention in medical field. Therefore we have to develop a clear understanding of "team medical-care". The purpose of this study is to investigate things regarded as "team medical-care" by co-medical, recognition of other category of license, the advantages of "team medical-care" and recognition against the "team medical-care" by different category of license. Total 652 nurses, clinical radiological technologists and clinical laboratory technologists who are working at 8 hospitals in Okayama prefecture were examined by using questionnaire and 470 members (72.1%) responded to it. They were nurses 207 (44.0%), clinical radiological technologists 91 (19.4%), clinical laboratory technologists 172 (36.6%). Majority of respondents recognized that the medical conference consisted of different category of license promoted the "team medical-care", and ninety percent of participants prompted more frequent medical conference. In both by category of license and years of experience, they regarded doctor, nurse, physical therapist and occupational therapist as the menber of the "team medical-care" at high rate. The respondences which have over 15 years of experience regarded all category of license as the menber of the "team medical-care", while the respondences which have less than 10 to 14 years of experience showed disperision of the recongnition. The advantages of "team medical-care" were highly perceived as "patient-centered medical care" by all category of license

    A new cancer diagnostic system based on a CDK profiling technology

    AbstractA series of molecular pathological investigations of the molecules that stimulate the cyclin dependent kinases (CDK1, 2, 4, and 6) have led to enormous accumulation of knowledge of the clinical significance of these molecules for cancer diagnosis. However, the molecules have yet to be applied to clinical cancer diagnosis, as there is no available technology for application of the knowledge in a clinical setting. We hypothesized that the direct measurement of CDK activities and expressions (CDK profiling) might produce clinically relevant values for the diagnosis. This study investigated the clinical relevance of CDK profiling in gastrointestinal carcinoma tissues by using originally developed expression and activity analysis methods. We have established novel methods and an apparatus for analyzing the expression and activities of the CDK molecules in lysate of tumor tissue in a clinical setting, and examined 30 surgically dissected gastrointestinal carcinomas and corresponding normal mucosal specimens. We demonstrate here that remarkably elevated CDK2 activity is evident in more than 70% of carcinoma tissues. Moreover, a G1-CDK activity profiling accurately mirrored the differences in proliferation between tumor and normal colonic tissues. Our results suggest that CDK profiling is a potent molecular–clinical approach to complement the conventional pathological diagnosis, and to further assist in the individualized medications

    Factors Associated with Cancer-Related Pain Requiring High-Dose Opioid Use in Palliative Cancer Patients

    Background:There are no universal tools to predict the necessity of high-dose opioid use for cancer-related pain. Early recognition and interventions for intractable cancer pain could minimize the distress of palliative patients.Objective:We sought to identify the clinical factors associated with high-dose opioid use in advanced cancer patients to recognize palliative patients who would develop intractable cancer pain, as early as possible.Setting/Subjects:Among 385 in-hospital cancer patients from April 1, 2014 to July 31, 2019, who were referred to the palliative care team for cancer-related pain, clinical factors significantly correlated to high-dose opioid use were retrospectively analyzed.Measurements:We conducted a multiple logistic regression analysis to identify variables significantly related to high-dose opioid use (>120 mg/day oral morphine equivalent dose).Results:Independent factors of high-dose opioid use included younger age (odds ratio [OR] 0.965, 95% confidence interval [CI] 0.944-0.986, p = 0.001), respiratory cancers (OR 1.882, 95% CI 1.069-3.312, p < 0.001), and opioid switch (OR 2.869, 95% CI 1.497-5.497, p = 0.001). The percentage of correct classifications of the regression equation was 86.9%.Conclusions:Younger age, respiratory cancers, and opioid switch were related to high-dose opioid use. Our findings may help palliative caregivers to deal with intractable cancer pain in palliative patients, and thus relieve their distress

Preliminary research was mainly conducted in areas of consumer products use, housing facilities, as well as the cultivation of public opinion and attitudes towards low energy life. The high accumulation of systems and functions, the rational and rigid differentiations of tasks and areas, typical of city life, aggravate the alienation of the individual and accelerate the consumption rate of material and energy resource. Further research is necessary, therefore, to relocate the areas in which the needs for privacy and property can be reformulated to embrace a more organic and cyclic concept of life
