    Laser Monotherapy in Plantar Fasciitis

    Introdução: A fasceíte plantar (FP) constitui a causa mais frequente de dor no retropé. Está recomendada uma prova terapêutica inicial conservadora. Objetivos: Caraterizar fatores demográficos e sintomáticos, terapêutica prévia e resultados de monoterapia com laser. Material e Métodos: Estudo observacional retrospetivo (12 meses). O tratamento aplicado foi laser infravermelho (AsGa 904nm; densidade de energia 20J/cm2; potência de pico 200mW). Fez-se análise estatística descritiva e comparativa (nível de significância de 0,05). Resultados: Seguiram-se 32 doentes com FP, com idade média de 54,28 anos (24–86; DP 13,95), 66% mulheres, Índice de Massa Corporal (IMC) médio de 30,18Kg/m2 (17,91-41,13; DP 5,19), 75% unilaterais, tempo médio de evolução de 6,28 meses (0,2-24; DP 6,82), 21 tinham feito previamente farmacoterapia e 12 outros tratamentos de MFR. O nível de dor inicial (END) médio foi 7,31 (5-10; DP 1,38). Dos 26 doentes tratados (6 perdas), 24 (92,3%) reportaram melhoria. O número médio de sessões foi 28,5 (11– 60; DP 14,01). O nível médio de dor final foi 0,75 (0-5; DP 2,37), com melhoria média de 89,7% (28,6%-100%; DP 0,16) do nível de dor. A intensidade de dor inicial foi maior em doentes com IMC superior (p=0,002). Não se encontraram outras relações estatisticamente significativas. Conclusões: A FP caraterizou-se por um nível de dor significativo (moderado a grave). A obesidade pareceu ser um fator de risco. A taxa de melhoria com o tratamento com laser foi muito satisfatória. A terapia com laser de baixa intensidade IV constitui uma boa opção terapêutica, a justificar avaliação suplementar por ensaios prospetivos controlados e randomizados

    The genus Alternaria is one of the most common black moulds and appears to be increasing as a causative agent of subcutaneous phaeohyphomycosis, particularly among immunosuppressed patients. A 53-year-old patient who had received a kidney transplant presented with multiple verrucous lesions on the distal extremities. Positive histopathology and cultures, in addition to rDNA ITS region sequencing, identified the fungal isolate as Alternaria infectoria. Oral itraconazole was administered for 10 months. A follow-up at 15 months demonstrated no signs of infection. Clinical manifestations of cutaneous alternariosis vary significantly and only a few cases have been described in the literature. Although optimal treatment options remain controversial, this case of phaeohyphomycosis was successfully treated with itraconazole monotherapy

    BCG Revaccination Does Not Protect Against Leprosy in the Brazilian Amazon: A Cluster Randomised Trial

    BCG is a vaccine developed and used to protect against tuberculosis, but it can also protect against leprosy. In Brazil, children receive BCG at birth, and since 1996 a trial has been conducted to find out if a second dose of BCG administered to schoolchildren gives additional protection against tuberculosis. We use this trial to find out if such vaccination protects against leprosy. The trial was conducted in the Brazilian Amazon, involving almost 100,000 children aged 7–14 years who had received neonatal BCG. Half of them received a second dose of BCG at school, and the other half did not. We followed the children for 6 years and observed that there were as many new cases of leprosy in the vaccinated children as in the unvaccinated children. Therefore, we concluded that a second dose of BCG given at school age in the Brazilian Amazon offers no additional protection against leprosy

    Melanin in Fonsecaea pedrosoi: a trap for oxidative radicals

    <p>Abstract</p> <p>Background</p> <p>The pathogenic fungus <it>Fonsecaea pedrosoi </it>constitutively produces the pigment melanin, an important virulence factor in fungi. Melanin is incorporated in the cell wall structure and provides chemical and physical protection for the fungus.</p> <p>We evaluated the production of nitric oxide (NO) in macrophages, the oxidative burst and the inducible nitric oxide synthase (i-NOS) activity in interactions between activated murine macrophages and <it>F. pedrosoi</it>. Experiments were carried out with or without tricyclazole (TC) treatment, a selective inhibitor of the dihydroxynaphthalene (DHN)-melanin biosynthesis pathway in <it>F. pedrosoi</it>. The paramagnetisms of melanin and the TC-melanin were analysed by electron spin resonance. The fungal growth responses to H<sub>2</sub>O<sub>2 </sub>and to S-nitroso-N-acetylpenicillamine (SNAP), a nitric oxide donor, were also evaluated.</p> <p>Results</p> <p>Melanised <it>F. pedrosoi </it>cells were more resistant to both H<sub>2</sub>O<sub>2 </sub>and NO. Nitrite was not detected in the supernatant of macrophages incubated with melanised fungal cells. However, i-NOS expression was unaffected by the presence of either untreated control <it>F. pedrosoi </it>or TC-treated <it>F. pedrosoi</it>. In addition, the inhibition of the DHN-melanin pathway by TC improved the oxidative burst capability of the macrophages.</p> <p>Conclusion</p> <p>The NO-trapping ability of <it>F. pedrosoi </it>melanin is an important mechanism to escape the oxidative burst of macrophages.</p

    Development and validation of exhaled breath condensate microRNAs to identify and endotype asthma in children

    Detection and quantification of microRNAs (miRNAs) in exhaled breath condensate (EBC) has been poorly explored. Therefore we aimed to assess miRNAs in EBC as potential biomarkers to diagnose and endotype asthma in school aged children. In a cross sectional, nested case control study, all the asthmatic children (n = 71) and a random sample of controls (n = 115), aged 7 to 12 years, attending 71 classrooms from 20 local schools were selected and arbitrarily allocated to the development or validation set. Participants underwent skin-prick testing, spirometry with bronchodilation, had exhaled level of nitric oxide determined and EBC collected. Based on previous studies eleven miRNAs were chosen and analyzed in EBC by reverse transcription-quantitative real-time PCR. Principal component analysis was applied to identify miRNAs profiles and associations were estimated using regression models. In the development set (n = 89) two clusters of miRNAs were identified. After adjustments, cluster 1 and three of its clustered miRNAs, miR-126-3p, miR-133a-3p and miR-145-5p were positively associated with asthma. Moreover miR-21-5p was negatively associated with symptomatic asthma and positively associated with positive bronchodilation without symptoms. An association was also found between miR-126-3p, cluster 2 and one of its clustered miRNA, miR-146-5p, with higher FEF25-75 reversibility. These findings were confirmed in the validation set (n = 97) where two identical clusters of miRNAs were identified. Additional significant associations were observed between miR-155-5p with symptomatic asthma, negative bronchodilation with symptoms and positive bronchodilation without symptoms.     Cell functional enviromics: Unravelling the function of environmental factors

    <p>Abstract</p> <p>Background</p> <p>While functional genomics, focused on gene functions and gene-gene interactions, has become a very active field of research in molecular biology, equivalent methodologies embracing the environment and gene-environment interactions are relatively less developed. Understanding the function of environmental factors is, however, of paramount importance given the complex, interactive nature of environmental and genetic factors across multiple time scales.</p> <p>Results</p> <p>Here, we propose a systems biology framework, where the function of environmental factors is set at its core. We set forth a "reverse" functional analysis approach, whereby cellular functions are reconstructed from the analysis of dynamic envirome data. Our results show these data sets can be mapped to less than 20 core cellular functions in a typical mammalian cell culture, while explaining over 90% of flux data variance. A functional enviromics map can be created, which provides a template for manipulating the environmental factors to induce a desired phenotypic trait.</p> <p>Conclusion</p> <p>Our results support the feasibility of cellular function reconstruction guided by the analysis and manipulation of dynamic envirome data.</p

    Increased CK5/CK8-Positive Intermediate Cells with Stromal Smooth Muscle Cell Atrophy in the Mice Lacking Prostate Epithelial Androgen Receptor

    Results from tissue recombination experiments documented well that stromal androgen receptor (AR) plays essential roles in prostate development, but epithelial AR has little roles in prostate development. Using cell specific knockout AR strategy, we generated pes-ARKO mouse with knock out of AR only in the prostate epithelial cells and demonstrated that epithelial AR might also play important roles in the development of prostate gland. We found mice lacking the prostate epithelial AR have increased apoptosis in epithelial CK8-positive luminal cells and increased proliferation in epithelial CK5-positive basal cells. The consequences of these two contrasting results could then lead to the expansion of CK5/CK8-positive intermediate cells, accompanied by stromal atrophy and impaired ductal morphogenesis. Molecular mechanism dissection found AR target gene, TGF-β1, might play important roles in this epithelial AR-to-stromal morphogenesis modulation. Collectively, these results provided novel information relevant to epithelial AR functions in epithelial-stromal interactions during the development of normal prostate, and suggested AR could also function as suppressor in selective cells within prostate
