Capacity to make health care decisions: its importance in clinical practice
Background. Assessment of capacity plays a pivotal role in determining when decisions need to be made on behalf of an individual. It therefore has major clinical management implications for health care professionals and civil liberties implications for the person concerned. In many countries, there is a presumption that adults have the capacity to make health care decisions. However, in persons with a mental disability, capacity may be temporarily or permanently impaired.
Methods. A selective review is presented which considers: (i) the broad approaches taken to determining capacity; (ii) the abilities commonly assessed in determining capacity; and (iii) the principles underlying health care decision-making for adults who are without capacity.
Results. Capacity is a functional concept, determined by the person's ability to understand, retain, and weigh up information relevant to the decision in order to arrive at a choice, and then to communicate that choice. We have reviewed the studies that examined decision-making abilities in people with dementia, chronic mental illness or intellectual disabilities. Approaches to decision-making in adults who lack capacity include: anticipatory decisions made through advance health care statements or decisions by proxy based on ‘best interests’ or ‘substituted judgement’.
Conclusions. The understanding of clinical and legal aspects of capacity is still developing. This paper examines current concepts of capacity and decision-making on behalf of those without capacity. We propose a framework, in line with current ethical and legal guidelines, as an aid to clinicians when they are seeking consent for a health care intervention.published_or_final_versio
A study of the water retention curve of lime-treated London Clay
© 2016 The Author(s) This paper investigates the drying and wetting soil water retention curves (SWRCs) of statically compacted lime-stabilised London Clay specimens. A series of tests were performed using the contact filter paper method, pressure plate apparatus and a suction-controlled triaxial system incorporating the axis translation technique. These investigated the water retention of the soil under different boundary and stress-state conditions and simultaneously determined the volume change in the soil during drying and wetting. Factors relevant to the lime treatment of soils, such as curing period and method (air vs. water curing), were also considered. Finally, the hysteresis of the SWRC of the chemically treated soil (for which there appears to be a lack of information in the international literature) was investigated. The results showed that the treatment with lime increased the volumetric stability but reduced the water retention ability due to a more open structure enabled by the flocculation and chemical bonding effects. Curing period and method effect appears to be small. Hysteresis was noted to some degree in all instances.

Effects of testing techniques on the SWRC of a partially saturated soil

Vaccination against Foot-and-mouth disease : do initial conditions affect its benefit?
When facing incursion of a major livestock infectious disease, the decision to implement a vaccination programme is made at the national level. To make this decision, governments must consider whether the benefits of vaccination are sufficient to outweigh potential additional costs, including further trade restrictions that may be imposed due to the implementation of vaccination. However, little consensus exists on the factors triggering its implementation on the field. This work explores the effect of several triggers in the implementation of a reactive vaccination-to-live policy when facing epidemics of foot-and-mouth disease. In particular, we tested whether changes in the location of the incursion and the delay of implementation would affect the epidemiological benefit of such a policy in the context of Scotland. To reach this goal, we used a spatial, premises-based model that has been extensively used to investigate the effectiveness of mitigation procedures in Great Britain. The results show that the decision to vaccinate, or not, is not straightforward and strongly depends on the underlying local structure of the population-at-risk. With regards to disease incursion preparedness, simply identifying areas of highest population density may not capture all complexities that may influence the spread of disease as well as the benefit of implementing vaccination. However, if a decision to vaccinate is made, we show that delaying its implementation in the field may markedly reduce its benefit. This work provides guidelines to support policy makers in their decision to implement, or not, a vaccination-to-live policy when facing epidemics of infectious livestock disease

Stall Inception in Low-Pressure Ratio Fans
A combined experimental and computational test program, with two low-pressure ratio aero-engine fans, has been used to identify the flow mechanisms at stall inception and the subsequent stall cell growth. The two fans have the same rotor tip clearance, annulus design, and downstream stators, but different levels of tip loading. The measurement data show that both the fans stall via spike-type inception, but that the growth of the stall cell and the final cell size is different in each fan. The computations, reproducing both the qualitative and quantitative behavior of the steady-state and transient measurements, are used to identify the flow mechanisms at the origin of stall inception. In one fan, spillage of tip leakage flow upstream of the leading edge plane is responsible. In the other, sudden growth of casing corner separation blockage leads to stall. These two mechanisms are in accord with the findings from core compressors. However, the transonic aerodynamics and the low hub-to-tip radius ratio of the fans lead to the following two findings: first, the casing corner separation is driven by shock-boundary layer interaction and second, the spanwise loading distribution of the fan determines whether the spike develops into full-span or part-span stall and both types of behavior are represented in the present work. Finally, the axial momentum flux of the tip clearance flow is shown to be a useful indicator of the leakage jet spillage mechanism. A simple model is provided that links the tip loading, stagger, and solidity with the tip clearance axial momentum flux, thereby allowing the aerodynamicist to connect, qualitatively, design parameters with the stall behavior of the fan.</jats:p
In the eye of the beholder: owner preferences for variations in cats' appearances with specific focus on skull morphology
Changes in the popularity of cat breeds are largely driven by human perceptions of, and selection for, phenotypic traits including skull morphology. The popularity of breeds with altered skull shapes appears to be increasing, and owner preferences are an important part of this dynamic. This study sought to establish how and why a range of phenotypic attributes, including skull shape, affect preferences shown by cat owners. Two questionnaires were distributed on-line to cat owners who were asked to rate preferences for pictures of cats on a 0–10 scale. Veterinarian consensus established the skull types of the cats pictured (i.e., level of brachycephaly (BC) or dolichocephaly (DC)). Preferences were then explored relative to cat skull type, coat and eye color, and coat length. Generalized estimating equations identified relationships between physical characteristics and respondent ratings. Further sub-analyses explored effects of respondents' occupation, location and previous cat ownership on rating scores. Overall, cats with extreme changes in skull morphology (both BC and DC) were significantly less preferred than mesocephalic cats. Green eyes, ginger coat color and medium length coat were most preferred. Current owners of a BC or DC pure bred cat showed significantly greater preference for cats with similar features and significantly lower preference for the opposite extreme. Respondents from Asia were significantly more likely to prefer both BC and DC cats as compared to respondents from other locations. Finally, those in an animal care profession, as compared to other professions, provided a significantly lower preference rating for BC cats but not for DC cats. This work, despite the acknowledged limitations, provides preliminary evidence that preferences for cat breeds, and their associated skull morphologies, are driven by both cultural and experiential parameters. This information may allow for better targeting of educational materials concerning cat breeds
Ground source heat pumps and their interactions with underground railway tunnels in an urban environment: A review
© 2015 Elsevier Ltd. All rights reserved. Ground source heat pumps (GSHPs) can provide an efficient way of heating and cooling buildings due to their high operating efficiencies. The implementation of these systems in urban environments could have further benefits. In such locations the ground source heat is potentially more accessible through alternative sources such as through underground railways (URs). The heat from the ground surrounding an UR tunnel could be exploited to enhance the operation of GSHPs operating in heating mode. To achieve this, the interactions of GSHPs with neighbouring URs must be fully understood but there is little exploration of these in current literature. This paper focuses on the potential benefit of understanding such interactions. It starts with a summary of typical and alternative heat sources for heat pumps and then it highlights the reasons why URs can be regarded as one of the most attractive ones. Then the paper reviews the current approaches used to model GSHPs and URs. Based on that review the paper suggests a method for the combined analysis of GSHPs and URs. The reasons why London is a sensible choice for a case study are also described. Summary of results from a preliminary investigation are also presented.

Motivation of owners to purchase pedigree cats, with specific focus on the acquisition of brachycephalic cats
Background: Cats are globally popular pets and pedigree cats are increasingly prevalent, with brachycephalic breeds being the most registered breeds. How owners decide upon and acquire their cats is poorly understood. Moreover, there are growing concerns about the health and welfare of brachycephalic (BC) dogs and recent studies are raising the awareness of health and welfare problems in BC cats. Methods: An online survey investigated owners’ motivations, perceptions and behaviours prior to, during and following acquisition of non-pedigree (NP), extreme brachycephalic pedigree (BC; i.e., Persian and Exotic Shorthair) and mild to non-BC pedigree (P) cats. Results: The survey received 1367 valid responses (NP n = 882, P n = 400, BC n = 85 (6.2%)). There were marked differences between NP, P and BC owners’ perception of their cats’ health and welfare, reason(s) for acquisition and its process. Owners of NP were less influenced by appearance, behaviour and other features than P or BC owners. In contrast, P and BC owners were highly influenced by appearance, with P owners also placing greater importance on good breed health than BC owners. BC owners were less likely to recommend their breeds to prospective cat owners, apparently concerned by high maintenance requirements. Conclusion: Further research is needed to determine how decision-making is constructed and how it may be improved, especially in respect of welfare outcomes for extreme BC cats given the increased weighting given to appearance over health

Short acquisition time PET quantification using MRI-based pharmacokinetic parameter synthesis
Positron Emission Tomography (PET) with pharmacokinetic (PK) modelling is a quantitative molecular imaging technique, however the long data acquisition time is prohibitive in clinical practice. An approach has been proposed to incorporate blood flow information from Arterial Spin Labelling (ASL) Magnetic Resonance Imaging (MRI) into PET PK modelling to reduce the acquisition time. This requires the conversion of cerebral blood flow (CBF) maps, measured by ASL, into the relative tracer delivery parameter (R 1 ) used in the PET PK model. This was performed regionally using linear regression between population R 1 and ASL values. In this paper we propose a novel technique to synthesise R 1 maps from ASL data using a database with both R 1 and CBF maps. The local similarity between the candidate ASL image and those in the database is used to weight the propagation of R 1 values to obtain the optimal patient specific R 1 map. Structural MRI data is also included to provide information within common regions of artefact in ASL data. This methodology is compared to the linear regression technique using leave one out analysis on 32 subjects. The proposed method significantly improves regional R 1 estimation (p < 0.001), reducing the error in the pharmacokinetic modelling. Furthermore, it allows this technique to be extended to a voxel level, increasing the clinical utility of the images
