    6-Year Experience of Outpatient Total and Completion Thyroidectomy

    Outpatient thyroidectomy has become slowly accepted with various published reports predominantly examining partial or subtotal thyroidectomy. Concerns regarding the safety of outpatient total and completion thyroidectomy remain, especially in regards to vocal fold paralysis, hypocalcemia, and catastrophic hematoma. We aimed to evaluate the safety of outpatient thyroid surgery in a large cohort. We conducted a retrospective review comparing outcomes in those who underwent outpatient (n=251) versus inpatient (n= 291) completion or total thyroidectomy between February 2009 and February 2015. Outpatient completion and total thyroidectomy had lower rates of temporary hypocalcemia (6% vs. 24.4%; p\u3c 0.001) and no significant difference in rates of return to emergency department (1.2% vs. 1.4%), hematoma formation (0.8% vs. 0.7%), temporary (2% vs. 4.1%) or permanent (0.4% vs. 0.7%) vocal fold paralysis, or permanent hypocalcemia (0.4% vs. 0%) compared to the inpatient group. Outpatients requiring calcium replacement had shorter duration of postoperative calcium supplementation (44.4±59.3 days vs. 63.3 ±94.4 days; p \u3c 0.001).     Glycated hemoglobin and incident type 2 diabetes in singaporean Chinese adults: The Singapore Chinese Health Study

    Background: The American Diabetes Association recently included glycated hemoglobin in the diagnostic criteria for diabetes, but research on the utility of this biomarker in Southeast Asians is scant. The aim of this study was to evaluate the association between percent HbA1c and incident diabetes in an Asian population of adult men and women without reported diabetes. Methods: Data analysis of 5,770 men and women enrolled in the Singapore Chinese Health Study who provided a blood sample at the follow-up I visit (1999-2004) and had no cancer and no reported history of diabetes or cardiovascular disease events. Diabetes was defined as self-report of physician diagnosis, identified at the follow-up II visit (2006-2010). Results: Hazard ratios (and 95%confidence intervals) for incident diabetes by 5 categories of HbA1c were estimated with Cox regression models and continuous HbA1c with cubic spline analysis. Compared to individuals with an HbA1c ≤ 5.7% (≤39 mmol/mol), individuals with HbA1c 5.8-5.9% (40-41 mmol/mol), 6.0-6.1% (42-43 mmol/mol), 6.2-6.4% (44-47 mmol/mol), and ≥ 6.5% (≥48 mmol/mol) had significantly increased risk for incident diabetes during followup. In cubic spline analysis, levels below 5.7% HbA1c were not significantly associated with incident diabetes. Conclusions: Our study found a strong and graded association with HbA1c 5.8% and above with incident diabetes in Chinese men and women

    Offloading in P4 Switch Integrated with Multiple Virtual Network Function Servers

    Software Defined Networking (SDN) and Network Function Virtualization (NFV) are two transformative technologies that offer distinct benefits. SDN virtualizes the control plane by separating it from the data plane, while NFV virtualizes the data plane by moving network functions from hardware and implementing them in software. Therefore, combining SDN and NFV can fully exploit the benefits of both technologies. As Programming Protocol-independent Packet Processors (P4) become popular due to its flexibility, traditional SDN switches are being replaced by P4 switches. In the P4+NFV architecture, network functions can be provided in both P4 switches (PNF) and NFV servers (VNF). However, to minimize packet delay, the offloading problem between P4 switches and NFV needs to be addressed. The novelty of our paper lies in investigating the offloading problem and evaluating the impact of employing multiple VNFs with varying computing capacities within the P4+NFV architecture. We also use M/M/1 queuing theory to derive the average packet delay and propose an optimization solution based on gradient descent to find out the optimal offloading probabilities of various VNF servers. Results show that optimal offloading from P4 switch to multiple VNFs can reduce the average packet delay from 4.76% to 40.02%
