    A prospective analysis of glove perforation in primary and revision total hip and total knee arthroplasty.

    Literature in regard to glove perforation rates in revision total joint arthroplasty (TJA) is scarce. Our purpose was to determine the incidence of perforation in revision TJA. Gloves from all scrubbed personnel were tested based on the American Society for Testing and Materials. A total of 3863 gloves were collected from 58 primary and 36 revision arthroplasty cases. Surgeons had a 3.7% outer-glove perforation rate in primary TJA compared with 8.9% in revision TJA. When both gloves were perforated, the outer-glove perforation was recognized intraoperatively 100% of the time, and the inner-glove perforation was noted only 19% of the time. The surgeon has the highest rate of glove perforation. Outer-glove perforations should prompt careful inspection of the inner glove

    Total knee arthroplasty using computer-assisted navigation in patients with deformities of the femur and tibia: A report of 5 cases

    Anatomic aberrations of the femur and tibia secondary to trauma, congenital defects, and prior surgery present challenges for the reconstructive knee surgeon because of an altered mechanical axis and distorted anatomic landmarks. Five patients with arthritis of the knee and extra-articular femoral and/or tibial deformity, retained hardware, or intramedullary (IM) implants underwent total knee arthroplasty using a computer navigation system. The navigation system obviated the need for an IM guide, and the normal mechanical axis of the patients was restored. Extensive dissection for hardware removal or osteotomy was not necessary in these patients. In these 5 cases, a navigation system proved to be an effective tool for restoration of limb alignment in the presence of significant extra-articular deformities and/or IM hardware. Thus, it provides an alternative approach to the traditional IM instrumentation for treating these patients in an effective manner

    The Impact of Caregiver and Community Education On Influenza Vaccination Acceptance

    Introduction: Pediatric populations are particularly vulnerable to influenza, and rely on parental approval of the pediatric influenza vaccine (PIV). We conducted a survey to better understand our community’s perspective on the PIV. Methods: An urban pediatric clinic’s records were gathered for the 2017-2019 flu seasons (i.e., pre-survey period). Surveys were distributed during the 2019-2020 flu season to assess vaccination status and their reasoning for acceptance or rejection of the PIV for their child(ren). Children vaccination rates were divided into 3 groups: 6 months-3 years, 3-5 years, and 5-18 years. Results: During the 2017-2018 flu season, the overall vaccination rate (n=1791) was 34.1%. For the 2018-2019 flu season, the overall vaccination rate (n=1795) was 41.7%. During the 2019-2020 flu season, the overall vaccination rate (n=1620) increased to 45.3%. This was a statistically significant increase from 2018-2019 flu season (p=0.035). The most common reason to vaccinate was, I do not want my child to get the flu/the shot reduces the likelihood of child getting the flu (52.2% in \u3c5 years age group, 56.5% in \u3e5 years age group). The most common reason to not vaccinate was, I do not want the flu shot for my child (35.7% in \u3c5 years age group, 38.6% in \u3e5 years age group). Discussion: Understanding community perceptions will allow for efficacious educational materials and allow healthcare providers to tailor their strategies for the sake of preventive medicine and community health

    Protest events are a hallmark of social movement tactics. Large crowds in public spaces send a clear message to those in authority. Consequently, estimating crowd size is important for clarifying how much support a particular movement has been able to garner. This is significant for policymakers and constructing public opinion alike. Efforts to accurately estimate crowd size are plagued with issues: the cost of renting aircraft (if done by air), the challenge of visibility and securing building access (if done by rooftops), and issues related to perspective and scale (if done on the ground). Airborne camera platforms like drones, balloons, and kites are geospatial affordances that open new opportunities to better estimate crowd size. In this article we adapt traditional aerial imaging techniques for deployment on an “unmanned aerial vehicle” (UAV, popularly drone) and apply the method to small (1,000) and large (30,000+) events. Ethical guidelines related to drone safety are advanced, questions related to privacy are raised, and we conclude with a discussion of what standards should guide new technologies if they are to be used for the public good

    Knee Injuries in NFL Linemen - Correlation with BMI Dr. Michael Lem, MD; Dr. John Bianchi, DO; Austin Irwin, OMS II; Dr. Robert Farrell, MD; Cole Siegenfeld, OMS II Purpose: Knee injuries are among the most common injuries suffered in the NFL which in turn can decrease a player\u27s playing time in a given season and thus negatively impact future performance. Understanding characteristics like BMI can help modify and identify players at risk for knee injuries. Methods and Study Design: Data collected for this study was extracted from the official NFL database of injuries. Reported knee injuries for offensive and defensive lineman were collected and organized by week during the 2009-2022 seasons; final data was calculated for players with a BMI over 39.95. Pro-football-reference.com was cited for height, weight, career length and draft status. BMI was calculated based on height and weight. Comparisons were made between BMI and game status. Results: Among the players collected, offensive tackles were the most likely to appear on the injury report. Players with a BMI above 40 appeared most commonly as Probable (n=182), followed by Questionable (157), Out (134) then Doubtful (26). The average BMI for the players listed as probable was x=42.4 (SD=2.69); questionable x=41.2 (1.37); doubtful x=41.0 (.856); out x=40.9 (.991). There is a significant difference in the player’s BMI between “probable” game status and both “questionable” (p=.023) and “out” (p\u3c .001). Players with a BMI= +37 appeared as either out or doubtful 30.07% total. Players with a BMI under 37 appeared as either out or doubtful 30.81%. Conclusions: Based on our data, BMI may play a protective role on injury report placement for knee injuries. The abundance of “probable” game status also supports the notion that the injuries studied are lower-grade. More players appear on the injury list towards the end of the season, which may indicate there is a persisting effect to the injuries

    Repeat Neuro-Imaging in Patients Presenting with Traumatic Extra-Axial Intracranial Hemorrhage

    Introduction: The management of extra-axial intracranial hemorrhage patients is complicated and lacks a systemic algorithm to determine the timing and necessity of head computed tomography (HCT). However, repeat HCTs weakly predict the need for an intervention after mild traumatic brain injury (TBI). Objective: This study focused on assessing the safety, efficacy, and necessity of obtaining serial HCTs in patients presenting with subdural hemorrhages. Methods: A retrospective chart review was conducted of patients with subdural hemorrhages and a GCS between 13-15 on admission (n=116). The total number of HCTs, time between repeat HCTs, duration of hospital stay, and factors necessitating surgery were studied. Fischer’s exact was used to evaluate the association between 1 HCT or 2-3 HCTs and the need for surgical intervention. Results: There was no statistical difference found in the need for an intervention between patients having one or greater than one HCT in the first 24 hours. The average age was 69 years old. The gender breakdown comprised of 49 females and 67 males. 80% (n = 93) of the patients did not require surgery during hospital stay. Discussion: Our results suggest that there is no association found between the number of HCTs and surgical intervention. This study demonstrates the need for improved variables to assess TBI. We recognize the limitations found in this study and future analysis will need to assess other prognostic indicators to better predict the need for intervention in mild TBI patients
