    Testing evapotranspiration estimates based on MODIS satellite data in the assessment of the groundwater recharge of karst aquifers in southern Italy

    In many Italian regions, and particularly in southern Italy, karst aquifers are the main sources of drinking water and play a crucial role in the socio-economic development of the territory. Hence, estimating the groundwater recharge of these aquifers is a fundamental task for the proper management of water resources, while also considering the impacts of climate changes. In the southern Apennines, the assessment of hydrological parameters that is needed for the estimation of groundwater recharge is a challenging issue, especially for the spatial and temporal inhomogeneity of networks of rain and air temperature stations, as well as the variable geomorphological features and land use across mountainous karst areas. In such a framework, the integration of terrestrial and remotely sensed data is a promising approach to limit these uncertainties. In this research, estimations of actual evapotranspiration and groundwater recharge using remotely sensed data gathered by the Moderate Resolution Imaging Spectrometer (MODIS) satellite in the period 2000–2014 are shown for karst aquifers of the southern Apennines. To assess the uncertainties affecting conventional methods based on empirical formulas, the values estimated by the MODIS dataset were compared with those calculated by Coutagne, Turc, and Thornthwaite classical empirical formulas, which were based on the recordings of meteorological stations. The annual rainfall time series of 266 rain gauges and 150 air temperature stations, recorded using meteorological networks managed by public agencies in the period 2000–2014, were considered for reconstructing the regional distributed models of actual evapotranspiration (AET) and groundwater recharge. Considering the MODIS AET, the mean annual groundwater recharge for karst aquifers was estimated to be about 448 mm·year−1 . In contrast, using the Turc, Coutagne, and Thornthwaite methods, it was estimated as being 494, 533, and 437 mm·year−1, respectively. The obtained results open a new methodological perspective for the assessment of the groundwater recharge of karst aquifers at the regional and mean annual scales, allowing for limiting uncertainties and taking into account a spatial resolution greater than that of the existing meteorological networks. Among the most relevant results obtained via the comparison of classical approaches used for estimating evapotranspiration is the good matching of the actual evapotranspiration estimated using MODIS data with the potential evapotranspiration estimated using the Thornthwaite formula.     The etiology of reticular macular disease (RMD), a sub-phenotype of age-related macular degeneration (AMD), is controversial and has not been clarified. RMD is suspected to be a multifactorial, complex disease with genetic, environmental, and systemic factors playing an important role in its origin. Findings from combinations of different imaging modalities suggest that the pattern that characterizes this condition is associated with an alteration of the choriocapillaris blood flow. If the choroid is indeed affected in RMD, the possible linkage with inflammatory or other systemic diseases could be better supported

    Risk factors associated with reticular pseudodrusen versus large soft drusen

    PURPOSE: To investigate genetic, environmental, and systemic risk factors in prospectively identified subjects with the age-related macular degeneration (AMD) phenotypes of (1) reticular pseudodrusen without large soft drusen and (2) large soft drusen without reticular pseudodrusen. DESIGN: Prospective case-case comparison. METHODS: In a clinical practice setting, patients with AMD were sequentially screened using clinical examination and scanning laser ophthalmoscopy imaging to prospectively identify subjects (n = 73) with the phenotypes of (1) reticular pseudodrusen without large soft drusen (n = 30) or (2) large soft drusen without reticular pseudodrusen (n = 43). Subjects were genotyped for 2 alleles associated with AMD, age-related maculopathy susceptibility 2 (ARMS2) and complement factor H (CFH). A questionnaire was administered to collect history of smoking, hypertension, diabetes, and hyperlipidemia, as well as personal and family history of AMD. RESULTS: The reticular pseudodrusen group was older (median age 87 vs 81 years, P = .04) and had more female subjects (83.3% vs 48.8%, P = .003), later ages of AMD onset (83 vs 70 years, P = .0005), and a greater frequency of hypertension (76.7% vs 55.8%, P = .08). No significant differences were found in the distribution of the ARMS2 risk allele (P = .4) between the reticular pseudodrusen (homozygous = 20.0%; heterozygous = 56.7%) and large soft drusen (homozygous = 19.0%; heterozygous = 42.9%) phenotypes, or in the distribution of the CHF risk allele (P = .7) between the reticular pseudodrusen (homozygous = 26.7%; heterozygous = 56.7%) and large soft drusen (homozygous = 21.4%; heterozygous = 66.7%) phenotypes.     Concurrent trastuzumab with adjuvant radiotherapy in HER2-positive breast cancer patients: acute toxicity analyses from the French multicentric study

    Background: Trastuzumab (T) combined with chemotherapy has been recently shown to improve outcome in HER2-positive breast cancer (BC). The aim of this study was to evaluate the toxic effects of concurrent radiation therapy (RT) and T administration in the adjuvant setting. Patients and methods: Data of 146 patients with stages II-III HER2-positive BC were recorded. Median age was 46 years. In all, 32 (23%) and 114 (77%) patients received a weekly and a 3-week T schedule, respectively. A median dose of 50 Gy was delivered after surgery. Internal mammary chain (IMC) was irradiated in 103 (71%) patients. Results: Grade >2 dermatitis and esophagitis were noted in 51% and 12%, respectively. According to the Common Toxicity Criteria v3.0 scale and HERA (HERceptin Adjuvant) trial criteria, respectively, 10% and 6% of the patients had a grade ≥2 of left ventricular ejection fraction (LVEF) decrease after RT. Multivariate analyses revealed two independent prognostic factors: weekly T administration (for LVEF decrease) and menopausal status (for dermatitis). Higher level of T cumulative dose (>1600 mg) was only borderline of statistical significance for acute esophagitis toxicity. Conclusion: We showed that weekly concurrent T and RT are feasible in daily clinical practice with, however, a decrease of LVEF. Cardiac volume sparing and patient selections for IMC irradiation are highly recommended. Longer follow-up is warranted to evaluate late toxic effect

    Esta pesquisa integra o Projeto Orla, dos espaços públicos de lazer da Orla de Atalaia, Aracaju/SE, construído no grupo Observatório da Mídia Esportiva da Universidade Federal de Sergipe. A Nova Orla de Atalaia reúne um número diversificado de espaços destinados às práticas corporais de esporte e lazer, fato que atrai pessoas de diferentes gostos e estilos que formam grupos sociais aos quais denominamos tribos. Considerando a acessibilidade, localização e estrutura do espaço, buscou-se identificar quais os seus interesses dos grupos em relação aos espaços públicos de lazer da Orla de Atalaia. Na pesquisa de caráter Qualitativo/descritivo foram utilizados como instrumentos de coleta de dados a Entrevista Semi-estruturada e o Diário de Campo, bem como alguns recursos tecnológicos como: câmara fotográfica e a filmadora. As observações e análises nortearam a pesquisa a três temáticas de discussão: a questão pública e privada do acesso aos espaços; a versão mercadológica fetichizada do esporte e do lazer; e as práticas não convencionais. Alguns resultados demonstram a acessibilidade e a qualidade das estruturas como fatores primordiais de interesse às tribos e que o lazer pode ser meio de produção cultural e de formação de grupos, bem como uma mercadoria, um fetiche do espetáculo consumismo

    Programmatic Impact of QuantiFERON-TB Gold In-Tube Implementation on Latent Tuberculosis Diagnosis and Treatment in a Public Health Clinic

    Background: QuantiFERON-TB Gold In-Tube (QFT-GIT) is considered an alternative to the tuberculin skin test (TST) for the diagnosis of tuberculosis (TB) infection, but the programmatic impact of QFT-GIT implementation is largely unknown. In March, 2010, the Baltimore City Health Department (BCHD) introduced routine QFT-GIT testing for individuals referred to the TB program for suspected latent TB infection (LTBI). Design: Retrospective study comparing LTBI diagnosis and treatment during the 13 months before and after QFT-GIT implementation at the BCHD TB clinic. Results: 607 and 750 individuals were referred by community-providers for suspected LTBI in the pre- and post-QFT-GIT periods, respectively. Most individuals in the pre- and post-QFT-GIT periods were referred on the basis of a positive TST (597/607 [98%] vs. 690/750 [92%], respectively) and were foreign-born (363/607[59%] vs. 507/750[68%], respectively). BCHD performed QFT-GIT testing for 375/543 (69%) eligible individuals in the post-QFT-GIT period, of which 185 (49%) were positive, 178 (47%) were negative, 1 (0.25%) was indeterminate, and 11 (3%) did not yield results. Concordance of QFT-GIT with TST was low (183/352[52%]). Foreign-born individuals had higher proportions of QFT-GIT positivity (57%) than US-born individuals (36%; AOR 3.3 [95%CI 1.7–6.2]). Significantly fewer individuals received a final diagnosis of LTBI in the post-QFT-GIT period (397/567 [70%]) compared to the pre-QFT-GIT period (445/452 [98%], p,0.001).
