    Application of isotope-selective non-dispersive infrared spectrometry for the evaluation of the 13C-urea breath test: comparison with three concordant methods

    The aim of this work was to compare the performance of isotope-selective non-dispersive infrared spectrometry (IRIS) for the 13C-urea breath test with the combination of the 14C-urea breath test (14C-UBT), urease test and histologic examination for the diagnosis of H. pylori (HP) infection. Fifty-three duodenal ulcer patients were studied. All patients were submitted to gastroscopy to detect HP by the urease test, histologic examination and 14C-UBT. To be included in the study the results of the 3 tests had to be concordant. Within one month after admission to the study the patients were submitted to IRIS with breath samples collected before and 30 min after the ingestion of 75 mg 13C-urea dissolved in 200 ml of orange juice. The samples were mailed and analyzed 11.5 (4-21) days after collection. Data were analyzed statistically by the chi-square and Mann-Whitney test and by the Spearman correlation coefficient. Twenty-six patients were HP positive and 27 negative. There was 100% agreement between the IRIS results and the HP status determined by the other three methods. Using a cutoff value of delta-over-baseline (DOB) above 4.0 the IRIS showed a mean value of 19.38 (minimum = 4.2, maximum = 41.3, SD = 10.9) for HP-positive patients and a mean value of 0.88 (minimum = 0.10, maximum = 2.5, SD = 0.71) for negative patients. Using a cutoff value corresponding to 0.800% CO2/weight (kg), the 14C-UBT showed a mean value of 2.78 (minimum = 0.89, maximum = 5.22, SD = 1.18) in HP-positive patients. HP-negative patients showed a mean value of 0.37 (minimum = 0.13, maximum = 0.77, SD = 0.17). IRIS is a low-cost, easy to manage, highly sensitive and specific test for H. pylori detection. Storing and mailing the samples did not interfere with the performance of the test

    Comportamento alimentar de ratos adultos submetidos à restrição protéica cujas mães sofreram desnutrição durante a lactação Feeding behavior of adult rats submitted to protein malnutrition whose mothers received protein restricted diets during lactation

    Foi avaliado o consumo alimentar de animais adultos submetidos à restrição protéica, cujas mães receberam dieta hipoprotéica ou hipocalórica na lactação: controle (C) ração normal com 23% de proteína; restrição protéica (RP) 8% de proteína; restrição energética (RE) 23% de proteína, em quantidade restrita à ingerida pelo grupo restrição protéica. Após o desmame todos os filhotes receberam ração normal até 60 dias e nesta época foram submetidos, por 21 dias, ao seguinte tratamento: (C/C)-filhotes de mães C recebendo ração normal; (restrição protéica/controle)-filhotes de mães Controle, recebendo ração hipoprotéica; (C/RP)-filhotes de mães RP recebendo ração normal; (RP/RP)-filhotes de mães RP recebendo ração hipoprotéica; (C/RE)-filhotes de mães RE recebendo ração normal; (RP/RE)-filhotes de mães RE recebendo ração hipoprotéica. Os filhotes de mães RP consumiram menos ração até 57 dias (p<0,01), enquanto os filhotes de mães RE normalizaram a ingestão aos 37 dias. Aos 81 dias, os animais submetidos à restrição protéica consumiram menos ração (p<0,01). A dieta materna na lactação modificou o consumo alimentar e o peso corporal da prole na vida adulta, estando, possivelmente, a restrição protéica associada a uma alteração permanente no controle hipotalâmico da seleção de nutrientes da prole.<br>This study evaluated food intake of adult animals whose mothers received protein- or energy-restricted diets during lactation as follows: control (C)- 23% of protein; protein-restricted (PR) - 8% of protein; energy-restricted (ER) - 23% of protein, restricted in quantity. After weaning all pups received the control diet until 60 days, when they were separated into groups, with free access to the following diets during 21 days: (C/C)- pups of control dams fed control diet; (PR/C)-pups of control dams fed protein-restricted diet; (C/PR)- pups of protein-restricted dams fed control diet; (PR/PR)-pups of protein-restricted dams fed protein-restricted diet; (C/ER)-pups of energy-restricted dams fed control diet; (PR/ER)-pups of energy-restricted dams fed protein-restricted diet. Pups of protein-restricted dams consumed less food until day 57 (p<0.01), while pups of energy-restricted dams normalized their ingestion at 37 days. At 81 days, all malnourished animals consumed less food than the control (p<0.01). The kind of diet consumed in the lactation period changed the food intake and the body weight of the offspring in the adulthood. So, the protein restriction can possibly be associated with a permanent alteration in the hypothalamic control of nutrients selection of the offspring

    Trata-se de investigar os significados atribuídos por pacientes portadores do vírus da hepatite C sobre a doença e o tratamento dietético. Para tanto, adota-se a metodologia da pesquisa qualitativa em que o sujeito interage com sua narrativa sobre o objeto de estudo e os pesquisadores analisam cuidadosamente seus enunciados. As acepções sobre o adoecer e as mudanças dos hábitos alimentares e de vida dos pacientes foram analisadas por meio da obtenção de entrevistas em profundidade. Ao trazer os sentidos expressos sobre a doença, cria-se a possibilidade de compreensão dos aspectos culturais sobre a alimentação, a nutrição e a dietética; um desafio para os profissionais de saúde e nutrição, que precisam relatar a seus pacientes a necessidade de seguir hábitos alimentares mais saudáveis, implicando alterações nem sempre simples de serem realizadas. Consideram-se hábitos e representações do ato da alimentação, como parte da identidade cultural do sujeito. Justifica-se este estudo pela necessidade de situar maior interação entre profissionais, estudantes de saúde e pacientes de modo a ampliar a compreensão dos fenômenos que cercam a experiência da doença pelos pacientes. Conclui-se que os pacientes de hepatite C atendidos no ambulatório do Hospital Universitário da cidade de Salvador e o profissional podem atingir os objetivos de aconselhamentos nutricionais, se trilharem direções dialógicas. Com isso, observou-se a adesão à orientação dietética sem rupturas bruscas e sofrimentos dos pacientes.<br>This paper investigates the meanings attributed by patients to hepatitis C on disease and dietary treatment. To this end, we adopt the methodology of qualitative research in which the individual interacts with his narrative on the subject of study and the researchers carefully examine their statements. The meanings of the illness and changes in dietary habits and life of the patients were analyzed by obtaining in-depth interviews. By bringing the senses expressed about the disease, it creates the possibility of understanding cultural factors on diet, nutrition and dietetics - a challenge for health professionals and nutrition, which need to report their patients the need to follow dietary healthier, involving changes not always simple to perform. Habits and representations of the act of feeding are considered part of the cultural identity of the subject. This study is justified by the need to place greater interaction among professionals, students, patients and health so as to broaden the understanding of the phenomena that surround the experience of illness by patients. We conclude that outpatients with hepatitis C at the University Hospital in the city of Salvador and the professional can achieve the goals of nutritional advice, if they follow dialogic lines. Thus, there was compliance with dietary counseling without sudden breaks and suffering of patients
