From Theory to Practice: An International Approach to Establishing Prehabilitation Programmes

PurposeThis article focuses on the following:*The importance of prehabilitation in people with cancer and the known and hypothesised benefits.*Exploration of the principles that can be used when developing services in the absence of a single accepted model of how these services could be established or configured.*Description of approaches and learning in the development and implementation of prehabilitation across three different countries: Canada, the Netherlands and the United Kingdom, based on the authors’ experiences and perspectives.Recent FindingsPractical tips and suggestions are shared by the authors to assist others when implementing prehabilitation programmes. These include experience from three different approaches with similar lessons.Important elements include the following: (i) starting with a small identified clinical group of patients to refine and test the delivery model and demonstrate proof of concept; (ii) systematic data collection with clearly identified target outcomes from the outset; (iii) collaboration with a wide range of stakeholders including those who will be designing, developing, delivering, funding and using the prehabilitation services; (iv) adapting the model to fit local situations; (v) project leaders who can bring together and motivate a team; (vi) recognition and acknowledgement of the value that each member of a diverse multidisciplinary team brings; (vii) involvement of the whole team in prehabilitation prescription including identification of patients’ levels of risk through appropriate assessment and need-based interventions; (viii) persistence and determination in the development of the business case for sustainable funding; (ix) working with patients ambassadors to develop and advocate for the case for support; and (x) working closely with commissioners of healthcare.SummaryPrinciples for the implementation of prehabilitation have been set out by sharing the experiences across three countries. These principles should be considered a framework for those wishing to design and develop prehabilitation services in their own areas to maximise success, effectiveness and sustainability

Updated Nucleosynthesis Constraints on Unstable Relic Particles

We revisit the upper limits on the abundance of unstable massive relic particles provided by the success of Big-Bang Nucleosynthesis calculations. We use the cosmic microwave background data to constrain the baryon-to-photon ratio, and incorporate an extensively updated compilation of cross sections into a new calculation of the network of reactions induced by electromagnetic showers that create and destroy the light elements deuterium, he3, he4, li6 and li7. We derive analytic approximations that complement and check the full numerical calculations. Considerations of the abundances of he4 and li6 exclude exceptional regions of parameter space that would otherwise have been permitted by deuterium alone. We illustrate our results by applying them to massive gravitinos. If they weigh ~100 GeV, their primordial abundance should have been below about 10^{-13} of the total entropy. This would imply an upper limit on the reheating temperature of a few times 10^7 GeV, which could be a potential difficulty for some models of inflation. Partnerships between an At-Risk Youth CrossFit Program and Local Community Organizations: Focusing on the Antecedents to Partnership Development

A large body of research has established that sport intervention programs can have social, emotional and health benefits for at-risk youth. While research has focused on the positive outcomes associated with these programs, little attention has been given to program inputs. It is recognized that community partnerships can help intervention programs achieve their goals. Yet, how are such partnerships formed and what can help to promote the successful formation of partnerships? This paper provides a detailed account of the partnership implementation process undertaken to develop and deliver a health promotion physical activity program for at-risk youth through the medium of CrossFit in a low socioeconomic area in a rural community in the southeastern United States. Developing successful partnerships serves as a valuable component to help organizations obtain resources and skills needed to initiate and continue programs for underserved populations. The scholars identify and explain how critical success factors such as personal contact, partnership complementarity and fit and the promotion of high levels of commitment and trust, serve as important starting points for developing and maintaining strong community partnerships

Betting Against Beta (and Gamma) Using Government Bonds

Purportedly consistent with "risk parity" (RP) asset allocation, recent studies document compelling "low risk" trading strategies that exploit a persistently negative relation between Sharpe ratios (SRs) and maturity along the U.S. Treasury (UST) term structure. This paper extends this evidence on betting against beta with government bonds (BABgov) in four respects. First, out-of-sample tests suggest that excess returns may have waned somewhat recently and that the pattern seems most pronounced for USTs given data on ten other previously unexamined government bond markets. Second, BABgov appears robust when hedged ex-ante against covariance with equities and thereby does not resemble selling volatility, but these results nonetheless belie a possible tension rather than consistency between leverage constraints and lowrisk investing: namely, that investors bid longer-dated UST prices higher (lower) under BAB (RP). Third, the fact that Gaussian affine term structure models of USTs also imply an inverted SR schedule suggests that investors cannot, in fact, realize abnormal returns if they are fully hedged to the underlying model factors, and BABgov excess returns are indeed not robust to exante constraints on exposure to the yield curve's principal components. Fourth, some evidence suggests that previous BABgov results reflect coskew preferences, alternative BABgov strategies hedged to coskew risks ex-ante forgo substantial returns, and there is no indication that investors can earn excess returns betting against gamma. However, the sign of investors' coskew preferences in government bond markets remains ambiguous
