    Treatment of hyper-granulated limb wounds in horses

    This study was performed to investigate the different methods of treating hyper granulation tissue on experimentally induced wounds in equine limbs. Wounds were induced by removal of a skin patch and subcutaneous tissue for about 5-7 cm width and 6-8 cm in length from the dorsal and lateral aspect of the fore and hind limbs below the carpal and tarsal joints. The wounds were left open without treatment and the animals were trained 2-2.5 hours every day for about 3-5 weeks until hyper granulation tissue was developed. The schedule for the treatment of hyper granulation was divided into five groups each contained eight wounds of hyper granulation tissue; each main group was divided into two subgroups. The subgroups of first, second, third, fourth and fifth groups were treated by the following schedules: bandage alone; copper sulphate ointment 10%; silver nitrate ointment 2%; red mercury ointment 11%; and laser therapy (at a total dose of 9.72 Joule / cm2) respectively. While the second subgroups were treated by surgical resection of the hyper granulation tissue, followed by the same treatments applied on the first subgroup. The bandage for all experimental groups was changed every 48 hours until healing was occurred. The clinical and histological observation of the first group revealed that the healing take long period comparing with other groups. The mean of wound healing were 65 days in non surgical removal of hyper granulation tissue subgroup, while 57 days in surgical removed of hyper granulation tissue subgroup. The results of the second, third, fourth groups revealed that the caustic material especially red mercury has a role in healing processes through depressing the hyper granulation tissue. The mean of wound healing of the second group was 42.25 days in non surgical removal of hyper granulation tissue subgroup while 37.25 days in surgically removed hyper granulation tissue subgroup. In the third group the mean of wound healing was 45.75 days in non surgical removal of hyper granulation tissue subgroup while 44.75 days in surgically removed hyper granulation tissue subgroup. While in the fourth group the mean of wound healing was 39 days in non surgical removal of hyper granulation tissue subgroup while 36 days in surgically removed hyper granulation tissue subgroup. In the fifth group the clinical and histological observation revealed that the using of laser lead to reduce the period for wound healing significantly comparing with other groups. The mean of wound healing was 25 days in non surgical removal of hyper granulation tissue subgroup while 20 days in surgically removed hyper granulation tissue subgroup, so that the laser was the best in this study and the using of surgical removal is better than of non surgical removal

    Health-promoting behaviors and social support of women of reproductive age, and strategies for advancing their health: Protocol for a mixed methods study

    <p>Abstract</p> <p>Background</p> <p>Determining the health-promoting behaviors of women during the important period of reproduction provides valuable information for designing appropriate intervention programs for advancing women's health. There is no study on the health-promoting behaviors of women of reproductive age in Iran. Thus, the aim of this study is to explore these health-promoting behaviors for the purpose of developing comprehensive and culturally sensitive health advancement strategies for Iranian women.</p> <p>Methods/Design</p> <p>This study has a sequential explanatory mixed methods design. The follow-up explanation model is used to elaborate the quantitative results by collecting qualitative data from participants who could best assist in elucidating the results. The study is conducted in two sequential phases. The first phase is a population-based cross-sectional survey in which 1350 Iranian women of reproductive age are selected by proportional random multistage cluster sampling of the 22 main municipal sectors of Tehran, Iran. Questionnaires are completed through a face-to-face interview. The second phase is a qualitative study in which participants are selected using purposive sampling in the form of extreme case sampling on the basis of health-promoting behavior scores. The qualitative phase is based on data collected from focus group discussions or individual in-depth interviews. A conventional qualitative content analysis approach is used, and the data are managed with a computer-assisted program. Women's health-promoting strategies are developed using the qualitative and quantitative results, a review of the related literature, and the nominal group technique among experts.</p> <p>Discussion</p> <p>The findings of this mixed methods sequential explanatory study, obtained using a culturally sensitive approach, provide insights into the health behavioral factors that need to be considered if preventive strategies and intervention programs are to be designed to promote women's health in the community.</p

    Motorcyclists' reactions to safety helmet law: a qualitative study

    <p>Abstract</p> <p>Background</p> <p>Extensive body of the literature reveals that proper use of helmets is an effective way to reduce the severity of injuries and fatalities among motorcyclists. However, many motorcyclists do not use safety helmet properly. This study aimed to empirically explore reactions of motorcyclists to the safety helmet laws, in Iran.</p> <p>Methods</p> <p>Qualitative data were collected via four focus groups and 11 in-depth interviews. Participants were 28 male motorcyclists who never used a safety helmet during rides, and 4 male police officers. All transcripts, codes and categories were read for several times to exhaust identifiable major themes. During this process data were reduced from text to codes and themes.</p> <p>Results</p> <p>Five major themes emerged from the data analyses, including themes related to the following: (1) circumventing or dodging police officers; (2) simulating a helmet wearing behavior; (3) accepting the probability of receiving a ticket; (4) taking advantage of the police neglect and carelessness; and (5) using a cheap or convenient helmet.</p> <p>Conclusion</p> <p>Our findings suggest certain levels of reckless driving among the participating motorcyclists in this study. They also point to a system of law enforcement that operates haphazardly and fails to consistently penalize those who deviate from it. Further studies are needed to investigate how "risks" are perceived and relate to "reactions", and how a 'culture of masculinity' may encourage risk tolerance and a disposition toward lawlessness and carelessness among male motorcyclists. Also, there is a need for the development and implementation of multidimensional interventions that would offer socio-culturally sensitive educational and motivational messages to the motorcyclists and the in-service traffic-enforcement officers in Iran.</p

    Second asymptomatic carotid surgery trial (ACST-2): a randomised comparison of carotid artery stenting versus carotid endarterectomy

    Background: Among asymptomatic patients with severe carotid artery stenosis but no recent stroke or transient cerebral ischaemia, either carotid artery stenting (CAS) or carotid endarterectomy (CEA) can restore patency and reduce long-term stroke risks. However, from recent national registry data, each option causes about 1% procedural risk of disabling stroke or death. Comparison of their long-term protective effects requires large-scale randomised evidence. Methods: ACST-2 is an international multicentre randomised trial of CAS versus CEA among asymptomatic patients with severe stenosis thought to require intervention, interpreted with all other relevant trials. Patients were eligible if they had severe unilateral or bilateral carotid artery stenosis and both doctor and patient agreed that a carotid procedure should be undertaken, but they were substantially uncertain which one to choose. Patients were randomly allocated to CAS or CEA and followed up at 1 month and then annually, for a mean 5 years. Procedural events were those within 30 days of the intervention. Intention-to-treat analyses are provided. Analyses including procedural hazards use tabular methods. Analyses and meta-analyses of non-procedural strokes use Kaplan-Meier and log-rank methods. The trial is registered with the ISRCTN registry, ISRCTN21144362. Findings: Between Jan 15, 2008, and Dec 31, 2020, 3625 patients in 130 centres were randomly allocated, 1811 to CAS and 1814 to CEA, with good compliance, good medical therapy and a mean 5 years of follow-up. Overall, 1% had disabling stroke or death procedurally (15 allocated to CAS and 18 to CEA) and 2% had non-disabling procedural stroke (48 allocated to CAS and 29 to CEA). Kaplan-Meier estimates of 5-year non-procedural stroke were 2·5% in each group for fatal or disabling stroke, and 5·3% with CAS versus 4·5% with CEA for any stroke (rate ratio [RR] 1·16, 95% CI 0·86–1·57; p=0·33). Combining RRs for any non-procedural stroke in all CAS versus CEA trials, the RR was similar in symptomatic and asymptomatic patients (overall RR 1·11, 95% CI 0·91–1·32; p=0·21). Interpretation: Serious complications are similarly uncommon after competent CAS and CEA, and the long-term effects of these two carotid artery procedures on fatal or disabling stroke are comparable.     Effect of magnesium oxide nanoparticles locally or systemic with hydroxyapatite on regeneration of radial fracture in rabbits: Radiological study

    Study aimed to develop and test effect of magnesium oxide nanoparticles (MgONPs) and hydroxyapatite (HA) with gel on repair of distal radius transverse fractures. MgONPs concentrations 200 μg /ml were dissolved in 1 ml distilled water, stirred for 10 minutes, 0.5 mg HA in 1 ml gel, shaking for 15 minutes. These materials were tested in vitro. Forty adult male rabbits, at 1.5 to 2 years old and weighing 1.7-2.3 kg. Rabbits were divided into 2 groups: Rabbits were given intramuscularly 40 mg/kg B.W ketamine hydrochloride, 5 mg/ kg B.W xylazine to anesthetic before operations. A 5 cm incision was made in the skin of forelimb cranio-medially, transverse fracture was induced. MgONPs/HA/gel applied locally group (A). While HA/gel was applied locally, MgONPs were intraperitoneally injected group (B). Clinical examination revealed excellent apatite with minor swelling at the fracture site. At 2nd, 4th, 6th, and 8th weeks, Radiographic evaluation revealed group A had periosteal reaction within fracture site, at 2nd week post-operatively, group B had active periosteal reaction at fracture site. At 4th week, callus bridge connected two ends of fracture in group A. At same time, a visible callus crossed two ends of fracture in group B

    Study's purpose of this study is to conduct synthesis and evaluate the effect of hydroxyapatite (HA) with hydrogel locally magnesium oxide nanoparticles (MgONPS) locally or intraperitoneally (IP) on the healing of the distal third radial fracture. Concentrations of MgONPs 200μg/ml, dissolved in 1 cc distilled water and the solution stirred by a stirrer for 10 min. HA 0.5 mg in 1ml hydrogel and the solution stirring at the vortex for 15 min. These materials were evaluated in vitro to ensure their suitability with the tissues. Seventy-five healthy adult male rabbits, aged about 1.5- 2 years old with average weighting  1.7- 2.3 Kg. B.W were used. Rabbits were divided into three groups randomly (n=25), group A (HA mixed hydrogel applied locally), group B (HA mixed with hydrogel and MgONPs applied locally) and group C (HA mixed hydrogel applied locally and MgONPs IP). Animals were anesthetized by i.m 40 mg/ kg B.W ketamine hydrochloride and 5mg/ kg B.W xylazine. A 5cm incision had made cranio-medially in the skin of the forelimb (right forelimb) and exposure radius and ulna. The macroscopic evaluation revealed that all groups at 2nd week showed bone reaction in different degrees

    Comparative study of three methods of esophageal anastomosis in dogs

    This study was performed to compare three methods of esophageal anastomosis. Twenty four healthy adult dogs were used in this study. The animals were divided into three groups; each one consisted of 8 animals. In group 1; two layers were used to perform the esophageal anastomosis. The first layer represented simple interrupted suture to close the mucosa with knot inside the lumen, and the second layer represented horizontal mattress interrupted suture to close the other layers of esophagus. While in group 2; one layer of cross interrupted mattress suture was used to close all layers of esophageal wall, and in group 3; one layer of Schmieden's suture was used to close all layers of esophageal wall. The results of clinical, radiological and histopathological studies after 15 and 30 days of surgical operation revealed that most of the animals showed different degrees of difficulty concerning the moderate dysphagia and regurgitation. The radiological study showed significant difference of stenosis. The best results were recorded in the second group where the mean degree of stenosis was 7.69%, however the mean degree of stenosis was 42.80% in the first group, while the mean degree of stenosis in the third groups was 37.81%, through 30 days. The histopathological study of group 2 showed rapid healing of the site of anastomosis, lack of granulation tissue and consequently the less degree of stricture and other complications as compared with groups 1 and 3. The Schmieden's suture was characterized by its standard short time as compared with group 1 and 2, although accompanied by some complications. In conclusion this study revealed that the cross mattress suture used in the second group characterized by faster healing and minimal amount of fibrous tissue formation manifested by decrease in moderate degree of stenosis as compared with the two other suture patterns used in the first and third groups

    A comparative study of laparoscopic partial hepatectomy by clamp-crush and ultrasurgical techniques in rams

    Th e p res en t study was designed to evaluate the feasibility and safety of laparoscopic partial hepatectomy via two techniques in rams. Tw enty-fo ur adult l ocal b reed rams were used, which divid ed rando mly and equ ally i nto two grou ps. All ani mals s ubjected to laparoscopic partial hepatectomy. operations were performed under the effect of general anesthesia by using 5 % thiopental sodium at a dose rate of 15 mg/kg B.W., intravenously and maintenance was done by inhalation (halothane) (2- 2.5%). Animals placed in a reverse Trendelenburg (30º) and left lateral position. Pneumoperitoneum was achieved under (10- 12mmHg). Four (10 mm) ports were made on ventral abdominal wall. Liver parenchyma was controlled with modified liver clamp. In first group, partial hepatectomy was done by Clamp-crush forceps. While in second group, Ultra- surgical device was used. Following operations, certain parameters such as rectal temperature, respiratory rate, heart rate, food intake, and secondary health problemes had been recorded. Also the operative and resection times and blood loss were estimated. Liver enzymes (alinine transaminase, aspartate transaminase, alkaline phosphate, total serum bilirubin and total serum proteins were measured. Histopathological examination of liver biobsies was performed at 3 rd, 7 th, 15 th days and four months post-surgery. The main intra-operative complication was the bleeding. Result indicated that the vital parameters were within the normal rang. Shorter operative and resection times were noticed in second group when compared with first group. There were no significant differences in the mean values of blood loss in both groups. Also no bile collection were seen in the abdominal cavity or subphrinic region during the exploratory laparoscopy at (3, 7
