    Treatment options for vasomotor symptoms in menopause: focus on desvenlafaxine.

    Vasomotor symptoms (VMS), including hot flashes and night sweats, occur in as many as 68.5% of women as a result of menopause. While the median duration of these symptoms is 4 years, approximately 10% of women continue to experience VMS as many as 12 years after their final menstrual period. As such, VMS have a significant impact on the quality of life and overall physical health of women experiencing VMS, leading to their pursuance of treatment to alleviate these symptoms. Management of VMS includes lifestyle modifications, some herbal and vitamin supplements, hormonal therapies including estrogen and tibolone, and nonhormonal therapies including clonidine, gabapentin, and some of the serotonin and serotonin-norepinephrine reuptake inhibitors. The latter agents, including desvenlafaxine, have been the focus of increased research as more is discovered about the roles of serotonin and norepinephrine in the thermoregulatory control system. This review will include an overview of VMS as they relate to menopause. It will discuss the risk factors for VMS as well as the proposed pathophysiology behind their occurrence. The variety of treatment options for VMS will be discussed. Focus will be given to the role of desvenlafaxine as a treatment option for VMS management

    Perspectives of application for nanoparticles prepared by CO2 laser pyrolysis: from ceramic nanocomposites to nanofluids

    Nanoparticles are one of the main ingredients for the realization of a wide range of nanostructured materials and devices with potential applications in several research areas and industrial sectors, hence their synthesis is a critical step in the development of nanotechnologies. Here we report on the preparation of several types of nanoparticles by laser pyrolysis of gas phase or vapour phase precursors, a very flexible and scalable synthesis route. A critical insight is given into the perspectives of practical applications of these nanoparticles in a number of fields ranging from the fabrication of ceramic nanocomposites to wear resistant coatings, from fluorophores for bio-imaging to nanofluids for efficient thermal management, from nanocoatings for cultural heritage preservation to the realization of a new class of inks for ink-jet printing applications

    What antarctic plants can tell us about climate changes: Temperature as a driver for metabolic reprogramming

    Global warming is strongly affecting the maritime Antarctica climate and the consequent melting of perennial snow and ice covers resulted in increased colonization by plants. Colobanthus quitensis is a vascular plant highly adapted to the harsh environmental conditions of Antarctic Peninsula and understanding how the plant is responding to global warming is a new challenging target for modern cell physiology. To this aim, we performed differential proteomic analysis on C. quitensis plants grown in natural conditions compared to plants grown for one year inside open top chambers (OTCs) which determine an increase of about 4 °C at midday, mimicking the effect of global warming. A thorough analysis of the up and downregulated proteins highlighted an extensive metabolism reprogramming leading to enhanced photoprotection and oxidative stress control as well as reduced content of cell wall components. Overall, OTCs growth seems to be advantageous for C. quitensis plants which could benefit from a better CO2 diffusion into the mesophyll and a reduced ROS‐mediated photodamage

    Burnout, well-being and defensive medical practice among obstetricians and gynaecologists in the UK: cross-sectional survey study

    Objectives: To determine the prevalence of burnout in doctors practising obstetrics and gynaecology, and assess the association with defensive medical practice and self-reported wellbeing. Design: Nationwide online cross-sectional survey study; December 2017-March 2018. Setting: Hospitals in the United Kingdom Participants: 5661 practising Obstetrics and Gynaecology consultants, specialty and associate specialist doctors and trainees registered with the Royal College of Obstetricians and Gynaecologists Primary and Secondary Outcome Measures: Prevalence of burnout using the Maslach Burnout Inventory and defensive medical practice (avoiding cases or procedures, overprescribing, over-referral) using a 12-item questionnaire. The odds ratios of burnout with defensive medical practice and self-reported wellbeing. Results: 3102/5661 doctors (55%) completed the survey. 3073/3102 (99%) met the inclusion criteria (1462 consultants, 1357 trainees and 254 specialty and associate specialist doctors). 1116/3073 (36%) doctors met the burnout criteria, with levels highest amongst trainees (580/1357 [43%]). 258/1116 (23%) doctors with burnout reported increased defensive practice compared to 142/1957 (7%) without (adjusted odds ratio 4.35, 95% CI 3.46 to 5.49). Odds ratios of burnout with wellbeing items varied between 1.38 and 6.37, and were highest for anxiety (3.59, 95% CI 3.07 to 4.21), depression (4.05, 95% CI 3.26 to 5.04), and suicidal thoughts (6.37, 95% CI 95% CI 3.95 to 10.7). In multivariable logistic regression, being of younger age, white or ‘other’ ethnicity, and graduating with a medical degree from the UK or Ireland had the strongest associations with burnout. Conclusions: High levels of burnout were observed in obstetricians and gynaecologists and particularly amongst trainees. Burnout was associated with both increased defensive medical practice and worse doctor wellbeing. These findings have implications for the wellbeing and retention of doctors as well as the quality of patient care, and may help to inform the content of future interventions aimed at preventing burnout and improving patient safety

    Tropospheric and ionospheric anomalies induced by volcanic and saharan dust events as part of geosphere interaction phenomena

    In this work, we assessed the possible relation of ionospheric perturbations observed by Detection of Electro-Magnetic Emissions Transmitted from Earthquake Regions (DEMETER), Global Positioning System total electron content (GPS TEC), National Oceanic and Atmospheric Administration (NOAA)-derived outgoing longwave-Earth radiation (OLR), and atmospheric chemical potential (ACP) measurements, with volcanic and Saharan dust events identified by ground and satellite-based medium infrared/thermal infrared (MIR/TIR) observations. The results indicated that the Mt. Etna (Italy) volcanic activity of 2006 was probably responsible for the ionospheric perturbations revealed by DEMETER on 4 November and 6 December and by GPS TEC observations on 4 November and 12 December. This activity also affected the OLR (on 26 October; 6 and 23 November; and 2, 6, and 14 December) and ACP (on 31 October-1 November) analyses. Similarly, two massive Saharan dust episodes, detected by Robust Satellite Techniques (RST) using Spinning Enhanced Visible and Infrared Imager (SEVIRI) optical data, probably caused the ionospheric anomalies recorded, based on DEMETER and GPS TEC observations, over the Mediterranean basin in May 2008. The study confirmed the perturbing effects of volcanic and dust events on tropospheric and ionospheric parameters. Further, it demonstrated the advantages of using independent satellite observations to investigate atmospheric phenomena, which may not always be well documented. The impact of this increased detection capacity in reducing false positives, in the framework of a short-term seismic hazard forecast based on the study of ionospheric and tropospheric anomalies, is also addressed

    Object-oriented Programming Laws for Annotated Java Programs

    Object-oriented programming laws have been proposed in the context of languages that are not combined with a behavioral interface specification language (BISL). The strong dependence between source-code and interface specifications may cause a number of difficulties when transforming programs. In this paper we introduce a set of programming laws for object-oriented languages like Java combined with the Java Modeling Language (JML). The set of laws deals with object-oriented features taking into account their specifications. Some laws deal only with features of the specification language. These laws constitute a set of small transformations for the development of more elaborate ones like refactorings
