    Rimegepant for the treatment of migraine.

    Migraine is a common form of primary headache, affecting up to 1 in every 6 Americans. The pathophysiology is an intricate interplay of genetic factors and environmental influence and is still being elucidated in ongoing studies. The trigeminovascular system is now known to have a significant role in the initiation of migraines, including the release of pain mediators such as CGRP and substance P. Traditional treatment of migraine is usually divided into acute and preventive treatment. Acute therapy includes non-specific therapy, such as NSAIDs and other analgesics, which may provide relief in mild to moderate migraines. 5-HT1 agonists may provide relief in severe migraine, but are not universally effective and carry a significant side-effect profile with frequent redosing requirement. Prophylactic therapy may reduce the occurrence of acute migraine attacks in selected patients, but does not completely eliminate it. More recently, CGRP antagonism has been studied and shown to be effective in both abortion and prevention of migraine. Novel medications, targeting CGRP, divide into CGRP antibodies and receptor antagonists (gepants). Rimegepant, a second-generation gepant, has shown efficacy in several clinical trials in treating acute migraine. Ongoing trials are also evaluating its role in migraine prophylaxis, and results are promising. It is also generally safer for use than existing options, does not appear to increase the chance of developing chronic migraines, and carries a very tolerable side effects profile. It is a part of a growing arsenal in migraine treatment, and may present the silver bullet for treatment of this disease

    Pattern selection in the absolutely unstable regime as a nonlinear eigenvalue problem: Taylor vortices in axial flow

    A unique pattern selection in the absolutely unstable regime of a driven, nonlinear, open-flow system is analyzed: The spatiotemporal structures of rotationally symmetric vortices that propagate downstream in the annulus of the rotating Taylor-Couette system due to an externally imposed axial through-flow are investigated for two different axial boundary conditions at the in- and outlet. Unlike the stationary patterns in systems without through-flow the spatiotemporal structures of propagating vortices are independent of parameter history, initial conditions, and system's length. They do, however, depend on the axial boundary conditions, the driving rate of the inner cylinder and the through-flow rate. Our analysis of the amplitude equation shows that the pattern selection can be described by a nonlinear eigenvalue problem with the frequency being the eigenvalue. Approaching the border between absolute and convective instability the eigenvalue problem becomes effectively linear and the selection mechanism approaches that one of linear front propagation.     Investigation of enhanced double weight code in point to point access networks

In this paper, an investigation and evaluation to enhanced double weight (EDW) code is performed, a new technique for code structuring and building using modified arithmetical model has been given for the code in place of employing previous technique based on Trial Inspections. Innovative design has been employed for the code into P2P networks using diverse weighted EDW code to be fitting into optical CDMA relevance applications. A new developed relation for EDW code is presented, the relation is based on studying and experimenting the effect of input transmission power with code weight, and the relation developed using numerical analysis method. This relation makes the estimation for the system input power needed more efficient. The results of the code has been explained by eye diagram and parametric illustrations from the simulated results. The result shows a magnificent performance of the code during high number of users and weight. On the other hand, the relation developed for power measurement helps to prevent power loss and consumption

    Review of Interventional Therapies for Refractory Pediatric Migraine.

    This is a review of the latest and seminal evidence in pediatric migraine. It covers the etiology and pathophysiology known today, and then will review treatment options, efficacy and safety, quality of data and indications. Though migraine is usually regarded as an infliction in adults, it is not uncommon in the pediatric population and affects up to 8% of children. Children may experience migraine differently than adults, and present not only with headache but also frequent gastrointestinal symptoms. They are frequently shorter in duration than in adults. Traditional migraine treatment in adults is less effective in children. In this population, adjunct therapies - such as interventional techniques - should be considered when traditional treatment fails, including Botulinum Toxin A (BTA) injections, peripheral nerve and ganglion blocks. BTA injections are FDA approved for migraine prophylaxis in adults, but currently not in children; however, recent evidence shows efficacy and safety in pediatric migraine management. Nerve blocks stop nociceptive afferent fibers through injection of local anesthetics, and it may be associated with the local injection of corticosteroids. Although more common in adults, recent data suggests they are safe and effective in children and adolescents. Blocking the sphenopalatine ganglion can be achieved through nasal approach, and achieves a similar action by blocking the entire ganglion. Interventional techniques may provide a key component in the alleviation of this otherwise debilitating chronic migraine pain.     Na incorporation in Mo and CuInSe from production processes

    Abstract Results of characterization of thin "lms of Mo deposited by DC magnetron sputtering on soda-lime glass (Mo/SLG) and CuInSe (CIS) on Mo/SLG are presented. The primary objective of the work was to clarify the factors determining the concentration of Na in commercial-grade CIS. Mo "lms were deposited by three laboratories manufacturing CIS thin "lm solar cells. Analysis was by secondary ion mass spectrometry, scanning electron microscopy and X-ray di!raction. Changes in Mo deposition parameters in general a!ected the Na level but there was no obvious link to any single Mo deposition parameter. Oxygen content directly a!ected the Na level. The Na behavior was not obviously connected to "lm preferred orientation. Selenization of the Mo layers was also examined. Elemental Se vapor was found to produce signi"cantly less selenization than H Se. The amount of selenization was also strongly dependent upon Mo deposition conditions, although a speci"c source of the change in reaction rate was not found. Na distributions in the CIS deposited on the Mo were not limited by the di!usivity of the Na. The Na concentration in the CIS was increased by annealing the Mo "lms both with and without intentionally added Na.     Perceptions and utilization of primary health care services in Iraq: findings from a national household survey

    <p>Abstract</p> <p>Background</p> <p>After many years of sanctions and conflict, Iraq is rebuilding its health system, with a strong emphasis on the traditional hospital-based services. A network exists of public sector hospitals and clinics, as well as private clinics and a few private hospitals. Little data are available about the approximately 1400 Primary Health Care clinics (PHCCs) staffed with doctors. How do Iraqis utilize primary health care services? What are their preferences and perceptions of public primary health care clinics and private primary care services in general? How does household wealth affect choice of services?</p> <p>Methods</p> <p>A 1256 household national survey was conducted in the catchment areas of randomly selected PHCCs in Iraq. A cluster of 10 households, beginning with a randomly selected start household, were interviewed in the service areas of seven public sector PHCC facilities in each of 17 of Iraq's 18 governorates. A questionnaire was developed using key informants. Teams of interviewers, including both males and females, were recruited and provided a week of training which included field practice. Teams then gathered data from households in the service areas of randomly selected clinics.</p> <p>Results</p> <p>Iraqi participants are generally satisfied with the quality of primary care services available both in the public and private sector. Private clinics are generally the most popular source of primary care, however the PHCCs are utilized more by poorer households. In spite of free services available at PHCCs many households expressed difficulty in affording health care, especially in the purchase of medications. There is no evidence of informal payments to secure health services in the public sector.</p> <p>Conclusions</p> <p>There is widespread satisfaction reported with primary health care services, and levels did not differ appreciably between public and private sectors. The public sector PHCCs are preferentially used by poorer populations where they are important providers. PHCC services are indeed free, with little evidence of informal payments to providers.</p

    Removing impulse noise images by using decision tree algorithm

    Presently, a digital image is obtained through a suitable digital imaging system. Although the current technology has tremendously advanced, no imaging system is perfect in generating images that are identical to their originals because of the inherent physical limitations. During the filtering of noisy image, important features such as edges, lines and other fine image details embedded in the image tends to blur because of filtering operation. The enhancement of noisy data, however, is a very critical process because the sharpening operation can significantly increase the noise. In this proposed approach we aim to develop image processing technique that can meet this new requirement, which are high quality and high speed. Furthermore, prevent the noise accretion during the sharpening of the image details, and compare the restored images via proposed method with other kinds of filters

    Replication of Infectious Bronchitis Virus in the Chicken Mesenchymal Stem Cells

    ABSTRACT The susceptibility of the chicken mesenchymal stem cells to infectious bronchitis virus was characterized after twenty consecutive passages in chicken mesenchymal stemm cells. Virus replication was monitored by cytopathic observation, indirect immunoperoxidase, and reverse transcription polymerase chain reaction. At 72 h post-infection (p.i.) in third passage, the cytopathic effect was characterized by rounding up of cell, monolayer detachment, intracytoplasmic brownish colouration was readily observed by from 24h p.i in third passage, and at all times the extracted viral RNA from IBV-infected monolayers was demonstrated by reverse transcription polymerase chain reaction. Tissue culture effective dose 50 was used to measure virus titration performed on chicken mesenchymal stem cells and the titres in twenty passages was 108.6 TID 50 /ml. The results obtained in this study suggested that the chicken mesenhymal stem cells can be used for adaptation IBV and may be considered a step forward for the use of these cells in the future for IBV vaccine production
