    Mid-term outcomes after distally locked-to-standard primary stem exchange in 29 hip-prosthesis patients

    AbstractBackgroundCementless locked femoral stems are used for revision surgery in patients with bone loss to induce spontaneous bone reconstruction, allowing subsequent replacement by a standard primary stem. The small number of patients and short follow-ups available to date preclude a valid assessment of this strategy.HypothesisAfter distally locked stem revision, replacement by a standard primary stem does not induce complications, and the quality of the bone reconstruction allows strong fixation of a regular primary stem.Materials and methodsWe retrospectively evaluated 29 patients in whom a distally locked femoral stem was replaced by a standard primary stem between 1998 and 2010 (cemented in 27, cementless in 2 cases). The reason for the procedure was stem breakage, stem migration, or thigh pain. Mean patient age was 63years (range, 39–78years). Outcomes were evaluated based on the Postel-Merle d’Aubigné [PMA] score and Harris Hip Score [HHS]. In addition, radiographs were obtained to assess prosthesis fixation and the Hofmann cortical index measured the bone reconstruction.ResultsThe distally locked stem was removed via a postero-lateral approach without femoral osteotomy in all the 29 cases. In one patient, an intra-operative fracture occurred during femoral preparation. Mean follow-up after the exchange procedure was 75months (range, 3–188months). Postoperative ccomplications occurred in 9 (32%) patients and consisted of chronic infection in 2 patients (after 3 and 76months), post-traumatic peri-prosthetic fractures treated with internal fixation in 3 patients (after 100, 138, and 182months), aseptic loosening in 3 patients (after 13, 39, and 122months), and recurrent instability in one patient (after 63months). All cause revision stem survival after 75months was 72% (95% confidence interval, 47%–87%). In the 19 patients who still had their revision stem at last follow-up, the mean PMA score was 16.7 (range, 13–18) and the mean HHS was 88.2 (range, 59–99). The Hofmann index remained unchanged [36.5% (range, 28%–58%) before the exchange and 32.9% (range, 20%–57%) after the exchange; P=0.129].DiscussionThis study confirms the feasibility of substituting a distally locked stem with a standard primary stem. No specific complications occurred and no technical difficulties arose when extracting the long stems. However, the 32% complication rate and, more specifically, the occurrence of loosening in 10% (3/29) of patients mandates caution in the use of this technique, which should not be proposed routinely, and suggests a need for considering cementless fixation of the standard primary stem.Level of evidenceLevel IV, retrospective study

    Long-term Site Fidelity and Individual Home Range Shifts in Lophocebus albigena

    We investigated long-term site fidelity of gray-cheeked mangabey (Lophocebus albigena) groups in Kibale National Park, Uganda. Concurrently, we monitored shifts in home range by individual females and subadult and adult males. We documented home range stability by calculating the area of overlap in successive years, and by recording the drift of each group’s monthly centroid from its initial location. Home ranges remained stable for 3 of our 4 groups (overlap over 10 yr >60%). Core areas were more labile, but group centroids drifted an average of only 530 m over the entire decade. Deviations from site fidelity were associated with dispersal or group fission. During natal dispersal, subadult males expanded their home ranges over many months, settling ≤4 home ranges away. Adult males, in contrast, typically dispersed within a few days to an adjacent group in an area of home range overlap. Adult males made solitary forays, but nearly always into areas used by their current group or by a group to which they had previously belonged. After secondary dispersal, they expanded their ranging in the company of their new group, apparently without prior solitary exploration of the new area. Some females also participated in home range shifts. Females shifted home ranges only within social groups, in association with temporary or permanent group splits. Our observations raise the possibility that male mangabeys use a finder-joiner mechanism when moving into new home ranges during secondary dispersal. Similarly, females might learn new resource locations from male immigrants before or during group fission

    A Glutathione Peroxidase, Intracellular Peptidases and the TOR Complexes Regulate Peptide Transporter PEPT-1 in C. elegans

    The intestinal peptide transporter PEPT-1 in Caenorhabditis elegans is a rheogenic H+-dependent carrier responsible for the absorption of di- and tripeptides. Transporter-deficient pept-1(lg601) worms are characterized by impairments in growth, development and reproduction and develop a severe obesity like phenotype. The transport function of PEPT-1 as well as the influx of free fatty acids was shown to be dependent on the membrane potential and on the intracellular pH homeostasis, both of which are regulated by the sodium-proton exchanger NHX-2. Since many membrane proteins commonly function as complexes, there could be proteins that possibly modulate PEPT-1 expression and function. A systematic RNAi screening of 162 genes that are exclusively expressed in the intestine combined with a functional transport assay revealed four genes with homologues existing in mammals as predicted PEPT-1 modulators. While silencing of a glutathione peroxidase surprisingly caused an increase in PEPT-1 transport function, silencing of the ER to Golgi cargo transport protein and of two cytosolic peptidases reduced PEPT-1 transport activity and this even corresponded with lower PEPT-1 protein levels. These modifications of PEPT-1 function by gene silencing of homologous genes were also found to be conserved in the human epithelial cell line Caco-2/TC7 cells. Peptidase inhibition, amino acid supplementation and RNAi silencing of targets of rapamycin (TOR) components in C. elegans supports evidence that intracellular peptide hydrolysis and amino acid concentration are a part of a sensing system that controls PEPT-1 expression and function and that involves the TOR complexes TORC1 and TORC2

    Sexual Signalling in Propithecus verreauxi: Male “Chest Badge” and Female Mate Choice

    Communication, an essential prerequisite for sociality, involves the transmission of signals. A signal can be defined as any action or trait produced by one animal, the sender, that produces a change in the behaviour of another animal, the receiver. Secondary sexual signals are often used for mate choice because they may inform on a potential partner's quality. Verreaux's sifaka (Propithecus verreauxi) is characterized by the presence of two different morphs of males (bimorphism), which can show either a stained or clean chest. The chest becomes stained by secretions of the sternal gland during throat marking (rubbing throat and chest on a vertical substrate while smearing the scent deposition). The role of the chest staining in guiding female mate choice was previously hypothesized but never demonstrated probably due to the difficulty of observing sifaka copulations in the wild. Here we report that stained-chested males had a higher throat marking activity than clean-chested males during the mating season, but not during the birth season. We found that females copulated more frequently with stained-chested males than the clean-chested males. Finally, in agreement with the biological market theory, we found that clean-chested males, with a lower scent-releasing potential, offered more grooming to females. This “grooming for sex” tactic was not completely unsuccessful; in fact, half of the clean-chested males copulated with females, even though at low frequency. In conclusion, the chest stain, possibly correlated with different cues targeted by females, could be one of the parameters which help females in selecting mates

    Die Charakterisierung der Pseudokomponenten anhand der Kohlenstoffzahl der Molekuehle erlaubt die Berechnung der Waermetoenung und des Wasserstoffverbrauches der Reaktion. Grundlage der Charakterisierung bilden Daten, die im Routinebetrieb einer Raffinerie anfallen. Ausgehend von einem adiabatischen eindimensionalen Modell des Reaktors werden die Produktauftrennung sowie das Kreislaufoel- und Quenchgassystem betrachtet, so dass sich die Simulationsergebnisse direkt mit Anlagenmesswerten vergleichen lassen. Zur Durchfuehrung der Modellrechnungen wurde ein FORTRAN-Programm entwickelt, mit dem die verfuegbaren Daten einer kompletten Betriebsperiode ausgewertet und so die Abhaengigkeiten modellspezifischer Parameter von der Katalysatorstandzeit ermittelt wurden. (orig.)The characterisation of the pseudo-components by means of the carbon number of the molecule enables one to calculate the heat tone and hydrogen consumption of the reaction. The basis of the characterisation is formed by the data which occur in routine operation of a refinery. Starting from an adiabatic one-dimensional model of the reactor, the product separation and the circuit oil and quenching gas system are considered, so that the simulation results can be directly compared with plant measurements. To carry out the model calculations, a FORTRAN program was developed, by which the available data of a complete operating period can be evaluated and thus the relationships of parameters specific to the model with the catalyst life were determined.     Berenty Reserve—A Gallery Forest in Decline in Dry Southern Madagascar—Towards Forest Restoration

    Berenty Reserve, a fully protected gallery forest beside the Mandrare River is renowned for its lemurs, but the continuous canopy of the main forest is shrinking, fragmenting and degrading. The aim of this study, before any restoration can be considered, is to investigate why canopy-cover is declining and define the forest’s vegetation status and composition. Our study includes analysis of tamarind age (the dominant species) and regeneration, forest extent, climate and soil. Measurement of trunk circumference and annual rings indicated a median age of 190 years, near the accepted maximum for tamarinds. There is no regeneration of tamarind seedlings under the canopy and an invasive vine, Cissus quadrangularis suffocates any regeneration on the forest margins. A vegetation survey, based on fifteen transects, broadly characterized three forest areas: continuous canopy near the river, transitional canopy with fewer tall trees, and degraded dryland; the survey also provided a list of the 18 most common tree species. Ring counts of flood-damaged roots combined with measurement to the riverbank show that erosion rates, up to 19.5 cm/year, are not an immediate threat to forest extent. The highly variable climate shows no trend and analysis of forest soil indicates compatibility with plant growth
