    Poor care and the professional duty of the registered nurse

    Concerns have been raised in recent years about standards of care in the UK. Notable failures have been identified in the care of vulnerable older adults. This article identifies and discusses some logical steps which might be taken to minimise the risk of individual and systemic care failure in settings for older adults. These steps include frank discussion about ageism to promote empowerment and respect for older people; ensuring robust policies are in place that support and encourage the reporting of poor care; and ensuring that registered practitioners are aware of their accountability for their actions and also their omissions should they witness poor care. In addition to reducing the risk of poor care, these steps could contribute to having a more confident, competent and empowered workforce

    Objectif: Déterminer la prévalence de la dysfonction de la valve aortique, de la dilatation de l'aorte proximale et des interventions au niveau de la valve aortique et de l'aorte ascendante chez les adultes avec une coarctation de l'aorte. Contexte: La dysfonction de la valve aortique et la dilatation de l'aorte proximale sont rares chez les enfants et les adolescents avec une coarctation de l'aorte. A long terme, les adultes pourraient être plus à risque de développer ce type de pathologie. Méthode: Nous avons rétrospectivement passé en revue tous les adultes avec une coarctation de l'aorte corrigée ou pas suivis au « Boston Children's Hospital » entre 2004 et 2010. Résultats: 216 adultes (56 % d'hommes) avec une coarctation ont été identifiés. L'âge médian à la dernière évaluation était de 28 (de 18 à 75) ans. Une bicuspidie aortique était présente dans 66% des cas. Au dernier contrôle, 3% avaient une sténose aortique modérée ou sévère et 4% avaient une insuffisance aortique modérée à sévère. Une dilatation de la racine de l'aorte ou de l'aorte ascendante était présente dans 28%, respectivement 42% des patients. Une dilatation au moins modérée de la racine de l'aorte ou de l'aorte ascendante (score Z > 4) était présente dans 8%, respectivement 14%. Les patients avec une bicuspidie aortique étaient plus sujets à avoir une dilatation au moins modérée de la racine de l'aorte ou de l'aorte ascendante comparés à ceux sans bicuspidie (20% contre 0%; p<0.001). L'âge était associé à une dilatation de l'aorte ascendante (p=0.04). Au dernier suivi, 6% avait nécessité une intervention au niveau de la valve aortique et 3% un remplacement de la racine de l'aorte ou de l'aorte ascendante

    The 1995 Attempted Derailing of the French TGV (High-Speed Train) and a Quantitative Analysis of 181 Rail Sabotage Attempts, MTI Report 09-12

    On August 26, 1995, the Saturday of the final and busiest weekend of France’s summer holiday season, terrorists attempted to derail the TGV (Train à Grande Vitesse) between Lyon and Paris by planting a bomb. Fortunately, their crude triggering mechanism failed to detonate the bomb, and subsequent analysis indicates that even had the bomb gone off, the explosion would not have derailed the train. The TGV episode, one of a continuing series of case studies by the Mineta Transportation Institute, points to a continuing problem: Since 1995, terrorists have attempted to derail trains on at least 144 occasions. Because of the expansion of high-speed rail systems in Europe, Asia, and North America, where 15 high-speed rail projects are in preparation or under way in the United States alone, this case study has been expanded to include a chronology and statistical analysis of attempted derailments worldwide. This analysis examines the geographic distribution of the attempts, the methods used by the saboteurs, and the outcomes. Although based on a small universe of events, it underscores both the attractiveness to terrorists of attacking transportation systems—a successful attack can result in high body counts, significant disruption, dramatic images, and enormous publicity, all things sought by terrorists—and the difficulties of achieving success

    Supplementing ESL Classroom Activities With Social Media For Young Adult Learners

    The classroom material currently utilized in English as a second language (ESL) classrooms is often disconnected from the real world. Publishers of ESL instructional materials often attempt to bridge this gap by including topics on popular news, films, songs, and famous persons at the time of printing. However, these are often outdated as quickly as they can be printed. As such, many ESL teachers face the challenge of keeping ESL classroom material relevant to the real world while seeking to maintain students’ interests in what they are learning. The purpose of the project is to make use of social networking sites, a readily available technology that students are already familiar with, and adapt classroom material to them as a way to increase students\u27 motivation and learner autonomy. The project is intended to serve as a guide for teachers seeking to enhance their lessons and engage their students through the means of authentic and familiar mediums.The project focuses on four popular social networking sites, Facebook, Instagram, Snapchat, and Twitter, to demonstrate how social networking sites can be incorporated into the classroom. For each type of social networking site, five classroom activities are designed, targeting the key language areas of grammar, vocabulary, listening, speaking, reading, and writing. Each language area has two types of activities, an introduction activity and a review activity. The introduction activity familiarizes students with how the social networking site will be used to complete the target language activity. The review activity can take the form of a game or is an expansion of the introduction activity. There is a growing trend in education towards the use of technology in the classroom. This project highlights one way in which this can be done while using the same curriculum and minimizing the need to introduce students to any new technology. In this manner, students can become more motivated and enthusiastic about their lessons, have the tools to take charge of their own learning, are exposed to more authentic texts and situations, and are provided with more learning opportunities in which to expand their knowledge of English
