    Meeting UK heat demands in zero emission renewable energy systems using storage and interconnectors

    Providing heat without emissions is a critical challenge to reach the 2050 UK net-zero target. Here, we simulate high renewable zero-emission energy system architectures with heat supply based on the major options of district heating, heat pumps, and electrolytic hydrogen boilers. We adopt a novel whole system modelling approach that combines meteorology-driven hourly simulations of demand and supply with storage, flexible technologies, and interconnections on the European scale. Our results show that systems with heat supply based on consumer or district heat pumps require about four times less electricity per unit of heat, with a heat cost about half of that from electrolytic hydrogen boilers. Furthermore, we compare trade-offs between investment in different infrastructure components. For example, we find that, compared to the reference scenario, increasing renewable capacity by 33%, or interconnections by 200%, can lower system storage capacity by up to 50%

    Evolving classification of intensive care patients from event data

    Objective: This work aims at predicting the patient discharge outcome on each hospitalization day by introducing a new paradigm—evolving classification of event data streams. Most classification algorithms implicitly assume the values of all predictive features to be available at the time of making the prediction. This assumption does not necessarily hold in the evolving classification setting (such as intensive care patient monitoring), where we may be interested in classifying the monitored entities as early as possible, based on the attributes initially available to the classifier, and then keep refining our classification model at each time step (e.g., on daily basis) with the arrival of additional attributes. / Materials and methods: An oblivious read-once decision-tree algorithm, called information network (IN), is extended to deal with evolving classification. The new algorithm, named incremental information network (IIN), restricts the order of selected features by the temporal order of feature arrival. The IIN algorithm is compared to six other evolving classification approaches on an 8-year dataset of adult patients admitted to two Intensive Care Units (ICUs) in the United Kingdom. / Results: Retrospective study of 3452 episodes of adult patients (≥ 16 years of age) admitted to the ICUs of Guy’s and St. Thomas’ hospitals in London between 2002 and 2009. Random partition (66:34) into a development (training) set n = 2287 and validation set n = 1165. Episode-related time steps: Day 0—time of ICU admission, Day x—end of the x-th day at ICU. The most accurate decision-tree models, based on the area under curve (AUC): Day 0: IN (AUC = 0.652), Day 1: IIN (AUC = 0.660), Day 2: J48 decision-tree algorithm (AUC = 0.678), Days 3–7: regenerative IN (AUC = 0.717–0.772). Logistic regression AUC: 0.582 (Day 0)—0.827 (Day 7). / Conclusions: Our experimental results have not identified a single optimal approach for evolving classification of ICU episodes. On Days 0 and 1, the IIN algorithm has produced the simplest and the most accurate models, which incorporate the temporal order of feature arrival. However, starting with Day 2, regenerative approaches have reached better performance in terms of predictive accuracy

    Sexual Dimorphism in Cellular and Molecular Features in Human ACTH-Secreting Pituitary Adenomas.

    Background. Cushing\u2019s disease presents gender disparities in prevalence and clinical course. Little is known, however, about sexual dimorphism at the level of the corticotrope adenoma itself. The aim of the present study was to evaluate molecular features of ACTH-secreting pituitary adenomas collected from female and male patients with Cushing\u2019s disease. (2) Methods. We analyzed 153 ACTH-secreting adenomas collected from 31 men and 122 women. Adenomas were established in culture and ACTH synthesis and secretion assessed in basal conditions as well as during incubation with CRH or dexamethasone. Concurrently, microarray analysis was performed on formalin-fixed specimens and differences in the expression profiles between specimens from male and female patients identified. (3) Results. ACTH medium concentrations in adenomas obtained from male patients were significantly lower than those observed in adenomas from female patients. This could be observed for baseline as well as modulated secretion. Analysis of corticotrope transcriptomes revealed considerable similarities with few, selected differences in functional annotations. Differentially expressed genes comprised genes with known sexual dimorphism, genes involved in tumour development and genes relevant to pituitary pathophysiology. (4) Conclusions. Our study shows for the first time that human corticotrope adenomas present sexual dimorphism and underlines the need for a gender-dependent analysis of these tumours. Differentially expressed genes may represent the basis for gender-tailored target therapy

    Envisioning happy places for all : a systematic review of the impact of transformations in the urban environment on the wellbeing of vulnerable groups

    Urban planning and design can impact mental health, but it is unclear how ever-growing and changing cities can sustain the psychological wellbeing of vulnerable groups, who are among the most mentally sensitive to spatial inequalities. This systematic review synthesised quantitative and qualitative studies on urban design interventions and their impact on wellbeing in vulnerable groups. Using the Preferred Reporting Items for Systematic Reviews and Meta-analyses (PRISMA) guidelines, we searched five online databases from inception to May 2020. A total of 10 papers were included. We found mixed evidence of benefits for wellbeing linked to urban regeneration projects or focused interventions (green spaces, transport, security). Interventions that were centred around participation, sustainable living, and quality of design (e.g., perceived sense of safety) were associated with increased residential satisfaction and wellbeing, particularly among low-income communities and women. Risk of bias was low to medium, but there was high methodological heterogeneity; studies were mainly from Western countries, and none of the included studies investigated the experiences of people with disabilities, migrants, or racial minorities. This review highlights the importance of inclusive and sustainable design interventions to create happy places for all strata of society, although further investigation is warranted

    An incidental pancreatic finding at 18F-choline PET/CT: Chronic mass-forming pancreatitis

    We present a case of a chronic mass-forming pancreatitis (CMFP) detected by 18F-choline (FCH) PET/CT in a male affected by prostate cancer. FCH PET/CT scan showed a focal uptake in the uncinate process of the pancreas, later diagnosed as a CMFP at biopsy. Although the physiological distribution of FCH in the pancreas, a careful interpretation of the images in this area is warranted

    The impact of social and weather drivers on the historical electricity demand in Europe

    Climate change, technological innovation, as well as electrification of energy services to meet carbon targets, have a significant impact on electricity demand magnitude and patterns. Increasing generation from renewable energy sources is already changing supply variability at the hourly and seasonal timescales. Our aim is to conduct a rigorous study of European historical demand, to understand its relationship with social and weather drivers and, therefore, to gain insights into appropriate storage needs. Daily activity profiles exhibit notable differences across European countries, with some of them reporting a consistent demand reduction, up to 25%, during school closures and Christmas festivities. In addition, temperature sensitivity differentiates demand by countries’ latitude (north vs. south), and by technologies (electric heating vs. other fuel based heating). Assuming a 100% renewables scenario, European countries would display quite distinct periodicities and amplitudes of their residual electricity demands. Annual load curves and temperature sensitivities of nations with high electric heating or cooling demand can assist in the prediction of future electricity and other fuel consumption under increased electrification and climate change scenarios. Fourier periodicity and residual demand analysis suggest that, in addition to grid storage, European countries with mutual energy needs – in terms of seasonal demand and generation surplus – might benefit from international trade to balance unmet demand. Our study of consumption variability in response to social and weather drivers constitutes a valuable resource to formulate country-specific demand scenarios, as well as to improve the design of energy system models

    Riassunto La Malattia di Parkinson è la seconda malattia neurodegenerativa più diffusa al mondo dopo l’Alzheimer. Le persone affette sono ad alto rischio di caduta, con conseguenti disabilità fisiche e psicologiche, con aumento della paura di cadere e di uscire di casa, e riduzione dell’autonomia individuale. I disturbi dell’equilibrio rispondono in maniera incompleta al trattamento farmacologico, mentre si fanno sempre più numerose le evidenze scientifiche che dimostrano come la pratica del Taiji-Quan quale “attività fisica adattata” sia efficace nel migliorare l’equilibrio e dunque la qualità di vita delle persone con Malattia di Parkinson. Il presente lavoro vuole proporre un Programma di Allenamento Adattato di Taiji-Quan, ben definito, ripetibile, completo di significato rieducativo specifico per ogni singolo esercizio, da associare al Programma Farmacologico nel Progetto Riabilitativo Individuale delle persone con Malattia di Parkinson. Tale protocollo è attuabile anche in modalità telematica come tele-riabilitazione
