Facteurs associés à la mauvaise qualité de la Gestion Active de la Troisième Phase de l’Accouchement dans les maternités du district de Dassa Zoumè- Glazoué au Bénin
L’objectif était d’étudier les facteurs associés à la mise en oeuvre de la Gestion Active de la Troisième Phase de l’Accouchement (GATPA) dans le district sanitaire de DAGLA au Bénin. Une étude analytique a été menée en juin 2011 portant sur les parturientes ayant accouché par voie basse dans sept maternités pratiquant la GATPA, les prestataires formés et le personnel de soutien. L’association entre la mauvaise qualité de cette intervention et les facteurs associés a été étudiée en utilisant le test de Khi carré de Pearson et la régression logistique pas à pas avec un seuil significatif fixé à 0.05 à l’aide de Epi Info 3.5.1. La mauvaise qualité de la GATPA a été de 63,2%. Les facteurs associés étaient : la qualification des prestataires (p = 0.002), la supervision (p = 0.034), les relations interpersonnelles (p < 0.001), la satisfaction des parturientes (p = 0.042). Dans le modèle final, la qualification (p = 0.019) et les relations interpersonnelles (p < 0.001) ont été les facteurs associés à la mauvaise qualité de la GATPA. Quelques dysfonctionnements ont été observés au niveau de l’organisation des services. Le renforcement des compétences techniques, la supervision régulière des prestataires et les relations soignants-soignés pourront contribuer à l’amélioration de la qualité de la GATPA dans le district sanitaire de DAGLA.Mots clés: Accouchement, gestion active, troisième phase, qualité
Prédiction de l’insulino-résistance par les ratios de lipoprotéines chez les adultes béninois
Des controverses subsistent sur force prédictive de l’insulino-résistance par les ratios de lipoprotéines chez les Afro-américains. L’objectif est de déterminer la force prédictive de l’insulino-résistance (IR) par les ratios triglycérides-par-HDL-cholestérol (TG/HDL-C) et cholestérol total-par-HDL-cholestérol (CT/HDL-C) chez des adultes béninois. L’étude est de type transversal, issue d’une enquête longitudinale sur le risque cardiométabolique et a inclut 416 sujets âgés de 29 à 69 ans. Les mesures anthropométriques et les dosages de la glycémie à jeun, de l’insulinémie, du CT, du HDL-C et des TG sont réalisés. L’IR est définie par le 75ème percentile des valeurs de « l’indice d’évaluation de modèle d'homéostasie pour l’insulino-résistance» (HOMA IR). Les valeurs prédictives de l’IR par les ratios TG/HDL-C et CT/HDL-C sont déterminées par les aires sous les courbes (AUC) de la « fonction d’efficacité du récepteur» (ROC). Les concentrations plasmatiques moyennes des lipides ont été en général plus élevées chez les IR que chez les non IR sauf pour les triglycérides : 0,9±0,5 mmol/L versus 0,8±0,4 mmol/L (p=0,138) chez les femmes, le cholestérol total : 4,7±1,7 mmol/L versus 4,3±1,2 mmol/L (p=0,161) et le HDL-Cholestérol : 1,2±0,5 mmol/L versus 1,34±0,5 mmol/L (p=0,105) chez les hommes. Les AUC ROC de prédiction de l’IR par le TG/HDL-C sont inférieures à  0,70 chez les femmes et les hommes, les obèses et les non obèses. L’AUC ROC de prédiction de l’IR par le CT/HDL-C est supérieure à 0,70 seulement chez les femmes : 0,71 (0,64-0,78). Le ratio TG/HDL-C n’est pas un prédicteur de l’IR dans la population d’étude alors que le ratio CT/HDL-C prédit l’IR chez les femmes seulement. Ces résultats contribueront au dépistage précoce de l’IR chez les femmes aux fins de la mise en place de mesures préventives.Mots clés : Prédiction, insulino-résistance, lipoprotéines, dyslipidémies, Bénin
A Formalization of The Natural Gradient Method for General Similarity Measures
In optimization, the natural gradient method is well-known for likelihood
maximization. The method uses the Kullback-Leibler divergence, corresponding
infinitesimally to the Fisher-Rao metric, which is pulled back to the parameter
space of a family of probability distributions. This way, gradients with
respect to the parameters respect the Fisher-Rao geometry of the space of
distributions, which might differ vastly from the standard Euclidean geometry
of the parameter space, often leading to faster convergence. However, when
minimizing an arbitrary similarity measure between distributions, it is
generally unclear which metric to use. We provide a general framework that,
given a similarity measure, derives a metric for the natural gradient. We then
discuss connections between the natural gradient method and multiple other
optimization techniques in the literature. Finally, we provide computations of
the formal natural gradient to show overlap with well-known cases and to
compute natural gradients in novel frameworks
Étude chimique de l’extrait aqueux d’Allium subvillosum (L.) (Alliaceae) et l’évaluation de sa toxicité chez les souris
Allium subvillosum (L.) fait partie de la pharmacopée marocaine, dont les bulbilles sont couramment utilisées comme dépuratifs et antidotes. Nous avons mené une étude chimique et biologique de l’extrait aqueux de la partie souterraine, constituée par des petites bulbilles à odeur d’ails, dont le but est de valoriser cette plante, aussi bien par la caractérisation chimique que l’étude toxicologique, et pour fixer ainsi une marge de sécurité de son emploi par la population locale. Les résultats chimiques obtenus ont révélé la présence des flavonoïdes, des alcaloïdes, des stérols, des triterpènes, des tanins, des composés réducteurs, des hétérosides cardiotonique et des saponosides. Et l’étude de la toxicité aiguë, réalisée chez des souris, a montré que l’extrait aqueux administré par voie intrapéritonéale est moyennement toxique avec une DL50 = 562.34± 342.64 mg/kg du poids corporel (PC). Alors que la même substance est faiblement toxique avec une DL50 = 3235.93 ± 878.34 mg/kg du PC, lorsqu’elle est administrée par voie orale.Mots clés: Pharmacopée marocaine, toxicité aiguë, voie intrapéritonéale, voie orale, phytochimie
Fast Optimal Transport Averaging of Neuroimaging Data
Knowing how the Human brain is anatomically and functionally organized at the
level of a group of healthy individuals or patients is the primary goal of
neuroimaging research. Yet computing an average of brain imaging data defined
over a voxel grid or a triangulation remains a challenge. Data are large, the
geometry of the brain is complex and the between subjects variability leads to
spatially or temporally non-overlapping effects of interest. To address the
problem of variability, data are commonly smoothed before group linear
averaging. In this work we build on ideas originally introduced by Kantorovich
to propose a new algorithm that can average efficiently non-normalized data
defined over arbitrary discrete domains using transportation metrics. We show
how Kantorovich means can be linked to Wasserstein barycenters in order to take
advantage of an entropic smoothing approach. It leads to a smooth convex
optimization problem and an algorithm with strong convergence guarantees. We
illustrate the versatility of this tool and its empirical behavior on
functional neuroimaging data, functional MRI and magnetoencephalography (MEG)
source estimates, defined on voxel grids and triangulations of the folded

Niveau de contamination des produits maraîchers par les substances toxiques sur le site de Houéyiho au Bénin
Il est admis que l’utilisation inappropriée des intrants agricoles et des pesticides a des effets sur la qualité des produits maraîchers. C’est pour étudier le niveau de contamination des produits maraîchers sur le site de Houéyiho que la présente étude a été menée. Il s’agit d’une étude transversale descriptive et analytique qui a opté pour une approche éco systémique basée sur la transdisciplinarité, la participation des producteurs et le genre/éthique. Des prélèvements de produits maraîchers (Daucus carota, Vernonia amygdalina, Lactuca sativa), d’eau et de sol ont été effectués à différents niveaux du site. Les résultats de cette étude révèlent que leniveau de contamination des produits maraîchers est élevé en zinc (198,84 mg/kg), en plomb (17,99 mg/kg), en fer (1042,33 mg/kg) et en cuivre (11,02 mg/kg) par rapport aux normes exigées par CODEX STAN 193- 1995 révisé en 2007 et qui sont respectivement de 5 mg/kg, 0,3 mg/kg, 2 mg/kg, 3 mg/kg. Des efforts restent à fournir pour réduire davantage ces concentrations à travers des sensibilisations, à l’utilisation effective du compost fabriqué sur le site pour minimiser les risques encourus par la population. Uniqueness of the compactly supported weak solutions of the relativistic Vlasov-Darwin system
We use optimal transportation techniques to show uniqueness of the compactly
supported weak solutions of the relativistic Vlasov-Darwin system. Our proof
extends the method used by Loeper in J. Math. Pures Appl. 86, 68-79 (2006) to

Calculus and heat flow in metric measure spaces and applications to spaces with Ricci bounds from below
This paper is devoted to a deeper understanding of the heat flow and to the
refinement of calculus tools on metric measure spaces (X,d,m). Our main results
are:
- A general study of the relations between the Hopf-Lax semigroup and
Hamilton-Jacobi equation in metric spaces (X,d).
- The equivalence of the heat flow in L^2(X,m) generated by a suitable
Dirichlet energy and the Wasserstein gradient flow of the relative entropy
functional in the space of probability measures P(X).
- The proof of density in energy of Lipschitz functions in the Sobolev space
W^{1,2}(X,d,m).
- A fine and very general analysis of the differentiability properties of a
large class of Kantorovich potentials, in connection with the optimal transport
problem.
Our results apply in particular to spaces satisfying Ricci curvature bounds
in the sense of Lott & Villani [30] and Sturm [39,40], and require neither the

Evaluation of the Performance of Expanded Immunization Programme Supply Chain and Logistics Management in Southern Benin Rural Health District
Abstract The objective was to evaluate the performance of the expanded immunization programme's (EPI) supply chain of and logistics management in Comé health district in 2015. This cross-sectional and evaluative study concerned the central deposit of vaccines and 19 health care centers randomly selected. Data on the performance of EIP supply chain of and logistics management were collected from 5 to 30 March 2015 using direct observation, document exploitation, questionnaire and individual interview in 20 health workers directly involved in EIP activities and 59 mothers of children aged 0-11 months. The performance of the supply chain and logistics management was assessed through three components "structure", "process" and "results" using predetermined score of two scales seeking for achievement of criterions included in components. The overall performance of the EIP's supply chain and logistics in the health district of Comé was rated fair (score = 75.2%). The performance level was good for "structure" (score = 90.3%), acceptable for the "process" (score = 79, 85%) and poor for the "results" of the management of supply chain and logistics (score = 59.48%). The level of performance EIP supply chain and logistics was sub-optimal. Adequate measures should be considered to improve the component "result" of the EIP supply chain and logistics management in the health district of Comé
Determinants of Adherence to Recommendations of the Dietary Approach to Stop Hypertension in Adults with Hypertension Treated in a Hospital in Benin
Abstract The dietary approach to stop hypertension (DASH) is an effective nutritional strategy to prevent and treat cardiovascular disease. Optimal benefit from dietary recommendations in management of hypertension depends on the compliance. This analytic cross sectional study aimed at establishing determinants of DASH among adults with hypertension treated at hospital in Benin. The study included 150 hypertensive adults selected during medical visit for blood pressure monitoring at hospital Saint-Luc in Cotonou from June 3 rd to July 1 st , 2014. Data on consumption of sodium, fruits and vegetables, alcohol, saturated and trans fat rich products were collected by questionnaire. A score of adherence to DASH was built. Determinants of adherence to DASH were identified using logistic regression model. Only 20% of subjects showed adherence to DASH. Better knowledge on hypertension OR=5.18 (95%IC 1.98-13.22) and healthy dietary habits and lifestyle prior to diagnosis of hypertension OR=4.26 (95%IC 1.67-13.18) increased the likelihood of adherence to dietary recommendations for hypertension management. Nutrition education and information of patients on hypertension and its complications during medical consultations may increase their adherence to dietary recommendations for management of the disease
