    Effect of Growth Hormone (hGH) Replacement Therapy on Physical Work Capacity and Cardiac and Pulmonary Function in Patients with hGH Deficiency Acquired in Adulthood.

    The effects of 6 months of replacement therapy with recombinant human GH (hGH) on physical work capacity and cardiac structure and function were investigated in 20 patients with hGH deficiency of adult onset in a double blind, placebo-controlled trial. The GH dose of 12.5 micrograms/kg BW was self-administered daily sc. Oxygen consumption (VO2), CO2 production, and ventilatory volumes were measured during exercise on a bicycle spiroergometer. M-Mode echocardiography was performed using standard techniques. The VO2 max data, expressed per kg BW (mL/min.kg BW) showed a significant increase from 23.2 +/- 2.4 to 30.0 +/- 2.3 (P < 0.01) in the hGH-treated group, whereas the VO2 max data, expressed per lean body mass (milliliters per min/kg lean body mass) did not change significantly in either group. Maximal O2 pulse (milliliters per beat) increased significantly from 15.2 +/- 5.6 to 19.6 +/- 3.3 mL/beat (P < 0.01), but remained constant in the placebo group. The maximal power output (watts +/- SE) increased significantly (P < 0.01) from 192.5 +/- 13.5 to 227.5 +/- 11.5 in the hGH-treated group, but remained constant in the placebo group. Cardiac structure (left ventricular posterior wall, interventricular septum thickness, left ventricular mass, left ventricular end-systolic dimension, and left ventricular end-diastolic dimension) as well as echocardiographically assessed cardiac function did not change significantly after 6 months of treatment in either group. We conclude that hGH replacement in hGH-deficient adults improves oxygen uptake and exercise capacity. These improvements in pulmonary parameters might be due to an increase in respiratory muscle strength and partly to the changes in muscle volume per se observed during hGH replacement therapy. Furthermore, an increased cardiac output might contribute to the improvement in exercise performance during hGH treatment. According to our data, hGH replacement therapy leads to an improvement of exercise capacity and maximal oxygen uptake, but has no significant effect on cardiac structure

    Does an Analysis of the Pulsatile Secretion Pattern of Adrenocorticotropin and Cortisol Predict the Result of Transsphenoidal Surgery in Cushing’s Disease.

    The endocrinological, surgical, and histological findings of patients with ACTH-dependent Cushing's disease were correlated with the pulsatile secretion pattern of ACTH and cortisol and the outcome after transsphenoidal pituitary surgery. A total of 28 patients were studied. The preoperative pulsatile secretion of ACTH and cortisol was assessed by sampling blood at 20-min intervals over 24 h. The pulsatile pattern of secretion was analyzed by the Cluster program. In 21 patients, an ACTH-secreting pituitary adenoma was identified and resected. Of these patients, 18 underwent clinical remission, and their cortisol secretion was suppressed to a normal level by low dose dexamethasone. Histological examinations in the patients with persistent disease revealed normal pituitary in 6 cases, nodular hyperplasia in 1, and ACTH-secreting pituitary adenoma in 3 cases. Analysis of the pulsatile pattern of ACTH and cortisol secretion did not reveal significant differences in timing, frequency, and/or amplitude of ACTH and cortisol pulses in normalized patients and those with persistent disease after surgery. It is concluded that analysis of the secretory pattern is not a suitable method for predicting the outcome of ranssphenoidal surgery in patients with ACTH-dependent Cushing's disease

    Local Equation of State and Velocity Distributions of a Driven Granular Gas

    We present event-driven simulations of a granular gas of inelastic hard disks with incomplete normal restitution in two dimensions between vibrating walls (without gravity). We measure hydrodynamic quantities such as the stress tensor, density and temperature profiles, as well as velocity distributions. Relating the local pressure to the local temperature and local density, we construct a local constitutive equation. For strong inelasticities the local constitutive relation depends on global system parameters, like the volume fraction and the aspect ratio. For moderate inelasticities the constitutive relation is approximately independent of the system parameters and can hence be regarded as a local equation of state, even though the system is highly inhomogeneous with heterogeneous temperature and density profiles arising as a consequence of the energy injection. Concerning the local velocity distributions we find that they do not scale with the square root of the local granular temperature. Moreover the high-velocity tails are different for the distribution of the x- and the y-component of the velocity, and even depend on the position in the sample, the global volume fraction, and the coefficient of restitution.Comment: 14 pages, 14 figures of which Figs. 13a-f and Fig. 14 are archived as separate .gif files due to upload-size limitations.     Understanding image-text relations and news values for multimodal news analysis

    The analysis of news dissemination is of utmost importance since the credibility of information and the identification of disinformation and misinformation affect society as a whole. Given the large amounts of news data published daily on the Web, the empirical analysis of news with regard to research questions and the detection of problematic news content on the Web require computational methods that work at scale. Today's online news are typically disseminated in a multimodal form, including various presentation modalities such as text, image, audio, and video. Recent developments in multimodal machine learning now make it possible to capture basic “descriptive” relations between modalities–such as correspondences between words and phrases, on the one hand, and corresponding visual depictions of the verbally expressed information on the other. Although such advances have enabled tremendous progress in tasks like image captioning, text-to-image generation and visual question answering, in domains such as news dissemination, there is a need to go further. In this paper, we introduce a novel framework for the computational analysis of multimodal news. We motivate a set of more complex image-text relations as well as multimodal news values based on real examples of news reports and consider their realization by computational approaches. To this end, we provide (a) an overview of existing literature from semiotics where detailed proposals have been made for taxonomies covering diverse image-text relations generalisable to any domain; (b) an overview of computational work that derives models of image-text relations from data; and (c) an overview of a particular class of news-centric attributes developed in journalism studies called news values. The result is a novel framework for multimodal news analysis that closes existing gaps in previous work while maintaining and combining the strengths of those accounts. We assess and discuss the elements of the framework with real-world examples and use cases, setting out research directions at the intersection of multimodal learning, multimodal analytics and computational social sciences that can benefit from our approach

    Review of the flood risk management system in Germany after the major flood in 2013

    Widespread flooding in June 2013 caused damage costs of €6 to 8 billion in Germany, and awoke many memories of the floods in August 2002, which resulted in total damage of €11.6 billion and hence was the most expensive natural hazard event in Germany up to now. The event of 2002 does, however, also mark a reorientation toward an integrated flood risk management system in Germany. Therefore, the flood of 2013 offered the opportunity to review how the measures that politics, administration, and civil society have implemented since 2002 helped to cope with the flood and what still needs to be done to achieve effective and more integrated flood risk management. The review highlights considerable improvements on many levels, in particular (1) an increased consideration of flood hazards in spatial planning and urban development, (2) comprehensive property-level mitigation and preparedness measures, (3) more effective flood warnings and improved coordination of disaster response, and (4) a more targeted maintenance of flood defense systems. In 2013, this led to more effective flood management and to a reduction of damage. Nevertheless, important aspects remain unclear and need to be clarified. This particularly holds for balanced and coordinated strategies for reducing and overcoming the impacts of flooding in large catchments, cross-border and interdisciplinary cooperation, the role of the general public in the different phases of flood risk management, as well as a transparent risk transfer system. Recurring flood events reveal that flood risk management is a continuous task. Hence, risk drivers, such as climate change, land-use changes, economic developments, or demographic change and the resultant risks must be investigated at regular intervals, and risk reduction strategies and processes must be reassessed as well as adapted and implemented in a dialogue with all stakeholders
