    Home care providers to the rescue:a novel first-responder programme

    To describe the implementation of a novel first-responder programme in which home care providers equipped with automated external defibrillators (AEDs) were dispatched in parallel with existing emergency medical services in the event of a suspected out-of-hospital cardiac arrest (OHCA).We evaluated a one-year prospective study that trained home care providers in performing cardiopulmonary resuscitation (CPR) and using an AED in cases of suspected OHCA. Data were collected from cardiac arrest case files, case files from each provider dispatch and a survey among dispatched providers. The study was conducted in a rural district in Denmark.Home care providers were dispatched to 28 of the 60 OHCAs that occurred in the study period. In ten cases the providers arrived before the ambulance service and subsequently performed CPR. AED analysis was executed in three cases and shock was delivered in one case. For 26 of the 28 cases, the cardiac arrest occurred in a private home. Ninety-five per cent of the providers who had been dispatched to a cardiac arrest reported feeling prepared for managing the initial resuscitation, including use of AED.Home care providers are suited to act as first-responders in predominantly rural and residential districts. Future follow-up will allow further evaluation of home care provider arrivals and patient survival

    Økologisk mat i offentlig sektor

    I 2005 var 3,5 % av jordbruksarealene økologiske, og det var registrert 2 496 godkjente, økologiske produsenter i Norge. Andelen økologisk produksjon varierer mellom ulike landsdeler og produksjoner. Produksjonen av økologisk melk var på 25,6 mill. liter, eller om lag 1,7 % av totalproduksjonen av melk i 2005, mens for eksempel innveid mengde storfeslakt bare utgjorde 0,6 % av totalproduksjonen. Det er et klart vekstpotensial for økologisk produksjon i Norge, og for mange økologiske produkter kan det også være mulig å øke andelen som faktisk når markedet som økologisk. Storhusholdningsmarkedet i Norge kan deles inn ifølgende virksomheter: Institusjonsmarkedet som i hovedsak er offentlig, kantine og catering som er både privat og offentlig og hotell- og restaurantmarkedet som i hovedsak er privat. Storkjøkken på institusjoner serverer mat til pasienter, pleietrengende og andre, mens kantinene selger maten til sine kunder. Økologiske varer er noe dyrere enn konvensjonelle. Mens institusjonskjøkken må kompenseres for de økte kostnadene ved å øke andelen økologiske matvarer, kan kantiner velge å hente inn merkostnaden ved økologiske råvarer ved å ta en høyere pris. Hvor mye prisen kan øke ved omlegging til økologiske uten at etterspurt mengde reduseres, er ikke kjent. Hvilken mat som serveres vil ha noe å si for mulighetene til å substituere en eksisterende meny med en annen. Eksempelvis kan det tenkes at en kantine som i hovedsak serverer tørrmat lettere vil kunne oppnå et mål om en viss andel økologiske varer ved å bytte til relativt rimelige leveranser av økologiske bakervarer og økologisk ost og melk; mens et storkjøkken som i hovedsak serverer varmmat vil kunne ha vanskeligheter med å få relativt rimelige leveranser av økologiske poteter, kjøtt, fisk og grønnsaker.     Randomised controlled trial of exercise training during lactation on breast milk composition in breastfeeding people with overweight/obesity: a study protocol for the MILKSHAKE trial

    Breast milk from people with overweight/obesity may differ in composition compared with that from normal-weight people. Exercise training can modify breast milk composition in rodent models, with a beneficial impact demonstrated on the offspring’s metabolism, but whether these findings translate to humans is unclear. This trial aims to determine the effect of an exercise intervention on breast milk composition and whether an exercise-induced modification of breast milk impacts the infants’ growth and body composition. Effect of Exercise Training on Breastmilk Composition is a randomised, controlled trial with two parallel groups, one exercise group and one control group, with a 1:1 allocation. We will include a minimum of 62 exclusively breastfeeding participants, 6 weeks postpartum. The exercise intervention lasts 8 weeks and comprises 25 supervised endurance exercise sessions with moderate or high intensity. The primary outcome measure is the change in the relative concentration of the human milk oligosaccharide 3′sialyllactose in breast milk from baseline at 6 weeks postpartum to the end of the intervention period. Secondary outcomes include breast milk concentrations of other metabolites, cytokines, hormones and microRNA, maternal health outcomes, infant growth, infant gut microbiome and infant circulating microRNA. Maternal and infant outcomes will be measured before, during and after the intervention period, with a follow-up of the infants until they are 24 months old. Trial registration number NCT05488964

    This paper investigates how reliable affective responses can be obtained using objective biometric measures for media audience research. We use Galvanic Skin Response (GSR) to detect sixteen respondents’ arousal levels and as an objective measure to show how self-reporting disrupts the experience of respondents watching video content. The subjective experiences from nine subjects were captured by self-reporting via the widely used SAM pictogram scale every three minutes. We found that interruptions induced by the self-reporting events cause them to consistently exhibit arousal peaks. Our post test measures show a negative effect on the subjects’ overall experience of the video and their empathy and identification with the main characters compared to the remaining subjects. We observed a 30 second return period from abnormal arousal levels after the self-reporting interruptions
