    Dementia with Lewy bodies versus nonconvulsive status epilepticus in the diagnosis of a patient with cognitive dysfunction, complex visual hallucinations and periodic abnormal waves in EEG: a case report

    BACKGROUND: The diagnosis of dementia with Lewy bodies (DLB) is often challenging in elderly individuals, not only for its various clinical features, sometimes, but also for its rare changes of periodic synchronous discharges (PSD) in electroencephalogram ( EEG). So, we reported one case of DLB and gave a detailed analysis. CASE PRESENTATION: A Chinese patient (Female, 56 years old) presented with progressive cognitive decline and complex visual hallucinations. Several days after admission, she gradually showed focal myoclonic jerks. Mini-Mental State Examination (MMSE) score was 19/30, EEG revealed PSD, Cerebrospinal fluid and 14-3-3 brain protein was negative, Magnetic resonance imaging (MRI) showed diffuse atrophy. To differentiate the PSD derived from DLB or from late-onset Absence Status Epilepticus, we have given the treatment with intravenous valproate (1200 mg/24 h) and diazepam 20 mg under the EEG monitor, a clinical improvement was absent and PSD in EEG did not disappear. Two weeks later, the patient showed akinetic-rigid syndrome and PSD in EEG persisted for a long time. According to her history and therapy, a clinical diagnosis of DLB has been made, but no autopsy for confirmation, and in the following visit, she has a poor prognosis. CONCLUSION: PSD in EEG may occasionally be recorded in neurodegenerative disorders such as AD, DLB other than CJD or NCSE. Hence it should not dissuade clinicians from the diagnosis of DLB where the clinical and neuropsychological findings were consistent with suggested diagnostic criteria for DLB

    Fast and accurate X-ray fluorescence computed tomography imaging with the ordered-subsets expectation maximization algorithm.

    The ordered-subsets expectation maximization algorithm (OSEM) is introduced to X-ray fluorescence computed tomography (XFCT) and studied; here, simulations and experimental results are presented. The simulation results indicate that OSEM is more accurate than the filtered back-projection algorithm, and it can efficiently suppress the deterioration of image quality within a large range of angular sampling intervals. Experimental results of both an artificial phantom and cirrhotic liver show that with a satisfying image quality the angular sampling interval could be improved to save on the data-acquisition time when OSEM is employed. In addition, with an optimum number of subsets, the image reconstruction time of OSEM could be reduced to about half of the time required for one subset. Accordingly, it can be concluded that OSEM is a potential method for fast and accurate XFCT imaging

    Effect of drying processes on prenylflavonoid content and antioxidant activity of Epimedium koreanum Nakai

    Epimedium koreanum Nakai is a famous Chinese herbal medicine for the treatment of impotence, osteoporosis, immune suppression and cardiovascular diseases. Drying is the most common and fundamental procedure in post-harvest processing of E. koreanum, which contributes to the variations of flavonoid content, especially prenylflavonoids, the bioactive components. In present study, effect of drying processes on flavonoid content and antioxidant activity were investigated. High performance liquid chromatography coupled with diode-array detection and electrospray ionization quadrupole time-of-flight tandem mass spectrometry methods were employed. Twenty seven compounds were identified and 11 of them, including eight prenylflavonoids and three other types of flavonoids, were further quantified. The antioxidant activity of E. koreanum was evaluated by the 2,2-diphenyl-1-picrylhydrazyl free radical scavenging method. The results showed that content of the eight prenylflavonoids exhibited significant variations after different drying processes, especially icariin and baohuoside I. The variation tendency of antioxidant activity was positively correlated with the content of total flavonoid, afzelin and icariin.     S100A4/NF-κB axis mediates the anticancer effect of epigallocatechin-3-gallate in platinum-resistant ovarian cancer

    Summary: Resistance to cisplatin (cis-dichlorodiamineplatinum, DDP) in ovarian cancer is a significant clinical challenge. Epigallocatechin-3-gallate (EGCG) has shown promise in cancer therapy. However, its effects on DDP-resistant ovarian cancer remain understudied. This study aims to assess the impact of EGCG on DDP-resistant cells and elucidate the associated molecular mechanisms. DDP-resistant cell lines were utilized for biological characterization. EGCG effectively inhibited proliferation, mobility, and induced apoptosis in OC/DDP cells. It downregulated the expression of S100A4 and NF-κB while upregulating p53 expression. These effects were reversed upon overexpression of S100A4 or NF-κB. In vivo experiments confirmed tumor inhibition and KI67 inhibition by EGCG. Moreover, EGCG downregulated the expression of S100A4 and NF-κB while upregulating p53 in xenograft mice compared to those without EGCG treatment. This study suggests that EGCG suppresses cancer progression through the S100A4/NF-κB signaling pathway, involving interaction with p53. EGCG holds potential as an anticancer candidate for OC/DDP

    Exploration of cortical inhibition and habituation in insomnia: Based on CNV and EEG

    Objective: Cognitive dysfunction with abnormal cortical inhibition and habituation has frequently been found in patients with insomnia. And the so-called contingent negative variation (CNV) and EEG power spectral density (FFT) may be the best choice to explore the underlining pathophysiology.Methods: We used polysomnography (PSG) to record such objective PSG parameters. The amplitudes, latencies, areas of different CNV components such as oCNV, iCNV and tCNV, PINV have been selected and analyzed. Behavioral data such as manual reaction time (RT) has been analyzed. Spectral analysis was performed with fast Fourier transformation (FFT) on all channels to make a spectral analyses of EEG datas.Results: The A-latency located in CZ or PZ were statistically longer in insomnia group than control group, the iCNV-latency located in insomnia group were statistically shorter than control group. The iCNV-amplitude located in insomnia group was lower than control group. The oCNV-amplitude or the tCNV-amplitude located in insomnia group was higher than control group. The oCNV-square, tCNV-square, or PINV-square located in insomnia group were significant larger than control group. 131 or 132 activity distributed in bilateral hemisphere were significantly increased in insomnia group than control group with different distributions.Discussion: Our study revealed varied attentional and information processing in insomnia patients. Above all, we made a hypothesis with ceiling theory: Frontal lobe play an important role in maintaining cognitive processing, which needs much more energy consumption and leads to decreased fast EEG activity in frontal cortex, which contributes to reduced cortical inhibition, represented as abnormal CNV

    Compromised Dynamic Cerebral Autoregulation in Patients with Epilepsy

    Objective. The aim of this study is to analyze dynamic cerebral autoregulation (dCA) in patients with epilepsy. Methods. One hundred patients with epilepsy and 100 age- and sex-matched healthy controls were recruited. Noninvasive continuous cerebral blood flow velocity of the bilateral middle artery and arterial blood pressure were recorded. Transfer function analyses were used to analyze the autoregulatory parameters (phase difference and gain). Results. The overall phase difference of patients with epilepsy was significantly lower than that of the healthy control group (p=0.046). Furthermore, patients with interictal slow wave had significant lower phase difference than the slow-wave-free patients (p=0.012). There was no difference in overall phase between focal discharges and multifocal discharges in patients with epilepsy. Simultaneously, there was no difference in mean phase between the affected and unaffected hemispheres in patients with unilateral discharges. In particular, interictal slow wave was an independent factor that influenced phase difference in patients with epilepsy (p=0.016). Conclusions. Our study documented that dCA is impaired in patients with epilepsy, especially in those with interictal slow wave. The impairment of dCA occurs irrespective of the discharge location and type. Interictal slow wave is an independent factor to predict impaired dCA in patients with epilepsy. Clinical Trial Identifier. This trial is registered with NCT02775682
