    Autologous reconstructions are associated with greater overall medium-term care costs than implant-based reconstructions in the Finnish healthcare system : A retrospective interim case-control cohort study

    Purpose: Previous studies have mainly reported the short-term costs of different reconstruction techniques. Revision operations may increase costs in longer follow-up. Authors report medium-term data on different reconstruction methods. We hypothesised that the reconstruction method would affect not only the duration of reconstruction process but also total costs. Methods: The reconstruction database was reviewed from 2008 to 2019. Women with autologous (deep inferior epigastric perforator, transverse musculocutaneous gracilis and latissimus dorsi [LD] without implant) and implant-based (implant and LD with implant) reconstructions were included. Variables evaluated included age, body mass index, smoking, comorbidities, radiotherapy, complications and readmissions. Risk factors for multiple revision surgeries were analysed. Time to definitive reconstruction and related costs were also calculated. Results: In total, 591 patients with autologous reconstructions and 202 with implant-based reconstructions were included. The median follow-up time was 73 months. Definitive reconstruction was obtained in 443 days in implant-based reconstructions and in 403 days in autologous reconstructions (P = 0.050). Independent risk factors for multiple surgeries were younger age (P < 0.001) and comorbidity (P = 0.008). No statistically significant difference was observed in the rate of overall surgical procedures (P = 0.098), but implant-based reconstructions were more commonly associated with two or more planned operations (P = 0.008). Autologous reconstructions were associated with greater total cost (22052vs.22 052 vs. 18 329, P < 0.001). Conclusions: This review of reconstructions over a 12-year study period revealed that autologous reconstructions are associated with greater overall costs, but there is no statistically significant difference in reconstruction time or rate of surgical procedures. However, a full cost assessment between reconstructive techniques requires a much longer follow-up period.publishedVersionPeer reviewe

    Physical activity, physical fitness and cardiometabolic health among Finnish military workers

    Introduction: The Western lifestyle challenges national defence. Inactivity, obesity, high BP and elevated lipid and glucose levels as well as tobacco use all increase cardiometabolic risk. The present study was thus aimed at investigating the health and physical activity of employees in a military environment, concentrating on comparisons between soldiers and civilians. Methods and design: A total of 260 employees from 6 brigades were included in the present study. Health status was evaluated with body composition, cardiometabolic risk markers from laboratory samples and a questionnaire concerning lifestyle habits. Body composition was assessed by means of body mass, body mass index, fat percentage and waist circumference. Furthermore, physical activity was examined by the aid of accelerometer recordings for a 2-week period, and physical fitness via aerobic and muscle fitness tests. Finally, upper-quartile active and lower-quartile passive participants were compared, by incorporating mean daily step counts. Results: When standardised by gender, there were no differences between the soldiers and civilians except for the muscle fitness test, in which soldiers performed better. The mean (±SD) moderate to vigorous activity was 0.9±0.3 hours/day in male soldiers and 1.0±0.4 hours/day in male civilians, and respectively sedentary behaviour was 9.5±1.4 hours/day in male soldiers and 8.9±1.7 hours/day in male civilians. The mean (±SD) low-density lipoprotein values were 3.28±0.84 mmol/L in male soldiers and 3.36±0.86 mmol/L in male civilians. In comparing soldiers and civilians, statistically significant differences were observed in body composition, physical fitness, insulin, fasting glucose, triglycerides and high-density lipoprotein values between the upper-quartile active and lower-quartile passive participants, but no difference in low-density lipoprotein values was noticed. Conclusions: Sedentary behaviour and elevated low-density lipoprotein values seem to increase cardiometabolic disease risk among participants, even if they meet the weekly physical activity demands.Peer reviewe

    Day surgery in reduction mammaplasty – saving money or increasing complications?

    Background: The benefits of reduction mammoplasty procedures have been reported previously. However, to control the rise in public healthcare costs, we need to find ways of conducting these procedures safely and more cost-effectively. Our aim was to examine whether reduction mammaplasty performed in an outpatient setting has comparable surgical complication rates to those performed in an inpatient setting. We also investigated whether any savings gained from day surgery are still present after any possible indirect costs are considered. Methods: The study population comprised 276 patients who underwent reduction mammaplasty in a single center between January 2019 and February 2021. Data were collected from patient medical records. The costs associated with the primary procedure and any possible additional expenses were calculated. Basic statistical comparisons were performed for propensity score-matched data. Results: Complication rates, readmissions, number of contacts to the health care system, and need for additional surgical interventions were comparable between outpatients and inpatients. The basic costs for outpatients were 2990 euros per patient and 3923 euros for inpatients. Total costs after possible extra expenses were lower in day surgery as it was markedly more cost-effective than patients treated as inpatients. Conclusions: Reduction mammaplasties can be safely performed in an outpatient setting. Moreover, the emergence of complications is comparable to those performed in an inpatient setting. An outpatient setting produced significant cost savings not only in the immediate costs of primary surgery but also in the costs associated with possible complications and extra contacts to the healthcare system.publishedVersionPeer reviewe

    Factors associated with urinary and double incontinence in a geriatric post-hip fracture assessment in older women

    Background: Incontinence and hip fractures are common in older people, especially women, and associated with multiple adverse effects. Incontinence is a risk factor for falls. Aims: We aimed to investigate the prevalence of urinary (UI) and double incontinence (DI, concurrent UI and faecal incontinence), and to identify factors associated with UI and DI 6 months post-fracture. Methods: A prospective real-life cohort study was conducted consisting of 910 women aged >= 65 who were treated for their first hip fracture in Seinajoki Central Hospital, Finland, between May 2008 and April 2018. Continence status was elicited at baseline and 6 months postoperatively at our geriatric outpatient clinic where all participants underwent a multidisciplinary comprehensive geriatric assessment (CGA) consisting of an evaluation of cognition, nutrition, mood, mobility, and functional ability. Results: At baseline, 47% of the patients were continent, 45% had UI and 8% had DI, and at follow up, 38%, 52%, and 11%, respectively. The mean age of the patients was 82.7 +/- 6.8. Both UI and DI were associated with functional disability and other factors related to frailty. The associations were particularly prominent for patients with DI who also had the worst performance in the domains of CGA. We identified several modifiable risk factors: depressive mood (odds ratio [OR] 1.81; 95% confidence interval [CI] 1.16-2.84) and constipation (OR 1.48, 95% CI 1.02-2.13) associated with UI and, late removal of urinary catheter (OR 2.33, 95% CI 1.31-4.14), impaired mobility (OR 2.08, 95% CI 1.05-4.15), and poor nutrition (OR 2.31, 95% CI 1.11-4.79) associated with DI. ​​​​​​​Conclusions: This study demonstrates a high prevalence of UI and DI in older women with hip fracture and modifiable risk factors, which should be targeted in orthogeriatric management and secondary falls prevention. Patients with DI were found to be an especially vulnerable group.</p

    Urinary and double incontinence in older women with hip fracture - risk of death and predictors of incident symptoms among survivors in a 1-year prospective cohort study

    Objectives: To investigate the association of urinary incontinence (UI) and double incontinence (DI, concurrent UI and fecal incontinence) with one-year mortality among older female hip fracture patients and to identify predictors of incident UI and DI. Design: A prospective cohort study Setting and subjects: 1,468 female patients aged ≥ 65 treated for their first hip fracture during the period 2007–2019 Methods: Continence status was elicited at baseline and one-year post-fracture. Age- and multivariable-adjusted Cox proportional hazards and multinomial logistic regression models were used to determine the associations of incontinence with one-year mortality and to examine the associations of baseline predictors with incident UI and DI respectively. Results: Of the women with no incontinence, UI and DI, 78 (13%), 159 (23%) and 60 (34%), died during follow-up. UI (HR 1.72, 95% CI 1.31–2.26) and DI (HR 2.61, 95% CI 1.86–3.66) were associated with mortality after adjusting for age. These associations lost their predictive power in multivariable analysis while age over 90, living in an institution, impaired mobility, poor nutrition, polypharmacy, and late removal of urinary catheter remained associated with mortality. Of continent women, 128 (21%) developed UI and 23 (4%) DI during follow-up. In multivariable analysis, impaired mobility was associated with incident UI (OR 2.56, 95% CI 1.48–4.44) and DI (OR 4.82, 95% CI 1.70–13.7), as well as living in an institution (OR 3.44, 95% CI 1.56–7.61 and OR 3.90, 95% CI 1.17–13.0).     Human papillomavirus concordance between parents and their newborn offspring : Results from the Finnish Family HPV study

    BACKGROUND: The knowledge on vertical human papillomavirus (HPV) transmission is limited. We aimed to determine whether HPV transmission from parents to their offspring occur before or during birth. METHODS: Altogether, 321 mothers, 134 fathers and their 321 newborn offspring from the Finnish Family HPV study cohort were included. Parents' genital and oral brush samples and semen samples were collected for HPV testing at baseline (36 weeks of pregnancy). Oral, genital and umbilical samples from the newborn and placenta samples were collected for HPV testing immediately after delivery. HPV risk for the newborn was calculated from mother's and father's HPV status by using logistic regression analyses. RESULTS: Concordances between mothers' and their newborns' HPV genotype at any site were statistically significant with HPV6, HPV16, HPV18, HPV31 and HPV56; Odds Ratios (OR) ranged from OR 3.41 (95% CI: 1.80-6.48) for HPV16 to OR 634 (95% CI: 28.5-14087) for HPV31. Father-newborn HPV concordances were statistically significant with HPV6 and HPV31; OR 4.89 (95% CI: 1.09-21.9) and OR 65.0 (95% CI: 2.92-1448), respectively.ConclusionsThe genotype-specific HPV concordance between parents and their newborn is suggestive for vertical HPV transmission. However, transmission from the father to the newborn remains more uncertain.Peer reviewe

    <p>Abstract</p> <p>Background</p> <p>To show magnetic resonance imaging (MRI) texture appearance change in non-Hodgkin lymphoma (NHL) during treatment with response controlled by quantitative volume analysis.</p> <p>Methods</p> <p>A total of 19 patients having NHL with an evaluable lymphoma lesion were scanned at three imaging timepoints with 1.5T device during clinical treatment evaluation. Texture characteristics of images were analyzed and classified with MaZda application and statistical tests.</p> <p>Results</p> <p>NHL tissue MRI texture imaged before treatment and under chemotherapy was classified within several subgroups, showing best discrimination with 96% correct classification in non-linear discriminant analysis of T2-weighted images.</p> <p>Texture parameters of MRI data were successfully tested with statistical tests to assess the impact of the separability of the parameters in evaluating chemotherapy response in lymphoma tissue.</p> <p>Conclusion</p> <p>Texture characteristics of MRI data were classified successfully; this proved texture analysis to be potential quantitative means of representing lymphoma tissue changes during chemotherapy response monitoring.</p

    A New Approach to Measuring Work-Related Well-Being

    The main aim of this study was to develop a short questionnaire to assess work-related well-being from the organizational behaviour perspective. The short well-being questionnaire enables measuring longitudinal work-related well-being. Work-related well-being was assessed with a 147-item questionnaire covering both organizational and intrinsic factors of work-related well-being. The questionnaire consisted of 27 categories. The respondents were 114 women (65%) and 62 men (35%), mean age 39.2 years, in various occupations. From the extensive questionnaire a shorter questionnaire with 33 items was developed by principal component analysis. The Kaiser–Meyer–Olkin measure to test the sampling adequacy of 27 factor solutions varied from .62 to .91 and Cronbachʼs α was .74–.94. Most κ values of the shorter questionnaire were .50–.94 (p < .001). The reliability of the short version was comparable to that of the original questionnaire. The short one could also be suitable for Internet and mobile questionnaire applications
