    The COVID-19 pandemic has undoubtedly elevated the status of public health. Concurrently, public health ethics has been brought to the forefront of the field and its practices in several ways. Over the past year, SARS-CoV-2 has affected nearly every country. In both high and low resource settings, policymakers turned to public health to ease the tension placed on health care systems by COVID-19. Governments implemented measures ranging from lockdown orders to mask mandates, all with the primary goal of curbing the spread of SARS-CoV-2. The effectiveness of these interventions has become a salient topic in public health research, and much epidemiological data has been published to inform ongoing responses to the pandemic. However, little attention has been given to the broader consequences of these policies. More specifically, ethics has rarely been considered in the development and evaluation of COVID-19 policies. Without the proper inclusion of ethics and human rights in public health responses, equitable outcomes cannot be guaranteed. This thesis aims to apply an ethical framework to analyze two types of COVID-19 policies: the restriction of mass gatherings and school closures/reopening. These measures were analyzed using a public health ethics framework to assess their effectiveness and outcomes as well as to facilitate comparisons between China and Iran, two countries with vastly different political structures and experiences during the pandemic. The analysis revealed that, while these policies were effective to some degree, neither policy was ethically justified in either China or Iran due to the unequal distribution of benefits and burdens across populations which has induced ramifications that extend beyond the current pandemic. These results demonstrate that public health officials and political leaders have an obligation to serve all populations and aspects of health especially during a public health crisis; controlling a pandemic itself does not ensure full health for all. Ethics should play an essential role in public health to avoid past mistakes and guarantee the right to health around the world

    Identification of Cytotoxic Flavor Chemicals in Top-Selling Electronic Cigarette Refill Fluids.

    We identified the most popular electronic cigarette (EC) refill fluids using an Internet survey and local and online sales information, quantified their flavor chemicals, and evaluated cytotoxicities of the fluids and flavor chemicals. "Berries/Fruits/Citrus" was the most popular EC refill fluid flavor category. Twenty popular EC refill fluids were purchased from local shops, and the ingredient flavor chemicals were identified and quantified by gas chromatography-mass spectrometry. Total flavor chemical concentrations ranged from 0.6 to 27.9 mg/ml, and in 95% of the fluids, total flavor concentration was greater than nicotine concentration. The 20 most popular refill fluids contained 99 quantifiable flavor chemicals; each refill fluid contained 22 to 47 flavor chemicals, most being esters. Some chemicals were found frequently, and several were present in most products. At a 1% concentration, 80% of the refill fluids were cytotoxic in the MTT assay. Six pure standards of the flavor chemicals found at the highest concentrations in the two most cytotoxic refill fluids were effective in the MTT assay, and ethyl maltol, which was in over 50% of the products, was the most cytotoxic. These data show that the cytotoxicity of some popular refill fluids can be attributed to their high concentrations of flavor chemicals

    We characterized the flavor chemicals in a broad sample of commercially available electronic cigarette (EC) refill fluids that were purchased in four different countries. Flavor chemicals in 277 refill fluids were identified and quantified by gas chromatography-mass spectrometry, and two commonly used flavor chemicals were tested for cytotoxicity with the MTT assay using human lung fibroblasts and epithelial cells. About 85% of the refill fluids had total flavor concentrations >1 mg/ml, and 37% were >10 mg/ml (1% by weight). Of the 155 flavor chemicals identified in the 277 refill fluids, 50 were present at ≥1 mg/ml in at least one sample and 11 were ≥10 mg/ml in 54 of the refill fluids. Sixty-one% (170 out of 277) of the samples contained nicotine, and of these, 56% had a total flavor chemical/nicotine ratio >2. Four chemicals were present in 50% (menthol, triacetin, and cinnamaldehyde) to 80% (ethyl maltol) of the samples. Some products had concentrations of menthol ("Menthol Arctic") and ethyl maltol ("No. 64") that were 30 times (menthol) and 100 times (ethyl maltol) their cytotoxic concentration. One refill fluid contained cinnamaldehyde at ~34% (343 mg/ml), more than 100,000 times its cytotoxic level. High concentrations of some flavor chemicals in EC refill fluids are potentially harmful to users, and continued absence of any regulations regarding flavor chemicals in EC fluids will likely be detrimental to human health

    Prediction regions for interval-valued time series

    We approximate probabilistic forecasts for interval-valued time series by offering alternative approaches. After fitting a possibly non-Gaussian bivariate VAR model to the center/log-range system, we transform prediction regions (analytical and bootstrap) for this system into regions for center/range and upper/lower bounds systems. Monte Carlo simulations show that bootstrap methods are preferred according to several new metrics. For daily S&P500 low/high returns, we build joint conditional prediction regions of the return level and volatility.     West Hartford-Bloomfield Health District Community Needs Assessment: A Mixed Methods Study Identifying COVID-19 Knowledge, Perceptions, and Health Disparities

    Background: The West Hartford-Bloomfield Health District (WHBHD) is a regional health department serving the towns of West Hartford and Bloomfield in Connecticut. From February to April of 2020, elderly and Black residents accounted for 59% and 38% of COVID-19 related deaths, respectively. Since the district has not assessed community health needs previously and is interested in becoming accredited, a community needs assessment will serve as the first step in implementing effective and timely interventions to address needs, disparities, and misperceptions about the pandemic. Methods: A comprehensive survey was adapted from existing NIH COVID-19 research instruments. 779 responses were collected on the topics of community health and COVID-19 knowledge, practices and perceptions. Five semi-structured key informant interviews were conducted with faith-based organizations and local government leaders to corroborate findings from the survey. Quantitative analyses were performed via SAS and R, and qualitative data was summarized. Results: Demographic distributions significantly differed between West Hartford and Bloomfield in age, income, race, marital status and household size. While there were differences in community health problems, social/environmental problems, and health care barriers, both towns shared individual health problems and perceived certain populations as most underserved. Both towns have adequate COVID-19 knowledge regarding symptoms and transmissions, low perceived risk of infection, and positive attitude towards preventative actions and measures. There were significant differences in COVID-19 testing accessibility between towns (p = 0.002), with nearly double the proportion of West Hartford residents reporting difficulty accessing testing. COVID-19 vaccination status is also different between towns, with both reporting lack of available vaccines as the primary reason preventing or delaying vaccination. Conclusions: The mixed methods approach to conducting this community needs assessment has provided consequential preliminary findings that will be useful in refining the health district’s current and future responses to COVID-19 and other public health issues.https://elischolar.library.yale.edu/ysph_pbchrr/1055/thumbnail.jp

    Ethyl Maltol, Vanillin, Corylone and other Conventional Confectionery-related Flavour Chemicals Dominate in Some E-cigarette Liquids Labelled ‘tobacco’ flavoured

    Background The increased popularity of electronic cigarettes (e-cigarettes) has been linked to the abundance of flavoured products that are attractive to adolescents and young adults. In the last decade, e-cigarette designs have evolved through four generations that include modifications in battery power, e-cigarette liquid (e-liquid) reservoirs and atomiser units. E-liquids have likewise evolved in terms of solvent use/ratios, concentration and number of flavour chemicals, use of nicotine salts and acids, the recent increased use of synthetic cooling agents and the introduction of synthetic nicotine. Our current objective was to evaluate and compare the evolving composition of tobacco-flavoured e-liquids over the last 10 years. Methods Our extensive database of flavour chemicals in e-liquids was used to identify trends and changes in flavour chemical composition and concentrations. Results Tobacco-flavoured products purchased in 2010 and 2011 generally had very few flavour chemicals, and their concentrations were generally very low. In tobacco-flavoured refill fluids purchased in 2019 and Puff Bar Tobacco e-cigarettes, the total number and concentration of flavour chemicals were higher than expected. Products with total flavour chemicals \u3e10 mg/mL contained one to five dominant flavour chemicals (\u3e1 mg/mL). The most frequently used flavour chemicals in tobacco e-liquids were fruity and caramellic. Conclusions There is a need for continuous surveillance of e-liquids, which are evolving in often subtle and harmful ways. Chemical constituents of tobacco flavours should be monitored as they clearly can be doctored by manufacturers to have a taste that would appeal to young users

    DNA methylation at birth and lateral ventricular volume in childhood:a neuroimaging epigenetics study

    Background: Lateral ventricular volume (LVV) enlargement has been repeatedly linked to schizophrenia; yet, what biological factors shape LVV during early development remain unclear. DNA methylation (DNAm), an essential process for neurodevelopment that is altered in schizophrenia, is a key molecular system of interest. Methods:In this study, we conducted the first epigenome-wide association study of neonatal DNAm in cord blood with LVV in childhood (measured using T1-weighted brain scans at 10 years), based on data from a large population-based birth cohort, the Generation R Study (N = 840). Employing both probe-level and methylation profile score (MPS) approaches, we further examined whether epigenetic modifications identified at birth in cord blood are: (a) also observed cross-sectionally in childhood using peripheral blood DNAm at age of 10 years (Generation R, N = 370) and (b) prospectively associated with LVV measured in young adulthood in an all-male sample from the Avon Longitudinal Study of Parents and Children (ALSPAC, N = 114). Results: At birth, DNAm levels at four CpGs (annotated to potassium channel tetramerization domain containing 3, KCTD3; SHH signaling and ciliogenesis regulator, SDCCAG8; glutaredoxin, GLRX) prospectively associated with childhood LVV after genome-wide correction; these genes have been implicated in brain development and psychiatric traits including schizophrenia. An MPS capturing a broader epigenetic profile of LVV – but not individual top hits – showed significant cross-sectional associations with LVV in childhood in Generation R and prospectively associated with LVV in early adulthood within ALSPAC. Conclusions: This study finds suggestive evidence that DNAm at birth prospectively associates with LVV at different life stages, albeit with small effect sizes. The prediction of MPS on LVV in a childhood sample and an independent male adult sample further underscores the stability and reproducibility of DNAm as a potential marker for LVV. Future studies with larger samples and comparable time points across development are needed to further elucidate how DNAm associates with this clinically relevant brain structure and risk for neuropsychiatric disorders, and what factors explain the identified DNAm profile of LVV at birth.</p

    Manganese(I)-Catalyzed H-P Bond Activation via Metal-Ligand Cooperation

    Here we report that chiral Mn(I) complexes are capable of H-P bond activation. This activation mode enables a general method for the hydrophosphination of internal and terminal α,β-unsaturated nitriles. Metal-ligand cooperation, a strategy previously not considered for catalytic H-P bond activation, is at the base of the mechanistic action of the Mn(I)-based catalyst. Our computational studies support a stepwise mechanism for the hydrophosphination and provide insight into the origin of the enantioselectivity

    Interaction of Brn3a and HIPK2 mediates transcriptional repression of sensory neuron survival

    The Pit1-Oct1-Unc86 domain (POU domain) transcription factor Brn3a controls sensory neuron survival by regulating the expression of Trk receptors and members of the Bcl-2 family. Loss of Brn3a leads to a dramatic increase in apoptosis and severe loss of neurons in sensory ganglia. Although recent evidence suggests that Brn3a-mediated transcription can be modified by additional cofactors, the exact mechanisms are not known. Here, we report that homeodomain interacting protein kinase 2 (HIPK2) is a pro-apoptotic transcriptional cofactor that suppresses Brn3a-mediated gene expression. HIPK2 interacts with Brn3a, promotes Brn3a binding to DNA, but suppresses Brn3a-dependent transcription of brn3a, trkA, and bcl-xL. Overexpression of HIPK2 induces apoptosis in cultured sensory neurons. Conversely, targeted deletion of HIPK2 leads to increased expression of Brn3a, TrkA, and Bcl-xL, reduced apoptosis and increases in neuron numbers in the trigeminal ganglion. Together, these data indicate that HIPK2, through regulation of Brn3a-dependent gene expression, is a critical component in the transcriptional machinery that controls sensory neuron survival

    The longitudinal association between potential stressful life events and the risk of psychosocial problems in 3-year-old children

    BackgroundExperiencing certain potentially stressful life events can impact psychosocial well-being among school-aged children and adolescents. This study aims to evaluate the association between life events occurring before age 2 and risk of psychosocial problems at 3 years of age.MethodsAll parents invited for the regular well-child visit when their child was 2 years of age by the preventive Youth Health Care in the Rotterdam-Rijnmond area, the Netherlands, were invited to participate in this study. In total 2,305 parents completed the baseline questionnaire at child age 2-years; 1,540 parents completed the questionnaire at child age 3-years. The baseline questionnaire included a life events assessment (12 items), and tension caused by the event (range 0–3). At child age 3-years the questionnaire included the Strengths and Difficulties Questionnaire (SDQ) to assess risk of psychosocial problems. Logistic regression models were applied.ResultsIn the current study 48.5% of families experienced ≥1 life event before child age 2 years. Divorce and problems in the relationship between the parents received the highest perceived severity score [respectively 2.1 (SD = 0.8) and 2.0 (SD = 0.7)]. Children experiencing ≥1 event before the age of 2 years were at higher risk of psychosocial problems at 3 years of age, compared to children that had experienced no life event (1–2 events OR = 1.50, 95%CI: 1.09; 2.06, and &gt;2 events OR = 2.55, 95%CI 1.64; 4.00, respectively). When life events caused high perceived levels of tension, there was also an association with an increased risk of psychosocial problems at age 3-years (OR = 2.03, 95%CI 1.43; 2.88).ConclusionsApproximately half of children in our study experienced a potential stressful life event before the age of 2 years. Results suggest an association between experiencing a life event and risk of psychosocial problems at child age 3-years. These findings emphasize the need for child health care professionals to pay attention to life events taking place in the life of young children in order to provide appropriate support
