    Nucleotomía Percutánea Automatizada: nuestra Experiencia

    Ante las diferentes técnicas quirúrgicas en la patología del disco lumbar, los autores presentan su experiencia personal con la nucleotomía percutánea automatizada utilizando el nucleotomo de Surgical Dynamics. Sobre un total de 50 pacientes intervenidos se expone la técnica utilizada, con algunas variantes sobre la inicial de Onik, tal como la posición en sedestación del enfermo. Se puntualizan las indicaciones y contraindicaciones y se analizan los resultados obtenidos tras un mínimo de 6 meses de seguimiento. Se considera como una técnica útil, adecuada, relativamente inócua y que no sustituye sino que complementa a los procedimientos quirúrgicos convencionales.Faced with the different surgical techniques of lumbar disk pathology» the authors convery personal experience with automated percutaneous nucleotomy using the Surgical Dynamics. Based on the intervention of 50 patients, the technique used is explained, with some variations of the Onik procedure, such as the sitting position of the patient. The indications and contraindications are reviewed by the analysis. The results obtained with 6 months of follow up. The technique is considered usefull, proper, and relatively innocuous. It does not substitute but complements conventional surgical procedures

    Obstructive Sleep Apnoea Syndrome, Endothelial Function and Markers of Endothelialization. Changes after CPAP

    STUDY OBJECTIVES: This study tries to assess the endothelial function in vivo using flow-mediated dilatation (FMD) and several biomarkers of endothelium formation/restoration and damage in patients with obstructive sleep apnoea (OSA) syndrome at baseline and after three months with CPAP therapy. DESIGN: Observational study, before and after CPAP therapy. SETTING AND PATIENTS: We studied 30 patients with apnoea/hypopnoea index (AHI) >15/h that were compared with themselves after three months of CPAP therapy. FMD was assessed non-invasively in vivo using the Laser-Doppler flowmetry. Circulating cell-free DNA (cf-DNA) and microparticles (MPs) were measured as markers of endothelial damage and the vascular endothelial growth factor (VEGF) was determined as a marker of endothelial restoration process. MEASUREMENTS AND RESULTS: After three month with CPAP, FMD significantly increased (1072.26 ± 483.21 vs. 1604.38 ± 915.69 PU, p< 0.005) cf-DNA and MPs significantly decreased (187.93 ± 115.81 vs. 121.28 ± 78.98 pg/ml, p<0.01, and 69.60 ± 62.60 vs. 39.82 ± 22.14 U/μL, p<0.05, respectively) and VEGF levels increased (585.02 ± 246.06 vs. 641.11 ± 212.69 pg/ml, p<0.05). These changes were higher in patients with more severe disease. There was a relationship between markers of damage (r = -0.53, p<0.005) but not between markers of damage and restoration, thus suggesting that both types of markers should be measured together. CONCLUSIONS: CPAP therapy improves FMD. This improvement may be related to an increase of endothelial restoration process and a decrease of endothelial damage

    Nucleotomía percutánea automatizada: experiencia en 425 casos

    En el presente trabajo exponemos nuestra experiencia en el tratamiento de la hernia discal lumbar mediante Nucleotomía Percutánea Automatizada. El estudio comprende el periodo de Junio de 1988 a Diciembre de 1992. Se incluyen un total de 425 enfermos de edades comprendidas entre los 18 y 58 años. Los pacientes han sido evaluados a las 6 semanas, a los 3 y a los 6 meses tras la intervención. Los resultados han sido satisfactorios en el 71% de los casos. Solamente hubo una complicación de espondilodiscitis. En el 29% de los casos se obtuvieron malos resultados. Un porcentaje importante de los fracasos, se debieron a una mala selección de los pacientes desde el punto de vista de su perfil psicológico.Our experience using automated percutaneous nucleotomy for treatment for herniated disc is presented. A total of 42 5 patientes, aged between 18 and 58 years and operated from June 88 to December 1992, has been included. Patients were clinically assessed at 6 weeks, 3 months and 6 months after surgery. Satisfactory results were found in 71% of cases. As for complications, there was only a case of discitis. In 29% of patients, the outcome was poor. An important group of failures were due to bad selection of patients regadings psychological profile

    Immunohistochemical localization of low density lipoprotein receptor-related protein 1 and α2-Macroglobulin in retinal and choroidal tissue of proliferative retinopathies

    The immunolocalization of the low density lipoprotein receptor-related protein 1 (LRP1) and its ligand α 2-Macroglobulin (α2M) was examined in tissues from human donor eyes of normal, diabetic and sickle cell disease subjects. Streptavidin alkaline phosphatase immunohistochemistry was performed with a mouse anti-human LRP1 and rabbit anti-human α2M antibodies. Retinal and choroidal blood vessels were labeled with mouse anti-human CD34 antibody in adjacent tissue sections. Mean scores for immunostaining from the pathological and control eyes were statistically compared.LRP1 immunoreactivity was very weak to negative in the neural retina of normal subjects except in scattered astrocytes. LRP1 expression in diabetic eyes was detected in the internal limiting membrane (ILM), astrocytes, inner photoreceptor matrix, choriocapillaris and choroidal stroma. The ligand α2M, however, was limited mainly to blood vessel walls, some areas of the inner nuclear layer (INL), photoreceptors, RPE-Bruch's membrane-choriocapillaris complex, intercapillary septa, and choroidal stroma. In sickle cell eyes, avascular and vascular retina as well as choroidal neovascularization (CNV) were analyzed. In avascular areas, LRP1 immunoreactivity was in innermost retina (presumably ILM, astrocytes, and Muller cells) and INL as well as RPE-Bruch's membrane-choriocapillaris complex and choroidal stroma α2M was very weak in avascular peripheral retina compared to vascularized areas and limited to stroma in choroid. In contrast, in areas with CNV, LRP1 immunoreactivity was significantly decreased in overlying retina and in RPE-Bruch's membrane and choroidal stroma compared to the controls, while α2M was elevated in RPE-Bruch's membrane near CNV compared to normal areas in sickle cell choroid. The mean scores revealed that LRP1 and α2M in neural retina were significantly elevated in astrocytes and ILM in diabetic eyes (p ≤ 0.05), whereas in sickle cell eyes scores were elevated in ILM and INL (p ≤ 0.05). In addition, α2M immunoreactivity was in photoreceptors in both ischemic retinopathies. In choroid, the patterns of LRP1 and α2M expression were different and not coincident.This is the first demonstration of the presence of LRP1 and α2M in human proliferative retinopathies. Elevated LRP1 expression in sickle cell neural retina and diabetic inner retina and choroid suggests that LRP1 plays an important role in ischemic neovascular diseases. © 2010 Elsevier Ltd.Fil: Barcelona, Pablo Federico.     Reduction in the levels of CoQ biosynthetic proteins is related to an increase in lifespan without evidence of hepatic mitohormesis

    Mitohormesis is an adaptive response induced by a mild mitochondrial stress that promotes longevity and metabolic health in different organisms. This mechanism has been proposed as the cause of the increase in the survival in Coq7+/− (Mclk1+/−) mice, which show hepatic reduction of COQ7, early mitochondrial dysfunction and increased oxidative stress. Our study shows that the lack of COQ9 in Coq9Q95X mice triggers the reduction of COQ7, COQ6 and COQ5, which results in an increase in life expectancy. However, our results reveal that the hepatic CoQ levels are not decreased and, therefore, neither mitochondrial dysfunction or increased oxidative stress are observed in liver of Coq9Q95X mice. These data point out the tissue specific differences in CoQ biosynthesis.     P-P Total Cross Sections at VHE from Accelerator Data

    Comparison of P-P total cross-sections estimations at very high energies - from accelerators and cosmic rays - shows a disagreement amounting to more than 10 %, a discrepancy which is beyond statistical errors. Here we use a phenomenological model based on the Multiple-Diffraction approach to successfully describe data at accelerator energies. The predictions of the model are compared with data On the basis of regression analysis we determine confident error bands, analyzing the sensitivity of our predictions to the employed data for extrapolation. : using data at 546 and 1.8 TeV, our extrapolations for p-p total cross-sections are only compatible with the Akeno cosmic ray data, predicting a slower rise with energy than other cosmic ray results and other extrapolation methods. We discuss our results within the context of constraints in the light of future accelerator and cosmic ray experimental results.Comment: 26 pages aqnd 11 figure

    Nanostructured Europium Oxide thin films deposited by pulsed laser ablation of a metallic target in a He buffer atmosphere

    Nanostrucured Europium oxide and hydroxide films were obtained by pulsed Nd:Yag (532 nm) laser ablation of an Europium metallic target, in the presence of a 1 mbar Helium buffer atmosphere. Both the produced film and the ambient plasma were characterized. The plasma was monitored by an electrostatic probe, for plume expansion in vacuum or in the presence of the buffer atmosphere. The time evolution of the ion saturation current was obtained for several probe to substrate distances. The results show the splitting of the plume into two velocity groups, being the lower velocity profile associated with metal cluster formation within the plume. The films were obtained in the presence of helium atmosphere, for several target to substrate distances. They were analyzed by Rutherford backscattering spectrometry (RBS), X-Ray Diffraction (XRD) and Atomic Force Microscopy, for samples as-deposited and treated at 600 degrees C in air. The results show that the as-deposited samples are amorphous and have chemical composition compatible with Europium hydroxide. The thermally treated samples show X-Ray diffraction peaks of Eu_2O_3, with chemical composition showing excess oxygen. Film nanostructuring was shown to be strongly correlated to cluster formation, as shown by velocity splitting in probe current versus time plots.Comment: 7 pages, 11 figures, 31 reference
