    Comparisons of intellectual capacities between mild and classic adult-onset phenotypes of myotonic dystrophy type 1 (DM1)

    Background : Myotonic dystrophy type 1 (DM1) is an autosomal dominant genetic multisystem disorder and the commonest adult-onset form of muscular dystrophy. DM1 results from the expansion of an unstable trinucleotide cytosine-thymine-guanine (CTG) repeat mutation. CTG repeats in DM1 patients can range from 50 to several thousands, with a tendency toward increased repeats with successive generations (anticipation). Associated findings can include involvements in almost every systems, including the brain, and cognitive abnormalities occur in the large majority of patients. The objectives are to describe and compare the intellectual abilities of a large sample of DM1 patients with mild and classic adult-onset phenotypes, to estimate the validity of the Wechsler Adult Intelligence Scale-Revised (WAIS-R) in DM1 patients with muscular weakness, and to appraise the relationship of intelligence quotient (IQ) to CTG repeat length, age at onset of symptoms, and disease duration. Methods : A seven-subtest WAIS-R was administered to 37 mild and 151 classic adult-onset DM1 patients to measure their Full-Scale (FSIQ), Verbal (VIQ) and Performance IQ (PIQ). To control for potential bias due to muscular weakness, Standard Progressive Matrices (SPM), a motor-independent test of intelligence, were also completed. Results : Total mean FSIQ was 82.6 corresponding to low average IQ, and 82% were below an average intelligence. Mild DM1 patients had a higher mean FSIQ (U=88.7 vs 81.1, p<0.001), VIQ (U=87.8 vs 82.3, p=0.001), and PIQ (U=94.8 vs 83.6, p<0.001) than classic adult-onset DM1 patients. In both mild and classic adult-onset patients, all subtests mean scaled scores were below the normative sample mean. FSIQ also strongly correlate with SPM (r s =0.67, p<0.001), indicating that low intelligence scores are not a consequence of motor impairment. FSIQ scores decreased with both the increase of (CTG)n (r s =−0.41, p<0.001) and disease duration (r s =−0.26, p=0.003). Conclusions : Results show that intellectual impairment is an extremely common and important feature in DM1, not only among the classic adult-onset patients but also among the least severe forms of DM1, with low IQ scores compared to general reference population. Health care providers involved in the follow-up of these patients should be aware of their intellectual capacities and should adapt their interventions accordingly

    Facteurs prédictifs de l’intention de migrer chez les élèves du secondaire au Saguenay–Lac-Saint-Jean

    Au cours des dix dernières années, le phénomène de la migration des jeunes s’est accentué au Saguenay–Lac-Saint-Jean (SLSJ), comme en témoigne le solde migratoire négatif de 15 %. Au-delà des constats basés sur le bilan migratoire, la compréhension des déplacements de population nécessite une analyse des caractéristiques des individus qui ont l’intention de migrer avant qu’ils ne quittent leur région. L’analyse discriminante est utilisée ici pour déterminer les facteurs associés à l’intention de migrer d’un échantillon stratifié de 1901 élèves du secondaire du SLSJ. Les résultats confirment que davantage de filles que de garçons préféreraient vivre à l’extérieur du SLSJ une fois leurs études terminées et montrent que les aspirations scolaires, les relations familiales et les comportements des jeunes qui ont l’intention de migrer se différencient nettement selon le sexe. Cette étude met en évidence la prépondérance des dimensions psychosociale et culturelle eu égard à l’intention de migrer des jeunes du SLSJ, et la nécessité de tenir compte du genre pour saisir les tenants et aboutissants de la migration des jeunes des régions périphériques du Québec.Over the past ten years, the phenomenon of youth migration has intensified in the Saguenay–Lac-Saint-Jean (SLSJ) region, as shown by the negative migration balance of 15%. In addition to the examination of findings based on migration balances, the understanding of migratory movements also requires an analysis of the characteristics of individuals who intend to migrate before they effectively leave their region. Discriminant analysis is used to identify factors associated with the migration intentions of a stratified sample of 1,901 secondary school students in SLSJ. The results confirm that more girls than boys would prefer to live outside the SLSJ upon completion of their studies and also show that the educational aspirations, family relationships and behaviours of students who intend to migrate strongly differ according to gender. This study emphasizes the importance of psychosocial and cultural factors as predictors of migration intentions in secondary school students in SLSJ. One also has to take gender into account in order to fully comprehend the processes underlying youth migration from peripheral regions of Québec

    Development and aetiology of body dissatisfaction in adolescent boys and girls

    This longitudinal study aims to describe the development of body dissatisfaction (BD), measured with the Contour Drawing Rating Scale, between the ages of 14 and 18, and to identify factors associated with BD at age 18, among 413 adolescents. Between the ages of 14 and 18, the proportion of girls wanting to be thinner increased, although it remained unchanged among boys. A ratio of 1:2 girls and 1:5 boys reported having seriously tried to lose weight. Factors associated with BD in girls at age 18 were (1) wanting to be thinner, (2) body mass index (BMI), (3) weight control behaviours and (4) negative comments about weight. Factors associated with BD in boys at age 18 were (1) wanting to be thinner or bigger, (2) BMI, (3) having experienced sexual intercourse and (4) negative comments about weight. The high prevalence of BD and weight-related concerns suggest a need for early interventions

Cette façon de voir et de faire a pour effet de subordonner presque exclusivement le champ des interventions préventives au plan individuel et dans les dimensions sanitaires.L'auteur relate des paradoxes, obstacles et égarements dans l'application des programmes de prévention du VIH et propose un renouvellement des pratiques sociales en traitant d'enjeux plus globaux de société visant à la réduction des inégalités sociales et économiques et en renvoyant à la reconnaissance de l'émergence de la personne en tant que sujet de sa vie et au respect de la dignité de la personne humaine

    Be thinner or larger? Correlates of body dissatisfaction in a large sample of French-Canadian children

    Objective: To assess body dissatisfaction among children between 9 and 14 years and to examine factors (age, gender, Body Mass Index (BMI), perceived shape, and self-esteem) associated with wanting a thinner or a larger shape. Study design: Through at-school questionnaires, 1,515 preadolescent children (51.2% girls) were asked to fill out the Culture Free Self-Esteem Inventory and the Contour Drawing Rating Scale (body dissatisfaction). Trained assessors then weighed and measured the students individually. Results: Overall, 50.5% of girls wanted a thinner shape compared to 35.9% of boys. More boys wanted a larger shape compared to girls (21.1% vs. 7.2%). Most of the preadolescents who were overweight or obese were unsatisfied whereas 58.0% of girls and 41.6% of boys who were underweight were satisfied with their body. Results of a multinomial logistic regression revealed that age, gender, BMI, perceived shape, and self-esteem were significant correlates of the four body dissatisfaction contrasts (wanting a slightly thinner, much thinner, slightly larger, and much larger shape) and explained 50% of the variance. An interaction between gender and perceived shape was found, revealing that girls who perceived themselves as having a larger shape were more likely to desire a thinner shape than boys. Conclusions: The high prevalence rate of body dissatisfaction among children suggests that current approaches in our society to prevent problems related to body image must be improved. The different results between girls and boys highlight the need to take into account gender differences when designing prevention programs that aim to decrease body dissatisfaction
