COVID-19 exposes weaknesses in public health in the Peruvian Amazon and highlights opportunities for a One Health approach

The Amazon is home to important wildlife and a biodiversity hotspot of global importance. The ancestral knowledge kept by Indigenous communities about its fauna and flora contributes further to its irreplaceable value. The Peruvian Amazon was heavily struck by the COVID-19 epidemic with a cumulative incidence of 725, a mortality rate of 34 per 100,000 inhabitants, and a case fatality rate of 4.6% by the end of July 2020. In this work, we review scientific literature and media to trace the events that happened at the beginning of the COVID-19 epidemic in the Peruvian Amazon. Results are synthesized in three observations: (1) the evolution of the COVID-19 epidemic within the Peruvian Amazon and the response of the Peruvian health care system, (2) Confusing information about Ivermectin use for COVID-19 treatment and prevalent self-medication (3) The response of the traditional Indigenous health care system to the COVID-19 epidemic. These three observations are interdependent. There is an unexploited potential for integrative approaches linking traditional medical practices (TMP) and biomedical approaches and they may benefit from the interactions that occur between them. Synergies can also be explored between the human and animal health care sector, especially in terms of the use and stewardship of medicines. We conclude that there is a benefit of the One Health approach in the region, which can go through the common ambition to improve the integrated health of people, animals and ecosystems, facilitate the enhancement of equity and inclusion while improving access to health services and conserving biodiversity. One Health Impact Statement The Amazon region is home to wildlife flora and fauna and indigenous Amazon communities. The case presented in this work shows that an existing grassroots initiative has been reducing case fatality rates tenfold during the COVID-19 epidemic, while acting in respect towards nature and the environment, and using its resources. This is in stark contrast to the uncontrolled, ineffective self-medication with ivermectin in that same period, which may endanger the biodiversity hotspot through metabolic residues. While the current example seems to illustrate community resilience due to government negligence, it also shows the vastly unexplored potential of integrating biomedical and traditional indigenous knowledge in a solidaric and co-creative framework such as One Health

An exploratory study of treated-bed nets in Timor-Leste: patterns of intended and alternative usage

<p>Abstract</p> <p>Background</p> <p>The Timor-Leste Ministry of Health has recently finalized the National Malaria Control Strategy for 2010-2020. A key component of this roadmap is to provide universal national coverage with long-lasting insecticide-treated nets (LLINs) in support of achieving the primary goal of reducing both morbidity and mortality from malaria by 30% in the first three years, followed by a further reduction of 20% by end of the programme cycle in 2020 <abbrgrp><abbr bid="B1">1</abbr></abbrgrp>. The strategic plan calls for this target to be supported by a comprehensive information, education and communication (IEC) programme; however, there is limited prior research into household and personal usage patterns to assist in the creation of targeted, effective, and socio-culturally specific behaviour change materials.</p> <p>Methods</p> <p>Nine separate focus group discussions (FGDs) were carried out in Dili, Manatuto, and Covalima districts, Democratic Republic of Timor-Leste, in July 2010.</p> <p>These focus groups primarily explored themes of perceived malaria risk, causes of malaria, net usage patterns within families, barriers to correct and consistent usage, and the daily experience of users (both male and female) in households with at least one net. Comprehensive qualitative analysis utilized open source analysis software.</p> <p>Results</p> <p>The primary determinants of net usage were a widespread perception that nets could or should <it>only </it>be used by pregnant women and young children, and the availability of sufficient sleeping space under a limited number of nets within households. Both nuisance biting and disease prevention were commonly cited as primary motivations for usage, while seasonality was not a significant factor. Long-term net durability and ease of hanging were seen as key attributes in net design preference. Very frequent washing cycles were common, potentially degrading net effectiveness. Finally, extensive re-purposing of nets (fishing, protecting crops) was both reported and observed, and may significantly decrease availability of nighttime sleeping space for all family members if distributed nets do not remain within the household.</p> <p>Conclusions</p> <p>Emphasizing that net usage is acceptable and important for all family members regardless of age or gender, and addressing the complex behavioural economics of alternative net usages could have significant impacts on malaria control efforts in Timor-Leste, as the country's programmes make progress towards universal net coverage.</p

Comportamiento estructural en pórticos con adición de tapas metálicas tipo corona reciclado, AA.HH. Jicamarca anexo 22, 2018

La presenta tesis titulada Comportamiento estructural en pórticos con adición de tapas metálicas tipo corona reciclado, AA.HH. Jicamarca anexo 22, 2018 planteó el objetivo general de evaluar la influencia de las tapas metálicas tipo corona reciclado en con el fin de conocer su beneficio, al favorecer el comportamiento estructural en pórticos de concreto armado, en cuanto el enfoque se trató de un estudio cuantitativo de nivel aplicado ya que se basó en resultados e hipótesis. El diseño fue experimental ya que una manipulación de variables. Los resultados obtenidos favorecieron y a la vez fueron aceptables debido a que se realizó una correcta dosificación en la número 3, (muestra con un 30% de tapas metálicas tipo corona), que involucre un esfuerzo a compresión luego de 28 días de 32,5 Kg/cm2; el esfuerzo a flexión o flexotracción, de la muestra con aditivo reciclado de la que se alcanzó un 51,7 Kg/cm2 ; y en cuanto a los desplazamientos laterales evidenciaron ser menores que el de la muestra patrón (sin aditivo reciclado), resultando en la dirección con más alta fluencia la x un 0,0050 cm y un 0,00680 cm respectivamente. En conclusión, el esfuerzo a compresión que se alcanzó está dentro del rango de aceptable ya que posee un aditivo de mejoramiento de propiedades como también en el esfuerzo a flexión. Es importante resaltar que, en los resultados del desplazamiento lateral, en el caso de las muestras se caracterizaron por ser satisfactorio en el rango de 0,005 de derivas, al destacar que la muestra con aditivo reciclado logró menor desplazamiento lateral (deriva)
