    Molecules with multiple personalities: how switchable materials could revolutionise chemical sensing

    Worldwide, the demand for sensing devices that can conform with the requirements of large-scale wireless sensor network (WSN) deployments is rising exponentially. Typically, sensors should be very low cost, low power (essentially self-sustaining), yet very rugged and reliable. At present, functioning WSN deployments involve physical transducers only, such as thermistors, accelerometers, photodetectors, or flow meters, to monitor quantities like temperature, movement, light level and liquid level/flow. Remote, widely distributed monitoring of molecular targets remains relatively unexplored, except in the case of targets that can be detected directly using ‘non-contact’ techniques like spectroscopy. This paper will address the issues inhibiting the close integration of chemical sensing with WSNs and suggest strategies based on fundamental materials science that may offer routes to new sensing surfaces that can switch between different modes of behaviour (e.g. active-passive, expand-contract)

    Asking questions that are “close to the bone”: integrating thematic analysis and natural language processing to explore the experiences of people with traumatic brain injuries engaging with patient-reported outcome measures

    Introduction: Patient-reported outcomes measures (PROMs) are valuable tools for assessing health-related quality of life and treatment effectiveness in individuals with traumatic brain injuries (TBIs). Understanding the experiences of individuals with TBIs in completing PROMs is crucial for improving their utility and relevance in clinical practice. Methods: Sixteen semi-structured interviews were conducted with a sample of individuals with TBIs. The interviews were transcribed verbatim and analysed using Thematic Analysis (TA) and Natural Language Processing (NLP) techniques to identify themes and emotional connotations related to the experiences of completing PROMs. Results: The TA of the data revealed six key themes regarding the experiences of individuals with TBIs in completing PROMs. Participants expressed varying levels of understanding and engagement with PROMs, with factors such as cognitive impairments and communication difficulties influencing their experiences. Additionally, insightful suggestions emerged on the barriers to the completion of PROMs, the factors facilitating it, and the suggestions for improving their contents and delivery methods. The sentiment analyses performed using NLP techniques allowed for the retrieval of the general sentimental and emotional “tones” in the participants’ narratives of their experiences with PROMs, which were mainly characterised by low positive sentiment connotations. Although mostly neutral, participants’ narratives also revealed the presence of emotions such as fear and, to a lesser extent, anger. The combination of a semantic and sentiment analysis of the experiences of people with TBIs rendered valuable information on the views and emotional responses to different aspects of the PROMs. Discussion: The findings highlighted the complexities involved in administering PROMs to individuals with TBIs and underscored the need for tailored approaches to accommodate their unique challenges. Integrating TA-based and NLP techniques can offer valuable insights into the experiences of individuals with TBIs and enhance the interpretation of qualitative data in this population

    NHS England Always EventsÂŽ program: Developing a national model for co-production

    NHS England, the Institute for Healthcare Improvement, Picker and NHS Improvement started the work described in this article to strengthen how patients, carers and staff working together in co-design and co-production can make a real difference in improving experience of care. Always EventsÂŽ, which is an improvement methodology for the co-design and consistent implementation of those aspects of patient experience that matter most to patients in a health care setting, was chosen. The approach has been to first test the methodology with three organizations then to spread to a substantial proportion of acute health care providers, with concurrent scale-up within organizations that were early adopters in single settings, and then spread to other providers including care homes, primary care and integrated care systems. For organizations to be successful in embedding and sustaining Always Events, and to have a significant impact on improving experience, we learnt that health organizations should: a) co-produce with patients/service users from the very start and throughout every phase, b) integrate quality improvement, patient experience and person-centered care, c) identify an Executive Leader champion who can support the team to gain momentum from the beginning, d) keep an open mind and not be tempted to pre-determine what changes need to be made; patients/service users & staff know the challenges and have the best, simplest and most affordable ideas for improvement and finally, e) involving point of care staff from the beginning and co-designing the Always Event with them will support engagement and reduce resistance to change

    Increasing outcome measure completion in adult patients with a traumatic brain injury: Ideas from the research literature and evaluation of service change

    The use of Patient Reported Outcome Measures (PROMs) is increasingly important in clinical practice. Within the Department of Clinical Neuropsychology at Salford Royal Hospital they are used routinely to collect information on the overall physical, psychological and social adjustment of patients with a traumatic brain injury (TBI). In an attempt to increase PROMs completion rates, we implemented two major changes, reducing the number of PROMs completed and changing the collection method. We critically evaluated the influence that these changes have had on PROMs completion rates and discussed the potential barriers in PROMs completion rates in TBI patients, together with reflections for future improvements of the PROMs used and their administration methods
