Optimizing medication management for patients with cirrhosis: Evidence‐based strategies and their outcomes
Cirrhosis is a morbid condition associated with frequent hospitalizations and high mortality. Management of cirrhosis requires complex medication regimens to treat underlying liver disease, complications of cirrhosis and comorbid conditions. This review examines the complexities of medication management in cirrhosis, barriers to optimal medication use, and potential interventions to streamline medication regimens and avoid medication errors. A literature review was performed by searching PUBMED through December 2017 and article reference lists to identify articles relevant to medication management, complications, adherence, and interventions to improve medication use in cirrhosis. The structural barriers in cirrhosis include sheer medication complexity related to the number of medications and potential for cognitive impairment in this population, faulty medication reconciliation and limited adherence. Tested interventions have included patient self‐education, provider driven patient education, intensive case management including medication blister packs and smartphone applications. Initiatives are needed to improve patient, caregiver and provider education on appropriate use of medications in patients with cirrhosis. A multidisciplinary team should be established to coordinate care with close monitoring, address patient and caregiver concerns, and to provide timely access to outpatient evaluation of urgent/complex issues. The prevalence of insomnia in the general population in China: A meta-analysis
This is the first meta-analysis of the pooled prevalence of insomnia in the general population of China. A systematic literature search was conducted via the following databases: PubMed, PsycINFO, EMBASE and Chinese databases (China National Knowledge Interne (CNKI), WanFang Data and SinoMed). Statistical analyses were performed using the Comprehensive Meta-Analysis program. A total of 17 studies with 115,988 participants met the inclusion criteria for the analysis. The pooled prevalence of insomnia in China was 15.0% (95% Confidence interval [CI]: 12.1%-18.5%). No significant difference was found in the prevalence between genders or across time period. The pooled prevalence of insomnia in population with a mean age of 43.7 years and older (11.6%; 95% CI: 7.5%-17.6%) was significantly lower than in those with a mean age younger than 43.7 years (20.4%; 95% CI: 14.2%-28.2%). The prevalence of insomnia was significantly affected by the type of assessment tools (Q = 14.1, P = 0.001). The general population prevalence of insomnia in China is lower than those reported in Western countries but similar to those in Asian countries. Younger Chinese adults appear to suffer from more insomnia than older adults
Estimating the causal effect of a time-varying treatment on time-to-event using structural nested failure time models
In this paper we review an approach to estimating the causal effect of a
time-varying treatment on time to some event of interest. This approach is
designed for the situation where the treatment may have been repeatedly adapted
to patient characteristics, which themselves may also be time-dependent. In
this situation the effect of the treatment cannot simply be estimated by
conditioning on the patient characteristics, as these may themselves be
indicators of the treatment effect. This so-called time-dependent confounding
is typical in observational studies. We discuss a new class of failure time
models, structural nested failure time models, which can be used to estimate
the causal effect of a time-varying treatment, and present methods for
estimating and testing the parameters of these models
Living IoT: A Flying Wireless Platform on Live Insects
Sensor networks with devices capable of moving could enable applications
ranging from precision irrigation to environmental sensing. Using mechanical
drones to move sensors, however, severely limits operation time since flight
time is limited by the energy density of current battery technology. We explore
an alternative, biology-based solution: integrate sensing, computing and
communication functionalities onto live flying insects to create a mobile IoT
platform.
Such an approach takes advantage of these tiny, highly efficient biological
insects which are ubiquitous in many outdoor ecosystems, to essentially provide
mobility for free. Doing so however requires addressing key technical
challenges of power, size, weight and self-localization in order for the
insects to perform location-dependent sensing operations as they carry our IoT
payload through the environment. We develop and deploy our platform on
bumblebees which includes backscatter communication, low-power
self-localization hardware, sensors, and a power source. We show that our
platform is capable of sensing, backscattering data at 1 kbps when the insects
are back at the hive, and localizing itself up to distances of 80 m from the

Reconstruction of the insulin-like signalling pathway of Haemonchus contortus
Background: In the present study, we reconstructed the insulin/insulin-like growth factor 1 signalling (IIS) pathway for Haemonchus contortus, which is one of the most important eukaryotic pathogens of livestock worldwide and is related to the free-living nematode Caenorhabditis elegans.
Methods: We curated full-length open-reading frames from assembled transcripts, defined the complement of genes that encode proteins involved in this pathway and then investigated the transcription profiles of these genes for all key developmental stages of H. contortus.
Results: The core components of the IIS pathway are similar to their respective homologs in C. elegans. However, there is considerable variation in the numbers of isoforms between H. contortus and C. elegans and an absence of AKT-2 and DDL-2 homologs from H. contortus. Interestingly, DAF-16 has a single isoform in H. contortus compared with 12 in C. elegans, suggesting novel functional roles in the parasitic nematode. Some IIS proteins, such as DAF-18 and SGK-1, vary in their functional domains, indicating distinct roles from their homologs in C. elegans.
Conclusions: This study paves the way for the further characterization of key signalling pathways in other socioeconomically important parasites and should help understand the complex mechanisms involved in developmental processes
Semiclassical theory of electron drag in strong magnetic fields
We present a semiclassical theory for electron drag between two parallel
two-dimensional electron systems in a strong magnetic field, which provides a
transparent picture of the most salient qualitative features of anomalous drag
phenomena observed in recent experiments, especially the striking sign reversal
of drag at mismatched densities. The sign of the drag is determined by the
curvature of the effective dispersion relation obeyed by the drift motion of
the electrons in a smooth disorder potential. Localization plays a role in
explaining activated low temperature behavior, but is not crucial for anomalous

Economic transition and elections in Poland 1
Poland's economic and political transition, one of the most successful, has depended very heavily on job creation in new firms to replace the jobs lost in the formerly state-owned enterprises. This paper uses survey and aggregate data from three Polish elections to suggest that these de novo firms, the individuals they employ, and the residents in the local areas where they exist become an important constituency supporting pro-reform political parties and constraining the actions of parties less sympathetic to the reforms. The creation of this political constituency helps explain how countries can successfully pursue both economic and political reforms. Fatty acid oxidation is essential for egg production by the parasitic flatworm Schistosoma mansoni
Schistosomes, parasitic flatworms that cause the neglected tropical disease schistosomiasis, have been considered to have an entirely carbohydrate based metabolism, with glycolysis playing a dominant role in the adult parasites. However, we have discovered a close link between mitochondrial oxygen consumption by female schistosomes and their ability to produce eggs. We show that oxygen consumption rates (OCR) and egg production are significantly diminished by pharmacologic inhibition of carnitine palmitoyl transferase 1 (CPT1), which catalyzes a rate limiting step in fatty acid β-oxidation (FAO) and by genetic loss of function of acyl CoA synthetase, which complexes with CPT1 and activates long chain FA for use in FAO, and of acyl CoA dehydrogenase, which catalyzes the first step in FAO within mitochondria. Declines in OCR and egg production correlate with changes in a network of lipid droplets within cells in a specialized reproductive organ, the vitellarium. Our data point to the importance of regulated lipid stores and FAO for the compartmentalized process of egg production in schistosomes
