بررسی اثر ميکروکانال‌های laser-lok برروی تنش‌های وارد بر استخوان کرستال به‌روش آناليز المان محدود

مقدمه: در اين تحقيق با روش آناليز المان محدود به بررسی تأثير ميکروکانال­های Laser-Lok برروی توزيع تنش­های وارد بر استخوان کرستال در ناحيه فصل مشترک استخوان با کولار در ايمپلنت Laser-Lok مي­پردازيم و نتايج را با نمونه شاهد که فاقد اين ميکروکانال­ها است، مقايسه مي­نماييم تا مشخص گردد که طراحی خاص ميکروکانال­های Laser-Lok چه تأثيری برروی ميزان تنش­های وارد بر استخوان کرستال خواهد داشت. روش بررسی: برای انجام آناليز المان محدود در ابتدا مدل سه بُعدی ايمپلنت Laser-Lok و ايمپلنت شاهد و همچنين مدل سه­بُعدی استخوان فک پايين (منديبل) را تهيه کرديم. سپس هر دو ايمپلنت را به­طور جداگانه با استخوان منديبل در ناحيه پره­مولر، البته با درنظرگرفتن شرايط مرزی مناسب، مونتاژ نموديم. پس از مش­­بندی و درنظرگرفتن المان­های مطلوب با توجه به نيروهای وارد بر دندان درحين جويدن و درنظرگرفتن شرايط طبيعی، نيروی 100 نيوتنی را در دو حالت محوری و مايل با دو زاويه 15 و 45 درجه به مرکز اباتمنت هر دو ايمپلنت Laser-Lok و شاهد در شرايط کاملاً يکسان وارد کرديم. يافته­ها: نتايج به­دست آمده در اين تحقيق حاکی از کاهش تنش­های وارد بر استخوان کرستال در ايمپلنت Laser-Lok نسبت به ايمپلنت شاهد بود. به­طور کلی در تمامی حالات ايمپلنت Laser-Lok نسبت به نمونه شاهد، برتر نشان داد و تنش کمتری را به استخوان کرستال اطرافش وارد کرد. نتيجه گيری: اين نتايج نشان دهنده مؤثر بودن طراحی خاص ميکروکانال­های Laser-Lok برروی کولار در جهت کاهش تنش­های وارد بر استخوان کرستال اطراف بود. با توجه به فرضيه تأثير عوامل استرس­زا مبنی بر اينکه هرچه تنش­های وارد بر استخوان کرستال کمتر باشد احتمال تحليل استخوان در اين ناحيه نيز کمتر است، مي­توان ادعا کرد که اين طراحی منحصر به فرد ايمپلنت Laser-Lok در نهايت منجر به کاهش تحليل استخوان کرستال در اين ايمپلنت مي­گردد

Floating Hem-o-Lok Clips in the Bladder without Stone Formation after Robot-Assisted Laparoscopic Radical Prostatectomy

Hem-o-Lok clips (Weck Surgical Instruments, Teleflex Medical, Durham, NC, USA) are widely used in robot-assisted laparoscopic radical prostatectomy because of their easy application and secure clamping. To date, there have been some reports of intravesical migration of these clips causing urethral erosion, bladder neck contractures, and subsequent calculus formation. We report the first case of bladder migration of Hem-o-Lok clips without stone formation after robot-assisted laparoscopic radical prostatectomy. The Hem-o-Lok clips were found during urethral dilation with a guide wire for bladder neck contracture under cystourethroscopy. The Hem-o-Lok clips were floating in the bladder without stone formation and were removed by a cystoscopic procedure

"At-risk" or "Socially deviant"? Conflicting narratives and grassroots organizing of sex/entertainment workers and LGBT communities in Cambodia

Cambodia has experienced rapid economic development and increased globalization in the last two decades, which have influenced changes in sexual attitudes and politics. Yet deeply embedded patriarchal structures that promote adherence to traditional values, gender binaries, and sexual purity of women impede progress in the recognition of the rights of sex/entertainment workers and LGBT communities. Using the framework of sexual humanitarianism, this paper outlines the ways in which these constraints are compounded by two dominant conflicting narratives that place these groups as either at-risk and vulnerable or socially deviant, and deemed in need of interventions that protect and control. Drawing on over a decade of empirical research on the sex/entertainment industries, and broader gender/sexual landscape in Cambodia, as well as current social activism of the authors, this paper also describes the ways LGBT and sex worker communities are engaging in shared organizing and self-advocacy as strategies to address their needs and the consequences left in the wake of sexual humanitarian interventions. In order to contextualize their deeply-rooted legacy in Cambodia, the paper also provides an overview of past and contemporary gender/sexual norms and diversity, and concludes with a call for governments and policymakers to expand support for grassroots movements and to listen more closely to the voices of LGBT and sex worker communities so that the political and social needs of these groups can be addressed

Aging Gracefully: The PACE Approach to Caring for Frail Elders in the Community

Mountain Empire is one of the newest of more than 100 independent PACE organizations across the nation that serve both as health plans and as medical and long-term service providers to elders—offering meals, checkups, rehabilitation services, home visits, and many other supports that enable enrollees to preserve their independence. The model for PACE dates back to 1971, when a public health dentist and social worker from the San Francisco Public Health Department working in Chinatown-North Beach noticed that as their clients aged, many needed extra support but dreaded moving into nursing homes. They founded On Lok Senior Health Services as an alternative to institutional care that would allow elders to "age in place" in their homes; on lokis Cantonese for "peaceful, happy abode."On Lok's founders were particularly concerned about elderly clients who suffered when their various clinicians failed to work together, sometimes leading to complications that necessitated moves into institutional care. They designed On Lok to promote what was then an innovative approach: coordinating care from an interdisciplinary team of professionals who provide all primary care services and oversee specialists' services.A Medicare-funded demonstration spanning 1979 to 1983 found this approach had many benefits. Care teams were able to prevent or quickly address problems, resulting in better health and quality of life and producing 15 percent lower costs than traditional Medicare. In the decades since, the model has spread slowly, though enrollment has grown nearly 40 percent in the past three years. As of January 2016, there were 118 PACE organizations in 31 states serving some 39,000 elders

Pengumpulan data diperoleh melalui teknik observasi.\ud Sebelum melakukan penelitian, penulis telah melakukan observasi terlebih dahulu dan terlihat bahwa hasil belajar siswa masih rendah dengan nilai 6,00\ud Hasil penelitian tindakan kelas yang dilakukan sebanyak 2 siklus dengan menggunakan metode demonstarsi menunjukan: Hasil belajar siswa pada siklus I pertemuan 1 menunjukkan peningkatan dengan nilai 6,16, dan siklus I pertemuan ke 2 meningkat menjadi 7,00. Siklus II pertemuan 1 meningkat menjadi 7,50 dan siklus II pertemuan 2 meningkat menjadi 8,00

An effective lowest Landau level treatment of demagnetization in superconducting mesoscopic disks

Demagnetization, which is inherently present in the magnetic response of small finite-size superconductors, can be accounted for by an effective $\kappa$ within a two-dimensional lowest Landau level approximation of the Ginzburg-Landau functional. We show this by comparing the equilibrium magnetization of superconducting mesoscopic disks obtained from the numerical solution of the three-dimensional Ginzburg-Landau equations with that obtained in the ``effective'' LLL approximation.Comment: 5 pages, 4 figures, submitted to Phys. A Novel Mounting Device For Attaching Intracranial Probes To The Skull For Use In Experimental Research Models

A simple mount capable of securely holding a variety of intracranial probes to the skull was constructed from commonly available clinical consumables. Using this device the cerebral cortical blood flow of preterm lambs was measured using a laser Doppler flow probe, and cerebral pH and cortical electrical impedance were measured in newborn piglets using pH electrodes and Ag/AgCl wire electrodes. In both studies, the mount held the various probes for periods up to 6 h with no dislodgement or probe failure. The simple mount presented here can be adapted to a wide variety of intracranial probes and will hold them securely to the skull
