    A dual-tuned C/ H head coil for PET/MR hybrid neuroimaging:Development, attenuation correction, and first evaluation

    PURPOSE: This study aims to develop, implement, and evaluate a dual-tuned 13 C/1 H head coil for integrated positron emission tomography/magnetic resonance (PET/MR) neuroimaging. The radiofrequency (RF) head coil is designed for optimized MR imaging performance and PET transparency and attenuation correction (AC) is applied for accurate PET quantification.MATERIAL AND METHODS: A dual-tuned 13 C/1 H RF head coil featuring a 16-rung birdcage was designed to be used for integrated PET/MR hybrid imaging. While the open birdcage design can be considered inherently PET transparent, all further electronic RF components were placed as far as possible outside of the field-of-view (FOV) of the PET detectors. The RF coil features a rigid geometry and thin-walled casing. Attenuation correction of the RF head coil is performed by generating and applying a dedicated 3D CT-based template attenuation map (μmap). Attenuation correction was systematically evaluated in phantom experiments using a large-volume cylindrical emission phantom filled with 18-F-Fluordesoxyglucose (FDG) radiotracer. The PET/MR imaging performance and PET attenuation correction were then evaluated in a patient study including six patients.RESULTS: The dual-tuned RF head coil causes a mean relative attenuation difference of 8.8% across the volume of the cylindrical phantom, while the local relative differences range between 1% and 25%. Applying attenuation correction, the relative difference between the two measurements with and without RF coil is reduced to mean value of 0.5%, with local differences of ±3.6%. The quantitative results of the phantom measurements were corroborated by patient PET/MR measurements. Patient scans using the RF head coil show a decrease of PET signal of 5.17% ± 0.81% when compared to the setup without RF head coil in place, which served as a reference scan. When applying attenuation correction of the RF coil in the patient measurements, the mean difference to a measurement without RF coil was reduced to -0.87% ± 0.65%.CONCLUSION: A dual tuned 13 C/1 H RF head coil was designed and evaluated regarding its potential use in integrated PET/MR hybrid imaging. Attenuation correction was successfully applied. In conclusion, the RF head coil was successfully integrated into PET/MR hybrid imaging and can now be used for 13 C/1 H multinuclear hybrid neuroimaging in future studies.     Staphylococcus aureus α-Toxin Triggers the Synthesis of B-Cell Lymphoma 3 by Human Platelets

    The frequency and severity of bacteremic infections has increased over the last decade and bacterial endovascular infections (i.e., sepsis or endocarditis) are associated with high morbidity and mortality. Bacteria or secreted bacterial products modulate platelet function and, as a result, affect platelet accumulation at sites of vascular infection and inflammation. However, whether bacterial products regulate synthetic events in platelets is not known. In the present study, we determined if prolonged contact with staphylococcal α-toxin signals platelets to synthesize B-cell lymphoma (Bcl-3), a protein that regulates clot retraction in murine and human platelets. We show that α-toxin induced αIIbβ3-dependent aggregation (EC50 2.98 µg/mL ± 0.64 µg/mL) and, over time, significantly altered platelet morphology and stimulated de novo accumulation of Bcl-3 protein in platelets. Adherence to collagen or fibrinogen also increased the expression of Bcl-3 protein by platelets. α-toxin altered Bcl-3 protein expression patterns in platelets adherent to collagen, but not fibrinogen. Pretreatment of platelets with inhibitors of protein synthesis or the mammalian Target of Rapamycin (mTOR) decreased Bcl-3 protein expression in α-toxin stimulated platelets. In conclusion, Staphylococcus aureus-derived α-toxin, a pore forming exotoxin, exerts immediate (i.e., aggregation) and prolonged (i.e., protein synthesis) responses in platelets, which may contribute to increased thrombotic events associated with gram-positive sepsis or endocarditis

    Identification of novel autoantibodies in Sjögren's disease

    Introduction The diagnosis of Sjögren's disease (SjD) in patients without autoantibodies against Ro/SSA is a major challenge. We aimed to identify novel autoantibodies in SjD that may facilitate the diagnostic procedure for Ro/SSA negative SjD. Methods IgG and IgA autoantibody reactivity of 94 potential candidate autoantigens for SjD, selected from a discovery screen of 1,629 human antigens coupled to Luminex beads and prior knowledge about potential biological relevance, were examined in serum of SjD patients (n=347) using Luminex and ELISA technology. Healthy (HC, n=118) and non-Sjögren's sicca syndrome (NSS, n=44) individuals served as controls. To assess disease specificity, the novel autoantibodies were also measured in serum of patients with Rheumatoid Arthritis (RA, n=50), Systemic Lupus Erythematosus (SLE, n=49), and Systemic Sclerosis (SSc, n=37). Results 45 novel autoantibodies were significantly (p ≤ 0.05) more prevalent in SjD than in HC and were detected in up to 19% of the SjD cohort. The most common autoantibodies were against CCL4, M5, TMPO and OAS3. Some of the novel autoantibodies were associated with extraglandular disease manifestations, such as anti-TONSL or anti-IL6 with pulmonary involvement. We have developed a three and five marker panel for the detection of Ro/SSA negative patients, consisting of anti-FNBP4, anti-SNRPC, anti-CCL4, anti-M3 and anti-KDM6B, which had a sensitivity of up to 46% with a specificity of 95% (SjD vs. HC). Both panels discriminate these patients from HC, whereas the three-marker more effectively differentiates between Ro/SSA negative patients and NSS. Discussion Novel autoantibodies will facilitate the diagnosis of Ro/SSA negative patients with SjD, in particular our predictive panel will be useful in the diagnosis and differentiation of these patients from healthy and NSS individuals in a clinical context. In addition, the autoantibodies may also be useful for risk stratification of extraglandular manifestations

    Hintergrund Intensivmedizinische Aufenthalte führen bei vorbelasteten Menschen häufig zu Symptomen einer posttraumatischen Belastungsstörung (Post-ICU-PTBS). In der Nachsorge spielen HausärztInnen eine wichtige Rolle. Falldarstellung Eine 58-jährige Patientin entwickelt nach erlebter Sepsis eine Post-ICU-PTBS. Sie erhält durch ihre Hausärztin eine Kurzform der Narrativen Expositionstherapie (NET) und erfährt eine deutliche Symptombesserung. Schlussfolgerung Die angewandte Kurzform der NET kann für die Behandlung von leicht- bis mittelgradigen Symptomen einer Post-ICU-PTBS geeignet sein.Background Care in the intensive care unit (ICU) often introduces symptoms of post-traumatic stress disorder (post-ICU PTSD) in individuals with prior adverse experiences. General practitioners play an important role in follow-up care. Case report A 58-year-old woman developed post-ICU PTSD symptoms after she experienced sepsis. She received brief narrative exposure therapy (NET) from her general practitioner and experienced significant improvement of symptoms. Conclusion A brief version of NET might be appropriate for the treatment of patients with mild to moderate post-ICU PTSD symptoms

    Standardized monitoring of cytomegalovirus-specific immunity can improve risk stratification of recurrent cytomegalovirus reactivation after hematopoietic stem cell transplantation

    Recurrence of cytomegalovirus reactivation remains a major cause of morbidity and mortality following allogeneic hematopoietic stem cell transplantation. Monitoring cytomegalovirus-specific cellular immunity using a standardized assay might improve the risk stratification of patients. A prospective multicenter study was conducted in 175 intermediate- and high-risk allogeneic hematopoietic stem cell transplant recipients under preemptive antiviral therapy. Cytomegalovirus-specific cellular immunity was measured using a standardized IFN-γ ELISpot assay (T-Track® CMV). Primary aim was to evaluate the suitability of measuring cytomegalovirus-specific immunity after end of treatment for a first cytomegalovirus reactivation to predict recurrent reactivation. 40/101 (39.6%) patients with a first cytomegalovirus reactivation experienced recurrent reactivations, mainly in the high-risk group (cytomegalovirus-seronegative donor/cytomegalovirus-seropositive recipient). The positive predictive value of T-Track® CMV (patients with a negative test after the first reactivation experienced at least one recurrent reactivation) was 84.2% in high-risk patients. Kaplan-Meier analysis revealed a higher probability of recurrent cytomegalovirus reactivation in high-risk patients with a negative test after the first reactivation (hazard ratio 2.73; p=0.007). Interestingly, a post-hoc analysis considering T-Track® CMV measurements at day 100 post-transplantation, a time point highly relevant for outpatient care, showed a positive predictive value of 90.0% in high-risk patients. Our results indicate that standardized cytomegalovirus-specific cellular immunity monitoring may allow improved risk stratification and management of recurrent cytomegalovirus reactivation after hematopoietic stem cell transplantation. Die Beiträge geben Einblicke in die dynamische Entwicklung der Schnittstellen von Erwerbsarbeit und beruflicher Aus-, Fort- und Weiterbildung im Kontext der Digitalisierung Arbeits- und Lernmitteln. Der Band schließt damit an die 2019 ebenfalls im Universitätsverlag der Technischen Universität Berlin erschienene Publikation „Berufsbildung am Bau digital“ (hrsg. von Bernd Mahrin und Johannes Meyser) an. Das erste Kapitel erörtert grundsätzliche didaktische Fragen zu digital unterstütztem Lernen und Arbeiten einschließlich der Rahmenbedingungen. Im zweiten Kapitel schließen sich Beiträge zur Kapazitätsentwicklung, zu Standards und zu digitalen Werkzeugen an. Das dritte Kapitel widmet sich konkreten Einzellösungen mit starkem Praxisbezug und hohem Transferpotenzial zum digitalisierten Arbeiten und Lernen im Bausektor und im Metallbereich. Das abschließende vierte Kapitel präsentiert übergreifend nutzbare und frei zugängliche Online-Angebote wie einen Medienpool für Bildungszwecke, eine Lernmedien-Datenbank und ein hybrides Lernsystem mit virtuellem 3D-Gebäudemodell. The volume is thus connected to the publication “Berufsbildung am Bau digital” (edited by Bernd Mahrin and Johannes Meyser), which was published in 2019 by the University Press of the Technische Universität Berlin. The first chapter discusses fundamental didactic questions about digitally supported learning and working, including the framework conditions. The second chapter picks contributions on capacity development, standards, and digital tools out as central themes. The third chapter is dedicated to concrete specific solutions with strong practical relevance and high transfer potential for digitized work and learning in the construction sector and in the metal sector. The final fourth chapter presents comprehensive and freely accessible online offers such as a media pool for educational purposes, a learning media database and a hybrid learning system with a virtual 3D building model.
