    The perceptions and performance of family-centered care among pediatric nurses at a children's hospital in South Korea: a descriptive study

    Purpose This study aimed to examine pediatric hospital nurses' perceptions and performance of family-centered care. Methods A descriptive study design was used. This study surveyed 162 nurses who worked at a single tertiary children's hospital in South Korea. The modified Family-Centered Care Scale was used to assess nurses' perceptions and performance of family-centered care. Barriers to the implementation of family-centered care were described in an open-ended format. Results Pediatric hospital nurses had a higher score for perceptions (mean score=4.07) than for performance (mean score=3.77). The collaboration subscale had the lowest scores for both perceptions and performance. The perceptions of family-centered care differed significantly according to the nurses’ clinical career in the pediatric unit and familiarity with family-centered care, while performance differed according to clinical career only. Perceptions and performance were positively correlated (r=.594, p<.001). Barriers to implementation included a shortage of nursing personnel, a lack of time, and the absence of a family-centered care system. Conclusion To improve the performance of family-centered care, nurses’ perceptions of family-centered care should be improved by offering education programs and active support, including sufficient staffing, and establishing systems within hospitals

    Prognostic factors for aorta remodeling after thoracic endovascular aortic repair of complicated chronic DeBakey IIIb aneurysms

    ObjectivesThe use of thoracic endovascular aortic repair (TEVAR) for chronic DeBakey III type b (CDIIIb) aneurysms is controversial. We analyzed the potential prognostic factors affecting aorta remodeling after this procedure.MethodsA total of 20 patients with CDIIIb aneurysms underwent TEVAR, with full coverage of reentry tears at the descending thoracic aorta. The potential factors affecting false lumen (FL) remodeling were analyzed, including reentry tears (communicating channels visible on the computed tomography angiogram), large intimal tears below the stent graft (≥2 consecutive axial cuts on the computed tomography angiogram), visceral branches arising from the FL, and intercostal arteries (ICAs) arising from the FL.ResultsAll the patients had uneventful in-hospital courses; 2 patients (10%) required reintervention during the follow-up period. Thirteen patients (65%) had complete thrombosis of the FL at stent graft segment. Compared with the complete thrombosis group, the partial thrombosis group had more reentry tears (1.8 vs 2.3, P = .48), large intimal tears (0.8 vs 1.7, P < .05), visceral branches arising from the FL (1.2 vs 2.3, P < .05), and ICAs arising from the FL (3.8 vs 5.1, P = .35). Reentry tears, visceral branches, and ICAs from the FL were significant negative prognostic factors for FL shrinkage (P < .05).ConclusionsAlthough reentry tears above the celiac trunk were fully covered, the visceral branches and ICAs from the FL and all communicating channels below the celiac trunk kept the FL pressurized and were unfavorable prognostic factors for aorta remodeling after TEVAR for CDIIIb aneurysms

    On the Nature of Propagating Intensity Disturbances in Polar Plumes during the 2017 Total Solar Eclipse

    The propagating intensity disturbances (PIDs) in plumes are still poorly understood and their identity (magnetoacoustic waves or flows) remains an open question. We investigate PIDs in five plumes located in the northern polar coronal hole observed during the 2017 total solar eclipse. Three plumes are associated with coronal bright points, jets and macrospicules at their base (active plumes) and the other two plumes are not (quiet plumes). The electron temperature at the base of the plumes is obtained from the filter ratio of images taken with the X-ray Telescope on board Hinode and the passband ratio around 400 nm from an eclipse instrument, the Diagnostic Coronagraph Experiment (DICE). The phase speed (v_r), frequency (omega), and wavenumber (k) of the PIDs in the plumes are obtained by applying a Fourier transformation to the space-time (r-t plane) plots in images taken with the Atmospheric Imaging Assembly (AIA) in three different wavelength channels (171 A, 193 A, and 211 A). We found that the PIDs in the higher temperature AIA channels, 193 and 211 A, are faster than that of the cooler AIA 171 A channel. This tendency is more significant for the active plumes than the quiet ones. The observed speed ratio (~1.3) between the AIA 171 and 193 A channels is similar to the theoretical value (1.25) of a slow magnetoacoustic wave. Our results support the idea that PIDs in plumes represent a superposition of slow magnetoacoustic waves and plasma outflows that consist of dense cool flows and hot coronal jets.Comment: 18 pages, 8 figures, accepted for publication in Ap

    Distribution and antimicrobial resistance profiles of bacterial species in stray cats, hospital-admitted cats, and veterinary staff in South Korea

    Antimicrobial resistance is becoming increasingly important in both human and veterinary medicine. According to the One Health concept, an important step is to monitor the resistance patterns of pathogenic bacteria. In this study, the antimicrobial susceptibility patterns and trends of bacteria isolated from stray cats, hospital-admitted cats, and veterinary staff in South Korea between 2017 and 2018 were investigated. The minimum inhibitory concentrations of different antibiotics for Staphylococcus spp., Enterobacteriaceae, and Enterococcus spp. were determined to establish representatives of different antibiotic classes relevant for treatment or surveillance. For Coagulase-positive and Coagulase-negative Staphylococci, resistance to fluoroquinolones was below 13%, but resistance to ampicillin and penicillin was high (20–88%). A total of 9.5, 12.1, and 40.3% of staphylococcal isolates from stray cats, hospital-admitted cats, and veterinary staff, respectively, were confirmed to be mecA positive. For Enterobacteriaceae, resistance to carbapenems, fluoroquinolones, and 3rd generation cephalosporins was low (0–11.1%). The Enterococcus spp. isolates showed no resistance to vancomycin. The antimicrobial resistance rates of the Staphylococcus spp. and Enterobacteriaceae isolates from stray cats were usually lower than those of isolates from hospital-admitted cats and veterinary staff, but the Enterococcus spp. isolates revealed the opposite. Thus, the antimicrobial resistance varied across bacterial species according to the source from which they were isolated. Resistance to critically important compounds were low.     Implant surface factors and bacterial adhesion: a review of the literature

    The microbiota that forms on implant surfaces placed in the human body can be highly resistant to antimicrobial agents and in some cases cause life-threatening infections. Consequently, to limit bacterial attachment to these surfaces and thereby minimize the risk of implant infection, the process of biofilm formation and bacterial attachment must be well-understood. The oral environment is considered to be an excellent model for research into biofilm formation and implant infection, accounting for many studies carried out in the field of dental medicine. Those studies show that the roughness, free energy, and material characteristics of the implant surface largely determine initial bacterial adhesion.     What should medical students know about artificial intelligence in medicine?

    Artificial intelligence (AI) is expected to affect various fields of medicine substantially and has the potential to improve many aspects of healthcare. However, AI has been creating much hype, too. In applying AI technology to patients, medical professionals should be able to resolve any anxiety, confusion, and questions that patients and the public may have. Also, they are responsible for ensuring that AI becomes a technology beneficial for patient care. These make the acquisition of sound knowledge and experience about AI a task of high importance for medical students. Preparing for AI does not merely mean learning information technology such as computer programming. One should acquire sufficient knowledge of basic and clinical medicines, data science, biostatistics, and evidence-based medicine. As a medical student, one should not passively accept stories related to AI in medicine in the media and on the Internet. Medical students should try to develop abilities to distinguish correct information from hype and spin and even capabilities to create thoroughly validated, trustworthy information for patients and the public
