Primary aldosteronism - why is the diagnosis difficult?
Pierwotny hiperaldosteronizm (PA) jest zespołem
patogenetycznie niejednorodnym. Może on być
spowodowany pojedynczym lub mnogim gruczolakiem
bądź rakiem warstwy kłębkowatej nadnerczy,
przerostem tej warstwy w obu nadnerczach
lub tylko w jednym, defektem genetycznym steroidogenezy
(hiperaldosteronizm rodzinny typu I
i II) lub nadmiernym wytwarzaniem aldosteronu
przez pozanadnerczowo położoną tkankę nowotworową.
Klasyczne objawy hiperaldosteronizmu
pierwotnego (nadciśnienie tętnicze, hipokaliemia
uwarunkowana nerkową utratą potasu, alkaloza
nieoddechowa, kliniczne objawy niedoboru potasu
w ustroju) stwierdza się u zaledwie 20-40%
chorych, w tym najczęściej z gruczolakiem warstwy
kłębkowatej nadnerczy. U tych chorych udaje
się też prawie zawsze wykazać pełną autonomię
sekrecji aldosteronu, czego nie stwierdza się zawsze
w innych postaciach morfologicznych pierwotnego
aldosteronizmu. Wprowadzenie do rutynowej
diagnostyki oznaczania aktywności reninowej
osocza lub stężenia reniny w osoczu krwi oraz
tak zwanego współczynnika aldosteronowo-reninowego
w istotny sposób zwiększyło częstość rozpoznawania
pierwotnego aldosteronizmu wśród
chorych z nadciśnieniem tętniczym niewykazujących
hipokaliemii. Różnicowanie PA z monogenowymi
postaciami niskoreninowego nadciśnienia
tętniczego lub z tzw. niskoreninowym samoistnym
nadciśnieniem często wymaga przeprowadzenia
badań genetycznych. Pytanie: czy mamy do czynienia
z epidemią PA i wynikającą z niej potrzebą
opracowania nowych strategii diagnostycznych
i leczniczych - jest przedmiotem kontrowersyjnych
poglądów.Primary hyperaldosteronism (PA) is a pathogenically heterogenous
syndrome. It may be caused by: a) solitary or
multiple adenomata or carcinoma of the adrenal glomerular
zone, b) by unilateral or bilateral micro- or
macromodular hyperplasia of this zone, c) by a genetic
defect of steroidogenesis (famillial hyperaldosteronism type
I and II) oraz d) by extraadrenal aldosterone synthesis
(paraendocrinopathy). The classical signs and symptoms
of PA (aterial hypertension, hypokaliemia caused by
incressive renal K excretion, hypokalemic alkalosis, clinical
symptoms of K depletion) are found only in 20–40% of
patients with PA predominantly in those with solitary adenoma
of the adrenal glomerular zone. In these patients
almost always autonomous hypersecretion of aldosterone
may be found, while in the other pathological forms of PA
this is not always the case. Routine estimation of plasma
renin activity or renin concentration respectively and of
the aldosterone-renin ratio (ARR) significantly increased
the frequency of PA diagnosis in normokalemic hypertensive
patients. In patients with monogenic forms of low-renin hypertension and of so-called low-renin essential hypertension
performance of genetic examinations are often
necessary. The question, is there an epidemic of primary
aldosteronism which needs elaboration of new diagnostic
and therapeutic strategies in hypertensive patients - is
subject of continual controversy
New potential targets of pharmacological attack in patients with hypertension
Patogeneza nadciśnienia tętniczego jest niewątpliwie
wieloczynnikowa. Głównymi determinantami
ciśnienia tętniczego są wolemia i opór naczyń
krwionośnych. Te dwa ogniwa są przedmiotem
ataku farmakologicznego przez główne grupy leków
o działaniu hipotensyjnym, czyli diuretyków,
sympatykolityków, antagonistów wapnia i leków
blokujących kaskadę reninowo-angiotensynowoaldosteronową.
Poznanie nowych ogniw patogenezy
nadciśnienia tętniczego stało się przyczyną
poszukiwania nowych leków przeciwnadciśnieniowych.
Należą do nich leki oddziałujące na:
WNK kinazy, „tratwy lipidowe” i kaweole błon
komórkowych, kotransporter sodowo-potasowodwuchlorkowy
1 i 2 (NKCC1, NKCC2), receptorowe
i niereceptorowe kinazy tyrozynowe, krążące
we krwi „mikrocząsteczki”, hormonalną czynność
tkanki tłuszczowej, strukturę macierzy pozakomórkowej
naczyń, limfocyty.
W najbliższej przyszłości należy oczekiwać wyników
badań nad przydatnością leczniczą nowych leków
oddziałujących na wymienione ogniwa nadciśnienia
tętniczego.

multifactorial. The main determinants of blood pressure
are volemia and blood vessel resistance. These two determinants
are the target of all available antihypertensive
drugs (diuretics, sympathicolytics, Ca-blockers, blockers of
the RAA cascade). Identification of new pathogenetic factors
of arterial hypertension initiated studies of new drugs
targeting: WNK kinases, „lipid rafts” and caveolae of
plasma membranes, Na+, K+, 2Cl co-transporter 1 and
two (NKCC1 and NKCC2), receptor and nonreceptor tyrosine
kinases, circulating microparticles, endocrine function
of the adipose tissue, extracellular matrix of blood vessels,
lymphocytes.
In the near future we may expect data reporting on the
suitability of new antihypertensive drugs targeting the
above mentioned pathogenetic factors T.

fibrotyzacji nerek wydaje się nie ulegać wątpliwości.
Wiodącą rolę w regulacji gospodarki sodowej odgrywają
nerki, znajdujące się pod wpływem wielu czynników
modulujących nasilenie natriurezy. Przedmiotem
pracy jest zwięzła informacja dotycząca pozanerkowej
regulacji gospodarki sodowej i ciśnienia
tętniczego przez czujniki sodowe macierzy płynu
śródmiąższowego oraz śródbłonków naczyniowych.

Siarkowodór jest obok tlenku azotu i tlenku CO trzecim gazowym transmiterem występującym u człowieka. Wydaje się, że związek ten uczestniczy w regulacji ciśnienia tętniczego (wykazuje działanie hipotensyjne), patogenezie zmian miażdżycowych naczyń (spowalnia ich rozwój) i progresji różnych nefropatii (zwalnia progresję). Jego działanie wydaje się być ściśle powiązane z funkcją NO i CO, tworząc koncertowe trio lub „triumwirat” regulacyjny. Rola H2S w patogenezie stanów zapalnych nie została dotychczas jednoznacznie zdefiniowana. Również rola H2S w gospodarce energetycznej (asocjowanej z fosforylacją oksydacyjną) nie jest precyzyjnie określona. Należy się spodziewać, że odkrycia substancji blokujących lub stymulujących syntezę H2S zaowocują produkcją nowych leków oddziałujących na układ sercowo-naczyniowo-nerkowy

Wyniki badań doświadczalnych i klinicznych potwierdzają
jednoznacznie udział aldosteronu w procesie
włóknienia naczyń, szkliwienia kłębuszków
nerkowych i progresji chorób nerek oraz przebudowy
mięśnia sercowego u chorych na nadciśnienie tętnicze.
Stały się one podstawą wprowadzenia do praktyki
klinicznej leków antagonizujących działanie aldosteronu,
zapobiegających lub leczących zaburzenia
sercowo-naczyniowe oraz zwalniających progresję
różnych glomerulopatii. Szkodliwe działania aldosteronu
mogą być wyzwalane drogą stymulacji MR
i ekspresji genu MDM2 (szlak genomowy) lub generacji
reaktywnych rodników tlenowych i cytokin prozapalnych
(szlak niegenomowy). Potwierdzono skuteczność
leczniczą stosowania antagonistów MR
w nefropatii cukrzycowej i w innych glomerulopatiach,
u chorych po przebytym zawale serca oraz u chorych
z aktywnymi zmianami miażdżycowymi naczyń
krwionośnych. Tak więc antagoniści aldosteronu stanowią
ważną składową współczesnych strategii leczniczych
stosowanych u chorych na nadciśnienie tętnicze
i choroby nerek.

fibrosis, glomerulosclerosis, progression of different
glomerulopathies and heart muscle remodelling in hypertensive
patients has already been proven, both in clinical
and experimental studies. This fact was the reason why
aldosterone antagonists or blockers of mineralocorticosteroid
receptors (MR) became important drugs
used in prevention or/and treatment of cardiovascular injuries
in hypertensive patients and in patients with chronic
renal glomerulopathies. Most of the damaging effects of
aldosterone are induced by stimulation of the MRs and
expression of the MDM2 gene (genomic pathway), by
stimulation of proinflammatory cytokines and by generation
of reactive oxygen species - ROS (nongenomic pathway).
These injuring effects of aldosterone can be prevented
or attenuated by using specific or nonspecific aldosterone
antagonists. Therapeutic benefits of aldosterone
treatment have already been proven in patients with diabetic nephropathy and other glomerulopathies, in postacute
myocardial infarction patients and in patients with
active atherosclerotic vascular lesions. Thus aldosterone
antagonists seem to be important components of contemporary
therapeutic strategies used in hypertensive patients
and patients with kidney diseases.

w patogenezie nadciśnienia tętniczego, zmian morfologicznych
i czynnościowych naczyń krwionośnych
i serca, zaburzeń gospodarki wodno-elektrolitowej
i innych stanów chorobowych jest znana od wielu
lat. W ostatnich 10 latach wykryto nowe fakty odnośnie
roli układu RAA, mogące mieć istotne implikacje
lecznicze. Wśród nich wymienić należy: 1) odkrycie
receptorów prorenino-reninowych, 2) poznanie
funkcji konwertazy 2 angiotensyny-1-10,
angiotensyny-1-7 i jej receptora, 3) syntezę niektórych
ogniw układu RAA przez trzewną tkankę tłuszczową,
4) poznanie nowych szlaków patogenetycznych indukowanych
aldosteronem, niezwiązanych lub niekoniecznie
związanych z działaniem hipertensjogennym
tego hormonu lub jego wpływem na gospodarkę wodno-elektrolitową. Celem pracy było zwięzłe omówienie
patogenetycznych i leczniczych implikacji poznanych
nowych faktów dotyczących układu RAA.The role of the renin-angiotensin-aldosterone system
(RAAS) in the pathogenesis of arterial hypertension, cardiovascular
abnormalities, water-electrolyte disturbances,
and many other pathological settings has been known for
many years. In the last ten years new functions of the
RAAS were discovered which may be of essential therapeutic
relevance. Among them the following are worth
mentioning: discovery and function of prorenin-renin
receptors, angiotensin-1-7 and its receptor, converting
enzyme 2 of angiotensin-1-10, identification of visceral
fat tissue as a site of RAAS components synthesis and
discovery of many pathogenetic effects of aldosterone, not
related to the hypertensogenic action of this hormone. In
this overview potential pathogenetic and therapeutic implication
of the above mentioned new facts concerning
RAAS are discussed
Perinatal hospice care in the opinion of nurses and midwives
Objectives: Perinatal medicine is a relatively new, dynamically developing branch of medicine. Its main purpose is takingcare of a woman in the pre-conception period, pregnancy and delivery, as well as taking care of a newborn baby.The main aim of the study was to assess the state of knowledge and opinion on hospice perinatal care of professionallyactive nurses and midwives.Material and methods: An original and anonymous questionnaire containing 30 questions was used for the study. 572 nursesand midwives from the Silesian Voivodeship took part in the study. The obtained data were analyzed.Results: Only 31.6% of respondents defined the level of their knowledge of pregnancy and neonatal care as high. 12.8%of respondents were able to indicate the definition of perinatal care and accurately determine its goals. The women participatingin the study were in favor of enclosing the information about not attempting resuscitation (DNAR) in medicalrecord of children with incurable disease diagnosed in fetal life (99.3%).Conclusions: The study showed deficits in practical and theoretical knowledge of nurses and midwives in the area ofhospice perinatal care. Lack of proper preparation is also one of the most frequently mentioned difficulties in taking careof a child and family with poor prognosis
A new look at the etiopathogenesis ofadolescent idiopathic scoliosis
Adolescent idiopathic scoliosis is the most common form of spinal deformity in children. The aetiology of the condition has
not been elucidated. Currently, the multifactorial theory seems to be the most probable. Certain authors propose that
melatonin should be considered as a causative factor of adolescent idiopathic scoliosis. Their assumption is supported by a range of research studies conducted on animal models with removed pineal gland, which induced scoliosis. Melatonin has
been proven to exert direct and indirect effects on the development of the skeletal system. The role of calmodulin
or osteoprotegerin seems equally important. In patients with this condition, the levels of platelet calmodulin and calmodulin
in the specimens of the paraspinal muscles on the convex side of the curve have been shown to rise. Osteoprotegerin, in turn,
modifies osteoclastic and osteoblastic differentiation. These substances have a direct influence on the cellular calcium and
phosphate metabolism and can be potentially responsible for spinal deformity in adolescents. The role of oestrogens is being
investigated. Moreover, the role of growth factors or thrombospondins still remains obscure. Additionally, molecular tests have revealed a number of genes that can predispose to adolescent idiopathic scoliosis. It still needs to be determined which of the musculoskeletal disorders occur first in the development of scoliosis and which are secondary to the deformity. The identification of the aetiological factor and factors responsible for scoliosis progression determines the manner
of treatment
Podstawy immunopatogenetyczne i doświadczenia kliniczne zastosowania leków biologicznych w terapii przewlekłych kłębuszkowych zapaleń nerek u dzieci
Zespół nerczycowy u dzieci, rozwijający się najczęściej na podłożu zmian minimalnych, ogniskowego szkliwienia
kłębuszków nerkowych bądź w przebiegu chorób układowych, jest następstwem dysfunkcji układu
immunologicznego. Pośrednim tego dowodem jest ustępowanie klinicznych i laboratoryjnych objawów
choroby pod wpływem steroidów nadnerczowych oraz leków immunosupresyjnych. Występowanie steroidoopornych,
alkiloopornych, a także steroido- i cyklosporynozależnych przypadków stwarza konieczność
poszukiwania nowych form terapii. Dużą nadzieję wiąże się z grupą leków określanych mianem leków
biologicznych. Ich działanie jest precyzyjnie ukierunkowane na wybrane ogniwa układu immunologicznego,
zaangażowane w etiopatogenezę pierwotnych i wtórnych glomerulopatii. W nefrologii dziecięcej do chwili
obecnej znalazł zastosowanie jedynie rytuksymab – chimeryczne przeciwciało monoklonalne przeciw antygenowi
CD20 limfocytów B. Lek ten wywołuje deplecję limfocytów B, a w efekcie hamuje syntezę hipotetycznych
czynników przepuszczalności białka czy też produkcję patologicznych autoprzeciwciał. Stosowanie
rytuksymabu u dzieci uznawane jest za „terapię ratunkową”, dopuszczoną jedynie w przypadkach
niepowodzenia innych form leczenia. Wskazania obejmują: steroidooporny zespół nerczycowy na podłożu
FSGS, MCNS, nefropatie w przebiegu tocznia trzewnego, nawrót FSGS w nerce przeszczepionej. Pomimo
zadowalających wyników leczenia biologicznego, raportowanych przez większość autorów, nie w pełni
zrozumiały i przewidywalny jest zakres zaburzeń w układzie immunologicznym, indukowany jego podaniem.
Wywoływanie i podtrzymywanie remisji zespołu nerczycowego nie pozostaje w prostej zależności
ze stopniem i czasem trwania deplecji limfocytów B, a powodzenie leczenia bywa zależne od aktywności
choroby, nasilenia białkomoczu i stosowanego uprzednio leczenia immunosupresyjnego. Pełna ocena przydatności
i bezpieczeństwa leczenia biologicznego w glomerulopatiach u dzieci wymaga dalszych badań
z udziałem większej liczby chorych. Najistotniejsze wydają się właściwa kwalifikacja dzieci do tej formy
leczenia oraz podjęcie decyzji o jego zastosowaniu we właściwym momencie
Melatonin – apleiotropic hormone
Melatonin, a tryptophan derivative, is synthesised in mammals mainly in the pineal gland. It coordinates the biological clock
by regulating the circadian rhythm. Its production is dependent on light and its concentrations change with age. Thanks to its
specific chemical structure, melatonin is capable of crossing all biological barriers in the organism and affecting other tissues
and cells, both in indirect and direct ways. Its mechanism of action involves binding with membrane receptors, nuclear
receptors and intracellular proteins. Melatonin shows antioxidant activity. Moreover, its immunomodulatory and antilipid
effects as well as its role in secreting other hormones, such as prolactin, luteinizing hormone, follicle-stimulating hormone,
somatotropin, thyroliberin, adrenocorticotropin hormone or corticosteroids, are essential. In the recent years, research
studies have been mainly focussed on the potential influence of melatonin on the aetiology and development of various
disease entities, such as sleep disorders, gastrointestinal diseases, cancers, psychiatric and neurological conditions,
cardiovascular diseases or conditions with bone turnover disorders. Indications for melatonin use in paediatrics are being
discussed more and more frequently. Among others, authors debate on its use in dyssomnias in children with
neurodevelopmental disorders, such as attention deficit hyperactivity disorder, supportive treatment in febrile seizures and
epilepsy as well as potential use in paediatric anaesthesia. The molecular mechanism and broad-spectrum action of melatonin have not been sufficiently researched and its clinical relevance is often underestimated. This hormone is a promising link in achieving alternative therapeutic solutions
