    Stochastic optimal control and algorithm of the trajectory of horizontal wells

    AbstractThis paper presents a nonlinear, multi-phase and stochastic dynamical system according to engineering background. We show that the stochastic dynamical system exists a unique solution for every initial state. A stochastic optimal control model is constructed and the sufficient and necessary conditions for optimality are proved via dynamic programming principle. This model can be converted into a parametric nonlinear stochastic programming by integrating the state equation. It is discussed here that the local optimal solution depends in a continuous way on the parameters. A revised Hooke–Jeeves algorithm based on this property has been developed. Computer simulation is used for this paper, and the numerical results illustrate the validity and efficiency of the algorithm

    Improvement of k-means Clustering Algorithm for Analyzing the Morphology of Ice Ridge Sails

    An improved k-means clustering algorithm is proposed after analyzing the disadvantages of the traditional k-means algorithm. The cluster centers are initialized by combining the sample mean and standard deviation, the optimal cluster centers are searched by the hybridizing particle swarm optimization and traditional k-means algorithm, and the criterion function is improved during the iteration process to search the optimal number of clusters. The theory analysis and experimental results show that the improved algorithm not only avoids the local optima, also has greater searching capability than the traditional algorithm. This improved algorithm is used to analyze the morphology of the ridge sail (the upper surface of ice ridges). The comparison with the measured data shows that the influences of the geographical locations and the growing environments on the formation of ice ridges can be perfectly reflected by the clustered results

    Thermal Diffusivity Identification of Distributed Parameter Systems to Sea Ice

    A method of optimal control is presented as a numerical tool for solving the sea ice heat transfer problem governed by a parabolic partial differential equation. Taken the deviation between the calculated ice temperature and the measurements as the performance criterion, an optimal control model of distributed parameter systems with specific constraints of thermal properties of sea ice was proposed to determine the thermal diffusivity of sea ice. Based on sea ice physical processes, the parameterization of the thermal diffusivity was derived through field data. The simulation results illustrated that the identified parameterization of the thermal diffusivity is reasonably effective in sea ice thermodynamics. The direct relation between the thermal diffusivity of sea ice and ice porosity is physically significant and can considerably reduce the computational errors. The successful application of this method also explained that the optimal control model of distributed parameter systems in conjunction with the engineering background has great potential in dealing with practical problems

    Identification and Validation of Esophageal Squamous Cell Carcinoma Targets for Fluorescence Molecular Endoscopy

    Dysplasia and intramucosal esophageal squamous cell carcinoma (ESCC) frequently go unnoticed with white-light endoscopy and, therefore, progress to invasive tumors. If suitable targets are available, fluorescence molecular endoscopy might be promising to improve early detection. Microarray expression data of patient-derived normal esophagus (n = 120) and ESCC samples (n = 118) were analyzed by functional genomic mRNA (FGmRNA) profiling to predict target upregulation on protein levels. The predicted top 60 upregulated genes were prioritized based on literature and immunohistochemistry (IHC) validation to select the most promising targets for fluorescent imaging. By IHC, GLUT1 showed significantly higher expression in ESCC tissue (30 patients) compared to the normal esophagus adjacent to the tumor (27 patients) (p n = 17) and high-grade dysplasia (HGD, n = 13) is higher (p n = 7) and to the normal esophagus adjacent to the tumor (n = 5). The sensitivity and specificity of 2-DG 800CW to detect HGD and ESCC is 80% and 83%, respectively (ROC = 0.85). We identified and validated GLUT1 as a promising molecular imaging target and demonstrated that fluorescent imaging after topical application of 2-DG 800CW can differentiate HGD and ESCC from LGD and normal esophagus

    Safety and efficacy of Hypofractionated stereotactic radiosurgery for high-grade Gliomas at first recurrence: a single-center experience.

    BACKGROUND: The optimal treatment for recurrent high-grade gliomas (rHGGs) remains uncertain. This study aimed to investigate the efficacy and safety of hypofractionated stereotactic radiosurgery (HSRS) as a first-line salvage treatment for in-field recurrence of high-grade gliomas. METHODS: Between January 2016 and October 2019, 70 patients with rHGG who underwent HSRS were retrospectively analysed. The primary endpoint was overall survival (OS), and secondary endpoints included both progression-free survival (PFS) and adverse events, which were assessed according to Common Toxicity Criteria Adverse Events (CTCAE) version 5. The prognostic value of key clinical features (age, performance status, planning target volume, dose, use of bevacizumab) was evaluated. RESULTS: A total of 70 patients were included in the study. Forty patients were male and 30 were female. Forty-nine had an initial diagnosis of glioblastoma (GBM), and the rest (21) were confirmed to be WHO grade 3 gliomas. The median planning target volume (PTV) was 16.68 cm3 (0.81–121.96 cm3 ). The median prescribed dose was 24 Gy (12–30 Gy) in 4 fractions (2–6 fractions). The median baseline of Karnofsky Performance Status (KPS) was 70 (40–90). With a median follow-up of 12.1 months, the median overall survival after salvage treatment was 17.6 months (19.5 and 14.6 months for grade 3 and 4 gliomas, respectively; p = .039). No grade 3 or higher toxicities was recorded. Multivariate analysis showed that concurrent bevacizumab with radiosurgery and KPS \u3e 70 were favourable prognostic factors for grade 4 patients with HGG. CONCLUSIONS: Salvage HSRS showed a favourable outcome and acceptable toxicity for rHGG. A prospective phase II study (NCT04197492) is ongoing to further investigate the value of hypofractionated stereotactic radiosurgery (HSRS) in rHGG

    Empagliflozin inhibits coronary microvascular dysfunction and reduces cardiac pericyte loss in db/db mice

    BackgroundCoronary microvascular dysfunction (CMD) is a pathophysiological feature of diabetic heart disease. However, whether sodium-glucose cotransporter 2 (SGLT2) inhibitors protect the cardiovascular system by alleviating CMD is not known.ObjectiveWe observed the protective effects of empagliflozin (EMPA) on diabetic CMD.Materials and methodsThe mice were randomly divided into a db/db group and a db/db + EMPA group, and db/m mice served as controls. At 8 weeks of age, the db/db + EMPA group was given empagliflozin 10 mg/(kg⋅d) by gavage for 8 weeks. Body weight, fasting blood glucose and blood pressure were dynamically observed. Cardiac systolic and diastolic function and coronary flow reserve (CFR) were detected using echocardiography. The coronary microvascular structure and distribution of cardiac pericytes were observed using immunofluorescence staining. Picrosirius red staining was performed to evaluate cardiac fibrosis.ResultsEmpagliflozin lowered the increased fasting blood glucose levels of the db/db group. The left ventricular ejection fraction, left ventricular fractional shortening, E/A ratio and E/e′ ratio were not significantly different between the three groups. CFR was decreased in the db/db group, but EMPA significantly improved CFR. In contrast to the sparse and abnormal expansion of coronary microvessels observed in the db/db group, the number of coronary microvessels was increased, and the capillary diameter was decreased in the db/db + EMPA group. The number and microvascular coverage of cardiac pericytes were reduced in the db/db mice but were improved by EMPA. The cardiac fibrosis was increased in db/db group and may alleviate by EMPA.ConclusionEmpagliflozin inhibited CMD and reduced cardiac pericyte loss in diabetic mice

    Determinants of career aspirations of medical students in southern China

    <p>Abstract</p> <p>Background</p> <p>With recent changes in both the Chinese medical system and compensation of medical doctors, the career aspirations of Chinese medical students have become more diverse. Shantou University Medical College has conducted evaluations and instituted programs to enhance student preparedness to enter a variety of medical careers.</p> <p>Methods</p> <p>A survey was conducted with 85 students to evaluate medical career aspirations and their association with family background, personal skills, English language proficiency, and interest in biomedical research, which were considered as possible factors affecting their career interest.</p> <p>Results</p> <p>Chinese students aspire to traditional as well as nontraditional medical careers. A significant minority of students are now interested in nontraditional careers such as medical teaching or research. However, poor proficiency in the English language and lack of computer skills may limit their academic and career opportunities.</p> <p>Conclusion</p> <p>Career aspirations have changed among medical undergraduates. Although many wish to pursue a traditional clinical doctor career, many are interested in research and teaching careers. Factors such as family background, personal characteristics, school mentoring, and extracurricular support may play a role.</p
