    Crucial impacts on career choices: Research to understand the influences on young people’s choices in primary and secondary schools: Executive summary

    This is the executive summary which sets out the findings of European research undertaken by five project partners (The Czech Republic, The United Kingdom, Denmark, Greece and Spain) and lead by a team from the International Centre for Guidance Studies at the University of Derby in the UK. The research was conducted between September 2018 and July 2020. This project has illuminated the similarities and differences in the way young people approach career decision making and the influences which prevail. Whilst there are some differences between the partner countries, largely due to the economic or social conditions which prevail, there are many similarities.     Screening for frailty in older emergency patients and association with outcome

    Older people have a high incidence of adverse outcomes after urgent care presentation. Identifying high-risk older patients early is key to targeting interventions at those patients most likely to benefit. This study used the Frailsafe three-point screening questions amongst older Emergency Department (ED) attendees. Consecutive unplanned ED attendances in patients aged ≥75 were assessed for Frailsafe status. The primary outcome was mortality at 180 days. A Frailsafe screen was completed in 356 patients, of whom 194/356 (54.5%) were Frailsafe positive. The mean age was 85.8 for Frailsafe screen positive and 82.2 for Frailsafe screen negative patients (p 28 days (AOR 3.42, 95% CI 1.41–8.31, p = 0.007) and re-attendance within 30 days of discharge after admission (OR = 2.73, 95% CI 1.27–5.88, p = 0.01). Frailsafe screen results independently predict a range of outcomes amongst older ED attendees

    Perception of academic learning environments and perceived impact on articulation of employability; a mixed methods study

    This study reports on the findings of a mixed methods study that was undertaken to establish student perceptions of academic learning environments and the perceived impact of these on their articulation of employability skills. This was so student perspectives on employability could be used to inform reflection on pedagogic practices for their educators in higher education. Using a purposive sample of 250 students based in a recently modernised Sciences Complex Building in a Higher Education Institution (HEI), the study was cross sectional and descriptive by design. The social learning spaces researched were perceived by participants to provide optimal academic learning environments for their development of knowledge, skills and professionalism through certain signature pedagogies as they progressed through their programmes of study. Students also expressed the view that their acquisition of functional skills were significantly more important than any personal attributes/characteristics that they brought to programmes. What also mattered was whether the importance of certain graduate skills to the workplace had been made explicit to them so that they could see the relevance of their studies to practice.     Secondary messenger signalling influences Pseudomonas aeruginosa adaptation to sinus and lung environments

    Pseudomonas aeruginosa is a cause of chronic respiratory tract infections in people with cystic fibrosis (CF), non-CF bronchiectasis, and chronic obstructive pulmonary disease. Prolonged infection allows the accumulation of mutations and horizontal gene transfer, increasing the likelihood of adaptive phenotypic traits. Adaptation is proposed to arise first in bacterial populations colonizing upper airway environments. Here, we model this process using an experimental evolution approach. Pseudomonas aeruginosa PAO1, which is not airway adapted, was serially passaged, separately, in media chemically reflective of upper or lower airway environments. To explore whether the CF environment selects for unique traits, we separately passaged PAO1 in airway-mimicking media with or without CF-specific factors. Our findings demonstrated that all airway environments-sinus and lungs, under CF and non-CF conditions-selected for loss of twitching motility, increased resistance to multiple antibiotic classes, and a hyper-biofilm phenotype. These traits conferred increased airway colonization potential in an in vivo model. CF-like conditions exerted stronger selective pressures, leading to emergence of more pronounced phenotypes. Loss of twitching was associated with mutations in type IV pili genes. Type IV pili mediate surface attachment, twitching, and induction of cAMP signalling. We additionally identified multiple evolutionary routes to increased biofilm formation involving regulation of cyclic-di-GMP signalling. These included the loss of function mutations in bifA and dipA phosphodiesterase genes and activating mutations in the siaA phosphatase. These data highlight that airway environments select for traits associated with sessile lifestyles and suggest upper airway niches support emergence of phenotypes that promote establishment of lung infection.</p
