Bone metabolism and vitamin D status in patients with multiple sclerosis
Background
Vitamin D (VD), an important factor for bone health immobilization and immune regulation, has been shown to have low serum concentration in multiple sclerosis (MS) patients. Those patients have also multiple fracture risk factors, including progressive immobilization and long-term glucocorticoids treatment. The aim of the study was to analyze bone health (osteopenia or osteoporosis prevalence) and VD serum concentration in MS patients as well as the influence of disease activity and treatment on bone health.
Materials and methods
The study involved 72 MS patients: 52 women and 20 men. Mean age was 40.3±10.5 yrs, mean EDSS (Expanded Disability Status Scale) 3.3±1.9. Bone health was analyzed using standard densitometry in the lumbar spine and femoral neck. Serum levels of VD, calcium, phosphate and parathormone were assessed. We compared two groups of patients with multiple sclerosis: relapsing - remitting MS (RRMS) and progressive relapsing MS (PRMS).
Results
Densitometry revealed osteopenia in twenty-six (36.1%) patients and osteoporosis in eleven (15.3%), no bone fractures were presented. Sixty-eight MS patients (94.4%) had lower VD serum level if compared to population referential values. Thirteen patients (18.1%) had severe VD deficiency. Densitometry parameter (T-score of the lumbar spine) worsened with EDSS increase (r=−0.43, P=0.001). There was a statistically significant negative correlation between VD concentration and EDSS score (r=−0.31; P=0.009).
Conclusions
Our study indicates that patients with MS have high incidence of osteopenia and osteoporosis and vitamin D deficiency. Bone health disturbances studied by densitometry are related to the disability caused by MS
Symptomatic medulla compression by vertebral artery
Objective
Vertebral artery medulla compression syndrome (VAMCS) is a very rare condition manifesting as different neurological focal deficits. The case of a 36-year-old male with symptomatic brainstem compression by vertebral artery (VA) treated by means of microvascular decompression (MVD) and a review of the literature is presented.
Case report
On admission, a 36-year-old patient presented with hypoalgesia, hypothermesthesia and hemiparesis on the left side. Magnetic resonance imaging (MRI) of the head disclosed the right VA loop compressing the ventrolateral medulla and excluded other entities such as brain tumor, stroke and multiple sclerosis. Since displacement and significant compression of the right pyramidal tract was confirmed by diffusion tensor imaging (DTI), neurovascular compression syndrome was diagnosed. The patient underwent MVD of the medulla using a Gore-Tex implant as a separating material via the right far-lateral approach. The left hemiparesis and hemisensory loss remitted rapidly after the procedure. The post-procedural neurological improvement was maintained at one year follow-up. Based on a review of the literature, a total of 33 cases of surgically treated VAMCS has been reported so far.
Conclusion
VAMCS should be considered as the cause of neurological deficits when other pathological entities are ruled out. In symptomatic conflict of the VA with the medulla, microvascular decompression using a Gore-Tex implant can be an effective method of treatment. Nevertheless, a statistical analysis on all reported cases showed favorable results using the VA repositioning technique when compared with MVD (success rate 91% vs. 58%, p<0.05)
The use of sensory perception of blind and visually impaired masseurs for the purpose of early-stage breast cancer screening by palpation (the “Breast-POL” project)
Introduction. Methodological assumptions as well as the initial stage of the execution of the project entitled “Breast-POL” — the use of sensory perception of blind and visually impaired masseurs for the purpose of breast cancer screening by palpation were presented.
Materials and methods. A list of incentives to conduct this methodologically adequate study includes: 1. Encouraging and widely disseminated German reports (these reports, however, were of high generality and they did not provide the adequate methodology); 2. Theoretical data on the hypercompensation of other senses than vision in the blind; 3. Preliminary information on the involvement of the blind in breast cancer screening by palpation in developing countries.
Results. Compared to people with normal vision, a statistically significant higher sensitivity (63.0 vs 47.5%) and lower specificity (89.6 vs 93.7%) in the detection of pathological lesions, using a phantom, was reported in the blind.
Conclusion. Our study confirms that the detection of pathological breast lesions by the blind is highly effective, however, the ability to exclude such lesions was not of clinical significance
High normal blood pressure - clinical fact or myth?
W ostatniej dekadzie dyskutuje się nad klinicznym
znaczeniem wysokich prawidłowych wartości ciśnienia
tętniczego. Szacuje się, że rozpowszechnienie
ciśnienia wysokiego prawidłowego może wynosić
około 30% i wiąże się z większym niż ciśnienie
optymalne ryzykiem sercowo-naczyniowym. Częściej
również towarzyszą mu inne czynniki ryzyka
rozwoju chorób układu sercowo-naczyniowego,
w tym cechy typowe dla zespołu metabolicznego.
Według obecnych zaleceń ESH-ESC wysokie prawidłowe
ciśnienie tętnicze wymaga postępowania
niefarmakologicznego, o ile nie współistnieje z chorobami
nerek lub układu sercowo-naczyniowego.
Obecnie brakuje przekonujących danych o konieczności
terapii farmakologicznej osób z ciśnieniem
wysokim prawidłowym obciążonych niskim ryzykiem
sercowo-naczyniowym.During last decade, an increased interest in the clinical
significance of high normal blood pressure has arisen. Its
prevalence is estimated at around 30% and is associated
with increased cardiovascular risk. Other cardiovascular
risk factors, including metabolic syndrome, are also related
to high normal blood pressure values. According to ESH/
/ESC guidelines, lifestyle modifications alone should be
implemented in patients with high normal blood pressure,
unless it coexists with renal or cardiovascular diseases. Current
data regarding the necessity of pharmacotherapy in
patients with high normal blood pressure and low cardiovascular
risk are unconvincing
Jak rozpoznać wtórne postacie nadciśnienia tętniczego?
Nadciśnienie tętnicze u 95% chorych ma charakter pierwotny, co oznacza, że nie można
jednoznacznie ustalić jego przyczyn. Jednak u około 5% pacjentów wzrost ciśnienia
tętniczego jest spowodowany określonym procesem chorobowym. Wśród najczęstszych
schorzeń, których przebiegowi towarzyszy wzrost ciśnienia, wymienia się obturacyjny
bezdech senny, choroby miąższu nerek, zwężenie tętnic nerkowych oraz pierwotny hiperaldosteronizm.
Ustalenie rozpoznania wtórnej postaci nadciśnienia ma istotne znaczenie.
Po pierwsze, pozwala niekiedy na skuteczne usunięcie przyczyny nadciśnienia
i całkowitą normalizację ciśnienia. Po drugie, chroni przed innymi skutkami rozwijającej
się choroby. W rozpoznawaniu wtórnych postaci nadciśnienia mogą być pomocne
charakterystyczne cechy kliniczne rozpoznane na podstawie wywiadów i badania przedmiotowego.
Można wśród nich wymienić: młody lub późny wiek rozwoju nadciśnienia,
nagłe pogorszenie kontroli ciśnienia lub oporność na leczenie farmakologiczne. Podejrzenie
wtórnej postaci udaje się niekiedy potwierdzić, wykonując ogólnie dostępne badania
dodatkowe. Celem uzupełnienia diagnostyki i właściwego leczenia chorzy z wtórnymi
postaciami nadciśnienia tętniczego powinni być kierowani do ośrodków referencyjnych
The effect of short acting hyperoxia on heart rate and blood pressure variability in hypertensive and normotensive individuals
Wstęp Oddychanie 100-procentowym tlenem eliminuje toniczną aktywność chemoreceptorów kłębka szyjnego. We wcześniejszych badaniach wykazano, że krótkotrwała hiperoksja powoduje zmniejszenie wartości ciśnienia tętniczego oraz obniżenie aktywności układu współczulnego ocenianego metodą mikroneurografii. Celem obecnej pracy było sprawdzenie, czy zastosowanie krótkotrwałej hiperoksji wpłynie na aktywność współczulną ocenianą nieinwazyjnie za pomocą analizy zmienności rytmu serca i ciśnienia tętniczego. Materiał i metody Badaniami objęto 10 mężczyzn z pierwotnym nadciśnieniem tętniczym (wiek 39,1 ± 2,9 roku; BMI 27,6 ± 1,0 kg/m2) i 11 zdrowych mężczyzn (wiek 39,7 ± 2,7 roku; BMI 26,1 ± 1,0 kg/ /m2). Uczestnicy otrzymali do oddychania przez bezzwrotną maskę powietrze atmosferyczne, a następnie100-procentowy tlen. Z uzyskanych zapisów EKG i ciśnienia tętniczego analizowano zmienność rytmu serca (HRV) i ciśnienia (BPV). Wyniki Zastosowanie 100-procentowego tlenu do oddychania spowodowało zmniejszenie całkowitej gęstości widma zmienności ciśnienia rozkurczowego i skurczowego (PSD-dBP, PSD-sBP) oraz składowych widma o niskiej częstotliwości (LF-dBP i LF-sBP) w grupie chorych z nadciśnieniem tętniczym. Nie stwierdzono natomiast zmian w stosunku składowej widma o małej i dużej częstotliwości dla ciśnienia skurczowego i rozkurczowego (LF/HF dBP i LH/HF sBP) ani zmian we wskaźnikach zmienności rytmu serca. Wnioski Wyniki badania wskazują na to, że zahamowanie aktywności chemoreceptorów tętniczych poprzez zastosowanie 100-procentowego tlenu do oddychania może zmniejszać aktywność współczulną ocenianą zmiennością ciśnienia tętniczego.Background Previous studies have shown that hyperoxia- induced deactivation of carotid body chemoreceptors produces transient decrease in systolic and mean blood pressure and reduces sympathetic activation measured by microneurography. This study compares the effect of acute hyperoxia on blood pressure and heart rate variability between hypertensive and normotensive subjects. Material and methods Ten males with hypertension (age 39.1 ± 2.9 yrs.; BMI 27.6 ± 1.0 kg/m2) and 11 normotensives males (age 39.7 ± 2.7 yrs.; BMI 26.1 ± 1.0 kg/m2) received, via non-rebreathing mask ventilation, ambient air followed by 100% oxygen for 10 minutes. The blood pressure variability, heart rate variability, blood pressure respiratory rate, and oxygen saturation were recorded continuously. Results Ventilation with 100% oxygen resulted in the decrease in total power density of systolic and diastolic blood pressure variability (PSD-dBP, PSD-sBP) and low frequency bands (LF-dBP i LF-sBP) in hypertensive patients. No changes were noted in ratio of low and high frequency bands (LF/HF) and in parameters of heart rate variability (HRV) in both groups. Influence of endothelin on geometry of heart left ventricule in patients with essential hypertension
Wstęp Przerost mięśnia lewej komory serca stanowi
jedno z głównych powikłań narządowych nadciśnienia
tętniczego. W przebiegu nieleczonego nadciśnienia
tętniczego dochodzi do zmian geometrii lewej
komory. Wpływ wielu czynników hormonalnych na
przerost mięśnia lewej, w tym zmiany jej geometrii,
pozostaje niejasny. W bieżącej pracy zbadano wpływ
endoteliny i innych hormonów na przerost mięśnia
lewej komory u chorych na nadciśnienie tętnicze.
Materiał i metody Do badania włączono 122 nieleczonych
chorych (51K/71M) z łagodnym i umiarkowanym
nadciśnieniem tętniczym oraz 20 zdrowych
ochotników w wieku odpowiednio 45,5 ± 9 i 41,6 ±
6 lat. U wszystkich badanych pobrano krew w celu
oznaczenia endoteliny i katecholamin w pozycji leżącej
po 30 minutach odpoczynku. Całodobowy pomiar
ciśnienia wykonano stosując SpaceLabs 90207.
Echokardiografie wykonywano aparatem Hewlett
Packard AC 77020.
Wyniki W grupie chorych z przerostem mięśnia serca
stwierdzono niższe stężenie endoteliny i noradrenaliny
i wyższe adrenaliny niż u chorych bez przerostu.
Przy porównaniu chorych z prawidłową lewą
komorą, przerostem ekscentrycznym i koncentrycznym
nie stwierdzano istotnych różnic w stężeniu endoteliny
we krwi. W grupie chorych z przebudową
koncentryczną lewej komory stwierdzono istotnie
niższe stężenie endoteliny i noradrenaliny we krwi
w porównaniu z chorymi z przerostem ekscentrycznym
i koncentrycznym i wyższe adrenaliny w stosunku
do chorych z przerostem ekscentrycznym.
Wnioski Wydaje się, że endotelina może wywierać
wpływ na rozwój przerostu mięśnia lewej komory
we wczesnym okresie rozwoju powikłania.Background Left ventricular hypertrophy (LVH) stays main
end-organ complication of arterial hypertension. In a course
of untreated hypertension left ventricule geometry is
changed. An influence of different hormones on LVH and
left ventricular geometry is still unclear. In present study
a potential influence of endothelin and catecholamines on
LVH in hypertensive patients has been investigated.
Material and methods The study included 122 untreated subjects
with mild-to-moderate hypertension (51F/71M) and 20
healthy controls, in mean age 45.5 ± 9 yrs and 41.6 ± 6 yrs
respectively. Blood samples for endothelin and catecholamines
were taken in recumbent position after 30 min of rest. Twenty-four hour ambulatory blood pressure monitoring was obtained
using SpaceLabs 90207. Echocardiography was performed using
Hewlett Packard AC 77020.
Results Higher plasma endothelin and noradrenalin concentrations
and lower plasma adrenalin concentration were
present in hypertensive patients with LVH than in those
without LVH. Plasma endothelin concentration was similar
in patients with normal left ventricle, eccentric and concentric
hypertrophy. Patients with concentric remodeling
had higher plasma endothelin and noradrenalin concentrations
compare to patients with eccentric and concentric
hypertrophy and lower adrenalin concentration compare
to patients with eccentric hypertrophy. Conclusion Presented results may suggest that endothelin
influences LVH during its early stages of development
Late percutaneous coronary intervention for an occluded infarct-related artery in patients with preserved infarct zone viability: A pooled analysis of cardiovascular magnetic resonance studies
Background: The results of clinical trials assessing the effect of late opening of infarct-related artery (IRA) on left ventricular ejection fraction (LVEF) and size in stable patients are equivocal, which may be related to the fact that the presence of viability was not a requirement for randomization in these trials. The aim of the study was to assess the influence of late percutaneous coronary intervention (PCI) with optimal medical therapy (OMT) vs. OMT alone on cardiac function and remodeling in patients presenting infarct zone with preserved viability on cardiovascular magnetic resonance (CMR).Methods: The analysis included pooled data of 43 patients from 3 randomized studies. All patients underwent CMR before randomization, but only in 1 previously unpublished study was preserved viability required for randomization to treatment. Follow-up CMR was performed after 6–12 months.Results: Late PCI with OMT led to improved LVEF (+5 ± 7% vs. –1 ± 6%, p = 0.005), decreased left ventricular end-systolic volume (–11 ± 19 mL vs. 12 ± 40 mL, p = 0.02) and a trend towards a decrease in end-diastolic volume (–7 ± 27 mL vs. 15 ± 47 mL, p = 0.07) in comparison to OMT alone. Increased LVEF and decreased left ventricular volumes were observed after the analysis was restricted to patients with left anterior descending artery (LAD) occlusion.Conclusions: In patients with the presence of infarct zone viability, OMT with late PCI for an occluded IRA (particularly LAD) is associated with improvement of left ventricular systolic function and size over OMT alone
Subcutaneous implantable cardioverter-defibrillator therapy in Poland: Results of the Polish S-ICD Registry
Background: The use of a subcutaneous implantable cardioverter-defibrillator (S-ICD) has been expanding in Poland since 2014. The Polish Registry of S-ICD Implantations was held by the Heart Rhythm Section of the Polish Cardiac Society between May 2020 and September 2022 to monitor the implementation of that therapy in Poland.
Aims: To investigate and present the state-of-the-art of S-ICD implantation in Poland.
Methods: Implanting centers reported clinical data of patients undergoing S-ICD implantations and replacements, including: age, gender, height, weight, underlying disease, history of pacemaker and defibrillator implantations, indications for S-ICD, electrocardiographical parameters, procedural techniques, and complications.
Results: 440 patients undergoing S-ICD implantation (411) or replacement (29) were reported by 16 centers. Most patients were in New York Heart Association class II (218 patients, 53%) or I (150 patients, 36.5%). Left ventricular ejection fraction was 10-80%, median (IQR) 33% (25%‒55%). Primary prevention indications were present in 273 patients (66.4%). Non-ischemic cardiomyopathy was reported in 194 patients (47.2%). The main reason for the choice of S-ICD were: young age (309, 75.2%), risk of infective complications (46, 11.2%), prior infective endocarditis (36, 8.8%), hemodialysis (23, 5.6%), and immunosuppressive therapy (7, 1.7%). Electrocardiographic screening was performed in 90% of patients. The rate of adverse events was low (1.7%). No surgical complications were observed.
Conclusions: Qualification for S-ICD in Poland was slightly different when compared to the rest of Europe. The implantation technique was mostly consistent with the current guidelines. S-ICD implantation was safe, and the complication rate was low
