    Treatment of patient with open fracture of ulnar bone

    The article presents the result of treatment of patient P. with trauma caused by an attack of a bear during the hunting. Patient had open comminuted displaced fracture of the lower third of right ulnar bone with formation of large infected wound of right forearm and right hand with injury of flexor tendons of IV and V fingers, extensor tendon of V finger, ulnar nerve. At the admission to the Central District Hospital of Mama, primary surgical treatment of wound, plaster immobilization were conducted, wound weren't stitched up. At the treatment in Irkutsk Scientific Center of Surgery and Traumatology, secondary surgical treatment of wound of right forearm, necretomy, ultrasound treatment of wound, laceration repair of wound with local tissues, transosseous osteosynthesis of right forearm - hand were conducted. As a result of conducted treatment we achieved total reduction of necrotic and purulent process, repair of vast infected wound of forearm and hand, full adhesion of fracture of ulnar bone and restoration of a function of right arm. Using ultrasound treatment of wounds and transosseous osteosynthesis at openfractures of forearm bones and chronic purulent infection is a treatment of choice and can cause good clinical outcome

    IXODES TICKS AS VECTORS OF TICK-BORNE INFECTIONS IN THE SOUTH OF PRIMORSKY KRAI (KHASANSKY DISTRICT)

    The Khasan district, located in the south of the Primorsky Krai, borders with the Democratic People’s Republic of Korea (DPRK) and  the People’s Republic of China (PRC). This determines the special  importance in terms of epizootological observations of the causative  agents of vector-borne tick infections. To date, such studies have not  been carried out in the Khasan district.Aims: to show infections of ixodid ticks in the south of Primorsky Krai (in the Khasansky District), causative agents of viral and bacterial tick-borne infections.Materials and methods. During the period of 2017, 387 specimens of ixodid ticks were collected from the vegetation. The study of collected ticks was carried out using the real-time polymerase chain reaction method.Results. RNA of the TBE virus was detected in 1.1 %, Borrelia burgdorferi sensu lato DNA in 12.5 %, Borrelia miyamotoi DNA in 0.6 %, Anaplasma phagocytophillum DNA in 4 %, Ehrlichia chaffeensis /  Ehrlichia muris DNA in 1.7 %. In 8 %, rickettsia DNA was detected,  of which Rickettsia heilongjiangensis in 12 cases. Seven cases of  mixed infection of various species of ticks with two pathogens were detected.Conclusions. The data obtained suggest that the pathogens of infections transmitted by ixodid ticks can circulate not only  throughout the border area of the south of the Primorsky Krai  (Khasansky District) but also in the adjacent territories of the PRC and DPRK

    THE PARAMETERS OF THE HEMOSTATIC SYSTEM, GENE POLYMORPHISMS IN CHILDREN WITH THROMBOTIC EVENTS IN THE PEDIATRIC CLINIC OF NOVOSIBIRSK STATE REGIONAL CLINICAL HOSPITAL

    The aim of the research was to study the effect of genetic predictors of thrombotic/ischemic events in children with the developed complications and born mature or premature. Material and methods. A prospective follow-up monitoring of 47 patients (inpatients of the State Novosibirsk Regional Clinical Hospital) with thrombotic complications, the analysis of their medical records were carried out. The homeostasis system parameters, carriage of polymorphic variants of genes predisposing to thrombotic complications were accomplished for the children during the follow-up period. Results and discussion. Thrombotic and/or related ischemic events are more common in premature infants, herewith the severe cerebral ischemia and venous thrombosis localized in various vessels predominated over these complications. The state of the blood coagulation system in children with thrombotic/ischemic events is characterized by hypercoagulation shifts in the external and internal cascades of blood coagulation and accompanied by the increase in the D-dimers level. Among polymorphic variants of genes of hemostasis system the occurrence of polymorphic variant FV Leiden (rs6025) is higher in the study group compared to the European population (χ2 = 7.454; p = 0.025). The obtained data indicate the need for extended diagnostic studies of hemostasis system and genetic search for «thrombogenic» predisposition in children, especially premature, residing in intensive care units for the presence of thrombotic readiness in order to timely predict the development of possible thrombotic and ischemic complications

    Verification of a case of mixed infection with Lyme disease, tick-borne encephalitis and COVID-19

    Background. The relationship between pathogens of new diseases and tick-borne infections is an underinvestigated direction in the problem of infectious diseases.The aim. To show the features of identifying the markers of Borrelia burgdorferi, tickborne encephalitis and SARS-CoV-2 pathogens on the example of a case of a triple mixed infection (Lyme disease, tick-borne encephalitis and COVID-19) and using comprehensive studies.Methods. In 2019–2021, a comprehensive study of 7 blood samples from a patient with mixed infection was carried out. We used real-time polymerase chain reaction (PCR), enzyme immunoassay (ELISA) and determined antigen, IgM, IgG antibodies, and avidity index (AI) of IgG antibodies.Results. Ixodid tick-borne borreliosis was diagnosed in a patient 5 months after contagion. Only high-avid Lyme-IgG antibodies were detected. Low-avid Lyme- IgG antibodies appeared against the background of a reduced general condition. At the same time, high-avid IgG (cut-off index (COI) – 7.8) and IgM (COI = 1.2) antibodies to the TBE virus were detected. In July 2020, the patient was infected with SARSCoV-2. TBE virus which passed into the body simultaneously with Borrelia in the fall of 2019 was activated. Although the patient did not have specific symptoms of TBE, in subsequent blood samples (No. 4, 5, 6) we found TBEV antigen (optical density (OD) – 4.3; 1.9 and 2.0 respectively) and IgM (COI = 1.3; 0,9 and 0 respectively). These results were recognized as TBEV activation, which contributed to an increase in the avidity of IgG antibodies (AI = 65 %; 100 % and 63 % respectively). IgM antibodies to SARSCoV-2 virus were not detected, as opposed to the high levels of IgG (COI = 8.2; 8.1; 8.4 and 14.7 respectively).Conclusions. Therefore, using not only the common methods of diagnosing (PCR and ELISA), but also the determination of the antibody avidity degree, we have shown that when analyzing a case of a triple mixed infection, B. burgdorferi dominates in the human body and causes a long-term chronic course of the disease

    Comprehensive assessment of specific antibodies on infectious activity of tick-borne encephalitis virus

    Vaccines for prophylactic immunization provide the most reliable and effective protection against the vast majority of infectious diseases. Tick-borne encephalitis (TBE) represents a high-priority medical issue at the territory of the Eurasian continent. Of great importance is assessing a role of distinct antibody titers especially low titers, observed quite often in vaccinated individuals, sometimes posing obstacles in determining a threshold of seropositivity as well as the level of specific protection against TBE virus. We aimed at obtaining data to assess antiviral activity of virus-specific antibodies with distinct titer levels based on the in vitro, ex vivo and in vivo experimental studies with a highly virulent Far-Eastern strain of tick-borne encephalitis virus. The in vitro, ex vivo and in vivo comprehensive experimental studies with a highly virulent Far-Eastern strain of tick-borne encephalitis virus (TBEV) were conducted and the dynamics of antiviral activity of virus-specific antibodies at variable titers (1:100–1:3200) was measured (timeframe ranged within 1–96 hours p.i.) to provide a rationale for evaluating the antiviral immune response. It was found that the in vitro experiments demonstrated that the IgG at 1:100 titer exerted a weak anti-TBEV neutralizing effect at all time-points examined. The IgG 1:400 titer caused a 2 log PFU/mL decline in TBEV Dal strain yield at 72 h post-infection, whereas at 1:3200 titer it completely suppressed TBEV replication throughout the observation period. The ex vivo experiments with blood serum obtained from vaccinated subjects demonstrating a range of TBEV antibody titers (sera from vaccinated individuals with varying anti-TBEV antibody titers) and in vivo (outinbred white mice) experiments revealed a delayed virus elimination for antibody titers at 1:100 and 1:200 as well as rapid virus elimination (1–2 days p.i.) for antibody titers greater than 1:400. Thus, antibody titer at 1:400 may be considered as the universal anti-TBEV protection threshold. In order to properly conclude regarding the revaccination schedule it is advised to start with testing blood serum for durability of anti-TBEV immune response. Subjects with TBEV antibody titers at 1:100 and 1:200 should be strongly recommended to undergo a mandatory revaccination. Such an approach is believed to be the most effective way toward enhancing efficacy of vaccine-mediated protection against TBE

    The SPIN@FERMI collaboration has updated its 1991-95 Reports on the acceleration of polarized protons in Fermilab's Main Injector, which was commissioned by Fermilab. This Updated Report summarizes some updated Physics Goals for a 120-150 GeV/c polarized proton beam.
